Choroidal vascularity index in thyroidassociated ophthalmopathy:a crosssectional study

Solid pseudopapillary neoplasms (SPNs) are rare solid pancreatic tumors mainly affecting young women. Role of osteoprotegerin/receptor activator of nuclear factor kappa B/receptor activator of nuclear factor kappa B ligand axis in nonalcoholic fatty liver disease

Concomitantly with the increase in the prevalences of overweight/obesity, nonalcoholic fatty liver disease(NAFLD) has worldwide become the main cause of chronic liver disease in both adults and children. Patients with fatty liver display features of metabolic syndrome(Met S), like insulin resistance(IR), glucose intolerance, hypertension and dyslipidemia. Recently, epidemiological studies have linked obesity, Met S, and NAFLD to decreased bone mineral density and osteoporosis, highlighting an intricate interplay among bone, adipose tissue, and liver. Osteoprotegerin(OPG), an important symbol of the receptor activator of nuclear factor-B ligand/receptor activator of nuclear factor kappa B/OPG system activation, typically considered for its role in bone metabolism, may also play critical roles in the initiation and perpetuation of obesityrelated comorbidities. Clinical data have indicated that OPG concentrations are associated with hypertension, left ventricular hypertrophy, vascular calcification, endothelial dysfunction, and severity of liver damage in chronic hepatitis C. Nonetheless, the relationship between circulating OPG and IR as a key feature of Met S as well as between OPG and NAFLD remains uncertain. Thus, the aims of the present review are to provide the existent knowledge on these associations and to discuss briefly the underlying mechanisms linking OPG and NAFLD.

Photorefractive keratectomy after cataract surgery in uncommon cases: long-term results

AIM： To evaluate the efficacy and safety of the excimer laser correction of the residual refractive errors after cataract extraction with intraocular lens（IOL） implantation in uncommon ***： Totally 24 patients with high residual refractive error after cataract surgery with IOL implantation were examined. Twenty-two patients had a history of phacoemulsification and IOL implantation, and two had extra-capsular cataract extraction with IOL implantation. Detailed examination of preoperative medical records was done to explain the origin of the post-cataract refractive errors. All patients underwent photorefractire keratectomy（PRK） enhancement. The mean outcome measures were refraction, uncorretted visual acuity（UCVA）, best corrected visual acuity（BCVA） and corneal transparency and follow up ranged from 1 to 8 ***： The principal causes of residual ametropia was inexact IOL calculation in abnormal eyes with high myopia and congenital lens abnormalities, followed by corneal astigmatism both suture induced and preexisting. After cataract surgery and before the laser enhancement the mean spherical equivalent（SE） was-0.56±3 D ranging from-4.62 to ＋2.25 D in high myopic patients, instead it was-1±1.73 D ranging from-3.25 to ＋3.75 D in the astigmatic eyes, with a mean cylinder of-3.75±0 ranging from-3 to ＋5.50 D. After laser refractive surgery the mean SE was 0.1±0.73, ranging from-0.50 to ＋1.50 in the myopic group, and it was-0.50±0.57 ranging from-1.25 to ＋0.50 in astigmatic patients, with a mean cylinder of-0.25±0.75. In myopic patients the mean UCVA and BCVA were 0.038±0.072 logMAR and 0.018±0.04 respectively, both ranging from 0.10 to 0.0. In astigmatic patients, the mean UCVA and BCVA were 0.213±0.132 and 0.00±0.0 respectively, UCVA ranging from 0.50 to 0.22 and BCVA was 0.00. All patients presented normal corneal transparency. Advances in medical decision support systems for diagnosis of acute graft-versus-host disease: molecular and computational intelligence joint approaches

Acute graft-versus-host disease (aGVHD) is a serious systemic complication of allogeneic hematopoietic stemcell transplantation (HSCT) causing considerable morbidity and mortality. Acute GVHD occurs when alloreactivedonor-derived T cells recognize host-recipient antigens as foreign. These trigger a complex multiphase process thatultimately results in apoptotic injury in target organs. The early events leading to GVHD seem to occur very soon,presumably within hours from the graft infusion. Therefore, when the first signs of aGVHD clinically manifest, thedisease has been ongoing for several days at the cellular level, and the inflammatory cytokine cascade is fully ***, it comes as no surprise that progress in treatment based on clinical diagnosis of aGVHD has been limited in the past30 years. It is likely that a pre-emptive strategy using systemic high-dose corticosteroids as early as possible couldimprove the outcome of aGVHD. Due to the deleterious effects of such treatment particularly in terms of infection riskposed by systemic steroid administration in a population that is already immune-suppressed, it is critical to identifybiomarker signatures for approaching this very complex task. Some research groups have begun addressing this issuethrough molecular and proteomic analyses, combining these approaches with computational intelligence techniques,with the specific aim of facilitating the identification of diagnostic biomarkers in aGVHD. In this review, we focus on theaGVHD scenario and on the more recent *** also attempt to give an overview of the classical and noveltechniques proposed as medical decision support system for the diagnosis of GVHD.

Attempt to calculate the prevalence and features of chronic hepatitis C infection in Tuscany using administrative data

AIM To evaluate this prevalence in Tuscan populations that was known and unknown to the Tuscan Regional Health Service in *** Tuscan Health administrative data were used to evaluate hepatitis C virus(HCV) infected people known to the Regional Health Service. Residents in Tuscany with a HCV exemption code(070.54) were identified. Using the universal code attributed to each resident, these patients were matched with hospital admission codes identified by the International Classification of Diseases, Ninth Revision(ICD-9), Clinical Modification, and with codes for dispensing drugs to patients by local and hospital pharmacies. Individuals were considered only once. Capture-recapture analysis was used to evaluate the HCV-infected population unknown to the Regional Health *** In total, 14526 individuals were living on 31/12/2015 with an exemption code for HCV. In total, 9524 patients were treated with pegylated interferon + ribavirin and/or direct-acting antiviral drugs during the last 10 years, and 13879 total hospital admissions were noted in the last 15 years. After data linkage, the total number was 25918. After applying the Capture-Recapture analysis, the number of unknown HCV-infected people was 23497. Therefore, the total number of chronic HCVinfected people was 38643, excluding those achieved sustained virological response to previous *** Our results show a prevalence of HCV infected people of 1%. Tuscan administrative data could be useful for calculating health care costs and health planning in the coming years.

Thrombocytopenia in chronic liver disease:Physiopathology and new therapeutic strategies before invasive procedures

Chronic liver disease is characterized by several hematological derangements resulting in a complex and barely rebalanced haemostatic *** is the most common abnormality observed in these patients and recent advances have led to researchers focus the attention on the multifactorial origin of thrombocytopenia and on the key role of thrombopoietin(TPO)in its *** thrombocytopenia(platelet count<50000/μL)complicates the management of patients with chronic liver disease by increasing the potential risk of bleeding for invasive procedures,which may be therefore delayed or canceled even if *** the very last years,the development of new drugs which exceed the limits of the current standard of care(platelet transfusions,either immediately before or during the procedure)paves the way to a new scenario in the management of this population of *** agents,such as the TPOreceptor agonists avatrombopag and lusutrombopag,have been developed in order to increase platelet production as an alternative to platelet *** agents have demonstrated a good profile in terms of efficacy and safety and will hopefully allow reducing limitations and risks associated with platelet transfusion,without any delay in scheduled ***,it is expected that patients with chronic liver disease will be able to face invasive procedures with one more string in their bow.

Cereal-forage crop rotations and irrigation treatment effect on water use efficiency and crops sustainability in Mediterranean environment

Agricultural systems based on crop rotations favour sustainability of cultivation and productivity of the crops. Wheat-forage crops rotations (annual winter binary mixture and perennial alfalfa meadow) combined with irrigation are the agronomical techniques able to better exploit the weather resources in Mediterranean environments. The experiment aimed to study the effect of 18 years of combined effect of irrigation and continuous durum wheat and wheat-forage rotations on productivities of crops and organic matter of topsoil. The experiments were established through 1991-2008 under rainfed and irrigated treatments and emphasized on the effect of irrigation and continuous wheat and wheat-forage crop rotations on water use efficiency and sustainability of organic matter. The effect of irrigation increased 49.1% and 66.9% the dry matter of mixture and meadow, respectively. Continuous wheat rotation reduced seed yield, stability of production, crude protein characteristics of kernel and soil organic matter. The yearly gain in wheat after forage crops was 0.04 t (ha·yr)-1 under rainfed and 0.07 t (ha·yr)-1 under irrigation treatments. The crude protein and soil organic matter of wheat rotations, compared to those of continuous wheat under rainfed and irrigated was increase in term of point percentage by 0.8 and 0.5 in crude protein and 5.1 and 4.4 in organic matter, respectively. The rotations of mixture and meadow under both irrigated treatments increased the point of percentage of organic matter over continuous wheat (*** 8.5 in mixture and 12.5 and 9.5 meadow under rainfed and irrigation, respectively). Irrigation reduce the impact of weather on crop growing reducing water use efficiency (mean over rotations) for dry mater production (15.5 in meadow and 17.5 in mixture [L water (kg·dry·matter)-1 ]) and wheat seed yield. The effect of agronomic advantages achieved by forage crops in topsoil expire its effect after three years of continuous wheat rotation.
