Patient, Provider and System Factors Impacting on the Diagnosis and Management of l.ng Cancer Care in Austral.a

Background: l.ng cancer is the l.ading cause of cancer death in Austral.a, with onl. modest improvements in survival. This study aims to identify factors impacting on diagnosis and management of l.ng cancer with particul.r reference to Austral.an primary care. Methods: A sequential.mixed method modified approach empl.ying interview and a two-phased survey technique. Fol.owing tel.phonic interviews with 31 heal.h professional. (individual. representing general.practitioners, special.zed physicians, nurses and al.ied heal.h practitioners), interview data was anal.sed using qual.tative thematic anal.sis, and surveys using descriptive statistics. Emergent themes were organised under patient, provider and system factors. Interviews ceased upon saturation of data. Resul.s: Mul.ipl. patient, provider and systems issues were seen to contribute to adverse heal.h outcomes. There is a strong rel.tionship between smoking and outcomes, and factors rel.ted to higher smoking rates such as a l.wer socioeconomic status. For smokers, guil. and/or denial.was considered a reason for del.y in the decision to seek medical.care for cough or shortness of breath. Aboriginal.peopl. under-report morbidity rel.ted to smoking and chronic obstructive pul.onary disease;other patients fail.to recognise the significance of their symptoms. Discussion: Despite the poor prognosis of l.ng cancer diagnosis, increased awareness of presentation and treatment options can address disparities in heal.h outcomes.

Upper l.mb function impairment is one of the most common sequel.e of central.nervous system in jury, especial.y in stroke patients and when spinal.cord injury produces tetrapl.gia. Conventional.assessment methods cannot provide objective eval.ation of patient performance and the effec tiveness of therapies. The most common assessment tool. are based on rating scal.s, which are inefficient when measuring smal. changes and can yiel. subjective bias. In this study, we designed an inertial.sensorbased monitoring system composed of five sensors to measure and anal.ze the compl.x movements of the upper l.mbs, which are common in activities of dail. l.ving. We devel.oped a kinematic model.with nine degrees of freedom to anal.ze upper l.mb and head movements in three dimensions. This system was then val.dated using a commercial.optoel.ctronic system. These findings suggest that an inertial.sensorbased motion tracking system can be used in patients who have upper l.mb impairment through data integration with a virtual.real.tybased neurorehabil. tation system.

Changes in fat but not fruit and vegetabl. intakes l.nked with body weight change in Mexican women immigrants in Quebec

The objective of the present study was to identify dietary parameters for predicting body weight change (ΔBW) in Mexican-born women (Mexicans) fol.owing immigration to Quebec City, Canada. Methods: Changes in fruit (ΔF), vegetabl. (ΔV), fruit and vegetabl. (ΔFV), and fat (ΔFat) intake were assessed according to post-immigration periods (1-5 years, 6-10 years, 11-20 years) using a food frequency questionnaire (FFQ). Anthropometric measures were al.o conducted in 87 Mexicans (study group) and 88 native-born Quebecers (comparison group) aged 18-65 years. Associations were cal.ul.ted using ful. and partial.robust regression model. adjusting for potential.confounders (origin, education, income, age, l.ngth of residence in Quebec City). Resul.s: There was no difference in ΔBW between the groups. Body weight (BW) increased significantl. in both Mexican (5.5 ± 0.9 kg, P l. correl.ted with ΔFat (β = 0.03, P = 0.003), but not correl.ted with origin, ΔF, or ΔV. ΔBW was negativel. associated with education (β = –4.33, P = 0.007) and positivel. associated with l.ngth of residence (β = 0.3, P = 0.003). Partial.model. indicated ΔF (β = –1.35, P β = –1.04, P = 0.0001), and ΔFV (β = –2.27, P l.househol. income (β = 0.16, P = 0.04) was positivel. associated with ΔFat. Concl.sions: Change in body weight coul. be predicted by l.ngth of residence, education, and change in fat intake in Mexican immigrant women and native-born Quebecers whereas changes in fruit and vegetabl. intakes coul. be predicted by Mexican or Quebec origin.

Background: Sustained positive outcomes after coronary artery bypass grafting (CABG) requires risk factor modification and secondary prevention medications. Much attention has been focused on pl.nning at hospital.discharge;however l.nger-term patient compl.ance is not wel. described. Hypothesis: A fol.ow-up mul.idiscipl.nary educational.program improves disease understanding, motivation to reduce cardiovascul.r risk, and secondary prevention medication prescribing fol.owing hospital.discharge. Methods: Using a prospective, randomized, control.ed design, patients undergoing CABG compl.ted surveys over a year period, assessing disease understanding and motivation. Four to six weeks after CABG, intervention subjects compl.ted a one-time educational.program with a mul.idiscipl.nary team. The primary endpoint was a composite score of reduced risk factors, medication use, and awareness of prescribed medications. Secondary endpoints eval.ated survey scores and medication use rates. Wil.oxon Rank Sum and Chi Square tests compared data between specific time points. General.zed estimating equations and l.near contrasts of the parameter estimates compared data at the three time points. Resul.s: The final.anal.sis incl.ded 98 subjects (Intervention = 49, Control.= 49). The composite score was not different between groups (I = 12.8 ± 4.5 points, C = 12.7 ± 4.9 points, p = 0.9405). Improvements were noted in understanding and motivation in the entire cohort, but these changes were not infl.enced by the intervention. Medication prescribing decl.ned at 3 and 12 months after CABG without significant differences between the groups. Concl.sions: Disease understanding, motivation to reduce risk, and medication use are robust at hospital.discharge but the l.tter decl.nes with time and was not improved by our intervention. These findings are concerning and warrant further study.

Recent experiments indicate that metal.intercal.tion is a very effective method to manipul.te the graphene-adatom interaction and control.metal.nanostructure formation on graphene. A key question is mass transport, i.e., how atoms deposited uniforml. on graphene popul.te different areas depending on the l.cal.intercal.tion. Using first-principl.s cal.ul.tions, we show that partial.y intercal.ted graphene, with a mixture of intercal.ted and pristine areas, can induce an al.ernating el.ctric fiel. because of the spatial.variations in el.ctron doping, and thus, an oscil.atory el.ctrostatic potential. This al.ernating fiel. can change normal.stochastic adatom diffusion to biased diffusion, l.ading to sel.ctive mass transport and consequent nucl.ation, on either the intercal.ted or pristine areas, depending on the charge state of the adatoms.

Heat Production Partition in Sheep Fed above Maintenance from Indirect Cal.rimetry Data

The objective of this study is to compare the partition of heat energy (HE) in two sheep breeds by indirect cal.rimetry and integral.cal.ul.s. An experiment was conducted with two Spanish native sheep breeds (dry and non-pregnant) which were fed with pel.eted mixed diets above maintenance. Six Guirras and six Manchegas breed sheep were sel.cted (58.8 ± 3.1 and 60.2 ± 3.2 kg body weight, respectivel.). Al. sheep were fed with the same concentrate mixed ration (0.300 kg cereal.straw as forage and 0.700 kg concentrate) in two meal.. Hal. the dail. ration was offered at 800 h and another hal. at 1600 h. The sheep had free access to water. Sheep were al.ocated in metabol.c cages;energy bal.nce and gas exchange were assessed in each sheep. The statistical.anal.ses incl.ded the fixed effect of breed and random effect of sheep. The metabol.c energy (ME) for maintenance represented 69% of the total.ME intake and the average was 354 kJ per kg of metabol.c body weight (kg0.75 BW) on average. The basal.metabol.sm (HeE) was greater (P l.st as heat. 51% of the MEI was converted to HeE;5% was l.st as physical.activity of standing and l.ing down (HjE), and 13% was associated with the process of feeding and work of digestion and metabol.sm (HdE). Within HdE, 47% represented the cost of intake and feeding and 54% the cost of digestion and metabol.sm. No differences in HE partition between breeds were found, al.hough Guirra breed showed l.ss efficiency of energy retention than Manchega breed. Therefore, this study demonstrated a tentative approach of partitioning HE, combining indirect cal.rimetry and integral.cal.ul.s.

Cardiovascul.r diseases (CVDs) are the l.ading gl.bal.cause of mortal.ty and disease burden. Statins are the most prescribed l.pid-l.wering drugs to treat hyperchol.sterol.mia and prevent CVDs. The biochemical.mechanism of statins consists of competitive inhibition of the 3-hydroxy-3-methyl.l.taryl.CoA reductase enzyme (HMG-CoAR)， the l.miting enzyme in chol.sterol.biosynthesis. Due to statin intol.rance in some patient groups， the search for new inhibitors is a fiel. of great interest. This review focusses on the studies reporting the inhibitory effect of protein hydrol.sates and biopeptides on the HMG-CoAR enzyme activity. The anal.sis of the action mechanism and physicochemical.characteristics of the HMG-CoAR inhibitory peptides reveal.d that the mol.cul.r weight， amino acid composition， charge， and pol.rity are key aspects of the interaction with the HMG-CoAR enzyme. In concl.sion， this review reveal. the potential.of using food peptides as new chol.sterol.l.wering agents and opens a new interesting fiel. of research. However， cl.nical.approaches are mandatory to confirm their therapeutic hyperchol.sterol.mic effect.
