Affect and prefrontal hemodynamics during exercise under immersive audiovisual stimulation:Improving the experience of exercise for overweight adults

Objective:Research on methods of improving the affective experience of exercise remains limited,especially for low-active overweight *** investigated the effectiveness of a virtual-reality headset and headphones in improving affective responses over conventionally delivered audiovisual ***:Low-active,overweight adults(16 women,5 men;age:34.67±9.62 years;body mass index:28.56±4.95 kg/m^2;peak oxygen uptake for men:29.14±6.56 mL/kg/min,for women:22.67±4.52 mL/kg/min,mean±SD)completed 15-min sessions of recumbent cycling at the ventilatory threshold:(a)high immersion(HI,virtual reality headset and headphones),(b)low immersion(LI,television screen and speakers),and(c)***-exercise pleasure and post-exercise enj oyment were *** of the right dorsolateral pre frontal cortex(dIPFC)was assessed with near infrared ***:Higher pleasure was reported during HI than during LI and Control(Condition×Time interaction;p<0.001,ηp^2=0.43).Participants who reported a preference for low exercise intensity showed higher dIPFC oxygenation during Control,but this difference diminished during LI and HI(Condition×Time×Preference interaction;p=0.036,ηp^2=0.10).Conclusion:Compared with conventionally delivered audiovisual stimulation,using a virtual-reality headset strengthens the dissociative effect,further improving affective responses to exercise at the ventilatory threshold among overweight,low-active *** by competing with interoceptive afferents at the level of sensory input,audiovisual stimulation may lessen reliance on cognitive efforts to attenuate declining affect,as indicated by lower right dIPFC activity,particularly among participants disinclined toward high exercise intensity.

Concomitant gastric adenocarcinoma and stromal tumor in a woman with polymyalgia rheumatica

Gastrointestinal stromal tumors （GISTs） are rare neoplasms （1%） of the gastrointestinal tract and to our knowledge only rare cases of synchronous presentation of gastric carcinomas and GISTs are reported in the literature. A 72-year-old female with a simultaneous presentation of gastric adenocarcinoma and GIST is presented. Moreover, due to polymyalgia rheumatica the patient received corticosteroids as treatment for the last 3 years. The concomitant occurrence of these neoplasms may involve common carcinogenic factors and there could be an association with polymyalgia rheumatica either as a paraneoplastic presentation or due to its treatment with corticosteroids.

Intrahepatic cholangiocarcinoma(iCCA)is a subgroup of cholangiocarcinoma that accounts for about 10%-20%of the total *** with hepatitis B virus(HBV)is one of the most important predisposing factors leading to the formation of *** has been recently estimated based on abundant epidemiological data that the association between HBV infection and iCCA is strong with an odds ratio of about *** HBV-associated mechanisms that lead to iCCA are under intense *** diagnosis of iCCA in the context of chronic liver disease is challenging and often requires histological confirmation to distinguish from hepatocellular *** is currently unclear whether antiviral treatment for HBV can decrease the incidence of *** terms of management,surgical resection remains the mainstay of *** is a need for effective treatment modalities beyond resection in both first-and second-line *** this review,we summarize the epidemiological evidence that links the two entities,discuss the pathogenesis of HBV-associated iCCA,and present the available data on the diagnosis and management of this cancer.

Leptomeningeal metastases originated from esophagogastric junction/gastric cancer:A brief report of two cases

Leptomeningeal carcinomatosis is a very rare manifestation in patients diagnosed with esophagogastric junction and gastric cancer. Its prognosis is ominous and therapy outcomes are disappointing. Herein, we present two patients; one initially diagnosed with gastric cancer and leptomeningeal carcinomatosis but no other evidence of metastatic disease and the other one initially diagnosed with esophagogastric junction cancer, who recurred solitary with leptomeningeal seedings several years after the initial diagnosis and treatment. Furthermore, a thorough and short review of the literature is carried out.

Chromogranin A as a valid marker in oncology: Clinical application or false hopes?

Chromogranin A, due to its primary expression throughout the neuroendocrine system, is a widely accepted biomarker for the assessment of neuro-endocrine tumors. It has been traditionally used in the management of patients with tumors of gastro-enteropancreatic origin. Lately, it has also been implicated in various conditions and diseases, both benign and malignant. However, the paucity of data of adequate strength, as well as its relation with common physiologic conditions and its interaction with commonly prescribed medications, limit its clinical use in only a narrow spectrum. Herein, we present a thorough review to the most frequent conditions where its levels are affected, focusing specifically on its potential use as a prognostic and predictive biomarker in oncology.
