Application of laparoscopic cholecystectomy in patients with cirrhotic portal hypertension

BACKGROUND: Laparoscopic cholecystectomy (LC) has been widely adopted in treating benign gallbladder disea- ses. Cirrhosis and cirrhotic portal hypertension (CPH) are contraindicated for LC in its early period. In recent years, several studies have reported liberal use of LC in patients with cirrhosis. But its benefits and successful use in patients with CPH are less documented. This study was designed to evaluate the feasibility, safety and technical characteristics of LC in CPH patients. METHODS: In 38 patients with symptomatic gallbladder disease and CPH, 19 belonged to Child A class, 15 Child B class and 4 Child C class. Perioperative data of these pa- tients were collected and analyzed. RESULTS: LC was successfully performed in 36 patients, and 2 patients (5.3%) were converted to open cholecystec- tomy (OC) for difficulty in management of bleeding under laparoscopy and dense adhesion of Calot' s triangle. The sur- gical time was 62.6±15.2 minutes. The estimated amount of intraoperative hemorrhage was 75.5±15.5 ml. No blood transfusion was necessary. The time to resume diet was 18.3 ±6.5 hours. Seven postoperative complications in 5 patients (13.2%) included port-site infection (1 patient), respiratory infection (2), upper digestive tract bleeding (1), slight hepatic encephalopathy (1) and increased ascites (2). All patients were cured and discharged from the hospital within 5.6±2.4 days after LC. CONCLUSIONS: Despite LC is difficult for CPH patients, it is feasible and relatively safe. To make LC successful in patients with CPH, it is necessary for surgeons to acquaint with the technical characteristics of LC and emphasize me- ticulous perioperative management.

Background and objective The value of D-dimer in the risk stratification of patients with coronary artery disease(CAD) and the relationship between D-dimer and the diseased coronary arteries remains controversial or unclear,especially in the *** study was to evaluate the usefulness of D-dimer as a biomarker in assessing the vulnerable blood in the elderly patients with coronary *** Sixty elderly (≥60 years old) male patients with suspected CAD were enrolled in this prospective study. Patients were divided into CAD group (n=41,10 with stable angina and 31 with unstable angina) and control group (n=19) according to their coronary angiography Results Clinical information including age,body mass index (BMI),smoking index,and the complications of primary hypertension or diabetes,and CAD family history was *** blood was sampled serially for the determination of total cholesterol,HDL cholesterol,LDL cholesterol,triglycerides,apoA1,apoB,glucose,uric acid,homocysteine (Hcy),hs-CRP,soluble thrombomodulin (sTM),and markers of fibrinolytic system and hypercoagulability,such as D-dimer,fibrinogen, *** extent of coronary atherosclerosis was assessed,using the Gensini scoring system on the basis of coronary angiography. Results Compared with the controls,the patients with CAD had significantly higher levels of D-dimer.D-dimer level was significantly correlated to age,hs-CRP,Hcy,and *** with D-dimer levels in the top triplicate of D-dimer level had significantly higher prevalence of unstable angina compared with patients in the lowest triplicate (OR=4.8,Z=3.28,P=0.001).In an ordinal logistic regression,the OR value of developing more serious CAD augmented 3.1-fold with each increasing triplicate of D-dimer. Patients with unstable angina had a significantly higher level of D-dimer than the controls (P=0.005),and an increasing trend compared with patients with stable angina (P=0.059),whereas there was no difference between the patients

Polygala paniculata *** a medicinal plant that grows in the Brazilian Atlantic coast,known as‘barba-de-São-João’,‘barba-de-bode’,‘vassourinha branca’,and‘mimosa’.In this study,pollen viability was estimated by three different staining methods:2%acetic orcein,2%acetic carmine,and Alexander’s *** young inflorescences of twenty accessions were collected and fixed in a solution of ethanol:acetic acid(3:1)for 24 hours,then stored in ethanol 70%under *** slides per plant,two for each stain,were prepared by squashing,and 300 pollen grains per slide were *** viability was high(>70%)for most accessions of *** using the Alexander’s stain,which proved the most adequate method to estimate pollen viability.

Purpose: The tumour necrosis factor (TNF) level in aqueous humor after transscleral fixation of intraocular lens (IOL) implantation in rabbits and discuss the effect of TNF on postoperative anterior ocular ***: Twenty - seven pigmented rabbits were divided into three groups. Group 1: transscleral fixation of posterior chamber (PC) IOL implantation;Group 2; Lens of rabbits was removed without IOL implantation; Group 3; the comtrol group, without surgical intervention. On the 1st, 3rd, 7th and 14th postoperative days,aqueous humor samples were obtained. An modified double antibodies indirect sandwich ELISA was used to detected for the presence of TNF. The data were analyzed by using analysis of variance of SAS software. Results: It was found that TNF level in aqueous humor was increased after transscleral fixation of IOL implantation. TNF level reached its maximum on the 14th postoperative day in the IOL implanted group. TNF level on 1st, 3rd, 7th and 14th days postoperatively was
