Risk Factors for Fall in Elderly Patients:Follow-Up Study after Hospital Discharge

Introduction: Increased occurrences of falls after discharge are reported, especially in elderly people. Falls are a major cause of disability and identification of risk factors associated with falls is required to plan preventive actions. This study aims to determine the occurrence of falls in the three months after discharge and risk factors in elderly patients. Materials and Methods: 100 patients over 65 admitted to an Internal Medicine Ward participated. Questionnaires were given during hospitalization and three months after discharge. Follow-up information was unavailable for 31 patients (25 deceased, 6 unreachable). Results: Of those analyzed 52% were males with 80 ± 8.1 years (mean ± SD). Polymedication (p = 0.002), use of psychoactive drugs (p = 0.007), analgesics (p = 0.034) and walking devices (p = 0.006) were associated with a higher incidence of falls 6 months before hospitalization. Post-discharge follow-up was obtained for 69 patients: 18 reported falling during the follow-up. There was a higher risk of fall in patients with a history of falls in the 6 months before admission (p = 0.015 RR = 2.76). Patients who had one or more falls after discharge had a significantly shorter length of hospital stay compared to those who didn’t fall (p = 0.012). In multivariate logistic regression, we found that patients who were hospitalized more than 7 days had a lower risk of falling in the post-discharge period (OR = 0.195, p = 0.017) independently of the history of falls 6 months prior to admission. Conclusions: Further studies are required to validate the risk factors identified after discharge and to evaluate preventive measures. Elderly patients discharged from an Internal Medicine Ward should be screened to determine the risk of falls, specifically previous history of falls and medication. These patients should integrate a fall prevention program.

In this work we present the preparation and functionalization of gold nanoparticles (AuNPs) for the detection of presence of gp63 glycoprotein in the surface of Leishmania genus parasites. AuNPs were prepared by induced laser ablation in a clean and biologically suitable media. The nanoparticles were functionalized with anti-gp63 lgG antibody in order to study the interaction with the glycoprotein component gp63 (63 kDa) present on the membrane surface of Leishmania genus parasites. The functionalized AuNPs showed potential as a spectrometric indicator of the parasite existence, both by the detection of the presence of the gp63 in solution and through the specific interaction with the parasites in vitro. The specificity of the study opens a new line of research on the use of modified nanoparticles in the development of a fast and easy assay for Leishmaniosis diagnostics.

Comparison of Surgical Techniques Used in Treating Acromioclavicular Dislocation in Patients Participating in Sports: A Systematic Review

Introduction: The acromioclavicular joint is a superficial diarthrodial joint that surrounds the medial articular facet of the acromion and the distal portion of the clavicle. Due to its anatomy and biomechanics, it is highly susceptible to trauma and in young men who play contact sports, acromioclavicular dislocation is common. This article aimed to systematically review the literature and compare the surgical techniques used in the treatment of acromioclavicular dislocation in patients who practice sports. Methods: This systematic review was conducted according to the International Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines. Eligible studies for this systematic review included articles in English or Spanish published between 2013 and 2023, which mention the occurrence of acromioclavicular dislocation during sports practices. Additionally, only studies that addressed the surgical treatment of acromion-clavicular dislocation and contained original data on the topic were included. Results: We found 144 eligible studies after searching the LILACS and PubMed databases. Based on the inclusion and exclusion criteria and the reviewers’ consensus, we selected four studies for the systematic review. 133 patients with AC joint displacement were evaluated. Mean Age: approximately 31.90 years. 81.92 of these injuries occurred during sports practice. Surgical Procedures Used: titanium plates fixation (49 patients), arthroscopy (24), single tunnel technique (30) and coracoid sling technique (30). The results of the visual analog scale and Constant-Murley scores varied between the techniques used. Twenty-two complications after surgical treatment were identified. Conclusion: A significant variability of operative techniques can be used in the surgical approach of acromioclavicular dislocation, such as arthroscopy, single tunnel, coracoid sling and titanium plates. The cave-site of Gruta da Furninha is a coastal karstic cavity. In the late 19th century, excavation of the sedimentary infill of the cave allowed the identification of two lithostratigraphic units： a Holocene one dominated by sands, containing human bones and a Neolithic industry; and a Late Pleistocene one, containing a diverse set of fossils set and Paleolithic artifacts. This study mainly focuses on taphonomic and palaeoenvironmental data concerning the Pleistocene bird bones that were collected from six layers （at 11 m and 6.7 m asl）. A main result of this study was the discovery of a left humerus fragment, with osteological features of the Alcidae family; from comparison with upper arms of distinct species of this family, it was concluded that this fossil belongs to Penguin impennis. The Pleistocene birds of the Furninha cave were also compared with the avifauna that currently occurs in the region of Peniche peninsula and a climate and environmental interpretation of the Pleistocene fossiliferous set is provided.
