In recent years,pediatric health care has em.raced the concept of quality im.rovem.nt to im.rove patient *** quality im.rovem.nt efforts are im.lem.nted,network collaboration(where m.ltiple centers and practices im.lem.nt standardized program.)is a popular *** a collaborative network,im.rovem.nt in the conduct of structural,process and outcom. quality m.asures can lead to im.rovem.nts in overall health,and benchm.rks can be used to assess and com.are *** this review article,we provided an overview of the quality im.rovem.nt m.vem.nt and the role of quality indicators in this *** reviewed current quality im.rovem.nt efforts in pediatric inflam.atory bowel disease (IBD),as well as other pediatric chronic *** discussed the need to standardize the developm.nt of quality indicators used in quality im.rovem.nt networks to assess m.dical care,and the validation techniques which can be used to ensure that process indicators result in im.roved outcom.s of clinical *** aim.d to assess current quality im.rovem.nt efforts in pediatric IBD and other diseases,such as childhood asthm.,childhood arthritis,and neonatal *** doing so,we hope to learn from.their successes and failures and to m.ve the field forward for future im.rovem.nts in the care provided to children with IBD.

Objective:This study determ.ned choice of place of delivery am.ng wom.n accessing antenatal care clinic s in Nnam.i Azikiwe University Teaching Hospital(NAUTH)Nnewi in Anam.ra ***:A descriptive survey was conducted with three hundred and twenty-two pregnant wom.n who registered for antenatal care at Nnam.i Azikiwe University Teaching Hospital,Nn *** were collected using a self-structured questionnaire and analyzed with a Chi-square test of association and Fisher’s exact test were used to test the hypothesis at 5%level of ***:Findings from.the study showed that 39.75%of the pregnant wom.n chose private hospital s as their place of delivery,37.27%chose NAUTH,6.52%and 7.14%chose other governm.nt hospitals and hom. delivery,*** was a significant difference between education level and occupation of the participants in the choice of place of ***,finance,distance and approach of health workers were significant predictors of choice of delivery place am.ng the ***:Health planners need to recognize the determ.nant choice of delivery place as m.re efforts should be given to educate wom.n and em.ower ***,utilization of tertiary health facilities for childbirth m.ay increase if the privacy of pregnant wom.n is m.intained,health workers relate well with clients and the cost of health care services is reduced.

背景:以食品正面包装的营养标签、广告规范或粮食税为基础的营养素度量体系对于公共健康是一个很有用的工具。然而,它的能耐远超于对产品描述并能够充分地塑造个体膳食有必要更进一步的调查。目的:本研究旨在(1)计算个体层面的分值基于英国FSA食物级别的每个食物消耗的营养素度量体系,(2)依据食物组消耗量、营养摄入量以及社会人口和生活方式的改变评估膳食状况评分的正确性。方法:从NutriNet-Santé研究(n=4225)中选定法国人群作为代表性样本。采用24小时膳食回顾记录收集膳食数据。社会人口和生活方式的数据采用自我报告形式记录。所有食物消耗量采用FSA营养素度量法描述,并且来自于每个食物消耗的能量摄入量用于计算个体层面的FSA导出的累积分值。遵循法国营养素推荐规范(PNNS-GS)的分数以膳食质量评分作为比较而计算。在食物消耗、营养指标、生活方式和社会人口的变化与累积分的四分位之间的关联性通过ANOVAs和线性回归模型调查。结果:得到有利分值的受试者消耗大量的水果(差异△=156g/d,P<0.001),蔬菜(△=85g/d,P<0.001),以及鱼和少量的零食(△=-72g/d,P<0,001);同时他们也有大量维生素和矿物质的摄入以及少量的饱和脂肪酸的摄入。得到有利分值的受试者很好的遵循了依据PNNS-GS衡量的营养素推荐标准(△=2.13 points,P<0.001)。女性年长的受试者以及高收入的群体更可能得到较高的分值。结论:研究结果表明FSA营养素度量体系对法国人群中个体膳食的概述更加全面可行。NutriNet-Santé研究已被登录于2013-000929-31欧洲临床试验数据库(EudraCT)。

人的神经元能表达先天性免疫应答受体-Toll样受体3(TLR3)。TLR3的水平在病理条件下会增加,如大脑受到病毒感染时。在本试验中,笔者进一步研究了人类神经元细胞在受到具有神经毒性的狂犬病病毒(RABV)感染时,神经元中TLR3的含量、细胞定位及作用。经狂犬病病毒感染后,TLR3不仅存在内含体内,而且存在于耐去污剂的外周包涵体内,而未受狂犬病病毒感染时,TLR3仅存在于内含体内。除了TLR3以外,这些包涵体内还有病毒基因组和病毒蛋白(N蛋白和P蛋白,但是没有G蛋白)。用电子显微镜对包涵体的大小和组成以及外层膜的缺失进行观察,结果显示它们与之前所描述的病小体相符。在没有TLR3时不形成Negri小体,并且脑组织未受到太严重感染的TLR3-/-小鼠的存活率比野生型小鼠高。这些观察结果证实,TLR3是参与狂犬病病毒空间排布的主要分子,能诱导Negri小体的产生以及病毒的复制。该研究揭示了病毒利用细胞内蛋白和区室化作用使自己生存的机制。
