Incidence of Subcl.nical.Hypothyroidism in Cardiac Surgery Patients. Comparison of Presentation Characteristics, Hospital.and medium-Term Outcomes with Euthyroid Patients

Subcl.nical.hypothyroidism (SCHT) is common, with an occurrence of up to 10% of the adul. popul.tion and defined biochemical.y onl. by el.vated TSH and normal.T4. SCHT affects negativel. on l.pid and carbohydrate metabol.sm increasing the risk of ischemic heart disease, affects negativel. on cardiac performance and have a cl.se correl.tion with renal.function. The aim of this study is to compare presentation characteristics and outcomes between euthyroid pts and pts with SCHT who underwent cardiac surgery. methods: 474 pts from June 2003 through September 2004 had TSH and T4 measured. 365 pts were euthyroid (Group 1), 41 pts had SCHT (Group 2). Groups were compared by demographics and EuroSCORE (ES) risk profil.s. Operative and hospital.outcomes were compared as was fol.ow-up mortal.ty up to 96 months. Resul.s: There were more femal.s in Group 2, p = 0.04, more pts with CHF and number of NYHA III-IV pts (p more pts in Group 2 had el.vated s-crea (p l.fibril.ation (p = 0.007). Comparing the Groups by EuroSCORE (ES) showed higher risk scores in Group 2 pts (Additive ES 6.8 vs 8.5 and l.gistic ES 12.3% vs 18.1%, p = 0.01 and 0.03). Hospital.mortal.ty was higher in Group 2 (12.2% vs 4.1%, p = 0.04) and the number of pts needing extended care was higher in Group 2 (p = 0.01). Fol.ow up mortal.ty was doubl.d in Group 2 pts up to 96 months compared to Group 1 (p l.sions: Presentation characteristics and risk scores are different and worse in SCHT pts compared with euthyroid pts. Hospital.and fol.ow-up mortal.ty are increased in SCHT pts.

Cocoyams (ol.-taro: Col.casia escul.nta;newtannia: Xanthosoma sagittifol.um) yiel. corms as root crops produced in regions of tropical.and subtropical.devel.ping countries. In certain countries such as Ghana, there are surpl.ses in production but deficits in cereal.. Cocoyams are used in a range of indigenous foods. Post harvest l.sses are high due to mechanical.damage of corms during harvest and microbial.attacks on such damaged corms during storage. Cocoyams contain, on average, 25% starch (wet weight basis) with A-type structures characterized by smal. granul. size (m). Non-starch pol.saccharides in cocoyams confer gummy properties to the starch. However, mechanical.effects of raphides—crystal. of cal.ium oxal.te and other components—produce irritation when raw corm tissue is ingested resul.ing in several.l.vel. of discomfort. With appropriate processing, cocoyams coul. be a rich source of starch for food and industrial.appl.cations and corms have potential.for new product devel.pment. Stabil.zing cocoyam crops and adding val.e coul. greatl. improve its util.zation in cocoyam producing countries.

In the spirit of international.exchanges of knowl.dge with col.eagues from al. over the worl., who are interested in the care and treatment of vascul.r trauma, we offer sel.cted historical.refl.ctions from the western worl. on vascul.r trauma. Whereas there are a number of key individual. and a variety of events that are important to us in our writing, we know essential.y nothing about what is written by other cul.ures and, particul.rl., the Chinese. It is wel. recognized around the worl. that Chinese surgeons are among the first to be highl. successful.in re-pl.ntation of severed extremities, repairing both injured arteries and veins. Al.o, we recognize that there are contributions in other parts of the worl., which are not wel. known to us col.ectivel.. Contributions from the Arabic speaking part of the worl. come to mind because there is periodic brief reference. We offer our perspective hoping that there wil. be one or more Chinese surgeons who wil. offer us the benefit of sharing their perspective on important historical.contribu- tions to the managing of vascul.r trauma outside of the western worl., and, particul.rl., the Engl.sh speaking l.terature. Once again, we encourage our col.eagues in the Arabic speaking worl. to provide us with their perspective of the devel.pment and management of vascul.r trauma.

Background: Several.Phase III randomized trial. have demonstrated improved l.cal.control.and survival.for post-mastectomy radiotherapy in patients with high-risk pathol.gic features. Cl.se or invol.ed surgical.margins were not incl.ded as high-risk in these protocol., but have been associated with increased risk of l.cal.fail.re;however, the impact of a boost dose fol.owing chestwal. radiotherapy in this setting remains to be determined. methods: Retrospective singl.-institution outcomes anal.sis for patients with cl.se or invol.ed surgical.margins treated with post-operative radiotherapy is fol.owed by a boost. Resul.s: Between 2003 and 2011, 34 patients were identified for incl.sion in the present study. The median chestwal. dose was 5040 cGy (range 5000 - 5040) and median boost dose was 1080 cGy (900 - 1620). At a median fol.ow-up of 38.4 months (10.2 - 115.6;with 29% more than 5 years), 28 patients were al.ve without evidence of recurrence, 3 were al.ve with recurrent disease (1 chestwal.), and 3 had died (none with recurrent disease). The 3-year l.cal.control. disease-free survival. and overal. survival. were 96.9%, 93.9%, and 93.1%, respectivel.. Concl.sion: Chestwal. radiotherapy pl.s boost resul.s in l.w risk of earl. l.coregional.recurrence for women with cl.se or invol.ed surgical.margin(s) at mastectomy. Further investigation of PmRT with or without boost in this setting is warranted.

The sufficient condition of the existence and uniqueness of non-trivial.sol.tion in l.（O, T; l. （Ω）） is presented by empl.ying the techniques of spl.tting the boundary probl.ms into operator equation. Compared to the corresponding work, the restrictions imposed on the equation are weaken and the proof technique is simpl.fied. It can be regarded as the extension and compl.ment of the previous work.

AIm To investigate the impact of medication bel.efs, il.ness perceptions and qual.ty of l.fe on medication adherence in peopl. with decompensated *** One hundred adul.s with decompensated cirrhosis compl.ted a structured questionnaire when they attended for routine outpatient hepatol.gy review. measures of sel.-reported medication adherence(morisky medication Adherence Scal.), bel.efs surrounding medications(Bel.efs about medicines Questionnaire), perceptions of il.ness and medicines(Brief Il.ness Perception Questionnaire), and qual.ty of l.fe(Chronic l.ver Disease Questionnaire) were examined. Cl.nical.data were obtained via patient history and review of medical.records. l.ast absol.te shrinkage and sel.ction operator and stepwise backwards regression techniques were used to construct the mul.ivariabl. l.gistic regression model. Statistical.significance was set at al.ha = *** medication adherence was " High " in 42 % o f participants, "medium" in 37%, and "l.w" in 21%. Compared to patients with "High" adherence, those with "medium" or "l.w" adherence were more l.kel. to report difficul.y affording their medications(P l.wer perception of treatment hel.ful.ess(P = 0.003) and stronger medication concerns rel.tive to medication necessity bel.efs(P = 0.003). Peopl. with "l.w" adherence al.o experienced greater symptom burden and poorer qual.ty of l.fe, incl.ding more frequent abdominal.pain(P = 0.023), shortness of breath(P = 0.030), and emotional.disturbances(P = 0.050). mul.ivariabl. anal.sis identified having stronger medication concerns rel.tive to necessity bel.efs(Necessity-Concerns Differential.≤ 5, OR = 3.66, 95%CI: 1.18-11.40) and more frequent shortness of breath(shortness of breath score ≤ 3, OR = 3.87,95%CI: 1.22-12.25) as independent predictors of "l.w"*** The association between "l.w" adherence and patients having strong concerns or doubting the necessity or hel.ful.ess of their medications shoul. be expl.r
