Assessment of fetus during second trimester ultrasonography using HDlive software:What is its real application in the obstetrics clinical practice?

AIM To show imaging results from application of fourdimensional(4D) ultrasound lightening technique(HDlive^(TM)) in clinical obstetrics practice. METHODS Normal and abnormal fetuses at second and third trimester of pregnancy undergoing routine scan with 4D HDlive^(TM)(5DUS) in the rendering mode are described. Realistic features of fetal structures were provided by 5DUS in the rendering mode. Normal anatomy as well as pathology like cleft lip, hypoplastic face, micrognathia, low-set ears, corpus callosum, arthrogryposis, aortic arch, left congenital diaphragmatic hernia are highlighted in this study. Anatomical details of the fetuses were provided by 5DUS with higher quality imaging modality compared to those obtained using conventional 2D/3D ultrasound. RESULTS Realistic views of fetal anatomy details were displayed by means of 5DUS in the rendering mode, with high image quality obtained either in low-risk or in high-risk obstetrics population. Corpus callosum, esophagus, and aortic arch were obtained in normal fetuses. Cleft lip, cleft lip and palate, micrognathia, hypoplastic face, lowset ears, arthrogryposis, left congenital diaphragmatic hernia, exomphalos, and clitoris hypertrophy were clearly rendered by 5DUS *** The use of 5DUS in the rendering mode, when clinical available, was diagnostic in a variety of congenital anomalies, aided understanding of the parents-tobe and improved prenatal counseling and perinatal management.

Candida albicans isolated from urine:Phenotypic and molecular identification,virulence factors and antifungal susceptibility

Objective: To isolate Candida albicans(C. albicans) from the urine of hospitalized patients and assess the virulence factors and antifungal susceptibility profiles of the ***: Yeasts were identified using the chromogenic medium CHROMagar?, the VITEK~?2 system, hypertonic Sabouraud broth, tobacco agar, polymerase chain reaction,and DNA sequencing. The evaluated virulence factors were proteinase production,phospholipase production, and biofilm production on polystyrene. The broth microdilution technique was used to determine the minimum inhibitory ***: All yeasts isolated from urine were identified as C. albicans using both classical and molecular methods. Although 91.3% of the isolates showed no phospholipase activity, 56.5% showed strong proteinase activity and 91.7% produced biofilm. All microorganisms were sensitive to fluconazole, voriconazole and amphotericin B, but 56.5% of the yeasts showed resistance to ***: C. albicans isolates from urine have a high capacity for virulence and can be associated with infectious processes. Furthermore, the high percentage of isolates resistant to itraconazole is important because this antifungal agent is commonly used to treat fungal infections in the hospital environment.

DNA damage levels in systemic lupus erythematosus patients with low disease activity:An evaluation by comet assay

Oxidative stress has been implicated in the inflammatory process of Systemic Lupus Erythematosus (SLE), particularly by the formation of anti-DNA autoantibodies, which can lead to DNA damage. The aim of this study was to investigate, through comet assay, whether the level of DNA damage in SLE patients is different from that of healthy subjects. Twenty-five adult SLE patients with SLEDAI up to ten, and 25 healthy subjects were paired according to age, gender and Body Mass Index (BMI). Other anthropometric variables were also assessed. Comet assay was assessed as the marker of oxidative stress described as DNA Damage (DD) percentage. Waist Circumference (WC), Hip Circumference (HC) and BMI were also performed. Exclusion criteria for patients and controls comprised smoking and other chronic disorders. Level of damage index was remarkably higher in SLE patients than in controls, and no significant differences between the groups were found for age, BMI, WC and HC. No stratification concerning gender was performed, since there were just two males per group. No correlation was observed between BMI and DD (%). DD increased in SLE, which reflects the oxidant/antioxidant imbalance in these patients. These findings support an association between oxidative stress and SLE. This stronger correlation observed in patients with low disease activity may be useful in elucidating the mechanisms of disease pathogenesis.

Sugammadex in the Management of Sinus Tachycardia after Rocuronium Administration: A Case Report

In rare cases, rocuronium has been associated with dose-related tachycardia, probably by a cardiac muscarinic M2 receptor blockade mechanism. We report the case of a 30-year-old female who underwent excision of a branchial cyst under general anesthesia. This patient presented an episode of sinus tachycardia (130 bpm) shortly after anesthesia induction with propofol, sufentanyl, and rocuronium. Tachycardia could not be explained by any cause other than the use of rocuronium, which was reverted with sugammadex. Two minutes after sugammadex administration, heart rate normalized, corroborating our hypothesis that rocuronium induced the sinus tachycardia observed in our patient. The patient recovered well from the anesthetic-surgical procedure and showed no further cardiovascular, ventilatory, or neurological changes, being transferred to the post-anesthesia care unit, and finally discharged to the ward.

Quality of Drugs Based on Ferrous Sulfate Dispensed in a Basic Health and Commercial Pharmacy in a City in the Amazon Region

Background: For the WHO, anemia is a serious public health problem, with an emphasis on deprivation anemia, which has socioeconomic conditions as determining factors for its development. Among nutritional anemias, the most relevant is iron deficiency, whose main form of prevention and treatment is supplementation with ferrous sulfate dispensed in the public health system, with the need to periodically check whether the drug has the quality required by regulatory health agencies. Thus, the objective of this study was to analyze the quality of the ferrous sulfate medication, from the same laboratory, and dispensed in a basic health unit (BHU) and a commercial pharmacy (CF). Materials and Methods: An analytical, qualitative and quantitative study of ferrous sulfate heptahydrate (25 mg/mL in 100 mL syrup), carried out on 20 BHU and 10 from CF, both from the same laboratory, but in different batches. Samples were subjected to organoleptic analysis and verification of iron content, pH, volume and density. Data were compared with values recommended by the Brazilian pharmacopoeia 5th edition. Results: Three batches were evaluated: 4659 of from the CF;4574 from the BHU1, and 4576 from the BHU2, stored in accordance with RDC 304/19 of ANVISA. All showed compliance with some organoleptic characteristics;and stable values in pH (3.61 - CF;3.67 - BHU1;3.65 - BHU2), density (1.079 - CF;1.088 - BHU1;1.086 - BHU2) and individual volume (not less than 95% of the declared value). However, it was not possible to detect the odor of herbs;the average volume of batch BHU1 was below the declared value, and the iron content of both BHUs was above the maximum value of pharmacopoeia standards. Conclusion: Samples did not show full compliance with the physical and chemical parameters evaluated, suggesting deviation in the quality of the medication.
