Listening to Turkish classical music decreases patients’anxiety,pain,dissatisfaction and the dose of sedative and analgesic drugs during colonoscopy:A prospective randomized controlled trial

AIM： To determine whether listening to music decreases the requirement for dosages of sedative drugs, patients＇ anxiety, pain and dissatisfaction feelings during colonoscopy and makes the procedure more comfortable and acceptable. METHODS： Patients undergoing elective colonoscopy between October 2005 and February 2006 were randomized into either listening to music （Group 1, n = 30） or not listening to music （Group 2, n = 30）. Anxiolytic and analgesic drugs （intravenous midazolam and meperidine） were given according to the patients＇ demand. Administered medications were monitored. We determined their levels of anxiety using the State-Trait Anxiety Inventory Test form. Patients＇ satisfaction, pain, and willingness to undergo a repeated procedure were self-assessed using a visual analog scale. RESULTS： The mean dose of sedative and analgesic drugs used in group 1 （midazolam： 2.1 ± 1.4, meperidine： 18.1 ± 11.7） was smaller than group 2 （midazolam： 2.4 ± 1.0, meperidine： 20.6 ± 11.5）, but without a significant difference （P 〉 0.05）. The mean anxiety level in group 1 was lower than group 2 （36.7 ± 2.2 vs 251.0 ± 1.9, P 〈 0.001）. The mean satisfaction score was higher in group 1 compared to group 2 （87.8 ± 3.1 vs 58.1 ± 3.4, P 〈 0.001）. The mean pain score in group i was lower than group 2 （74.1 ± 4.7 vs 39.0 ± 3.9, P 〈 0.001）. CONCLUSION： Listening to music during colonoscopy helps reduce the dose of sedative medications, as well as patients＇ anxiety, pain, dissatisfaction during the procedure. Therefore, we believe that listening to music can play an adjunctive role to sedation in colonoscopy. It is a simple, inexpensive way to improve patients＇ comfort during the procedure.

Tata-box-binding protein-associated factor 15 as a new potential marker in gastrointestinal tumors

In this editorial,the roles of tata-box-binding protein-associated factor 15(TAF15)in oncogenesis,tumor behavior,and as a therapeutic target in cancers in the context of gastrointestinal(GI)tumors are discussed concerning the publication by Guo et ***15 is a member of the FET protein family with a comprehensive range of cellular ***,evidence has shown that TAF15 is involved in many diseases,including ***15 contributes to carcinogenesis and tumor behavior in many ***,its relationship with the mitogen-activated protein kinases(MAPK)signaling pathway makes TAF15 a new target for ***,the fact that there is few studies investigating the expression of TAF15 constitutes a potential limitation in GI system,the association of TAF15 expression with aggressive tumor behavior and,similar to other organ tumors,the influence of TAF15 on the MAPK signaling pathway emphasize that this protein could serve as a new molecular biomarker to predict tumor behavior and target therapeutic intervention in GI *** conclusion,more studies should be performed to better understand the prognostic and therapeutic role of TAF15 in GI tumors,especially in tumors resistant to therapy.

Prognostic significance of tumor budding,desmoplastic reaction,and lymphocytic infiltration in patients with gastric adenocarcinoma

A cross-layer fault tolerance management module for wireless sensor networks

It is a well-established fact that wireless sensor networks (WSNs) are very power constraint networks, but besides this, they are inherently more fault-prone than any other type of wireless network and their protocol design is very application specific. Major reasons for the faults are the unpredictable wireless communication channel, battery depletion, as well as fragility and mobility of the nodes. Furthermore, as traditional protocol design methods have proved inadequate, the cross-layer design (CLD) approach, which allows for interactions between different layers, providing more flexible and energy-efficient functionality, has emerged as a viable solution for WSNs. In this study we define a fault tolerance management module suitable to the requirements, limitations, and specifics of WSNs, encompassing methods for fault detection, fault prevention, fault management, and recovery. The suggested solution is in line with the CLD approach, which is an important factor in increasing the network performance. Through simulations the functionality of the network is evaluated, based on packet loss, delay, and energy consumption, and is compared with a similar solution not including fault management. The results achieved support the idea that the introduction of a unified approach to fault management improves the network performance as a whole.

Indoor^(222)Rn Levels and Effective Dose Estimation of Academic Staff in izmir-Turkey

Objective To investigate the annual effective doses from indoor radon received by academic staff in the Faculty building. Methods Measurements of indoor radon concentrations were performed in the Arts and Sciences Faculty of Dokuz Eylul University for two surveys of about 1 month duration respectively using the SSNTD （Solid State Nuclear Track Detectors） method with LR115 detectors. Time integrated measurements comprised different locations inside the faculty building： classrooms, toilets, canteen and offices. Homes of academic staff were also tested for radon. Results The aritthmetic mean radon concentration is 161 Bq m-3 with a range between 40 and 335 Bq m-3 in the Faculty. Six offices and three classrooms have a radon concentration above 200 Bq m-3. The results show that the radon concentration in classrooms is generally higher than in offices. Based on the measured indoor radon data, the annual effective doses received by staff in the Faculty were estimated to range from 0.79 to 4.27 mSv, according to UNSCEAR methodology. The annual effective doses received by staff ranged from 0.78 to 4.20 mSv in homes. On average, the Faculty contributed 56% to the annual effective dose. Conclusion Reported values for radon concentrations and corresponding doses are within the ICRP recommended limits for workplaces.
