Bilateral Spontaneous Achilles Tendon Rupture without Any Predisposing Factors

Besides spontaneous rupture of achilles tendon is a rare condition which is usually associated with corticosteroid or fluoroquinolon usage, spontaneous rupture of Achilles tendon without any risk factors is extremely rare. In this report we aimed to present clinical features of a case of bilateral spontaneous achilles tendon rupture and tried to investigate potential etiologic factors. A 54 year old man was admitted to our emergency department as complaining bilateral ankle pain at the retrocalcaneal region. He had no trauma, any comorbidities or any drug consumption history. Painful achilles tendon gaps proximal to his ankles had been palpated and bilateral Thompson’s test positivity had been noted (Figures 1 and 2). He was unable to rise upon his toes however he was able to walk. American Orthopedic Foot and Ankle Society Hindfoot Scoring was calculated as 78 points. The rupture had been verified with a magnetic resonance imaging and his laboratory findings were all within normal limits (Figures 3 and 4).The patient did not approve an operation so conservative treatment with dorsiflexion limiting brace was applied and strengthening and stretching exercises of gastrosoleus was suggested. Atraumatic and bilateral cases with achilles rupture should be well investigated whether any systemic factors are coexistent. Besides well known corticosteroids or fluoroquinolone exposure, endocrinologic and rheumatological diseases, hypercholesterolaemia should be evaluated in terms of predisposing causes. Patients who have to use corticosteroids or fluoroquinolone, should be informed about achilles tendon rupture and be suggested do regular physical exercises in order to reduce the rupture risk. Despite predisposing factors, sometimes nothing can be associated with achilles tendon rupture as in our case. Genetic predisposition or a degenerative process of a tendon due to age may be underlying cause like in rotator cuff tears.

The Effect of Dexmedetomidine on Oxygenation and Intrapulmonary Shunt during One Lung Ventilation

Background: The effect of intravenously administered dexmedetomidine on oxygenation during one-lung ventilation has not been studied. The hypothesis of this prospective, randomized study was that dexmedetomidine would be associated with an improvement in oxygenation during one-lung ventilation. The secondary outcome was the change in the intrapulmonary shunt. Materials and Methods: Thirty patients undergoing one-lung ventilation were included. Patients in Group D (n = 15) received a bolus dose of 1 μg·kg-1 dexmedetomidine at 10 min after induction of anaesthesia followed by an infusion of 0.4 μg·kg-1·hr-1 that was stopped at the end of the surgery, and those in the control group(Group C, n = 15) were given saline at the same dosage. Vital signs, PaO2, and pulmonary shunting (Qs/Qt) were compared. Results: During one-lung ventilation, the PaO2 in Group D increased significantly and Qs/Qt significantly decreased compared to Group C (PaO2, 203.7 ± 42.3 mmHg vs. 173 ± 37.6 mmHg [P = 0.04] and Qs/Qt 19.8% ± 4.5% vs. 31% ± 2.4% [P < 0.0001], Group D and Group C, respectively). Conclusion: Intravenous administration of dexmedetomidine achieved an improvement in oxygenation during one-lung ventilation. This was associated with a decrease in pulmonary shunt. Use of intraoperative dexmedetomidine during thoracic surgery can be suggested.

在发达国家,主动脉瓣狭窄(AS)是最常见的心脏瓣膜病,且随着人口老龄化,其发病率也越来越高。在临床决策中,对病程及病情严重性进行准确诊断非常重要。目前的诊疗指南虽然推荐使用跨瓣膜压差、最大流速及主动脉瓣口面积来判断疾病的严重程度,但疾病严重性分级不一致仍然是临床中常见的问题。近期研究表明,伴有低流速和/或低压差的患者病情较为严重,且预后较差。目前指南所推荐的诊治模式,可能会导致对病情估计不足,延误了心脏主动脉瓣膜置换术(AVR)的进行。因此,我们需对有这一富有挑战的临床疾病的患者认真进行评估,并制订个体化临床诊疗决策。

当前,定量检测循环免疫复合物(CIC)的方法已有许多种,其中最常用的是C1q、Raji细胞和胶固素分析法。本文作者利用聚乙二醇(PEG)能吸附固相支持物表面并能使免疫复合物(IC)沉淀的两方面特性,建立了另一种检测CIC的快速固相ELISA技术—EICA。作者对包被塑料微孔的PEG浓度进行了选择,并证明微孔表面吸附的PEG能与人工IC(BSA—抗BSA)结合,用热凝IgG(AHG)作为CIC的替代物测制了定量检测的参考曲线。EICA法试验步骤如下:于微孔中加入200μl9％的PEG(MW=6000,溶于0.01MTrispH7.8缓冲液),置37℃保温18—24小时;弃去包被液,加入25μl1:4稀释的待检血清或AHG(溶于1:4正常血清中),置37℃保温1.5小时;

Objective:To determine post-discharge mortality and associated factors of the first-wave multicenter Turkish Thoracic Society(TTD)-TURCOVID ***:In this retrospective cohort study,we analyzed the data of 18 of 26 centers included in the first TTD-TURCOVID study,and 1112 cases diagnosed with COVID-19 between 11 March and 31 July 2020 participated in the *** causes of death after COVID-19 discharge were ***:The mean age of the patients was(51.07±16.93)years,with 57.6%male *** the cohort group,89.1%of COVID-19 treatment locations were hospital wards,3.6%were intensive care units(ICUs),and 7.2%were community *** the longterm follow-up,the in-hospital mortality rate was 3.6%(95%CI 2.6-4.8),the post-discharge mortality rate was 2.8%(95%CI 1.9-3.9),and the total mortality was 6.3%(95%CI 5.0-7.8).After discharge,63.3%of mortality overall occurred during the first six *** rates in post-discharge follow-ups were 12.7%(95%CI 8.0-30.6)in cancer patients,10.8%(95%CI 6.3-22.9)in chronic obstructive pulmonary disease patients,11.1%(95%CI 4.4-22.7)in heart failure patients,7.8(95%CI 3.8-14.3)in atherosclerotic heart disease patients,and 2.3%(95%CI 0.8-5.6)in diabetes mellitus *** smokers/ex-smokers,the all-mortality rates were higher than in ***:This multicenter study showed that patients over 65 years of age,males,former/active smoker,ICU stay,lung,heart disease,and malignancy should be followed up for at least the first six months after discharge due to COVID-19.

Background： Metabolic syndrome （MetS） is one of the high cardiovascular （CV） situations. Endothelial dysfunction, which is a common finding in patients with MetS, is related with increased CV risk. In patients with MetS, the effect of the major CV risk factors, not included in the MetS definition, on endothelial dysfunction is not well known. The aim of this study was to determine the effect of major CV risk factors such as gender, smoking, family history, and biochemical parameters on endothelial dysfunction in patients with MetS. Methods： The study was performed between December 2010 and August 2014. A total of 55 patients （ 15 females and 40 males） with MetS and 81 healthy controls （37 females and 44 males） with a body mass index 〈25 kg/m2 were enrolled in the study. Endothelial dysfunction was measured by flow-mediated dilatation （FMD）, oxidative stress parameters; high-sensitivity C-reactive protein （hs-CRP）, oxidized low-density lipoprotein （ox-LDL）, endothelial nitric oxide synthase （e-NOS）, nitric oxide, and cell adhesion markers; yon Willebrand factor, and e-selectin. Platelet aggregation （endothelial adenosine diphosphate）, total platelet count, and mean platelet volume were additionally analyzed and demographic parameters were explored. Student＇s t-test, Mann-Whitney U-test, and Chi-square test were used to analyze the results. Results： The fasting blood glucose （z = 3.52, P = 0.001 ）, hs-CRP （z - 3.23, P = 0.004）, ox-LDL （z = 2.62, P= 0,013）, and e-NOS （z = 2.22, P = 0.026） levels and cardiac risk score （z - 5.23, P 〈 0.001） were significantly higher in patients with MetS compared with the control group. Smoking was correlated with decreased FMD （χ2 = 9.26, P = 0.002） in MetS patients but not in the control group. Conclusions： Increased ox-LDL, hs-CRP, and e-NOS are likely to be a result of oxidative stress, a condition in which an imbalance occurs between the production and inactivation of reactive nitrogen and oxygen species. Synovial Chondromatosis of the Elbow Joint in a 27-Year-Old Man:A Case Report

Introduction: Multiple intraarticular loose bodies are pathognomonic for the Synovial chondromatosis. This disorder affects a wide age range from adults to childhood with no sex dominance. Malignant transformation is rare to date, and there are approximately 34 cases in literature about the transformation of synovial chondromatosis to chondrosarcoma. Case: We present case of a 27-year-old patient who had synovial chondromatosis in his elbow. Discussion: Radiographic findings are usually pathognomonic for the diagnosis. Multiple calcified loose bodies can be seen in the joint in radiographs. Computed tomography (CT) optimally depicts the calcified intraarticular fragments and extrinsic bone erosion. However, because the radioopacity cannot be detected in 1/3 of the cases, diagnosis becomes difficult. Treatment modalities include arthroscopic or open extraction of the loose bodies and arthropasty in the presence of severe osteoarthritis. Conclusion: Synovial chondromatosis should be kept in the differential dignosis of the chronic monoarticular pain and painful limitations of the range of motions of the joints.

文学家创作作品成功与否的一个重要标准是其作品对人的关注程度如何,而文学对比研究也是文学批评中的重要议题之一。中国当代最负盛名的作家之一——贾平凹在其作品《秦腔》中以平实细腻的语言风格,集中表现了改革开放年代农村的价值观念在传统格局中的转换,展现出作者对社会转型期农村现状的思考。而土耳其作家奥尔罕·帕慕克则在其代表作《我的名字叫红》中揭示了伊斯兰文化与欧洲文化之间的碰撞与融合,同时也体现出了对传统文化的忧患意识。这两位作家,虽然有着不同的生活背景,但在描写异质文化冲突下人物的心理动态及其精神诉求时却有着异曲同工之妙。两部文学作品的文化语境,寄寓了两位作者共同的现实批判以及对未来不同的展望。在如今这个多元文化共生的时代,笔者选取两位极具代表性的生活在文化混杂背景之下的作家,以期为学界在此方面的研究起到抛砖引玉的作用。

Intestinal obstruction due to phytobezoars: An update

The term bezoar refers to an intraluminal mass in the gastrointestinal system caused by the accumulation of indigestible ingested materials, such as vegetables, fruits, and hair. Bezoars are responsible for 0.4%-4% of cases of mechanical intestinal obstruction. The clinical findings of bezoar-induced ileus do not differ from those of mechanical intestinal obstruction due to other causes. The appearance and localization of bezoars can be established with various imaging methods. Treatment of choice depends on the localization of the bezoar which makes the clinical findings.
