Insomnia is associated to depressive symptoms in patients with chronic heart failure

Introduction: Insomnia and depressive symptoms are common among patients with chronic heart failure (HF). Aim: The aim was to describe the prevalence of insomnia and depressive symptoms, as well as to examine the association between insomnia and depressive symptoms in patients with HF. Method: A crosssectional descriptive study including 212 patients with HF. All patients responded to questionnaires regarding sleeping difficulties (Uppsala Sleep Inventory-Chronic Heart Failure), daytime sleepiness (Epworth Sleepiness Scale) and depressive symptoms (The Mental Health Scale). Results: Patients with depressive symptoms (34%) had, compared to those without, significantly more major complaints of insomnia as indicated by the sub-types;non-restorative sleep (66% vs. 32%, p < 0.0001), difficulty in maintaining sleep (35% vs. 16%, p = 0.003), difficulty in initiating sleep (29% vs. 14%, p = 0.009) and early morning awakenings (25% vs. 10%, p = 0.004). The odds ratio (OR) to suffer from depressive symptoms;were for;non-restorative sleep 5.2 (CI 95%, 2.2 - 12.3), difficulties maintaining sleep 2.5 (CI 95%, 1.2 - 4.9), difficulties in initiating sleep 2.2 (CI 95%, 1.1 - 4.4) and early morning awakenings 2.4 (CI 95%, 1.1 - 5.4). When categorising insomnia into three severity groups, 1) non insomnia, 2) mild insomnia, and 3) severe insomnia, the OR for depressive symptoms for the mild insomnia and severe insomnia group were 2.2 (CI 95%, 1.1 - 4.2) and 7.4 (CI 95%, 2.4 - 22.8) respectively, compared to the non insomnia group. Conclusion: Insomnia is independently associated to depressive symptoms. Assessment of depressive symptoms and insomnia in patients with HF is important since treatment could be targeted to depressive symptoms only and/or to the sleep disturbance.

The rate and mechanism for cement hydration is important since it will affect the properties of the final product. The hydration process is known to be highly exothermic, i.e. heat is produced. The heat flow, expressed in W (J·s-1), reflects the rate of the overall hydration process, which can be monitored using TAM Air, an eight-channel heat conduction calorimeter. In this presentation some functional aspects and performance of TAM Air is discussed with special emphasis on cement hydration.

Objective: 1) To describe the prevalence of undiagnosed obstructive sleep apnea (OSA) and depressive symptoms in hypertensive men and women below 65 years of age, and 2) to describe the association of OSA to subjective sleep complaints, depressive symptoms and global perceived health. Design: Cross-sectional design focusing on nursing care outcomes of obstructive sleep apnea. Setting: Four primary care health centres in Sweden. PATIENTS: 411 consecutive patients (52% women), mean age 57.9 years (SD 5.9 years), with diagnosed hypertension (BP > 140/90). Main Outcome Measures: Prevalence of OSA and depressive symptoms, and association of OSA to sleep complaints, depressive symptoms and global perceived health. RESULTS: Mild, moderate and severe OSA was seen among 29%, 16% and 14% of patients, respectively. Depressive symptoms were seen in 16% of the total group, with a higher prevalence among men, compared to women, 21% vs. 12%. No differences were found regarding blood pressure, estimated sleep need, sleep sufficiency index, insomnia symptoms, daytime sleepiness or depressive symptoms with respect to different degrees of OSA. Apnea-hypopnea index was significantly associated to perceived health after adjustment for gender and comorbidities, but when depressive symptoms and non-restorative sleep were added to the model, 33% of the variance in global perceived health was explained. Conclusion: OSA is highly prevalent among patients with hypertension in primary care and does together with sleep complaints and depressive symptoms have a negative impact on global perceived health. Hypertensive patients without subjective sleep complaints or depressive symptoms may still have OSA.

Purpose:This study was aimed to determine associations of accelerometer-determined time and bouts of sedentary behavior,light physical activity(LPA),and moderate-to-vigorous PA(MVPA)with sarcopenia and incident falls over 12 ***:A total of 3334 Swedish 70-year-olds were assessed for sarcopenia,as defined by the revised definition of the European Working Group on Sarcopenia in Older *** were based on low scores for appendicular lean mass(dual-energy X-ray absorptiometry),hand grip strength,and the Timed Up and Go *** 7 days after baseline,total time and total number of bouts(≥10 min of continuous activity at a given intensity)of activity performed at sedentary,LPA,and MVPA intensities were assessed by *** falls were self-reported 6 months and 12 months after ***:Only 1.8%of participants had probable or confirmed *** multivariable adjustment for other levels of activity,only greater MVPA time was associated with a decreased likelihood of having low appendicular lean mass,low hand grip strength,and slow Timed Up and Go time as defined by the European Working Group on Sarcopenia in Older People criteria(all p0.05).Conclusion:Higher amounts of accelerometer-determined MVPA are consistently associated with a decreased likelihood of sarcopenia and its components,regardless of the length of bouts or amounts of sedentary behavior.

背景：血管紧张素转换酶（angiotensin-converting enzyme，ACE）抑制剂可降低心肌梗死（myocardial infarction，MI）的危险，但是还不清楚血管紧张素受体拮抗剂是否具有同样的效果。目的：评价血管紧张素受体拮抗剂坎地沙坦对心力衰竭患者MI和其他冠脉事件的影响。设计、地点和参试者：“坎地沙坦治疗心力衰竭：降低病死率和发病率评价”（Candesartan in Heart Failure：Assessment of Reduction in Mortality and Morbidity，CHARM）计划是一项随机、安慰剂对照研究。研究入选有纽约心脏病协会分级Ⅱ至Ⅳ级症状的患者（平均年龄，66[SD，11]岁）。在心力衰竭最佳治疗的基础上，随机接受坎地沙坦（目标剂量，32mg每天一次）或匹配的安慰剂。患者入选期为1999年3月至2001年3月。分组的7599例患者中，4004例（53％）有MI病史，1808例（24％）患有心绞痛。基线时，服用ACE抑制剂3125例（41％）；服用β-阻滞剂4203（55％）例；降脂药物3153例（42％）；阿斯匹林4246例（56％）；利尿药6286例（83％）。主要观测指标：该分析的主要终点为坎地沙坦或安慰剂组心力衰竭患者心血管死亡和非致死性MI复合发生率。结果：在37．7个月的中位随访期间，坎地沙坦组（775例患者[20．4％]）心血管死亡和非致死性MI主要终点的发生率明显低于安慰剂组（868[22．9％]）（风险比[hazard ratio，HR]，0．87；95％可信区间[confidence interval，CI]，0．79-0．96；P=0．004；所需治疗例数[numbe rneeded to treat，NNT]，40）。与安慰剂组（522[13．8％]）比较，坎地沙坦组（459[12．1％]）单纯非致死性MI也明显降低（HR，0．86；95％CI，0．75-0．97；P=0．02；NNT，59）。在CHARM试验预先设定的不同亚组和分析间，心血管死亡和非致死性MI危险的降低相似。坎地沙坦对不稳定心绞痛或冠脉血运重建导致的住院没有影响。结论：坎地沙坦能明显降低心力衰竭患者发生心血管死亡或非致死性MI复合终点的危险。

Rectal nitric oxide as biomarker in the treatment of inflammatory bowel disease: Responders versus nonresponders

AIM： To explore rectal nitric oxide （NO） as biomarker of treatment response in ulcerative colitis （UC） and Crohn＇s disease （CD）, and examine relationships between rectal NO, mucosal expression of NO synthases （NOS）, and pro-inflammatory cytokines. METHODS： Twenty-two patients with UC and 24 with CD were monitored during steroid treatment. Rectal NO levels were measured and clinical activities were assessed on days 1, 3, 7 and 28. Mucosal presence of NOS and pro-inflammatory cytokines were analyzed by immunohistochemistry and RT-PCR. RESULTS： Active UC and CD displayed markedly increased rectal NO levels （10950 ± 7610 and 5 040 ± 1 280 parts per billion （ppb）, respectively） as compared with the controls （154 ± 71 ppb, P 〈 0.001）. Rectal NO correlated weakly with disease activity in both UC and CD （r = 0.34 for UC and r = 0.48 for CD, P 〈 0.01）. In 12 patients, a steroid-refractory course led to colectomy. These patients had only slightly increased NO levels （UC： 620 ± 270 ppb; CD： 1260 ± 550 ppb） compared to those with a therapeutic response （UC： 18860 ± 530 ppb, P 〈 0.001, CD： 10060 ± 3200 ppb, P 〈 0.05）. CONCLUSION： Rectal NO level is a useful biomarker of treatment response in IBD as low NO levels predicts a poor clinical response to steroid treatment.
