Epidemiologic Characteristics of Patients with Cerebral Palsy

Aim: The aim of this study was to determine the demographical and clinical features of patients with cerebral palsy (CP) attending to our outpatient or inpatient clinics. Materials and Methods: Children admitted to Inonu University Faculty of Medicine Department of Physical Medicine and Rehabilitation outpatient or inpatient clinics, Malatya, Turkey with the diagnosis of CP during the study period were included in the study. Age, sex, etiological factors, clinical classifications, and epidemiological characteristics such as socio-economic status as well as the problems associated with CP were analyzed in all patients. Results: Of the total of 130 patients with mean ages 51.05 ± 36.06 months, 51 were girls and 79 were boys. The most common etiologic risk factors were bleeding and presence of threatened miscarriage in prenatal period;asphyxia, low birth weight and prematurity in perinatal period;and neonatal convulsions and hyperbilirubinemia in the postnatal period. Consanguineous marriage was present in 24.6% of families. In clinical classification it was seen that 45 subjects were spastic quadriplegic (34.6%), 41 were spastic diplegic (31.5%), 15 were dyskynetic (11.5%), 14 were spastic hemiplegic (10.8%), 10 were mixed type (7.7%) and 5 were hypotonic/ataxic (3.8%). Conclusion: Prenatal risk factors constitute the foremost risk factors in CP etiology. CP incidence can be lowered by close follow up of the newborns who has been considered as high risk newborns due to the presence of risk factors during pregnancies or deliveries, and by increasing the number of neonatal intensive care units. Besides, it is important to raise public awareness about consanguinity marriages in our country.

With the development of aircraft equipment, conventional navigation is the shift from performance based navigation (PBN). As is known, conventional navigation is based on ground-based navigation aids;however, PBN is based on aircraft avionics and performance. In this paper, a new method called Point Merge System (PMS) considered as one of PBN procedures will be introduced. PMS has many benefits related to fuel savings and emission reductions by implementing Continuous Descent Approaches (CDAs). A new PMS standard arrival route (STAR) model will be designed in radar simulation and it will be suggested.

Echocardiographic Measurement of Pulmonary Arterial Pressure in Patients with Sarcoidosis

Introduction: Pulmonary hypertension (PH) is a significant complication and is poor prognostic factor for sarcoidosis. We evaluated various tests of diagnosis and treatment as well as the associations between PH and value of six-minute walk test (6 MWT) in sarcoidosis patients. Methods: This study was conducted with fifty patients with sarcoidosis. We grouped patients according to echocardiography. Results: PH prevalence in sarcoidosis was found to be 10%. In cases with likely PH, FEV1%, FVC%, MMF% was found to be lower. Although DLCO was less than 80% in the group of likely PH and over 80% in the other, there were no statistical differences between two groups. Mean value of FVC/DLCO ratio was 1.1 ± 0.38 in group of likely PH. There was no significant difference between groups. Mean distance of 6 MWT was 464 ± 105 m in all cases. There were no statistical differences between two groups in 6 MWT. Significant desaturation with exercise was observed in cases with PH (p = 0.007). Conclusion: In all stages, patients who have longer disease duration and abnormal pulmonary function tests, should be examined about PH. Patients should undergo 6 MWT and presence of desaturation after 6 MWT, even without hypoxemia, should be looked after and dealt with accordingly.

PRKCI,the gene for protein kinase Cι(PKCι),is frequently amplified in ovarian cancer and recent studies have shown that PKCιparticipates in ovary ***,it is unknown whether PKCιis differentially involved in the growth/survival between PRKCI-amplified and non-amplified ovarian cancer *** this study,we analyzed ovarian cancer patient dataset and revealed that PRKCI is the only PKC family member significantly amplified in ovarian cancer and PRKCI amplification is associated with higher PKCι*** a panel of ovarian cancer cell lines,we found that abundance of PKCιis generally associated with PRKCI ***,silencing PKCιled to apoptosis in PRKCI-amplified ovarian cancer cells but not in those without PRKCI amplification,thus indicating an oncogenic addiction to PKCɩin PRKCI-amplified *** small-molecule inhibitors characterized to selectively block atypical PKCs did not offer selectivity nor sensitivity in PRKCI-amplified ovarian cancer cells and were even cytotoxic to non-cancerous ovary surface or fallopian tube epithelial cells,we designed an EpCAM aptamer-PKCιsiRNA chimera(EpCAM-siPKCιaptamer).EpCAM-siPKCιaptamer not only effectively induced apoptosis of PRKCI-amplified ovarian cancer cells but also greatly deterred intraperitoneal tumor development in xenograft mouse *** study has demonstrated a precision medicine-based strategy to target a subset of ovarian cancer that contains PRKCI amplification and shown that the EpCAM aptamerdelivered PKCιsiRNA may be used to suppress such tumors.

According to Michael Porter＇s Diamond Model, a business cluster is defined as geographic concentration of inter-connected companies and institutions working in a common industry. In addition, clusters encompass an array of collaborating and competing services and providers that create a specialized infrastructure, which supports the cluster＇s industry. The economic cluster model, represents a synergy, a dynamic relationship and a network between not only the companies that comprise a cluster but also the successful partnering of the stakeholders. Today in all over the world, successful clusters have established a greater competitive advantage and wealth creation for their regions when compared to companies not in a cluster. Given this success more policy makers and regions are considering fostering cluster development as building blocks of regional economies. Research is planned and performed for understanding the clustering in SMEs （small and medium enterprises） industrial estate in Istanbul if it gives a competitive advantage to the cluster members or not. It is performed to determine and satisfy the lack of knowledge about the small and medium enterprises＇ issues, technologic systems, production quality standards, capacity usage ratios, interactions with other firms, habits of following the technologic improvements and clustering effects on the members.

Transfer of single dose of intravitreal injection of ranibizumab and bevacizumab into milk of sheep

AIM： To investigate whether single-dose intravitreal injections of bevacizumab and ranibizumab transfer into milk. METHODS： This study included lactating 12 sheep and a single 3-month old suckling lamb of each sheep. Two groups consisting of 6 sheep and their lambs were constituted; the ranibizumab group and the bevacizumab group before the administration of intravitreal injections, blood and milk samples were obtained from all sheep and, following the injections, blood and milk samples of all sheep and blood samples of all lambs were collected at regular time points. Serum and milk concentrations of bevacizumab and ranibizumab were measured using an enzyme-linked immunosorbent assay（ELISA） kit. The limit of determination was 0.9 ng/m L for bevacizumab and 0.62 ng/m L for ***： At 6h after intravitreal injections, bevacizumab concentration was above the limit of determination in the blood of all sheep. At 3wk, when the study was terminated, bevacizumab concentrations were high in 4 sheep. Even though bevacizumab concentrations in milk showed fluctuations, the drug transferred into the milk of all sheep at detectable concentrations. Ranibizumab drug concentrations in the blood and milk of sheep and those in the blood of lambs were below the limit of determination by the ELISA ***： This sheep model study demonstrate that intravitreal injection of ranibizumab, which did not transfer into the milk of sheep and suckling lambs, is safer than bevacizumab during lactation period.

Does vitamin D affect disease severity in patients with ankylosing spondylitis？

Background Vitamin D has been found to have a role in the function of the immune system. There have been a lot of studies investigating a relation between vitamin D and disease activity in ankylosing spondylitis （AS）. However, there have not been any studies arranging AS in groups according to vitamin D levels and determining any differences among these patients in terms of disease activity, functional status, quality of life, and other clinical parameters. The aim of this study is to compare 25-hydroxy-vitamin D3 （25（OH）D3） levels in AS patients with those in normal healthy subjects and to determine the relationship between 25（OH）D3 levels and AS disease activity, functional status, and quality of life. Methods Ninety-nine consecutive patients and 42 healthy volunteers were included in this study. After a comparison between the patient group and the control group, the patient group was divided into normal, insufficient and deficient subgroups according to the plasma 25（OH）D3 levels for another comparison. Results The differences in the 25（OH）D3 level between the patient and the control groups were statistically insignificant. The number of AS patients whose 25（OH）D3 levels were classified as normal, insufficient, and deficient were 34, 29, and 36, respectively. Erythrocyte sedimentation rate （ESR）, C-reactive protein （CRP）, and Bath AS Disease Activity Index （BASDAI） scores were higher in the low （including insufficient and deficient） 25（OH）D3 level subgroups （P 〈0.05）. The Bath AS Functional Index （BASFI） and AS Quality of Life （ASQoL） scores were significantly different between the normal and the deficient subgroups （P 〈0.05）. Pain, BASDAI, ESR, and CRP were inversely correlated to the 25（OH）D3 levels （P 〈0.05）. Conclusions The plasma 25（OH）D3 levels may decrease in AS patients and this may negatively affect disease activity, functional status and quality of life.

Tumor budding as a potential prognostic marker in determining the behavior of primary liver cancers

Hepatocellular(HCC)and intrahepatic cholangiocarcinoma(ICC),the most common primary tumors of the liver,are among the most important causes of cancer deaths *** patients with primary liver tumors are frequently diagnosed at an advanced stage and have high mortality,many efforts have been made to identify new markers to determine their behavior and treatment,similar to those in other solid organ ***,morphological assessment of tumor budding(TB)has been revealed as a promising prognostic finding to predict tumor behavior and survival across several different tumor ***,the TB score in colorectal cancer has been revealed as an important parameter in pathology report protocols to determine the course of the *** the liver,despite enormous data showing that many mechanisms involved in TB are associated with tumor behavior in both HCC and ICC,studies focusing on the role of TB in predicting the behavior and prognosis of these tumors have started to be investigated very *** purpose of this review is to present data about TB in primary tumors of the liver,pointing out the potential role of this parameter in determining the course of the disease,and emphasize the need to increase the number of further studies focusing on the evaluation of this parameter with an overview of the mechanisms involved in TB.

Comparison of the Effects of Physiotherapy Group Exercise and Basketball on Quality of Life of Obese Children

Aim: To compare the effects of group exercises under physiotherapist control and basketball program on the quality of life of obese children. Methods: 45 obese children aged 10 years were randomly included to the physiotherapy (n=14), basketball (n:15), and control group (n:16). The children were assessed before and after 12-week study duration. The Pediatric Quality of Life Inventory (PedsQL) was used to determine and compare their quality of life (QoL). Results: No difference was found when the subheadings of the PedsQL were compared among the groups (p>0.05). When the changes were investigated within the groups, only significant increase was determined in the physical and emotional subheadings of the physiotherapy group (pConclusion: Although no change was determined in the quality of life, physiotherapy group exercise was found to be more successful to affect physical and emotional status of obese children.

Incidental Collision Tumor of Hepatocellular Carcinoma and Neuroendocrine Carcinoma

The composite tumors of the liver are very rare,including the coexistence of HCC(hepatocellular carcinoma)with NEC(neuroendocrine carcinoma).The rare occurrence of these tumors necessitates more reported cases in order to fully understand their clinical characteristics,behaviors and *** is described an incidental collision tumor of HCC-NEC,along with a review of the literature focusing on their clinicopathological findings and *** tumor presented here was found incidentally in the hepatectomy specimen of a 56-yearold man who had undergone liver transplantation for rapidly progressive liver failure because of alcoholic hepatitis and *** and laboratory examinations did not demonstrate tumor-related *** macroscopic examination,two sharply defined and distinctive areas(1.7 cm and 0.6 cm dimension respectively)were detected among the cirrhotic *** characteristic histopathological features and immunohistochemical findings allowed a diagnosis of HCCNEC to be *** was no evidence of recurrence and metastasis after 10months following *** present case and review revealed that these tumors are frequently found in older ages and *** serum markers are valuable in the discrimination of malignant tumors,their absence cannot completely rule out composite *** requires a comprehensive histopathological evaluation together with *** NEC component might influence the treatment strategy and eventually the outcome of the *** conclusion,the rare occurrence of HCC-NEC and the lack of diagnostic clinical signs and symptoms do not exclude their consideration in the differential diagnosis of liver tumors,especially in patients with the chronic liver disease.
