Inflammatory bowel diseases(IBDs)are a group of chronic inflammatory diseases that affect the gastrointestinal tract,including Crohn's disease(CD)and ulcerative *** is a treatment option,and more than half of the patients with CD will undergo surgical interventions over the course of the *** complications are common in IBD patients,the most frequent being intraabdominal sepsis,infection of the surgical site,and adynamic ileum,and nutritional status is a factor that can influence postoperative *** studies have shown that malnutrition,obesity,sarcopenia,and myosteatosis are predictors of surgical ***,most were retrospective studies with small patient samples and heterogeneity of clinical and nutritional assessment methods,which limit the extrapolation of ***,knowing the pathophysiological mechanisms of IBD and identifying the best parameters for assessing nutritional status are essential for prompt implementation of adequate nutritional interventions.

Acute Appendicitis Confined to an Incisional Hernia Following Renal Transplantation

We present a case of a 57 years old moderately obese woman with a known 12 cmincisional hernia, who subsequently developed an incarcerated acute appendicitis. The patient underwent an uneventful orthotopic liver and renal transplant five years prior, and was compliant with ongoing immunosuppression without rejection. She presented with 8 hours of acute onset right lower quadrant pain, associated with anorexia, documented fevers, and nausea. Noncontrast CT demonstrated a blind-ending tubular structure with an enhancing and thickened wall within a hernia defect of the right lower quadrant. The patient underwent emergent laparotomy and a non-perforated appendix was completely excised at its base. Discussion: There have been documented reports of an acute appendicitis associated with inguinal hernias, given the eponym Amyand’s hernia. Appendicitis may present within hernias, and there should be a low threshold for radiologic assessment of its components when there is clinical doubt about the symptoms associated with the hernia. Our recommendation prompts early use of non-contrast CT scan in transplant patients with known hernias on examination and abdominal tenderness over the renal allograft considering the high risk of perforation of acute appendicitis and strangulation.

Crohn's disease with involvement of the esophagus, stomach and duodenum has a prevalence of 0.5% to 4% in symptomatic adult patients, but some studies have shown that these results may be underestimated, since upper gastrointestinal endoscopy is not performed routinely in the initial evaluation of the disease in adult patients, as it is in the pediatric population. In general, involvement of the upper gastrointestinal tract in Crohn's disease occurs concomitantly with involvement of the lower gastrointestinal tract. The diagnosis depends on clinical,endoscopic, histological and radiological evaluation. The presence of aphthoid ulcers, longitudinal ulcers, bamboo-joint-like appearance, stenoses and fistulas are endoscopic findings suggestive of the disease, and it is important to exclude the presence of Helicobacter pylori infection. The primary histological findings,which facilitate the diagnosis, are the presence of a chronic inflammatory process with a predominance of lymphoplasmacytic cells and active focal gastritis. The presence of epithelioid granuloma, although less frequent, is highly suggestive of the disease in the absence of chronic granulomatous disease. Treatment should include the use of proton pump inhibitors associated with corticosteroids,immunomodulators and biological therapy according to the severity of the disease.

Direct-acting antiviral (DAA) therapy is often well-tolerated, and adverse events from DAA therapy are uncommon. We report a case of a woman who underwent orthotopic liver transplant for chronic hepatitis C infection and later devel-oped alloimmune hepatitis shortly after starting DAA therapy for recurrent hepatitis C infection. The patient developed acute alloimmune hepatitis approximately 2 weeks after starting treatment with sofosbuvir, velpatasvir, and voxilap-revir. This case report proposes a dysregulation of immune surveillance due to the DAA stimulation of host immunity and rapid elimination of hepatitis C viral load as a precipitating factor for the alloimmune process, leading to alloimmune hepatitis in a post-transplant patient who starts on DAA.

Objective: Drug resistance is considered one of the main threats for tuberculosis control. Our aim was to identify risk factors for drug resistance in tuberculosis patients in the Northern Portugal. Study Design and Methods: Retrospective case-control study. The medical records and drug susceptibility test data from TB patients diagnosed between 31 March 2009 and 1 April 2010 were examined. We enrolled 119 patients with any drug resistance to first line anti-TB drugs and 238 with drug-susceptible TB, matched by age group. Variables analyzed included: gender, country of origin, employment situation, site of disease, previous treatment, presence of diabetes mellitus, HIV infection, alcohol abuse, intravenous drug use, abuse of other drugs and smoking habits. Multivariate conditional logistic regression was used to identify independent predictors for drug-resistant TB. Results: Diabetes mellitus [adjusted odds ratio (OR): 3.54;95% CI: 1.45 - 8.66], intravenous drug use (OR: 4.77;95% CI: 1.24 - 18.32) and previous TB treatment (OR: 2.48;95% CI: 1.12 - 5.49) were found to be risk factors for drug-resistant disease development. Conclusions: Diabetes mellitus, prior tuberculosis treatment, and intravenous drug use were risk factors for drug-resistant disease. The association between diabetes and drug-resistant TB should be further explored. Identifying clinical predictors of drug resistance can allow prompt identification of patients at risk for drug-resistant TB.

Novel Simplified Method for Coalbed Methane Feasibility Evaluation

Coal during its carbonization process produces a gas. This gas, mainly formed by methane, can be used. This gas, coalbed methane (CBM), is usually mainly formed by methane and can be considered similar to natural gas as energy source. The evaluation of the techno-economic feasibility of the extraction of this gas depends on a large number of complex different factors. The work carried out covers the different aspects to simplify the first approach for CBM feasibility quantification considering a short number of indicators. A theoretical review and a state of the art description have been carried out, taking into account all the factors that can influence in the development of a CBM project. After that, technical feasibility has been used to evaluate total amount of gas that can be recovered. The last step was to evaluate economic feasibility to know how much gas could be economically profitable. Conclusions got have been used to develop a simple method for evaluating CBM economic feasibility considering just four easy known parameters of coal beds. These are: the rank, the thickness, the gas concentration, and the permeability.

Conditioned Taste and Place Preferences Induced by Electrical Stimulation of the External Lateral Parabrachial Nucleus: A General Reinforcing Mechanism?

This study examined the stimulus specificity of external lateral parabrachial (LPBe) rewarding stimulation by using two identical learning procedures that may dissociate conditioned reinforcement to either the place or the flavor stimulus. Animals were presented with two distinct flavors in two different positions (left and right) that were varied throughout the experimental sessions. In the first experiment, LPBe stimulation was associated with one or other flavor, while in the second it was conditioned to one or other place in which these flavors were offered. The results show that, despite stimulus interferences, the animals develop specific conditioned preferences for the flavor stimuli (experiment 2A), and also for the place of their presentation (experiment 2B). These data are discussed in the context of brain reward systems and the biological constraints that characterize some learning modalities.

Photodynamic therapy in cancer treatment - an update review

Cancer remains a worldwide health problem, being the disease with the highest impact on global health. Even with all the recent technological improvements, recurrence and metastasis still are the main cause of death. Since photodynamic therapy (PDT) does not compromise other treatment options and presents reduced long-term morbidity when compared with chemotherapy or radiotherapy, it appears as a promising alternative treatment for controlling malignant diseases. In this review, we set out to perform a broad up-date on PDT in cancer research and treatment, discussing how this approach has been applied and what it could add to breast cancer therapy. We covered topics going from the photochemical mechanisms involved, the different cell death mechanisms being triggered by a myriad of photosensitizers up to the more recent-on-going clinical trials.

The presence of a potentially hazardous facility in a community demands several safety procedures. Bringing risk communication among those actions may help the population that lives near the facility feel more confident and have the required knowledge on how to behave in an emergency situation. A research performed in Angra dos Reis, RJ, Brazil, where a nuclear power plant is located, shows that there is a lack of information and awareness about the emergency plan.
