Surgical treatment of metastatic bone disease of the distal extremities

Metastatic bone disease of the distal extremities,also known as acrometastasis,is very ***,there is very limited information regarding the clinical manifestations and methods of surgical *** current available literature shows that acrometastases are often encountered in the context of advanced disease and are thus associated with poor patient *** metastatic bone disease is generally uncurable,the goal of surgical treatment is to provide the patient with good function with as few complications as *** this article,we discuss the clinical manifestation of acrometastases,the methods of surgical intervention,and the expected clinical ***-surgically managed pathological fractures generally remain ununited;therefore,conservative treatment is reserved for patients with poor general condition or dismal *** current evidence suggests that in lesions of the lower arm and leg,osteosynthesis(plate and screw fixation or intramedullary nail)is the most common method of reconstruction,whereas local excision or amputation are more commonly used in cases of more distal lesions(such as ankle,foot and hand).Following surgery most patients receive adjuvant radiotherapy,even though its role is poorly *** collaboration between orthopedic surgeons and medical oncologists is necessary to improve patient care and treatment *** studies are needed in order to provide stronger clinical evidence and improve decision-making,in an effort to optimize the patients’quality of life and avoid the need for revision surgery.

Safety of anti-tumor necrosis factor therapy during pregnancy in patients with inflammatory bowel disease

Treatment of inflammatory bowel disease has significantly improved since the introduction of biological agents, such as infliximab, adalimumab, certolizumab pegol, and golimumab. The Food and Drug Administration has classified these factors in category B, which means that they do not demonstrate a fetal risk. However, during pregnancy fetuses are exposed to high anti-tumor necrosis factor(TNF) levels that are measurable in their plasma after birth. Since antibodies can transfer through the placenta at the end of the second and during the third trimesters, it is important to know the safety profile of these drugs, particularly for the fetus, and whether maintaining relapse of the disease compensates for the potential risks of fetal exposure. The limited data available for the anti-TNF drugs to date have not demonstrated any significant adverse outcomes in the pregnant women who continued their therapy from conception to the first trimester of gestation. However, data suggest that antiTNFs should be discontinued during the third trimester, as they may affect the immunological system of the newborn baby. Each decision should be individualized, based on the distinct characteristics of the patient and her disease. Considering all the above, there is a need for more clinical studies regarding the effect of antiTNF therapeutic agents on pregnancy outcomes.

The Reliability of Fibro-test in Staging Orthotopic Liver Transplant Recipients with Recurrent Hepatitis C

Background and Aims:Liver biopsy remains the gold stand-ard for staging of chronic liver disease following orthotopic liver *** assessment of fibrosis with Fibro-test(FT)is well-studied in immunocompetent populations with chronic hepatitis C virus *** aim of this study is to investigate the diagnostic value of FT in the assessment of hepatic fibrosis in the allografts of liver transplant recipients with evidence of recurrent hepatitis ***:We retro-spectively compared liver biopsies and FT performed within a median of 1 month of each other in orthotopic liver transplan-tation recipients with recurrent hepatitis ***:The study population comprised 22 patients,most of them male(19/22),and with median age of 62 *** all patients,there was at least a one-stage difference in fibrosis as assessed by liver biopsy compared to FT,while for the majority(16/22)there was at least a two-stage *** absence of correlation between the two modalities was statistically demonstrated(Mann-Whitney U test,p=0.01).In detecting significant fib-rosis(a METAVIR stage of F2 and above),an FT cut-off of 0.5 showed moderate sensitivity(77%)and negative predictive value(80%),but suboptimal specificity(61%)and positive predictive value(58%).Conclusions:In post-transplant pa-tients with recurrent hepatitis C,FT appears to be inaccurately assessing the degree of allograft fibrosis,therefore limiting its reliability as a staging tool.

Technical note for intraoperative determination of proper acetabular cup size in primary total hip arthroplasty

Evolution of endoscopic extraperitoneal radical prostatectomy （EERPE）： technique and outcome

Endoscopic extraperitoneal radical prostatectomy （EERPE） is a well-established and standardized technique for treating patients with localized prostate cancer. Nevertheless, the procedure is continuously being refined with the expansion of anatomical knowledge. The development of a nerve-sparing approach and improvements in currently used equipment are expected to yield better results in cosmesis and convalescence without sacrificing the procedure＇s established benefits in terms of potency, continence and oncological management. In this study, the technique and its evolution are presented in detail, along with an analysis of its clinical efficacy. We also consult the literature to compare EERPE to transperitoneal laparoscopic radical prostatectomy, and we also discuss new technical advancements regarding the use of robotic assistance during EERPE.

Dear Editor,We present the first reported case of nasal optic disc pit resulting in serous detachment of the macula. Optic disc pit is a rare congenital anomaly occurring in one out of 11 000 people. The appearance is a localized round or oval depression in that is grey, yellow or black in colour depending on the amount of glial tissue present.

Psychogenic non‑epileptic seizures (PNES)in the context of concurrent epilepsy-making the right diagnosis

Advanced cement based nanocomposites reinforced with MWCNTs and CNFs

Cementitious materials reinforced with well dispersed multiwall carbon nanotubes （MWCNTs） and carbon nanofibers （CNFs） at the nanoscale were fabricated and tested. The MWCNTs and CNFs were dispersed by the application of ultrasonic energy and the use of a superplasticizer. Mechanical and fracture properties including flexural strength, Young＇s modulus, flexural and fracture toughness were measured and compared with similarly processed reference cement based mixes without the nano-reinforcement. The MWCNTs and CNFs reinforced mortars exhibited superior properties demonstrated by a significant improvement in flexural strength （106%）, Young＇s modulus （95%）, flexural toughness （105%）, effective crack length （30%） and fracture toughness （120%）.
