AIM： To evaluate the optimal intervention point of a selective cyclooxygenase-2 （COX-2） inhibitor, Celecoxib, for inhibiting Helicobacter pylori （H pylori-associated gastric carcinogenesis in Mongolian gerbils （MGs）.METHODS： One hundred and twelve MGs were divided into six groups （A-F）. One hundred gerbils were inoculated with H pylor/（groups A-E）. Twelve gerbils were inoculated with vehicle broth only （group F）. After 4 wk, they were given N＇-methyl-N＇ -nitro-N-nitroso-guanidine （MNNG）（50 μg/mL） in the drinking water for 20 wk. in groups B-E, the animals were given the stock Celecoxib （10 mg/kg per day） diet from the 21st, 31st, 21st and 41st week respectively. The periods of administering Celecoxib were 30, 20, 20, and 15 wk respectively. On the 51st week, the animals were sacrificed for histological examination. Local PCNA expression was examined by the immunohistochemistry method. The expression of COX-2 protein was assessed by Western Blot. Analysis used the 2 test. The difference was regarded as significant when P value was less than 0.05. RESULTS： Seventeen percent （17/100） of H pyloriinfected MGs developed gastric cancer. All of these lesions were well-differentiated adenocarcinoma. The incidence rates of adenocarcinoma in groups A-F were 40%, 0%, 0%, 20%, 25%, and 0% respectively. The inflammatory scores were higher in group B than in other groups. There was no inflammatory response noted in group F. Celecoxib treatment resulted in a significant reduction in the proliferation of H pyloriinfected mucosal cells （groups B, C and D）（P 〈 0.01）. The expression of COX-2 protein was significantly attenuated in the groups which were Celecoxib-treated for more than 20 wk （groups B, C, D）. The groups treated with Celecoxib had a significantly lower rate of advanced gastric cancer （34% vs 75%, P 〈 0.001） There were no sudden deaths in any of the groups. Reappraisal of endoscopic papillary balloon dilation for the management of common bile duct stones

Although endoscopic sphincterotomy(EST) is still considered as a gold standard treatment for common bile duct(CBD) stones in western guideline, endoscopic papillary balloon dilation(EPBD) is commonly used by the endoscopists in Asia as the first-line treatment for CBD stones. Besides the advantages of a technical easy procedure, endoscopic papillary large balloon dilation(EPLBD) can facilitate the removal of large CBD *** indication of EPBD is now extended from removal of the small stones by using traditional balloon, to removal of large stones and avoidance of lithotripsy by using large balloon alone or after EST. According to the reports of antegrade papillary balloon dilatation, balloon dilation itself is not the cause of pancreatitis. On the contrary, adequate dilation of papillary orifice can reduce the trauma to the papilla and pancreas by the basket or lithotripter during the procedure of stone extraction. EPLBD alone is as effective as EPLBD with limited EST. Longer ballooning time may be beneficial in EPLBD alone to achieve adequate loosening of papillary orifice. The longer ballooning time does not increase the risk of pancreatitis but may reduce the bleeding episodes in patients with coagulopathy. Slowly inflation of the balloon, but not exceed the diameter of bile duct and tolerance of the patients are important to prevent the complication of perforation. EPBLD alone or with EST are not the sphincter preserved procedures, regular follow up is necessary for early detection and management of CBD stones recurrence.

Objective: This study evaluated the effectiveness of acupuncture treatment on postoperative pain in patients with degenerative lumbar spine disease, and explored the relationship between the postoperative analgesic effect of acupuncture and the sensation of acupuncture experienced by the ***: This retrospective study analyzed the medical records of 97 patients who had undergone an operation by the same surgeon due to degenerative lumbar disease. These patients were divided into acupuncture group(n = 32), patient-controlled analgesia(PCA) group(n = 27), and oral analgesia group(n = 38) according to the different postoperative analgesic methods. During their hospitalization, patients completed daily evaluations of their pain using a visual analogue scale(VAS), and injection times of supplemental meperidine were recorded. Also, the Chinese version of the Massachusetts General Hospital Acupuncture Sensation Scale(C-MASS) was used in the acupuncture ***: Each of the three treatment groups showed significant reductions in postoperative pain, as shown by reduced VAS scores. The acupuncture group, however, had less rebound pain(P < 0.05) than the other two groups. Both the acupuncture and PCA groups experienced acute analgesic effects that were superior to those in the oral analgesia group. In addition, the higher the C-MASS index on the second day after surgery, the lower the VAS score on the fourth day after surgery. There was also a significant difference in the“dull pain” in the acupuncture ***: The results demonstrated that acupuncture was beneficial for postoperative pain and discomfort after simple surgery for degenerative spinal disease. It is worth noting that there was a disproportionate relevance between the patient’s acupuncture sensation and the improvement of pain VAS score.

Although the prevalence of chronic hepatitis B virus (HBV) infection has declined in renal transplant recipients (RTRs), it remains a relevant clinical problem with high morbidity and mortality in long-term follow up. A thorough evaluation, including liver biopsy as well as assessment of HBV replication in serum (i.e. hepatitis B e antigen and/or HBV DNA) is required before transplantation. Interferon should not be used in this setting because of low efficacy and precipitation on acute allograft rejection. The advent of effective antiviral therapies offers the opportunity to prevent the progression of liver disease after renal transplantation. However, as far as we are aware, no studies have compared prophylactic and preemptive strategies. To date, the majority of RTRs with HBV-related liver disease have had a high virological and biochemical response to lamivudine use. However, lamivudine resistance is frequent with a prolonged course of therapy. Considering long-term treatment, antiviral agents with a high genetic barrier to resistance and lack of nephrotoxicity are suggested. The optimal strategy in RTRs with HBV infection remains to be established in the near future.

目的：1)运用统计学方法,建立痤疮古代外用方方药资料数据库,并藉此基础上探讨。2)对痤疮古代外治方用药特点,进行总结。为临床治疗提供理论指导。方法：以中医大型电子图书《中华医典》为载体,以古代痤疮多种病名为检索词,收取122首古代外用方,涉及中药153种。使用Microsoft Office-Excel软件,从外治剂型、药物使用频次、药物分类、药物来源、药物颜色、药物毒性、单方药、赋形剂等做统计,从统计结果归纳总结其用药特色。结果：1)本次所收集的122首外治方中散剂74首,膏剂18首,汤剂15首,洗面药粉剂15首。2)痤疮外治方剂使用率高的药物依序为胡粉、水银、白附子、白芷、白蔹、硫磺、密陀僧、杏仁等。3)根据药物特性进行分类,使用率最高依序为解表药、活血化瘀药、清热药、攻毒杀虫止痒药、拔毒化腐生肌药、化痰止咳平喘药、补虚药、理气药、祛风药、温里药、利水渗湿药、平肝息风药、泻下药、止血药、安神药、化湿药、涌吐药、收涩药。4)根据药物来源进行分类,也发现植物药共使用120种,矿物药共使用18种,动物药共使用16种。5)使用白色或有带白字的药物,共有23种药,占全部用药的15%。6)对于痤疮古代外治方使用有毒药物共有30种药,占全部用药的20%。7)在收集古代外治方122首中,发现单味药方有30首,占全部方的25%。8)本次研究痤疮外治方122首方剂,共有153种药,另外还有20种基底质做为赋形剂。其中使用频率最多的是猪脂、蜜和酒赋形剂。

Water-soluble and air-stable triruthenium carbonyl clusterRu3(CO)9(TPPMS)3 (TPPMS=sodium diphenylphosphinobenzene-msulphonate) was used as catalyst to hydroformylate Propylene with syngas inWater. The main product is n-butyraldehyde, side-products are isobumpdehyde and a small amount of 1-butyl and isobutyl alcohols. At120℃ and propylene, CO and H2 pnal Pressures of 0.7 MPa, 2.0 MPa and2.0 MPa respectively, catalytic turnover of 61.2 ***-1.h-1 (products/cluster) and product n/i ratio of 15.9 were obtained. For ethylenehydroformylation, the main product is propanal, side-Products are3-pentanone, 1-Propanol and 2-methyl-pent-2-en-1-al. The catalyst Wascharacterized before and after the reaction by IR and X-ray photoelectronspectroscopy and the results were dianssed as related to the possiblecatalytic active species.
