Gastric Bypass with Long Pouch and Transit Bipartition for Endoscopic Access to the Remaining Stomach

Introduction: obesity has a complex and multifactorial etiology, difficult treatment and increasing incidence rates in recent decades. The treatment involves clinical and pharmacological approaches and, in case of lack of results, surgical interventions. Roux-en-Y gastric bypass (RYGB) is one of these surgical interventions in which the stomach is divided, creating a small pouch, and the remaining portion of the stomach become excluded and left without endoscopic access. Objective: to evaluate the results of modified RYGB with long pouch and endoscopic access to the remaining stomach. Materials and Methods: prospective clinical trial with sample selected among patients seen at the Alberto Rassi General State Hospital of Goiânia (HGG) and indicated for bariatric and metabolic surgery confirmed by the medical and multidisciplinary team. The study was conducted from January 2020 to August 2021. Clinical history and laboratory test results of the selected patients were collected through consultations with the medical and multidisciplinary team. Results: twelve participants were included in the study. Of these, 11 (91.7%) were women and the mean age was 46.3 years. The weight before surgery was 112.17 kg (92.00 - 150.00) and the Body Mass Index (BMI) was 44.89 (35.06 - 74.39). After 18 months of surgery, the mean weight was 80.77 kg (±11.92) and the mean BMI was 29.46 (±11.00), showing a significant reduction in both (p = 0.003 and p = 0.002, respectively). All patients underwent endoscopic evaluation of the pouch, remaining stomach and duodenum at 12 months postoperatively. The mean percentage of lost excess weight loss was 68.21%. Conclusion: We conclude that the proposed changes in RYGB (GBLP + GIB – Roux-en-Y gastric bypass with long pouch and gastrointestinal bipartition) did not compromise weight loss or control of type 2 diabetes and other comorbidities and proved to be a safe and effective alternative without gastroduodenal exclusion, enabling a better postop

Modelling the test, trace and quarantine strategy to control the COVID-19 epidemic in the state of São paulo, Brazil

Testing for detecting the infection by SARS-CoV-2 is the bridge between the lockdown and the opening of *** this paper we modelled and simulated a test-trace-andquarantine strategy to control the COVID-19 outbreak in the State of São paulo,*** State of São paulo failed to adopt an effective social distancing strategy,reaching at most 59%in late March and started to relax the measures in late June,dropping to 41%in 08 ***,São paulo relies heavily on a massive testing strategy in the attempt to control the *** alternative strategies combined with economic evaluations were *** strategy included indiscriminately testing the entire population of the State,reaching more than 40 million people at a maximum cost of 2.25 billion USD,that would reduce the total number of cases by the end of 2020 by 90%.The second strategy investigated testing only symptomatic cases and their immediate contacts e this strategy reached a maximum cost of 150 million USD but also reduced the number of cases by 90%.The conclusion is that if the State of São paulo had decided to adopt the simulated strategy on April the 1st,it would have been possible to reduce the total number of cases by 90%at a cost of 2.25 billion US dollars for the indiscriminate strategy but at a much smaller cost of 125 million US dollars for the selective testing of symptomatic cases and their contacts.

Introduction: Hospital Recreation (HR) aims to reduce disasters caused by hospital admission. Objectives: To expose the behavior of children in cancer treatment (CA) during HR, and to verify which recreational activities generate the most adherence. Methods: The research is an experimental clinical trial of a non-probabilistic and intentional sample, with a quantitative and qualitative approach. Participants were 43 children (both sexes), with CA, admitted to the Pediatric Hospital Oncology Center—PHOC. The mean age of the children in fact was 7.5 years, and the data of mean and standard deviation between parenthood confer with the data of the results found during the research. The study lasted 12 weeks, with 38 sessions lasting 3 hours. Using games of activity: cognitive, manipulative, perceptive, creative and cooperative. Inclusion criteria: 1) being hospitalized in the PHOC, of the city of Natal;2) being released by the medical team to participate in the activities. Children who were on special care in the ICU were excluded from the study. The behavior of the children was evaluated by the observation matrix proposed by Urie Bonfenbrenner, approached by Krebs and Koller. At the end of the research, an interview was made with patients about the HR experience. Results: In the subjects the molecular behavior showed relevance of ~50%, and the cooperative of ~80% in relation to HR, and the cognitive activities showed an adhesion index of ~90%. Conclusion: The participants showed greater adherence in the cognitive activities, and the predominance of the molecular behavior in relation to hospital recreation, highlighting the cooperative behavior among the individuals. In this way, the activities of HR can complement conventional medical treatment.

With the expansion of eucalyptus crops to areas with severe water limitations,physiological studies involving eucalyptus clones to identify those that are tolerant to water stress become *** objective of this study was to assess morphological and physiological responses by eucalyptus clones subjected to drought stress and *** experiment consisted of three eucalyptus clones:VC865,I224 and I144 and two water regimes:control and water stress followed by rehydration,with six *** water potential,gas exchange,maximum quantum efficiency of photo systemⅡand plant height and stem diameter were evaluated under drought stress and ***6 d of rehydration,the number of leaves,leaf area and dry mass of root,leaf,stem and their total were *** clones showed intense reduction of gas exchange during the drought stress period,and only VC865 and 1144 showed rapid recovery with 3 d of *** 1224 showed greater reduction in height,stem diameter,number of leaves,water potential at midday(Ψ_(w)_(Midday)),and maximum quantum efficiency of photosystemⅡ(F_(v)/F_(m)).Clones VC865and T144 showed lower reductions inΨ_(wMidday)and F_(v)/F_(m) under ***865 had lower reductions in leaf number,leaf area and higher leaf dry mass,while clone I144 had higher height and lower reduction in root dry mass *** these clones showed higher water use efficiency with 3d of *** different phenotypic plasticities gave the clones VC865 and 1144 efficient mechanisms of acclimatization to stress and more drought tolerance,enhancing their greater capacity for recovery after stress,which allowed lower dry mass *** 1224,however,was more susceptible to drought stress,undergoing greater physiological damage with only partial recovery during rehydration.

直接乙醇燃料电池作为便携式移动电源受到越来越多的关注,如乙醇能量密度高、可再生、无毒、清洁、便于储存和运输等优点。然而,乙醇燃料电池的研究也面临一些严重问题,其中C−C键断裂是最为挑战的问题之一。近年来有报道表明,乙醇的完全氧化(包含C−C键断裂过程)并没有显示出燃料电池效率的增加,相反乙醇不完全氧化的催化剂会导致更明显的催化电流增加。同时金属Bi掺杂会对催化剂产生电子效应和几何效应并有效增加碱性体系中催化剂的活性位中心和抗CO中毒能力。因此,本工作将铋掺杂在铂和钯中制备出PtBi/C和PdBi/C作为乙醇燃料电池催化剂,同时研究Bi掺杂催化剂对乙醇燃料电池性能以及乙醇的不完全氧化产物醋酸盐的影响,为乙醇燃料电池电-化学品联产提供一定的依据。以硼氢化钠为还原剂,Vulcan XC-72(Cabot)导电炭为载体制备了Pt/C、Pd/C、PtBi/C和PdBi/C(20%质量比)炭载催化剂,其中PtBi和PdBi原子比均为95∶5。首先将金属前驱体溶于H_(2)O/异丙醇溶液(50/50,体积比)中,加入Vulcan XC-72导电炭黑充分混合,然后加入硼氢化钠在常温常压下充分反应,过滤洗涤干燥备用。其次,对所制备的催化剂进行TEM、XRD、XPS、电化学以及光谱电化学进行表征和测试。电化学、光谱电化学在三电极电解池中进行,将所制备的催化剂分散在超薄多孔电极上制备成工作电极,对电极为Pt片电极,参比电极为Ag/AgCl电极。电解液为碱性1 mol/L KOH的乙醇溶液,在电化学工作站上进行CV、LSV扫描以及在不同电位下测试溶液的ATR-FTIR谱。最后,燃料电池测试在一个面积为5 cm2的单电池上进行,阳极为2 mol/L乙醇+3 mol/L KOH溶液,流量为2 mL/min,阴极通入氧气,并研究催化剂的构成及性能对电池性能的影响。采用硼氢化钠还原法合成Pt/C、PtBi(95∶5)/C、Pd/C和PdBi(95∶5)/C。Pd/C和PdBi/C的晶体结构呈现FCC特征,Pt/C和PtBi/C的晶体结构也呈现FCC特征。XRD也揭示了铋原子对Pt晶体结构的影响。在1 mol/L KOH溶液中,异位XPS和循环伏安法显示这些金属具有较高的氧化态。透射电镜显示Pd/C有一定的纳米粒子聚集,PdBi/C有一定的有序结构。红外光谱(ATR-FTIR)结果表明,Bi效应由于吸附能力较弱,抑制了碳酸盐的生成,增加了醋酸盐的生成。由于醋酸盐优先生成,且不被碳酸盐离子毒害,PtBi/C具有最佳的电化学和DEFC性能。通过在Pt和Pd掺杂金属Bi制备具有电子调控的双金属炭载PtBi(95∶5)/C和PdBi(95∶5)/C催化剂,以增强乙醇氧化反应催化氧化过程。透射电镜(TEM)和X射线光电子能谱(XPS)结果表明,Bi含量不影响PdBi纳米颗粒的尺寸,但会影响PtBi纳米颗粒的尺寸。X射线衍射分析表明,Bi掺杂改变了Pt晶体结构中的晶格参数。此外,ATR-FTIR结果表明,碳酸盐的形成受到抑制,乙酸盐产量增加。极化曲线和功率密度曲线结果表明,PtBi/C催化剂具有更高的功率密度,几乎是Pt/C的6倍。在线性扫描伏安实验中,PtBi/C具有最高的电流密度(44 mW/cm2)和最低的起始电位(−0.6 V)。在电流-时间实验中,它也具有最高的最终电流密度。因此,在Pt和Pd中掺杂铋催化剂被证明是一种很有前途的燃料电池发电和醋酸盐的联产方法。

Instruments for Analysis of Knowledge Management in Operators Companies of Metropolitan Transport Systems

This research aimed to determine instruments for analysis of knowledge management in operators companies of metropolitan transport systems. How much the goal was an exploratory research, how much the procedure was a literature search, how much as nature was an applied research, and how much as approach was qualitative research？ The following instruments were identified and selected：（a） the methodology for the analysis of metropolitan transport systems; （b） the knowledge management maturity model （Siemens）; and （c） Documentation and Information Centre. The simulation, using the tools in the knowledge management to analyze the Company Metropolitan of S^io paulo--METRO, identified the need for improvements in eight areas and the problems：（1） rethink the knowledge management area; （2） develop new knowledge management policies; （3） define responsibilities in knowledge management area; （4） implement new knowledge management processes; （5） explore new ways and structures of knowledge; （6） consider the internal and external actors in knowledge management actions; （7） expand the dissemination of the collaborative culture; and （8） integrate in the knowledge management area current and future initiatives. The analysis shows that the transposition of the current situation to the desired situation for the company can be obtained with the implementation of an Information and Documentation Centre. The conclusion is that this set of tools allows to characterize, evaluate, and model the knowledge management in this business.

Intense Exercise in the Quality of Life of Breast Cancer Survivors:A Meta-Analysis

The treatment of breast cancer (BC) leaves emotional and functional sequels affecting the quality of life (QOL) of the survivors. We aim to investigate, intense exercises in the rehabilitation of BC survivors. Using the systematic search model in the PubMed databases;Lilacs Bireme and Scielo. Meta-Analysis used the Meta package implemented in software R (version 3.3.2) p < 0.05. We selected 29 articles, and after reading in the Integra, we excluded 27 texts being included in the meta-analysis only 2. In the QOL, the heterogeneity in the emotional function showed a difference of 75%, in the social function 36% and in the physics 32%. Using intense exercises in BC survivors during rehabilitation improves overall QOL, muscle structure, and preserves functional capacity.
