Cellular and molecular overview of gestational diabetes mellitus:Is it predictable and preventable?

BACKGROUND In contrast to overt diabetes mellitus(DM),gestational DM(GDM)is defined as impaired glucose tolerance induced by pregnancy,which may arise from exaggerated physiologic changes in glucose *** prevalence is reported to be as high as 20%among pregnancies depending on the screening method,gestational age,and the population *** and fetal effects of uncontrolled GDM include stillbirth,macrosomia,neonatal diabetes,birth trauma,and subsequent postpartum ***,it is essential to find the potential target population and associated predictive and preventive measures for future intensive peripartum *** To review studies that explored the cellular and molecular mechanisms of GDM as well as predictive measures and prevention *** The search was performed in the Medline and PubMed databases using the terms“gestational diabetes mellitus,”“overt diabetes mellitus,”and“insulin resistance.”In the literature,only full-text articles were considered for inclusion(237 articles).Furthermore,articles published before 1997 and duplicate articles were *** a final review by two experts,all studies(1997-2023)included in the review met the search terms and search strategy(identification from the database,screening of the studies,selection of potential articles,and final inclusion).RESULTS Finally,a total of 79 articles were collected for *** risk factors for GDM included maternal obesity or overweight,pre-existing DM,and polycystic ovary *** pathophysiology of GDM involves genetic variants responsible for insulin secretion and glycemic control,pancreaticβcell depletion or dysfunction,aggravated insulin resistance due to failure in the plasma membrane translocation of glucose transporter 4,and the effects of chronic,low-grade ***,many antepartum measurements including adipokines(leptin),body mass ratio(waist circumference and waist-to-hip ratio),and biomarkers(microRNA in extra

Outpatient varicocelectomy performed under local anesthesia

Aim： To report a series of varicocelectomy performed under pure local anesthesia. Methods： From July 1988 to June 2003, a total of 575 patients, aged between 15 and 73 years, underwent high ligation of the internal spermatic vein for treatment of a varicocele testis under a regional block in which a precise injection of 0.8 % lidocaine solution was delivered to involved tissues after exact anatomical references were made. A 100-mm visual analog scale （VAS） was used to assess whether the pain level was acceptable. Results： The surgeries were bilateral in 52 cases, and unilateral in 523 cases. All were successfully performed on an outpatient basis except in the case of two patients, who were hospitalized because their surgeries required general anesthesia. Overall, 98.6 % （567/575） of men could go back to work by the end of the first post-operative week and only 8 （1.4 %） men reported feeling physical discomfort on the eighth day. The VAS scores varied from 11 mm to 41 mm with an average of （18.5 ± 11.3） mm that was regarded as tolerable. Conclusion： This study has shown varicocelectomy under local anesthesia to be possible, simple, effective, reliable and reproducible, and a safe method with minimal complications. It offers the advantages of more privacy, lower morbidity, with no notable adverse effects resulting from anesthesia, and a more rapid return to regular physical activity with minor complications.

BACKGROUND Acquired hemophilia A (AHA) is a rare and potentially severe bleeding disordercaused by circulating autoantibodies against factor Ⅷ (FⅧ). In approximately50% of the patients, the condition is associated with autoimmune diseases,cancers, medication use, pregnancy, and the post-partum period. Bullous pemphigoid(BP) is a chronic autoimmune subepidermal blistering disease associatedwith tissue-bound and circulating autoantibodies against BP antigens 180 (BP180)and 230 (BP230). AHA-associated BP has a high mortality rate;hence, the understandingof this disease must *** SUMMARY A 69-year-old man presented with erythema, blisters, blood blisters, and crustsaccompanied by severe pruritus for more than 20 days, and ecchymosis andswelling on his left upper arm for 3 days. Pathological examination revealed asubepidermal blister that contained eosinophils. Laboratory tests showed that theBP180 autoantibody levels had increased, isolated activated partial thromboplastintime was notably prolonged (115.6 s), and coagulation FⅧ activity wasextremely low (< 1.0%). Furthermore, the FⅧ inhibitor titer had greatlyincreased (59.2 Bethesda units). Therefore, the patient was diagnosed as having BP associated with AHA, prescribed 0.05% topical halometasone cream, and transferred to a higher-level hospitalfor effective treatment;however, he died after 2 *** AHA associated BP is rare, dangerous, and has a high mortality rate. Therefore, its timely diagnosis and effectivetreatment are necessary.

Main observation and conclusion DNA circuits have been designed for implementation of various functions based on DNA strand displacement in cell-free settings,but their capabilities in biological environments remain *** this work,we report framework nucleic acid(FNA)-based circuits enabling intracellular logic computation for mRNA *** as rigid scaffolds enable to deliver our built DNA circuits into cells without aid of transfection reagents,evading a time-consuming and tedious process prior to analysis,and the pendant duplex DNA designed at one vertex of FNA as gate is suitable for four-way strand exchange,minimizing crosstalk with other nucleic acids in the cellular *** demonstrated that such FNA-based circuits can operate both in vitro and in vivo logic computation,including OR and AND logic ***,in situ mRNA imaging was also realized by exploiting native mRNA as scaffolds to bind multiple FNA-based gates for the enhanced signal-to-background *** hope that this FNA-based circuit can be applied for disease diagnosis,facilitating the development of biomedicine.

Anti-neuroinflammatory responses in lipopolysaccharide-stimulated BV2 microglial cells upon treatment with methanol extract of Panax ginseng root

OBJECTIVE:To investigate the anti-neuroinflammatory properties of Panax ginseng(***)root by measuring the levels of nitric oxide(NO),tumour necrosis factor-alpha(TNF-α),interleukin-6(IL-6),and interleukin-10(IL-10)in lipopolysaccharide(LPS)-stimulated BV2 microglia ***:Maximal non-toxic dose(MNTD)of methanol extract of *** root culture on BV2 microglia cells was first determined via 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay,followed by treatment and LPS stimulation of cells,and the measurement of NO using Griess assay and TNF-α,IL-6,and IL-10 using ELISA ***:The MNTD of *** root extract was determined to be(587±57)µg/*** that,NO and IL-6 levels were found to be insignificantly reduced by 6.88%and 0.14%respectively in stimulated cells upon treatment with *** with½MNTD yielded similar insignificant result,with only a reduction of 3.58%and 0.08%in NO and IL-6 levels ***,TNF-αand IL-10 levels were significantly downregulated by 15.64%and 34.96%respectively upon treatment with *** root extract at ***:Methanol extract of *** root culture did not show any significant anti-inflammatory effects on NO and IL-6 levels,but might potentially possess both anti-neuroinflammatory and pro-neuroinflammatory properties through the downregulation of TNF-αand IL-10 respectively.

Effect of substrate bias on microstructure and tribological performance of GLC films using hybrid HIPIMS technique
