Hilar cholangiocarcinomas are common tumors of the bile duct that are often unresectable at presentation. palliation, therefore, remains the goal in the majority of these patients. palliative treatment is particularly indicated in the presence of cholangitis and pruritus but is often also offered for high-grade jaundice and abdominal pain. Endoscopic drainage by placing stents at endoscopic retrograde cholangio-pancreatography(ERCp) is usually the preferred modality of palliation. However, for advanced disease, percutaneous stenting has been shown to be superior to endoscopic stenting. Endosonography-guided biliary drainage is emerging as an alternative technique, particularly when ERCp is not possible or fails. Metal stents are usually preferred over plastic stents, both for ERCp and for percutaneous bili-ary drainage. There is no consensus as to whether it is necessary to place multiple stents within advanced hi-lar blocks or whether unilateral stenting would suffice. However, recent data have suggested that, contrary to previous belief, it is useful to drain more than 50% of the liver volume for favorable long-term results. In the presence of cholangitis, it is beneficial to drain all of the obstructed biliary segments. Surgical bypass plays a limited role in palliation and is offered primarily as asegment Ⅲ bypass if, during a laparotomy for resec-tion, the tumor is found to be unresectable. photody-namic therapy and, more recently, radiofrequency abla-tion have been used as adjuvant therapies to improve the results of biliary stenting. The exact technique to be used for palliation is guided by the extent of the bili-ary involvement(Bismuth class) and the availability of local expertise.

Optimal integration of battery energy-storage system with high penetration of renewable energy in radial distribution network

An innovative approach to minimize excess sludge production in sewage treatment using integrated bioreactors

The present investigation deals with an application of integrated sequential oxic and anoxic bioreactor（SOABR） and fluidized immobilized cell carbon oxidation（FICCO） reactor for the treatment of domestic wastewater with minimum sludge generation. The performance of integrated SOABR-FICCO system was evaluated on treating the domestic wastewater at hydraulic retention time（HRT） of 3 hr and 6 hr for 120 days at organic loading rate（OLR）of 191 ± 31 mg/（L·hr）. The influent wastewater was characterized by chemical oxygen demand（COD） 573 ± 93 mg/L; biochemical oxygen demand（BOD5） 197 ± 35 mg/L and total suspended solids（TSS） 450 ± 136 mg/L. The integrated SOABR-FICCO reactors have established a significant removal of COD by 94% ± 1%, BOD5 by 95% ± 0.6% and TSS by 95% ± 4% with treated domestic wastewater characteristics COD 33 ± 5 mg/L; BOD59 ± 0.8 mg/L and TSS 17 ± 9 mg/L under continuous mode of operation for 120 days. The mass of dry sludge generated from SOABR-FICCO system was 22.9 g/m3. The sludge volume index of sludge formed in the SOABR reactor was 32 mL/g and in FICCO reactor it was 46 mL/g. The sludge formed in SOABR and FICCO reactor was characterized by TGA, DSC and SEM analysis. Overall, the results demonstrated that the integrated SOABR-FICCO reactors substantially removed the pollution parameters from domestic wastewater with minimum sludge production.

A brain tumor is the uncharacteristic progression of tissues in the *** are very deadly,and if it is not diagnosed at an early stage,it might shorten the affected patient’s life ***,their classification and detection play a critical role in *** Brain tumor detection is done by biopsy which is quite *** is usually not preferred at an early stage of the *** detection involvesMagneticResonance Imaging(MRI),which is essential for evaluating the *** paper aims to identify and detect brain tumors based on their location in the *** order to achieve this,the paper proposes a model that uses an extended deep Convolutional Neural Network(CNN)named Contour Extraction based Extended EfficientNet-B0(CE-EEN-B0)which is a feed-forward neural network with the efficient net layers;three convolutional layers and max-pooling layers;and finally,the global average pooling *** site of tumors in the brain is one feature that determines its effect on the functioning of an ***,this CNN architecture classifies brain tumors into four categories:No tumor,pituitary tumor,Meningioma tumor,andGlioma *** network provides an accuracy of 97.24%,a precision of 96.65%,and an F1 score of 96.86%which is better than already existing pre-trained networks and aims to help health professionals to cross-diagnose an MRI *** model will undoubtedly reduce the complications in detection and aid radiologists without taking invasive steps.

Techno-Economic Optimization of Novel Stand-Alone Renewable Based Electric Vehicle Charging Station near Bahria Town Karachi,Sindh pakistan

Electric vehicles (EVs) are the most interesting and innovative technology in the 21st century because of theirenormous advantages, both technically and economically. Their emissions rate compared to fuel-based vehicles isnegligible as they do not consume fuel and hence do not emit any harmful gases. However, their bulk production,adoption and lack of charging stations increase the stress of power stations due to modern-day lifestyles. If Electricvehicles demand increases drastically then conventional power stations will not bear their demand and if theygenerate electricity by conventional means it will be very costly and may further add greenhouse gases. Therefore,this research provides the techno-economic assessment of a stand-alone renewable-dependent electric vehiclecharging station, excluding any burden on electrical utility. The proposed study is carried out in Bahria Town,Karachi, a city in pakistan. In this study, HOMER pro software was utilized for techno-economic assessment. Ahybrid system comprising solar photovoltaic/Wind Turbine/Fuel cells and battery storage is included in the *** and wind resources were taken from NASA’s website, where charging stations will be integrated. The overallresults show promising in terms of total Net present Cost and the Cost of Energy which are 2.72M $ and 0.237 $,respectively. The total system generation is 3,598 Megawatt hours per year, and the total energy consumption is 885Megawatt hours per year.

Staghorn calculi are branched stones which occupy a majority portion of the pelvicaliceal *** untreated staghorn calculus over time can damage the kidney and deteriorate its function and/or cause life threatening *** stone clearance is an important goal in order to eradicate any infective focus,relieve obstruction,prevent recurrence and preserve the kidney *** nephrolithotomy(pCNL)is currently the accepted first-line treatment option for staghorn *** options available are single-tract pCNL with an auxiliary procedure like shockwave lithotripsy,single-tract pCNL with flexible nephroscopy,or multitract *** has its own pros and *** the ultimate goal of treatment for any patient with staghorn calculi should be safety,cost-effectiveness,and to achieve total stone *** this article,we review the management of staghorn calculi with multiple percutaneous(“multitract”)access,its advantages and disadvantages and its current position by studying the various published materials across the globe.

A new voltage control strategy of distribution static compensator(DSTATCOM)has been proposed in this paper for electric grid *** proposed control scheme combines two methods of DSTATCOM operation to improve its *** power factor and voltage magnitude as degree of freedom,the DSTATCOM provides features such as mitigation of voltage and current harmonics,balancing of source currents,improvement of power factor,voltage regulation during voltage sag and swell,reduction in inverter losses,and control of load power to achieve energy *** performance of proposed DSTATCOM control is better as compared to its conventional operation at any time of *** simulation and experimental results validate the performances.
