超声引导（ultrasound guidance，USG）锁骨下臂丛神经阻滞可提供实时的进针程度和局麻药分布图像。但在USG下行臂丛神经阻滞时，局麻药扩散情况能否取代神经刺激反应作为局麻药的注药指征，目前尚无正式的研究报道。因此，我们进行了此项前瞻性随机研究实验：以72例择期行手和前臂手术的患者作为观察对象，将患者分为单独应用超声引导组（U组）和超声引导与神经刺激联合应用组（S组），比较两组完成神经阻滞操作的时间以及锁骨下阻滞的效果。U组患者采用尽可能少的注药次数注入局麻药，在腋动脉的后方及两侧呈U形分布（其注射次数为1、2和3次的患者数分别为29、6和3例）。S组患者则应用0．3—0．6mA的刺激电流产生远端运动反应之后，行单次注药。注入的麻醉药液为1．5％的利多卡因与0．125％的布比卡因混合液内含1：200000的肾上腺素（最终浓度），药物容量为0．5ml／kg。与S组患者比较，U组患者的阻滞操作时间明显缩短（分别为3．1±1．6分钟及5．2±4．7分钟；P=0．006）。S组患者中，37％的患者局麻药液向腋动脉前方扩散，而63％的患者局麻药液向腋动脉后方扩散。注药后30分钟产生肌皮神经、正中神经、桡神经和尺神经区域完全性感觉阻滞效果的U组患者为86％，而S组患者中仅占57％（P=0．007）。U组患者单次注药与多次注药所产生的完全阻滞率相同，均为86％。U组患者需要追加给药率为8％，而S组为26％（P=0．049）。S组中有1例患者由于20分钟之后仍不能获得远端刺激反应而阻滞失败。由此，我们得出如下结论，在超声引导行锁骨下臂丛神经阻滞时，以观察药液分布情况作为注入局麻药的指征，可缩短操作时间，提高麻醉成功率。局麻药围绕腋动脉的后一侧方向扩散可预示阻滞成功，并免除了直接观察神经的必要。

AIM： To report anatomic and visual outcomes following silicone oil removal in a cohort of patients with complex retinal detachment, to determine association between duration of tamponade and outcomes and to compare patients with oil removed and those with oil in situ in terms of demographic, surgical and visual factors. METHODS： We reported a four years retrospective case series of 143 patients with complex retinal detachments who underwent intraocular silicone oil tamponade. Analysis between anatomic and visual outcomes, baseline demographics, duration of tamponade and number of surgical procedures were carried out using Fisher＇s exact test and unpaired two-tailed t-test. RESULTS： One hundred and six patients（76.2%） had undergone silicone oil removal at the time of review with 96 patients（90.6%） showing retinal reattachment following oil removal. Duration of tamponade was not associated with final reattachment rate or with a deterioration in best corrected visual acuity（BCVA）. Patients with oil removed had a significantly better baseline and final BCVA compared to those under oil tamponade（P=0.0001, 〈0.0001 respectively）. CONCLUSION： Anatomic and visual outcomes in this cohort are in keeping with those reported in the literature. Favorable outcomes were seen with oil removal but duration of oil tamponade does not affect final attachment rate with modern surgical techniques and should be managed on a case by case basis.

Surface texture and friction characteristics of diamond-ground concrete and asphalt pavements

Recently, diamond grinding has gained increasing attention as pavement preservation treatment to restore desired surface characteristics, particularly friction. Compared to other pavement preservation treatments such as surface overlays and high friction surface treat- ments, diamond grinding may cost less, save construction time, or require minimum maintenance. Diamond grinding produces longitudinal, continuous, and line-type texture that contains corrugations with evenly spaced ridges. The improved surface texture will immediately enhance pavement surface friction and reduce the possibility of hydroplaning in rainy weather. However, little information has been documented on the texture charac- teristics and long-term friction performance of diamond-ground pavements. A field evalu- ation was conducted to examine the surface texture and friction characteristics in diamond- ground concrete and asphalt pavements by the authors. Five pavement test sections, including two diamond-ground concrete pavements, one diamond-ground concrete bridge deck, one diamond-ground asphalt pavement, and one transversely tined concrete pave- ment, were selected for evaluation. Laser scanner testing was performed to capture both macro and microtexture profiles. Locked wheel testing was performed to measure the fric- tion numbers. The test results were examined and compared so as to evaluate two performances, after construction and long-term friction performance of diamond-ground pavements. It was found that longitudinal diamond grinding can provide durable, satisfactory surface friction performance for both concrete and asphalt pavements.

Remote sensing was used in a series of experiments over a three-year period to obtain spectral reflectance data for studying differences in vegetation indices between grasses, broadleaf plants, and grass/broadleaf plant mixtures. Selected non-crop winter and spring host plants of tarnished plant bug, Lygus lineolaris, were planted in field-plot experiments. Multispectral reflectance data were aerially acquired with both a Real-Time Digital Airborne Camera System (RDACS) sensor and Geospatial Systems (Duncan Tech) MS-2100 multispectral camera. The following six vegetation indices of spectral reflectance were evaluated in this study: normalized difference vegetation index, ratio vegetation index, green normalized difference vegetation index, green vegetation index, green ratio vegetation index, and Ashburn vegetation index. Vegetation indices calculated with imagery data for the grasses and broadleaves differed significantly;there appeared to be more discriminating differences between vegetation indices for grasses and broadleaf plants when the indices were based on a combination of green and near infrared or green and red spectral bands than when the indices were based on red and near infrared spectral bands. Insect data from these studies confirm that tarnished plant bug prefers broadleaf host plants, but can use Italian ryegrass for food and reproduction. The narrow temporal window of host suitability for Italian ryegrass may limit its significance. Herbicide destruction of broadleaf host plants in early spring prevented the tarnished plant bug population increases that occurred in untreated plots. Findings of this study would be useful in site-specific vegetation management decisions in programs such as the prototypical area-wide tarnished plant bug management experiment conducted in the Delta region of Mississippi.

OMEGA is a system for cultivating microalgae using wastewater contained in floating photobioreactors (PBRs) deployed in marine environments and thereby eliminating competition with agriculture for water, fertilizer, and land. The offshore placement in protected bays near coastal cities co-locates OMEGA with wastewater outfalls and sources of CO2-rich flue gas on shore. To evaluate the feasibility of OMEGA, microalgae were grown on secondary-treated wastewater supplemented with simulated flue gas (8.5% CO2 V/V) in a 110-liter prototype system tested using a seawater tank. The flow-through system consisted of tubular PBRs made of transparent linear low-density polyethylene, a gas exchange and harvesting column (GEHC), two pumps, and an instrumentation and control (I&C) system. The PBRs contained regularly spaced swirl vanes to create helical flow and mixing for the circulating culture. About 5% of the culture volume was continuously diverted through the GEHC to manage dissolved oxygen concentrations, provide supplemental CO2, harvest microalgae from a settling chamber, and add fresh wastewater to replenish nutrients. The I&C system controlled CO2 injection and recorded dissolved oxygen levels, totalized CO2 flow, temperature, circulation rates, photosynthetic active radiation (PAR), and the photosynthetic efficiency as determined by fast repetition rate fluorometry. In two experimental trials, totaling 23 days in April and May 2012, microalgae productivity averaged 14.1 ± 1.3 grams of dry biomass per square meter of PBR surface area per day (n = 16), supplemental CO2 was converted to biomass with >50% efficiency, and >90% of the ammonia-nitrogen was recovered from secondary effluent. If OMEGA can be optimized for energy efficiency and scaled up economically, it has the potential to contribute significantly to biofuels production and wastewater treatment.

Architects and planners have been at the forefront of envisioning a future built environment for millennia. How ever,fragmental view s that emphasize one facet of the built environment,such as energy,environment,or groundbreaking technologies,often do not achieve expected outcomes. Buildings are responsible for approximately one-third of w orldw ide carbon emissions and account for about 40% of primary energy consumption in the U. S. In addition to achieving the very ambitious goal of reducing building-associated greenhouse gas emissions by75% by 2050,buildings must improve their functionality and performance to meet current and future human,societal,and environmental needs in a changing w orld. In this article,w e introduce a new framew ork to guide potential evolution of the building stock in the next century,based on greenhouse gas emissions as the common thread to investigate the potential implications of new design paradigms,innovative operational strategies,and disruptive technologies. This framew ork emphasizes integration of multidisciplinary know ledge,scalability for mainstream buildings,and proactive approaches considering constraints and unknow *** framew ork integrates the interrelated aspects of the built environment through a series of quantitative metrics that aim to improve environmental outcomes w hile optimizing building performance to achieve healthy,adaptive,and productive buildings.
