A methodology for calibration of traffic micro-simulator for urban heterogeneous traffic operations

Traffic micro-simulation is a widely accepted tool in many countries for the evaluation and assessment of alternative design ***,for several developing countries,replicating heterogeneous,non-lane based traffic in a micro-simulation framework is gaining increased importance and still remains a challenge due to its *** present study demonstrates a methodology to calibrate a traffic micro-simulation model giving due consideration to vehicle-class specific driver behavior in an urban Indian scenario for a midblock section and an intersection approach in *** sensitive parameters affecting the driver behavior were identified for every vehicle type using Latin Hyper cube design,taking vehicle class specific travel time as a performance *** regression models were developed for each vehicle class considering the sensitive driving behavior *** models highlight that the dependency of measure of effectiveness(MOE)of one vehicle type is not only limited to its own driver behavior but also on parameters of other vehicle classes.A genetic algorithm based optimization was adopted to obtain optimal parameter sets for different vehicle *** optimum values were found to vary significantly across all vehicle classes at a 95%confidence *** and multi-criteria calibration principles are also implemented to yield much more realistic results and subsequently minimizing weighted error for all vehicle classes.

Biloma: An unusual complication in a patient with pancreatic cancer

The term biloma describes an encapsulated collection of bile within the abdomen, usually secondary to bile duct disruption. The commonest causes reported in the literature are iatrogenic （secondary to hepatobUiary surgery）, trauma or complications due to choledocho- lithiasis. A few cases have been reported as complica- tions of cholangiocarcinoma or acute cholecystitis. We report the case of a 64-year-old man initially diagnosed with a non-obstructive malignancy of the pancreas, who developed a spontaneous intrahepatic biloma 8 mo later. This was identified following a 1-wk history of fever, rigors and icterus. The biloma was identified on computed tomography and subsequently drained under ultrasound guidance. Forty-eight hours later, a stent was inserted endoscopically into his common bile duct and he made an uneventful in-hospital recovery. We believe this is the first documented case of spontaneous intrahepatic biloma to occur secondary to pancreatic malignancy.

Intraductal papillary mucinous neoplasm(IPMN)is an increasingly reported *** pancreatic calcification is generally thought to be a sign of chronic pancreatitis,but it may occur simultaneously with IPMN leading to diagnostic *** report a case of a patient initially diagnosed with chronic calcifying pancreatitis who was later shown to have a malignant *** case illustrates potential pitfalls in the diagnosis of IPMN in the case of extensive pancreatic calcification as well as clues that may lead the clinician to suspecting the *** possible mechanisms of the relation between pancreatic calcification and IPMN are also reviewed.

Indigenous plant medicines for health care:treatment of Diabetes mellitus and hyperlipidemia

Medicinal plants have played an important role in treating and preventing a variety of diseases throughout the world. Metabolic syndrome had become a global epidemic, defined as a cluster of three of five criteria： insulin resistance and glucose intolerance, abdominal obesity, hypertension, low high-density cholesterol, and hypertriglyeeridemia. The current review fo- cuses on Indian medicinal plant drugs and plants used in the treatment of diabetes and hyperlipidemia. Though there are various approaches to reduce the ill-effects of diabetes and hyperlipidemia and its secondary complications, plant-based drugs are preferred due to lesser side effects and low cost. The current review focuses on twenty-three medicinal plants used in the treatment of Diabe- tes mellitus and nine medicinal plants used in the treatment of hyperlipidemia. The wealth of knowledge on medicinal plants points to a great potential for research and the discovery of new drugs to fight diseases, including diabetes and hyperlipidemia.

Significant scarcity of a donor pool exists for heart transplantation(HT)as the prevalence of patients with end-stage refractory heart failure is increasing *** the discovery of effective direct-acting antiviral and favorable short-term outcomes following HT,the hearts from hepatitis C virus(HCV)patient are being utilized to increase the donor ***-term outcomes with regards to graft function,coronary artery vasculopathy,and kidney and liver disease is comparable in HCV-negative recipients undergoing HT from HCVpositive donors compared to HCV-negative donors.A significant high incidence of donor-derived HCV transmission was observed with great success of achieving sustained viral response with the use of direct-acting *** accepting HCV-positive organs,the donor pool has expanded with younger donors,a shorter waitlist time,and a reduction in waitlist ***,the longterm outcomes and impact of specific HCV genotypes remains to be *** reviewed the current literature on HT from HCV-positive donors.

Aim:The aim of this study is to compare disease-free survival(DFS)and overall survival(OS)in patients with stage I cervical cancer(≤4cms,lymph node-negative)undergoing open radical hysterectomy(ORH)*** invasive radical hysterectomy(MIRH).Methods:All patients undergoing radical hysterectomy between January 2012-December 2018 from the largest tertiary referral cancer centre were included.A 1:1 propensity matching was done based on four independent prognostic factors to compare DFS and OS with the route of ***:One hundred and ninety-nine patients were included during the study *** median age of the cohort was 50 *** median follow-up of patients was 47 *** 1:1 propensity matching,a total of 174 patients were analysed for DFS and OS in ORH(n=87)and MIRH(n=87)*** measure was used in two-thirds of the patients during ***-nine patients(16.7%)had *** the matched cohort(n=174),the DFS at 36 and 60 months was 84.8%(78.1%-89.6%)and 81%(73.4%-86.6%)respectively and the OS was 96.5%(91.7%-98.5%)and 95.6%(90.3%-98%)*** was no statistically significant difference in DFS or OS between ORH and ***:The present study showed no difference in oncological outcomes in MIRH compared to *** audits on patient characteristics such as screening/vaccination history along with surgical technique/load and matching for crucial risk factors should be factored in future studies to eliminate the possible methodological errors.

Dentinogenic ghost cell tumor: A case report

BACKGROUND Dentinogenic ghost cell tumor(DGCT) is an uncommon locally invasive odontogenic neoplasm. It is considered to be a solid variant of calcifying odontogenic cyst(COC). This tumor makes up for only 2%-14% of all COCs and less than 0.5% of all odontogenic tumors which owes to its rarity. The purpose of this paper was to describe a case of DGCT and the treatment adopted in our case,and to provide a review of this case in the indexed *** SUMMARY In this article, we discussed a case of 18 year old male who reported with a chief complaint of a recurrent swelling and dull aching pain in upper left back region of the jaw. Computed tomography scan was carried out which revealed hypodense lesion with a few hyperdense flecks within it suggesting the presence of calcification. On incisional biopsy, diagnosis of COC was given. After segmental resection of the lesion, histopathogically odontogenic epithelium was noted along with calcifications, ghost cells and dentinoid material. Special staining was done with van Gieson and it showed pink areas of dentinoid material and yellow colour represented ghost cells. Hence, amalgamation of careful clinical examination, use of advanced radiographic imaging and detailed histopathological examination confirmed the diagnosis of DGCT. The patient was followed up for one year and there was no recurrence of the lesion or signs of any residual *** Radical treatment should be carried out along with mandatory long-term follow up in order to avoid recurrence in aggressive lesions.

Sustained,high level transgene expression in mammalian cells is desired in many cases for studying gene ***,stable transgene expression has been accomplished by using retroviral or lentiviral ***,such viral vector-mediated transgene expression is often at low levels and can be reduced over time due to low copy numbers and/or chromatin remodeling *** piggyBac transposon has emerged as a promising nonviral vector system for efficient gene transfer into mammalian *** its inherent advantages over lentiviral and retroviral systems,piggyBac system has not been widely used,at least in part due to their limited manipulation ***,we seek to optimize piggyBac-mediated transgene expression and generate a more efficient,user-friendly piggyBac *** engineering a panel of versatile piggyBac vectors and constructing recombinant adenoviruses expressing piggyBac transposase(PBase),we demonstrate that adenovirusmediated PBase expression significantly enhances the integration efficiency and expression level of transgenes in mesenchymal stem cells and osteosarcoma cells,compared to that obtained from co-transfection of the CMV-PBase *** further determine the drug selection timeline to achieve optimal stable transgene ***,we demonstrate that the transgene copy number of piggyBac-mediated integration is approximately 10 times higher than that mediated by retroviral *** the engineered tandem expression vector,we show that three transgenes can be simultaneously expressed in a single vector with high ***,these results strongly suggest that the optimized piggyBac system is a valuable tool for making stable cell lines with sustained,high transgene expression.
