Clinical prognostic factors for disabling Crohn's disease: A systematic review and meta-analysis

AIM: To identify demographic and clinical factors asso-ciated with disabling Crohn's disease (CD). METHODS: A systematic review and meta-analysisof observational studies, focusing on the factors that can predict the prognosis of different outcomes of CD was undertaken. PubMed, ISI Web of Knowledge and Scopus were searched to identify studies investigat-ing the above mentioned factors in adult patients with CD. Studies were eligible for inclusion if they describe prognostic factors in CD, with inclusion and exclusion criteria defined as follows. Studies with adult patients and CD, written in English and studying association between clinical factors and at least one prognosis out-come were included. Meta-analysis of effects was un-dertaken for the disabling disease outcome, using odds ratio (OR) to assess the effect of the different factors in the outcome. The statistical method used was Mantel-Haenszel for fixed effects. The 16-item quality assess-ment tool (QATSDD) was used to assess the quality of the studies (range: 0-42). RESULTS: Of the 913 papers initially selected, sixty studies were reviewed and three were included in the systematic review and meta-analysis. The global QA-TSDD scores of papers were 18, 21 and 22. Of a total of 1961 patients enrolled, 1332 (78%) were classified with disabling disease five years after diagnosis. In two studies, age at diagnosis was a factor associated with disabling disease five years after diagnosis. Individu-als under 40 years old had a higher risk of developing disabling disease. In two studies, patients who were treated with corticosteroids on the first flare developed disabling disease five years after diagnosis. Further, perianal disease was found to be relevant in all of the studies at two and five years after diagnosis. Finally, one study showed localization as a factor associated with disabling disease five years after diagnosis, with L3 being a higher risk factor. Characterization of the Topography and Wettability of English Weed Leaves and Biomimetic Replicas

The topography and wettability of the underside of English weed （Oxalis pes-caprae） leaves and of their biomimetic replicas are investigated. Polyvinyl siloxane molds were cast from the leaves and then filled with an epoxy pre-polymer to produce replicas. The particular topographical structures of leaves and replicas were evaluated by optical microscopy and Scanning Electron Microscopy （SEM） analysis. The static wettability of leaves and replicas was assessed by contact angle measurements, while the dynamic wettability was characterized by estimating contact angle hysteresis and studying the dynamic behavior of impacting water droplets. A smooth glass slip and its replica were used as control surfaces. The replica moulding method used was able to transfer the characteristic pattern of irregular 100 μm - 200 μm × 60 μm convex papillae interspersed with stomata of the original leaf to the epoxy replicas. The static contact angle of 143°± 3° and the contact angle hysteresis of 2~ indicate that the underside of the English weed leaf is close to superhydrophobic. The lower contact angles （130° ± 4°） and higher hysteresis （31°） observed for the replica when compared with the original leaves were associated to an inaccurate replication of the chemistry and structures of the three-dimensional wax projections covering the plant surface. Also, trichomes in the original leaves could not be accurately reproduced due to their flexibility and fragility. Differences in wetting behavior were also evident from droplet impact experiments, with rebound regimes prevailing in the original leaves and regimes characterized by higher adhesion and larger dissipation predominating in the replicas. Nevertheless, the morphological features of the leaf transferred to the replica were sufficient to promote a clear hydrophobic behavior of the replica when compared with the smooth epoxy reference surface.

The chemical preparation, crystal structure and infrared spectroscopic characterization of the triaqua（4-amino-6-methoxypyrimidine） cuprate（II） sulfate, [Cu（C5H7N3）（H2O）3]SO4, is reported. The compound crystallizes in the noncentrosymmetric orthorhombic space group P212121 with lattice parameters a = 7.9025（3）, b = 11.1189（4）, c = 12.9720（4） , V = 1139.81（7） ~3 and Z = 4. The Cu（II） cation is fivecoordinated, in an early half-way between square pyramidal and trigonal bipyramidal fashion, by two nitrogen atoms of the 4-amino-6-methoxypyrimidine ligand and three water oxygen atoms. In the atomic arrangement, the organic ligands and the 5-connected Cu centers are linked with each other to give a 1-D corrugated hybrid chain running along the b-axis direction. The chains are interconnected by the SO4^（2-） anions via O–H...O, O–H...S, C–H...O and N–H...O hydrogen bonds to form layers spreading parallel to the（011） plane. The vibrational absorption bands were identified by infrared spectroscopy. Quantitative measurements of the second harmonic generation（SHG） of a powdered sample at 1064 nm were performed and a relative efficiency of 5.2 times the KDP standard was observed. Magnetic properties were also defined to characterize the complex. Magnetic measurements revealed that this material had a onedimensional antiferromagnetic character. The magnetic parameters were g = 2.11 and 2J/k B = -36 K.

Austenitic stainless steels, when exposed to welding conditions or aging for length of service, it＇s observed the formation of numerous deleterious phases, such as several kinds of carbides type MC, M6C, M7C3, M23C6, and intermetallic secondary phases （sigma, chi, laves）, which cause the process of intergranular corrosion. The aim of this work was verifying the formation of the types of carbides and/or intermetallic phases existing in the stainless AISI 304 at 800 ℃, varying the timing of heat treatment between 30, 360 and 1,440 min. The optical microscopy analysis revealed the predominant formation of the carbide type M23C6. The results of DL-EPR （double loop electrochemical potentiokinetic reactivation） tests showed a gradual increase in the precipitation of this carbide with the increase of treatment time. The potentiodynamic polarization showed that the precipitation of this carbide reduce the formation of the Cr2O3 passive layer, suggesting that the precipitate carbide to be predominantly of the Cr23C6 type.

Phytochemical investigations of the ethanolic extract of leaves of Clutia lanceolata(Family:Euphorbiaceae)resulted in the isolation of four compounds viz.3,4-dihydroxy-2-methylbenzoic acid(1),2,20-dihydroxy-1,10-binaphthyl(2),1,3,8-trihydroxy-6-methylanthracene-9,10-dione(3)and 5-hydroxy-1,7-bis(4-hydroxy-3-methoxyphenyl)hepta-1,4,6-trien-3-one(4).Although all the isolated compounds were known but this was the first report from this plant *** structures were established on the basis of chemical and physical evidences *** analysis,FT-IR,1 H-NMR,13CNMR and mass spectral *** of compound 2 and 4 was further authenticated by single-crystal X-ray analysis and density functional theory *** isolated compounds(1–4)were screened for AChE enzyme inhibition assay in which compound 3 and 4 were found to be more potent AChE *** docking study of potent AChE inhibitor was performed to find the probable binding mode of the compounds into the active site of ***,the isolated compounds were also screened for in vivo cytotoxicity by brine shrimp lethality assay.

The aim of this work is evaluate the intergranular corrosion on UNS S31803 steel, with heat treatments at 800 ℃, varying treatment times of 30 mins, 360 mins and 1,440 mins. The results confirm the formation of o phases and secondary austenite （γ2）. For the analysis of the influence of o and γ2 phases, metallographic analysis were conducted through optical microscopy, potentiokinetic reactivation electrochemical techniques and potentiodynamic polarization in NaCI 3.5% solution. Microstructural analysis has shown a formation of γ2 and o phase in heat treatment, due to diffusion of chromium and molybdenum from δ phase to y phase, precipitating on δ/γ and δ/δ interfaces. The DL-EPR （Double loop electrochemical potentiokinetic reactivation） results have shown an increase of the DOS （degree of sensitization） for long periods of time on heat treatment. The results of potentiodynamic polarization showed a reduction of the corrosion and pitting potentials, followed by an increase of the current density when the UNS S31830 steel is heat treated during long periods of time.

Although uncommon, sporadic nonampullary duodenal adenomas have a growing detection due to the widespread of endoscopy. Endoscopic therapy is being increasingly used for these lesions, since surgery, considered the standard treatment, carries significant morbidity and mortality. However, the knowledge about its risks and benefits is limited, which contributes to the current absence of standardized recommendations. This review aims to discuss the efficacy and safety of endoscopic mucosal resection(EMR) and endoscopic submucosal dissection(ESD) in the treatment of these lesions. A literature review was performed, using the Pubmed database with the query: "(duodenum or duodenal)(endoscopy or endoscopic) adenoma resection", in the human species and in English. Of the 189 retrieved articles, and after reading their abstracts, 19 were selected due to their scientific interest. The analysis of their references, led to the inclusion of 23 more articles for their relevance in this subject. The increased use of EMR in the duodenum has shown good results with complete resection rates exceeding 80% and low complication risk(delayed bleeding in less than 12% of the procedures). Although rarely used in the duodenum, ESD achieves close to 100% complete resection rates, but is associated with perforation and bleeding risk in up to one third of the cases. Even though literature is insufficient to draw definitive conclusions, studies suggest that EMR and ESD are valid options for the treatment of nonampullary adenomas. Thus, strategies to improve these techniques, and consequently increase the effectiveness and safety of the resection of these lesions, should be developed.

BACKGROUND Esophagogastric junction outflow obstruction(EGJOO) is a rare syndrome,characterized by an elevation of the integrated relaxation pressure of the lower esophageal sphincter, not accompanied by alterations in esophageal motility that may lead to the criteria for achalasia. We were unable to find any prior report of the combination of Heller myotomy with anterior partial fundoplication(Dor) as the treatment for EGJOO. We herein report a case of EGJOO treated with laparoscopic Heller myotomy combined with Dor *** SUMMARY A 26-year-old man presented with a 3-year history of solid dysphagia and a 30-kg weight loss. He was treated with oral nifedipine, isosorbide, and omeprazole,without resolution of symptoms. An upper gastrointestinal series(barium swallow) revealed a "bird's beak" sign. Esophagogastroduodenoscopy was positive for Los Angeles grade A peptic esophagitis. High-resolution esophageal manometry was compatible with EGJOO. Esophageal pH monitoring showed pathological acid reflux both in orthostatic and decubitus position. An 8-cm laparoscopic Heller myotomy combined with an anterior 220° Dor fundoplication was performed. Solid diet was introduced on postoperative day 2, and the patient was discharged home the same day. At 17-mo follow-up, he reported no symptoms. Barium swallow was compatible with complete radiologic *** esophageal manometry and upper endoscopy showed normal findings 9 mo after the *** Surgical treatment with Heller myotomy and Dor fundoplication is a potential treatment option for EGJOO refractory to medical treatment.

To understand the influence of environmental factors on the metabolic diversity of phytochemical compounds from extracts of leaves and bark of Anacardium occidentale,collected in two different geographic areas,ultra-performance liquid chromatography and mass spectrometry were used,as well as the composition of physicochemical properties of both soils at the collection site(“Lavras da Mangabeira”and“Crato”).It was possible to observe physicochemical differences between the soils of the two collection areas and some *** present granulometry with a sandy texture,with high values of total sand,with acid pH,and variation in porosity *** micronutrients with the highest accumulation in the soil,with emphasis on the predominance of P(Phosphorus)and Al(Aluminium)mainly in the 20-40 cm layer in ***-three phytochemical compounds were identified in the leaf and bark extracts,of which 28 were present in the leaves and 20 in the bark *** this total,13 were present in at least one of the parts of the plant(leaf or bark),and most were present in both Crato and Lavras da Mangabeira,presenting mainly phenolic acids,flavonoids and tannins in all extracts *** extracts showed similarities between the groups of *** together,this study may be relevant to assess environmental influences on the production of secondary metabolites.
