＜正＞The actual joint prosthesis production is still very empirical. Modern tools for mechanical investigating enable to take in account the complexity of the muscular system, the Dynamics of efforts generated by the biomechanical system. Experimental and numerical methods association enable to determine the mechanical behaviour of the muscular-skeletal system. The goal of this research is to determine the dynamic muscular and intra-joint forces applied on the hip joint during walking on flat ground. The hip joint dynamic behaviour was studied during level walking. Four steps were considered: Trajectory measurements, 3D solid motion reconstruction, Hip joint external force and torque using inverse dynamics, internal forces on joint and muscles computation by optimisation method. The trajectory measurements concerned foot, leg, thy, pelvis, and provided accelerations values needed by the dynamic relationships. The Simplex optimisation method used a non linear programming algorithm;two minimizing conditions were tested: Muscular stress, and Joint force.

In patients with definitively unresectable colorectal liver metastases,whether the primary tumor should be resected prior to chemotherapy is an old *** retrospective studies(1-5)and a meta-analysis with individual data(6)had suggested that primary resection of the primary tumor followed by chemotherapy was associated with better survival than the chemotherapy-first *** prevention of complications related to the primary tumor(occlusion,perforation,bleeding)as well as a better response to chemotherapy were usual justifications to explain the superiority of primary tumor ***,the absence of any randomized study made it difficult to conclude on this question due to the numerous selection biases related to these retrospective analyses.

Despite ongoing efforts to reduce tobacco smoking, the smoking prevalence in many countries has remained stable for years. This may be a consequence of either lack of knowledge about effective ways to reduce smoking, or failing treatment of tobacco addiction in healthcare. This study explored gaps in the current understanding of smoking cessation and the challenges facing tobacco addiction management in order to formulate recommendations for future research and healthcare practice. A narrative review was written to determine areas in which more research is needed as well as areas in which sufficient knowledge is already available. Recommendations for future research were prioritised using a Delphi-procedure. Recommendations for healthcare practice were confirmed by expert’s assessment. Smoking is not widely acknowledged as an addiction and a relatively small number of smokers ask help from a healthcare professional when trying to stop smoking. Most healthcare professionals recognise the importance of advising patients to stop smoking, but experience certain barriers to actually do this. Overall, healthcare professionals need to be convinced that tobacco smoking is an addiction and should be treated likewise. If all healthcare professionals systematically advise their patients to give up smoking, eventually more smokers will successfully stop smoking.
