Ground level utility of Access, Watch, Reserve classification: Insights from a tertiary care center in North India

BACKGROUND The overuse and misuse of antimicrobials contribute significantly to antimicrobial resistance(AMR),which is a global public health *** has particularly high rates of AMR,posing a threat to effective *** World Health Or-ganization(WHO)Access,Watch,Reserve(AWaRe)classification system was introduced to address this issue and guide appropriate antibiotic ***,there is a lack of studies examining the prescribing patterns of antimi-crobials using the AWaRe classification,especially in North ***,this study aimed to assess the prescribing patterns of antimicrobials using the WHO AWaRe classification in a tertiary care centre in North ***,Obstetrics and Gynecology).Metronidazole and ceftriaxone were the most prescribed *** to the AWaRe classification,57.61%of antibiotics fell under the Access category,38.27%in Watch,and 4.11%in *** Access antibiotics were prescribed within the Medicine department,and the same department also exhibited a higher frequency of Watch antibiotics *** questionnaire survey showed that only a third of participants were aware of the AWaRe classification,and there was a lack of knowledge regarding AMR and the potential impact of AWaRe *** The research was carried out in accordance with the methodology presented in Figure 1.A total of n=123 patients were enrolled in this study,with each of them receiving antibiotic *** majority of these prescriptions were issued to inpatients(75.4%),and both the Medicine and Surgical departments were equally represented,accounting for 49.6%and 50.4%,*** the healthcare providers responsible for prescribing antibiotics,72%were Junior Residents,18.7%were Senior Residents,and 9.3%were *** findings have been summarized in Table *** prescriptions included 27 different antibiotics,with metronidazole being the most prescribed(19%)followed by ceftriaxone(17%).Th

Correlation of serum SARS-CoV-2 IgM and IgG serology and clinical outcomes in COVID-19 patients:Experience from a tertiary care centre

BACKGROUND The severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)virus has become a pandemic for the last 2 *** response to the virus leads to organ dysfunction and *** the severity of inflammatory response helps in managing critical patients using serology tests IgG and *** To investigate the correlation of the serology(IgM and IgG)with reverse transcriptase polymerase chain reaction(RT-PCR)status,disease severity[mild to critical],intensive care unit(ICU)admission,septic shock,acute kidney injury,and in-hospital *** We conducted a longitudinal study to correlate serum SARS-CoV-2 immunoglobulin M(IgM)and immunoglobulin G(IgG)serology with clinical outcomes in coronavirus disease 2019(COVID-19)*** analyzed patient data from March to December 2020 for those who were admitted at All India Institute of Medical Sciences *** and laboratory data of these patients were collected from the e-hospital portal and analyzed.A correlation was seen with clinical outcomes and was assessed using MS Excel 2010 and SPSS *** Out of 494 patients,the mean age of patients was 48.95±16.40 years and there were more male patients in the study(66.0%).The patients were classified as mild-moderate 328(67.1%),severe 131(26.8%),and critical 30(6.1%).The mean duration from symptom onset to serology testing was 19.87±30.53 ***-hospital mortality was observed in 25.1%of *** seropositivity rate(i.e.,either IgG or IgM>10 AU)was 50%.IgM levels(AU/mL)(W=33428.000,P≤0.001)and IgG levels(AU/mL)(W=39256.500,P≤0.001),with the median IgM/IgG levels(AU/mL),were highest in the RT-PCR-Positive group compared to RT-PCR-Negative clinical *** was no significant difference between the two groups in terms of all other clinical outcomes(disease severity,septic shock,ICU admission,mechanical ventilation,and mortality).CONCLUSION The study showed that serology levels are high in RT-PCR positive group compared

Practice patterns among ophthalmic surgeons in treating concomitant oculoplastic conditions in preoperative period:A questionnaire-based study

BACKGROUND Addressing oculoplastic conditions in the preoperative period ensures both the safety and functional success of any ophthalmic *** oculoplastic conditions,like nasolacrimal duct obstruction,have been extensively studied,whereas others,like eyelid malposition and thyroid eye disease,have received minimal or no *** To investigate the current practice patterns among ophthalmologists while treating concomitant oculoplastic conditions before any subspecialty ophthalmic *** A cross-sectional survey was disseminated among ophthalmologists all over *** survey included questions related to pre-operative evaluation,anaesthetic and surgical techniques preferred,post-operative care,the use of adjunctive therapies,and patient follow-up *** A total of 180 ophthalmologists responded to the *** practitioners(89%)felt that the ROPLAS test was sufficient during pre-operative evaluation before any subspecialty surgery was *** most common surgical techniques employed were lacrimal drainage procedures(Dacryocystorhinostomy)(63.3%),eyelid malposition repair(36.9%),and ptosis repair(58.7%).Post-operatively,47.7%of respondents emphasized that at least a 4-week gap should be maintained after lacrimal drainage procedures and eyelid ***-seven percent of ophthalmologists felt that topical anaesthetic procedures should be preferred while performing ocular surgeries in thyroid eye disease *** Approximately 50%of ophthalmologists handle prevalent oculoplastic issues themselves,seeking the expertise of an oculoplastic surgeon under particular *** ophthalmologists still favor using ROPLAS as a preliminary screening method before proceeding with cataract *** conditions and thyroid eye disease are not as commonly addressed before subspecialty procedures compared to issues like nasolacrimal duct obstruction and periocular infections.

Several challenges are encountered while integrating microgrids(MGs)with an existing utility *** are low inertia,intermittent nature of renewable energy resources(RESs),sensor and actuator faults,unbalanced and nonlinear loads,supply-demand mismatch,and uncertain switching functions of the power electronic *** control is fully dependent on the communication network,which is also susceptible to different types of failures such as noise in communication links,communication delay,limited bandwidth,packet dropout,and various malicious cyber *** few papers are available in the literature that focus on the review of control schemes for ***,this paper presents a comprehensive review of different robust and adaptive control schemes that address the challenges encountered due to communication constraints,uncertainties,and disturbances for different MG topologies such as AC,DC,and hybrid AC/DC MGs,as well as the current research trends in the field of robust and adaptive *** can be concluded that to achieve control objectives of MG and overcome the existing challenges,robust and adaptive controllers show significantly improved performance in terms of transient and steady-state behavior and robustness as compared to traditional controllers.
