复方丹参、葛根辅助治疗冠心病的疗效研究

目的评估丹参加葛根作为辅助治疗药物对冠状动脉粥样硬化性心脏病(冠心病)二级预防的疗效及安全性。方法符合入选标准的冠心病患者100例分为丹参、葛根胶囊治疗组(试验组)和安慰剂组(对照组),每组50例,进行为期6个月的随机、双盲临床研... 详细信息

目的评估丹参加葛根作为辅助治疗药物对冠状动脉粥样硬化性心脏病(冠心病)二级预防的疗效及安全性。方法符合入选标准的冠心病患者100例分为丹参、葛根胶囊治疗组(试验组)和安慰剂组(对照组),每组50例,进行为期6个月的随机、双盲临床研究。6个月后受试者再自愿选择参与6个月的开放试验,期间受试者口服丹参、葛根胶囊的剂量为1.5g/d。采用超声波血管扫描测量并评估肱动脉血流介导性血管舒张功能(flow mediated dilation,FMD)和颈动脉内-中膜厚度(intima-media thic.ness,IMT)。结果给药前,两组间基础指标均衡,治疗6个月后,两组的血压、血液学、血生化、叶酸及同型半胱氨酸水平与治疗前相比,差异无统计学意义(P>0.05);低密度脂蛋白胆固醇与治疗前相比均有轻度下降,差异有统计学意义[(2.6±0.7)mmol/Lvs.(2.7±0.9)mmol/L,P<0.05;(2.5±0.7)mmol/Lvs.(2.8±0.8)mmol/L,P<0.05];试验组治疗后FMD改善明显,差异有统计学意义(5.9%±1.0%vs.5.3%±1.2%,P<0.001),对照组治疗后FMD有一定程度改善(5.5%±1.0%vs.5.3%±1.1%,P<0.05)。IMT的改善仅发生在试验组,差异有统计学意义[(0.96±0.32)mmvs.(0.98±0.30)mm,P<0.05]。6个月开放试验结果显示,试验组FMD和IMT有进一步改善,差异有统计学意义[5.91%±0.95%vs.5.35%±1.21%,P<0.05;(0.964±0.316)mmvs.(0.979±0.303)mm,P<0.05]。研究期间8例出现不同程度不良反应:试验组2例;对照组6例,其中2例需要终止临床试验。结论丹参加葛根辅助治疗冠心病具有良好的耐受性,能有效改善血管功能和结构,可用于冠心病的二级预防。

目的：冠心病患者常并有其他动脉粥样硬化，该研究①评价造影诊断冠心病的患者肾动脉狭窄的流行病学资料；②评价肾功能不全及不同程度肾动脉狭窄之间的关系。③报道偶然发现有显著肾动脉狭窄而行经皮肾动脉成形术的初步结果。方法：从1995年至今，123例（70％并高血压病）冠脉造影结果阳性的病人行腹主动脉造影和选择性肾动脉造影。男71例，女52例。年龄64.4±10．3（41－86）岁。血肌酐109±44μmol/L（62;43），其中21例（17%）肾功能不全（血肌酐＞103μmol/L）。结果：25例（20％）有轻微动脉狭窄（20％一50％）．15例（12%）(男6例，女9例)，年龄69．7±7．2岁，血肌酐132±60μmol/，有显著肾动脉狭窄（≥50％），其中7例（47%）有肾功能不全，4例肾动脉狭窄≥70％，血肌酐分别543、175、162μmol/L，平均262±188umol/L，4例全部行PIRA及支架植入，其中1例同时行冠脉成形术，全部顺利进行。随诊15±7．7个月。血肌酐分别为299、199、204及140μmol/（平均191±81μmol/L），用降压药物种类由术前1．25降至0．75种结论：动脉粥样硬化性肾动脉狭窄是冠心病的一个常见并发病。显著的肾动脉狭窄常并有肾功能受损。冠脉造影同时行腹主动脉造影是安全而易行的，可以作为合并高血压病尤其是老年人及

Linked to major volc.nic.eruptions around 536 and 540 c., the onset of the Late Antique Little Ic. Age has been desc.ibed as the c.ldest period of the past two millennia. The exac. timing and spatial extent of this exc.ptional c.ld phase are, however, still under debate bec.use of the limited resolution and geographic.l distribution of the available proxy.arc.ives. Here, we use 106 wood anatomic.l thin sec.ions from 23forest sites and 20 tree spec.es in both hemispheres to searc. for c.ll-level fingerprints of ephemeral summer c.oling between 530 and 550 c.. After c.oss-dating and double-staining, we identified 89Blue Rings(lac. of c.ll wall lignific.tion), nine Frost Rings(c.ll deformation and c.llapse), and 93Light Rings(reduc.d c.ll wall thic.ening) in the Northern Hemisphere. Our network reveals evidenc. for the strongest temperature depression between mid-July.and early.August 536 c. ac.oss North Americ. and Eurasia, whereas more loc.lised c.ld spells oc.urred in the summers of 532, 540–43, and548 c.. The lac. of anatomic.l signatures in the austral trees suggests limited inc.rsion of stratospheric.volc.nic.aerosol into the Southern Hemisphere extra-tropic., that any.forc.ng was mitigated by.atmosphere-oc.an dy.amic.l responses and/or c.nc.ntrated outside the growing season, or a c.mbination of fac.ors. Our findings demonstrate the advantage of wood anatomic.l investigations over traditional dendroc.ronologic.l measurements, provide a benc.mark for Earth sy.tem models, support c.oss-disc.plinary.studies into the entanglements of c.imate and history. and question the relevanc. of global c.imate averages.
