Prognostic value of elevated cardiac and inflammatory biomarkers in patients with severe COVID-19:a single-center,retrospective study

Background:The outbreak of coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus 2 in India has been declared a public health *** patients with COVID-19 experience cardiac *** with COVID-19 admitted to the intensive care unit(ICU)with acute myocardial injury showed increased high-sensitivity troponin *** troponin levels may indicate myocardial injury and are commonly associated with ***:We conducted a retrospective observational study of 44 patients with severe COVID-19 in ICU during the second *** primary end point of our retrospective study was 28-day mortality,and the time of ICU admission was designated as day *** extracted and analyzed cardiac biomarkers,such as creatine kinase(Ck),creatine kinase-MB(Ck-MB),B-type natriuretic peptide(BNP),and high-sensitivity cardiac troponin I(hs-cTnI),and various inflammatory markers such as C-reactive protein(CRP)level,interleukin 6(IL-6),D-dimer,ferritin,lactate dehydrogenase,IL-6,and procalcitonin in patients with severe COVID-19 at ICU admission and 72 hours after ICU admission from our electronic medical record ***:The best cutoff of BNP were 326.8 and 398.5 pg/mL,Ck were 195.95 and 180.12 U/L,Ck-MB were 112.10 and 108.5 U/L,and hs-cTnI were 0.035 and 0.025 ng/mL,at ICU admission and 72 hours after ICU admission for predicting 28-day mortality among ***:In patients with severe COVID-19,Ck and hs-cTnI may be considered effective and valuable predictive cardiac biomarkers among nonsurvivors and predict poor prognosis.

Performance analysis of cognitive radio networks using channel-prediction-probabilities and improved frame structure

This paper focuses on potential issues related to the random selection of a sensing channel that occurs after the prediction phase in a Cognitive Radio Network (CRN).A novel approach (Approach-l)for improved selection is proposed,which relies on the probabilities of channels by which they are predicted ***,closed-form expressions are derived for the throughput of Cognitive Users (CUs)in the conventional and proposed approaches. In addition to this,a fimdamental approach for computing the prediction probabilities is also ***, a new challenging issue named "sense and stuck"was observed in the conventional *** proposed approach is validated by comparing the results achieved with the results of the conventional ***, to achieve the prediction probabilities,the pre-channel-state-information is a prerequisite,but it may be unavailable for particular scenarios;therefore,a modified selection method is introduced to avoid the sense and stuck *** algorithm to evaluate the throughput using the random,improved,and modified selection methods is presented with its space and time ***,for additional improvement in the throughput of the CU,a new frame structure is introduced,in which the spectrum prediction and sensing periods are exploited for simultaneous transmission of data via the underlay spectrtun access technique (Approach-2).The simulated results of Approach-2 are compared with our pre-obtalned results of Approach-I,which confirm significant improvement in the throughput.

A Comparison of SWAT Model Calibration Techniques for Hydrological Modeling in the Ganga River Watershed

The Ganga River, the longest river in India, is stressed by extreme anthropogenic activity and climate change, particularly in the Varanasi region. Anticipated climate changes and an expanding populace are expected to further impede the efficient use of water. In this study, hydrological modeling was applied to Soil and Water Assessment Tool （SWAT） modeling in the Ganga catchment, over a region of 15 621.612 km2 in the southern part of Uttar Pradesh. The primary goals of this study are： ① To test the execution and applicability of the SWAT model in anticipating runoff and sediment yield; and ② to compare and determine the best calibration algorithm among three popular algorithms-sequential uncertainty fitting version 2 （SUFI-2）, the generalized likelihood uncertainty estimation （GLUE）, and par-allel solution （ParaSol）. The input data used in the SWAT were the Shuttle Radar Topography Mission （SRTM） digital elevation model （DEM）, Landsat-8 satellite imagery, soil data, and daily meteorological data. The watershed of the study area was delineated into 46 sub-watersheds, and a land use/land cover （LULC） map and soil map were used to create hydrological response units （HRUs）. Models utilizing SUFI- 2, GLUE, and ParaSol methods were constructed, and these algorithms were compared based on five cat-egories： their objective functions, the concepts used, their performances, the values of P-factors, and the values of R-factors. As a result, it was observed that SUFI-2 is a better performer than the other two algo-rithms for use in calibrating Indian watersheds, as this method requires fewer runs for a computational model and yields the best results among the three algorithms. ParaSol is the worst performer among the three algorithms. Supplementation of L-ascorbic acid improves the in vitro development of buffalo(Bubalus bubalis)embryos and alters the expression of apoptosis-related genes

Objective:To study the effect of L-ascorbic acid supplementation on the in vitro development of buffalo embryos and evaluate the relative mRNA abundance of some pro-apoptotic,anti-apoptotic,and embryonic development-related ***:In experiment 1,we evaluated the effect of the addition of 0(control),50,and 100μM L-ascorbic acid to the in vitro maturation medium on the developmental competence in terms of blastocyst rate and relative mRNA abundance of some pro-apoptotic(BAX,BID),anti-apoptotic(BCL-XL,MCL1),and embryonic development(GDF9,BMP15)related *** on the results,we chose 50μM as the suitable dose of L-ascorbic acid for the subsequent *** further evaluated the blastocyst rates following the addition of 50μM L-ascorbic acid to the in vitro culture medium(experiment 2),and in vitro maturation and in vitro culture media(experiment 3).In all three experiments,the maturation and culture media devoid of L-ascorbic acid served as the control ***:The blastocyst rate after adding 50μM L-ascorbic acid to the in vitro maturation medium was significantly higher than the control group(P<0.05),whereas 100μM L-ascorbic acid exhibited a negative effect on the blastocyst *** blastocyst rates for embryos cultured in 50μM L-ascorbic acid in the in vitro culture medium alone and both in vitro maturation and in vitro culture media were significantly higher than their corresponding control groups(P<0.05).The relative mRNA abundance of BAX significantly decreased in blastocysts produced after the addition of 50μM L-ascorbic acid as compared with the control group(P<0.05),whereas,for MCL1,it significantly decreased in blastocysts produced after the addition of 100μM L-ascorbic acid(P<0.05).Conclusions:The supplementation of 50μM L-ascorbic acid to in vitro maturation and in vitro culture media supports in vitro embryonic development in buffaloes by improving developmental competence and altering the expression of apoptosis-related genes.

Developments in the Diagnostic Techniques of Infectious Diseases: Rural and Urban Prospective

Objectives: Diagnostics is the first step for the treatment and eradication of infectious microbial diseases. Due to ever evolving pathogens and emerging new diseases, there is an urgent need to identify suitable diagnostic techniques for better management of each disease. The success rate of specific diagnostic technique in any population depends on various factors including type of the microbial pathogen, availability of resources, technical expertise, disease severity and degree of epidemic of disease in the area. One of the important tasks of the policy makers is to identify and implement suitable diagnostic techniques for specific regions based on their specific requirements. In this review we have discussed various techniques available in the literature and their suitability for the target population based on above mentioned criteria. Methods: Diagnostic techniques evaluation of well documented representative microbial diseases;Tuberculosis (bacterial), Malaria (parasitic) and HIV (viral) were included in the study. Identification and collection of information and data was performed focusing on the diagnostic techniques used from the scientific publications from Pubmed, Science Access, Scopus, EMBASE and several regional databases. WHO and CDC database for Tuberculosis, Malaria and HIV were also included. These techniques were compared with respect to the financial resource availability, expertise and management, functional capacity, pathogen virulence and degree of epidemic in the population. Results and Conclusion: In case of Tuberculosis, ELISA and colorimetric techniques are successful in rural and urban communities with 80% - 90% sensitivity. Genotyping and SNP analysis are useful in drug resistant strains. Parasitic disease Malaria also follows the same trend with diagnostic techniques like RDTs being common in both population with fast results and around 90% sensitivity. Modeling of Discharge Distribution in Bend of Ganga River at Varanasi

Dynamics of river behavior plays a great role in meandering, sediment transporting, scouring, etc. of river at bend, which solely depends on hydraulics properties such as horizontal and vertical stress, spatial and temporal variation of discharge. Therefore understanding of discharge distribution of river Ganga is essential to apprehend the behavior of river cross section at bend particularly. The measurement of discharge is not very simple as there is no instrument that can measure the discharge directly, but velocity measurement at a section can be made. Velocity distribution at different cross sections at a time is also not easy with single measurement with the help of any instrument and method, so it required repetitions of the measurement. Velocity near the end of bank, top and bottom layer of natural streams is difficult to be measured, yet velocity distribution at these regions plays important role in characterizing the behavior of river. This paper deals with the new advanced discharge measurement technique and measured discharge data has been used for modelling at river bend. To carry out the distribution of discharge and velocity with depth in river Ganga, the length of river in study area was distributed into 14 different cross sections, M-1 to M-14, measured downstream to upstream and the measurement was done by using of ADCP (Acoustic Doppler Current Profiler). At each cross section, profiles were measured independently by an ADCP and data acquired from ADCP were further used for the regression modeling. A multiple linear regression model was developed, which showed a high correlation among the discharge, depth and velocity parameters with the root mean square error (R2) value of 0.8624.

Sweet lime(Citrus limetta)peels are a rich and valuable bioactive residue obtained from the citrus processing *** high moisture content of the peels can be reduced by drying and dried peels could be used as an ingredient to enhance the nutritional value of *** this study,different drying operations viz;infrared drying(IRD),vacuum drying(VD),tray drying(TD),hot-air multimode drying(HAD),and evacuated tube solar drying system without heat pipe(SD1)and with heat pipe(SD2)were explored for their effect on drying behavior and quality characters of sweet lime ***,total phenol,total flavonoid,DPPH radical scavenging activity,color profile,and Fourier Transform Infrared(FTIR)spectroscopy of fresh and dried sweet lime peel were *** results showed that the time taken for IRD was the lowest followed by VD and *** IRD was a suitable technique for retention of crude fibre,ash,and crude protein ***,VD exhibited a higher L*value and a lower b*value which indicated good retention of *** fresh peels exhibited total phenol,total flavonoid,and DPPH radical scavenging activity of 25.60 mg GAE/g,18.85 mg QE/g,and 32.79%,***,peels dried by IRD,TD,&HAD exhibited a significant increase in antioxidants activity compared to fresh peels and those subjected to other drying *** spectroscopy confirmed the presence of phenols in the dried *** of sweet lime peels could be an effective way to preserve the nutrients,phenols,flavonoids,and antioxidants.

背景尽管静脉置管时的疼痛经常让人备受困扰，但通常都是在没有任何镇痛措施下进行静脉置管的。临床上曾尝试用一些药物和非药物方法来减轻静脉置管的疼痛，但结果各异。本研究拟评价一次闪光对于减轻静脉置管痛的效果。方法90例拟择期行腹腔镜胆囊切除术的ASAⅠ～Ⅱ级的成年患者（15～60岁），随机分为3组，每组30例，组1为对照组；组2为分散注意力组，即在非优势手的手背静脉进行置管前给患者照相而不使用闪光；组3为闪光组，静脉置管前照相并且使用闪光；在照相后立即使用18G的穿刺针进行静脉置管。结果每组均有2例患者首次静脉穿刺失败，在数据分析中即予以剔除，闪光组疼痛的发生率为50％（28例中14例发生），其他2组为100％（28例全部发生），通过视觉模拟评分评估的疼痛严重程度分散注意力组和闪光组显著低于对照组，而闪光组也显著低于分散注意力组。结论静脉置管前使用闪光是一种减轻疼痛的安全而简单有效的方法。
