Patient, Provider and System Factors Impacting on the Diagnosis and Management of Lung Cancer Care in Australia

Background: Lung cancer is the leading cause of cancer death in Australia, with only modest improvements in survival. This study aims to identify factors impacting on diagnosis and management of lung cancer with particular reference to Australian primary care. Methods: A sequential mixed method modified approach employing interview and a two-phased survey technique. Following telephonic interviews with 31 health professionals (individuals representing general practitioners, specialized physicians, nurses and allied health practitioners), interview data was analysed using qualitative thematic analysis, and surveys using descriptive statistics. Emergent themes were organised under patient, provider and system factors. Interviews ceased upon saturation of data. Results: Multiple patient, provider and systems issues were seen to contribute to adverse health outcomes. There is a strong relationship between smoking and outcomes, and factors related to higher smoking rates such as a lower socioeconomic status. For smokers, guilt and/or denial was considered a reason for delay in the decision to seek medical care for cough or shortness of breath. Aboriginal people under-report morbidity related to smoking and chronic obstructive pulmonary disease;other patients fail to recognise the significance of their symptoms. Discussion: Despite the poor prognosis of lung cancer diagnosis, increased awareness of presentation and treatment options can address disparities in health outcomes.

Modelling dynamic pantograph loads with combined numerical analysis

Appropriate interaction between pantograph and catenary is imperative for smooth operation of electric *** heights of overhead lines to accommodate level crossings,overbridges,and tunnels pose significant challenges in maintaining consistent current collection performance as the pantograph aerodynamic profile,and thus aerodynamic load changes significantly with operational *** research aims to analyse the global flow characteristics and aerodynamic forces acting on individual components of an HSX pantograph operating in different configurations and orientations,such that the results can be combined with multibody simulations to obtain accurate dynamic insight into contact ***,computational fluid dynamics simulations are used to investigate the pantograph component loads in a representative setting,such as that of the recessed cavity on a Class 800 *** an aerodynamic perspective,this study indicates that the total drag force acting on non-fixed components of the pantograph is larger for the knuckle-leading orientation rather than the knuckle-trailing,although the difference between the two is found to reduce with increasing pantograph *** the aerodynamic loads acting on individual components with multibody tools allows for realistic dynamic insight into the pantograph *** results obtained show how considering aerodynamic forces enhance the realism of the models,leading to behaviour of the pantograph-catenary contact forces closely matching that seen in experimental tests.

Isolating higher yielding and more stable rice genotypes in stress environments: fine-tuning a selection method using production and resilience score indices

In Asia, the rice crop sustains millions of people. However, growing demand for this crop needs to be met while simultaneously reducing its water consumption to cope with the effects of climate change. Lowland cropping systems are the most common and productive but have particularly high water requirements. High-yielding rice genotypes adapted to drier environments(such as rainfed or aerobic rice ecosystems) are needed to increase the water use efficiency of cropping. Identifying these genotypes requires fast and more accurate selection methods. It is hypothesized that applying a new quantitative selection method(the score index selection method), can usefully compare rice yield responses over different years and stress intensities to select genotypes more rapidly and efficiently. Applying the score index to previously published rice yield data for 39 genotypes grown in no-stress and two stress environments, identified three genotypes(ARB 8, IR55419-04 and ARB 7) with higher and stable yield under moderate to severe stress conditions. These genotypes are postulated to be better adapted to stress environment such as upland and aerobic environments. Importantly, the score index selection method offers improved precision than the conventional breeding selection method in identifying genotypes that are well-suited to a range of stress levels within the target environment.

Transgenic cell lines of loblolly pine （Pinus taeda L.） were analyzed by a compact laser-tweezers-Raman-spectroscopy （LTRS） system in this investigation. A low power diode laser at 785 nm was used for both laser optical trapping of single transgenic cells and excitation for near-infrared Raman spectroscopy of the nuclei of synchronized cells, which were treated as single organic particles, at the S-phase of the cell cycle. Transgenic living cells with gfp and uidA genes were used as biological samples to test this LTRS technique. As expected, different Raman spectra were observed from the tested biological samples. This technique provides a high sensitivity and enables real-time spectroscopic measurements of transgenic cell lines. It could be a valuable tool for the study of the fundamental cell and molecular biological process by trapping single nucleus and by providing a wealth of molecular information about the nuclei of cells.

Microplastics are emergent contaminants threatening aquatic organisms including aquacultured *** study investigated the effects of high-density polyethylene(HDPE,100 to 125 mm)on yellow perch(Perca flavescens)based on integrative evaluation including growth performance,nutritional status,nutrient metabolism,fish health,and gut microbial *** test diets(0,1,2,4,or 8 g HDPE/100 g diet)containing 41%protein and 10.5%lipid were fed to j.venile perch(average body weight,25.9±0.2 g;n=15)at a feeding rate of 1.5%to 2.0%body weight *** feeding trial was conducted in a flow-through water system for 9 wk with 3 tanks per treatment and 15 yellow perch per *** mortality or HDPE accumulation in the fish was found in any *** gain and condition factor of fish were not significantly impacted by HDPE(P>0.05).Compared to the control group,fish fed the 8%HDPE diet had significantly decreased levels of protein and ash(P<0.05).In response to the increasing levels of HDPE exposure,the hepatosomatic index value,hepatocyte size,and liver glycogen level were increased,but lipid content was reduced in the liver *** to the control treatment,fish fed the 8%HDPE diet had significant accumulations of total bile acids and different metabolism pathways such as bile acid biosynthesis,pyruvate metabolism,and carnitine *** enterocyte necrosis was documented in the foregut of fish fed the 2%or 8%HDPE diet;and significant cell sloughing was observed in the midgut and hindgut of fish fed the 8%HDPE *** fed the 2%HDPE diet harbored different microbiota communities compared to the control *** study demonstrates that HDPE ranging from 100 to 125 mm in feed can be evacuated by yellow perch with no impact on ***,dietary exposure to HDPE decreased whole fish nutrition quality,altered nutrient metabolism and the intestinal histopathology as well as microbiota community of yellow *** results indicate that extende

Solving field equations exactly in f(R,T)−gravity is a challenging *** do so,many authors have adopted different methods such as assuming both the metric functions and an equation of state(EoS)and a metric ***,such methods may not always lead to well-behaved solutions,and the solutions may even be rej.cted after complete ***,very recent studies on embedding class-one methods suggest that the chances of arriving at a well-behaved solution are very high,which is *** the class-one approach,one of the metric potentials is estimated and the other can be obtained using the Karmarkar *** this study,a new class-one solution is proposed that is well-behaved from all physical points of *** nature of the solution is analyzed by tuning the f(R,T)−coupling parameterχ,and it is found that the solution leads to a stiffer EoS forχ=−1 than that forχ=*** is because for small values ofχ,the velocity of sound is higher,leading to higher values of Mmax in the M−R curve and the EoS parameterω.The solution satisfies the causality condition and energy conditions and remains stable and static under radial perturbations(static stability criterion)and in equilibrium(modified TOV equation).The resulting M−R diagram is well-fitted with observed values from a few compact stars such as PSR j.614-2230,Vela X-1,Cen X-3,and SAX ***,for different values ofχ,the corresponding radii and their respective moments of inertia have been predicted from the M−I curve.

We present a new class of solutions to the Einstein field equations for an anisotropic matter distribution in which the interior space-time obeys the Karmarkar condition. The necessary and sufficient condition required for a spherically symmetric space-time to be of Class One reduces the gravitational behavior of the model to a single metric function. By assuming a physically viable form for the grr metric potential we obtain an exact solution of the Einstein field equations which is free from any singularities and satisfies all the physical criteria. We use this solution to predict the masses and radii of well-known compact obj.cts such as Cen X-3, PSR j.348＋0432, PSR B0943＋10and XTE j.739-285.
