Descemet stripping automated endothelial keratoplasty in phakic eyes:incision modification reducing cataract formation

AIM： To evaluate the efficacy of a technical modification to reduce the incidence of traumatic cataract induced by Descemet stripping automated endothelial keratoplasty（DSAeK） performed in phakic eyes. MeTHODS： A retrospective cohort study. The records of all patients with a clear crystalline lens and endothelial failure that underwent modified DSAeK at our insitution were reviewed. In this modification, in order to avoid inadvertent touch of the insertion forceps against the exposed crystalline lens while passing across the anterior chamber, the incision sites were shifted from the standard 9 and 3 o＇clock positions, superiorly to the 10 and 2 o＇clock position respectively. Formation of typically traumatic, anterior subcapsular cataract in these patients was compared to that observed in a cohort including all the patients with a clear crystalline lens and endothelial failure that underwent conventional DSAeK at our ***： The study group included 49 eyes following modified DSAeK and the control group included 35 eyes following DSAeK with conventional incision sites. Anterior subcapsular cataract occurring 4 mo or less postoperatively was identified in 2 of 49（4%） eyes in the study group and 7 of 35（20%） eyes in the control group. The rates of traumatic cataract were significantly higher in the control group in comparison to the study group（P=0.03, RR=4.9, 95%CI 1.08-22.1）.CONCLUSION： Traumatic cataract formation following phakic DSAeK may be avoided with a simple modification to the position of the incision sites.

Steep-slope agricultural landscapes are under threat due to climate *** the one hand,the growing fre-quency of extreme high-intensity rainfall events concentrated in both temporal and spatial scales are causing flash floods or slope failure risk *** the other hand,future climate projections indicate a significant expansion of arid zones in the steep slope agricultural *** is evidence that these landscapes face a high risk of growing water *** their unique role in crop production,ecosystem diversity,and crop production,ecosystem diversity,and cultural heritage,understanding sustainable water resource manage-ment for mitigating climate change-induced drought has never been more urgent than *** these landscapes,unique indigenous knowledge of water conservation is adopted to manage water resources improving their re-silience *** is,therefore,necessary to promote water storage to mitigate floods or increase the resilience to prolonged drought(creating at the same time favourable conditions for biodiversity).Modern technological advances(e.g.,high-resolution remote sensing and GIS-based modelling)are crucial in supporting these activities and understanding earth’s surface processes.

Northwestward range expansion of the bumblebee Bombus haematurus into Central europe is associated with warmer winters and niche conservatism

Species range expansions are crucial for understanding niche formation and the interaction with the ***,we studied the bumblebee Bombus haematurus Kriechbaumer,1870,a species historically distributed from northern Serbia through northern Iran which has very recently started expanding northwestward into Central europe without human-mediated dispersal(i.e.,it is a natural spread).After updating the global distribution of this species,we investigated if niche shifts took place during this range expansion between newly colonized and historical *** addition,we have explored which climatic factors may have favored the natural range expansion of the *** results indicated that Bombus haematurus has colonized large territories in 7 european countries outside the historical area in the period from the 1980s to 2018,a natural expansion over an area that equals 20%of the historical *** addition,this bumblebee performs generalism in flower visitation and it occurs in different habitats,although a preference for forested areas clearly *** land-use associated with the species in the colonized areas is similar to the historical distribution,indicating that no major niche shifts occurred during the ***,in recently colonized localities,the range expansion was associated with warming temperatures during the winter and also during both queen overwintering and emergence *** findings document a case of natural range expansion due to environmental change rather than due to niche shifts,and specifically they suggest that warmer winters could be linked to the process of natural colonization of new areas.

Morphological integration and functional modularity in the crocodilian skull

We explored the morphological organization of the skull within Crocodylidae,analyzing functional and phylogenetic interactions between its 2 constituent functional modules:the rostrum and the *** used geometric morphometrics to identify localized shape changes,focusing on the differences between the major clades of the crown-group Crocodylia:Alligatoridae and *** used published bite performance data to correlate rostral function with postrostral *** skull modules appear more integrated within Alligatoridae than within *** effects on shape variation are more evident in Alligatoridae than in Crocodylidae,where functional parameters concerning the rostral morphology are proportionally more important than ***-snouted species are characterized by low structural performance,which is significantly associated with a reduction of the pterygoid-quadrate cranial nipper,suggesting that the nipper is important for the ingestion of large food items in generalist *** functional association is coupled with a significant evolutionary allometry at the clade level,while Alligatoridae and Crocodylidae show different degrees of evolutionary allometry for their entire shape and *** postrostrum is more conservative than the rostrum in terms of morphospace occupation,evolutionary allometry and phylogenetic signal.

Results of analytical surveys on pesticide residue levels in foods for the years 1986-1987 are presented. Analytical data obtained from regional laboratories are tabulated in terms of various commodity groups (vegetables, fruit, cereals, processed or derived foodstuffs, other commodities, legumes, environmental samples, others) and are compared with maximum legal tolerances established by the ministerial ordinance (Italian Ministry of Health) 6.6. 1985.On a total number of 4715 determinations of classes of compounds or of single compounds made on 1601 commodities samples, only N 53 (1.1 %) violative results appeared. No residue was found at levels exceeding regulatory limits or found in commodities where a tolerance had not been established for a specific pesticide/commodity combination.

Low-dose tenofovir is more potent than adefovir and is effective in controlling HBV viremia in chronic HBeAg-negative hepatitis B

AIM: To study the efficacy of tenofovir disoproxil fumarate (TDF) at low dose in a small open trial of chronic hepatitis B patients with advanced stage disease. MeTHODS: eleven patients were treated with TDF 75 mg for a median period of 80 (range, 24-576) wk and then 7 cases were shifted to an adefovir 10 mg treatment group. All patients had been pre-treated with lamivudine: 5 had YMDD resistant mutants and 6 wild- type virus. When TDF was started, 4 patients had low- level viremia and 6 were PCR-negative. ReSULTS: During TDF treatment, PCR remained negative in 10 patients, transaminase levels were normal and no significant viral breakthrough was observed. The drug was well tolerated in all cases. When TDF 75 mg was substituted with adefovir 10 mg, 3 out of 7 patients had a persistent viral rebound (2700-130 000 copies/mL), in whom lamivudine had to be reintroduced. CONCLUSION: Low-dose TDF monotherapy can control HBV viremia for an extended period of time without the emergence of resistance and is more potent than adefovir at the standard dosage. The use of a reduced dose of TDF could diminish the cost of therapy in low-income countries, but further studies in a larger population and in HBeAg-positive subjects are needed.

Radiofrequency ablation of liver tumors: Actual limitations and potential solutions in the future

Over the past decade,radiofrequency ablation(RFA) has evolved into an important therapeutical tool for the treatment of non resectable primary and secondary liver *** clinical benefit of RFA is represented in several clinical *** underline the safety and feasibility of this new and modern concept in treating liver *** has proven its clinical impact not only in hepatocellular carcinoma(HCC) but also in metastatic disease such as colorectal cancer(CRC).Due to the increasing number of HCC and CRC,RFA might play an even more important role in the ***,the refinement of RFA technology is as important as the evaluation of data of prospective randomized trials that will help define guidelines for good clinical practice in RFA application in the *** combination of hepatic resection and RFA extends the feasibility of open surgical procedures in patients with extensive *** effects of RFA such as biliary tract damage,liver failure and local recurrence remain an important task today but overall the long term results of RFA application in treating liver tumors are *** ablation of liver tumors due to insufficient technology of ablation needles,tissue cooling by the neighbouring blood vessels,large tumor masses and ablation of tumors in close vicinity to heat sensitive organs remain difficult tasks for *** solutions to overcome these limitations of RFA will include refinement of ultrasonographic guidance(accuracy of probe placement),improvements in needle technology(*** preventing charring)and intraductal cooling techniques.

Prioritization methodology for roadside and guardrail improvement:Quantitative calculation of safety level and optimization of resources allocation

The attention to road safety-related issues has grown fast in recent decades. The experi- ence gained with these themes reveals the importance of considering these aspects in the resource allocation process for roadside and guardrail improvement, which is a complex process often involves conflicting objectives. This work consists on defining an innovative methodology, with the objective of calculating and analysing a numerical risk factor of a road. The method considers geometry, accident rate, traffic of the examined road and four categories of elements/defects where the resources can be allocated to improve the road safety （safety barriers, discrete obstacles, continuous obstacles, and water drainage）. The analysis allows the assessment of the hazard index, which could be used in decision- making processes. A case study is presented to analyse roadsides of a 995 km long road network, using the cost-benefit analysis, and to prioritize possible rehabilitation work. The results highlighted that it is suitable to intervene on roads belonging to higher classes of risk, where it is possible to maximize the benefit in terms of safety as consequence of rehabilitation works （i.e., new barrier installation, removal and new barrier installation, and new terminal installation）. The proposed method is quantitative; therefore, it avoids providing weak and far from reliable results; moreover, it guarantees a broad vision for the problem, giving a useful tool for road management body.
