BACKGROUND Chronic liver disease(CLD)was associated with adverse clinical outcomes among people with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection.AIM To determine the effects of SARS-CoV-2 infection on the incidence and treatment strategy of hepatocellular carcinoma(hCC)among patients with CLD.METhODS A retrospective,territory-wide cohort of CLD patients was identified from an electronic health database in hong Kong.Patients with confirmed SARS-CoV-2 infection[coronavirus disease 2019(COVID-19)+CLD]between January 1,2020 and October 25,2022 were identified and matched 1:1 by propensity-score with those without(COVID-19-CLD).Each patient was followed up until death,outcome event,or November 15,2022.Primary outcome was incidence of hCC.Secondary outcomes included all-cause mortality,adverse hepatic outcomes,and different treatment strategies to hCC(curative,non-curative treatment,and palliative care).Analyses were further stratified by acute(within 20 d)and post-acute(21 d or beyond)phases of SARS-CoV-2 infection.Incidence rate ratios(IRRs)were estimated by Poisson regression models.RESULTS Of 193589 CLD patients(>95%non-cirrhotic)in the cohort,55163 patients with COVID-19+CLD and 55163 patients with COVID-19-CLD were included after 1:1 propensity-score matching.Upon 249-d median follow-up,COVID-19+CLD was not associated with increased risk of incident hCC(IRR:1.19,95%CI:0.99-1.42,P=0.06),but higher risks of receiving palliative care for hCC(IRR:1.60,95%CI:1.46-1.75,Ph acute and post-acute phases of infection,COVID-19+CLD were associated with increased risks of allcause mortality(acute:IRR:7.06,95%CI:5.78-8.63,Phepatic outcomes(acute:IRR:1.98,95%CI:1.79-2.18,Phough CLD patients with SARS-CoV-2 infection were not associated with increased risk of hCC,they were more likely to receive palliative treatment

On-chip optical nonreciprocal devices are vital components for integrated photonic systems and scalable quantum information processing.Nonlinear optical isolators and circulators have attracted considerable attention because of their fundamental interest and their important advantages in integrated photonic circuits.however,optical nonreciprocal devices based on Kerr or Kerr-like nonlinearity are subject to dynamical reciprocity when the forward and backward signals coexist simultaneously in a nonlinear system.here,we theoretically propose a method for realizing on-chip nonlinear isolators and circulators with dynamic nonreciprocity.Dynamic nonreciprocity is achieved via the chiral modulation on the resonance frequency due to coexisting self-and cross-Kerr nonlinearities in an optical ring resonator.This work showing dynamic nonreciprocity with a Kerr nonlinear resonator can be an essential step toward integrated optical isolation.

Objective: To evaluate transperineal laser ablation (TPLA) with Echolaser® (Echolaser® TPLA, Elesta S.p.A., Calenzano, Italy) as a treatment for benign prostatic hyperplasia (BPh) and prostate cancer (PCa) using the Delphi consensus method.Methods: Italian and international experts on BPh and PCa participated in a collaborative consensus project. During two rounds, they expressed their opinions on Echolaser® TPLA for the treatment of BPh and PCa answering online questionnaires on indications, methodology, and potential complications of this technology. Level of agreement or disagreement to reach consensus was set at 75%. If the consensus was not achieved, questions were modified after each round. A final round was performed during an online meeting, in which results were discussed and finalized.Results: Thirty-two out of forty invited experts participated and consensus was reached on all topics. Agreement was achieved on recommending Echolaser® TPLA as a treatment of BPh in patients with ample range of prostate volume, from 80 mL (80%), comorbidities (100%), antiplatelet or anticoagulant treatment (96%), indwelling catheter (77%), and strong will of preserving ejaculatory function (100%). Majority of respondents agreed that Echolaser® TPLA is a potential option for the treatment of localized PCa (78%) and recommended it for low-risk PCa (90%). During the final round, experts concluded that it can be used for intermediate-risk PCa and it should be proposed as an effective alternative to radical prostatectomy for patients with strong will of avoiding urinary incontinence and sexual dysfunction. Almost all participants agreed that the transperineal approach of this organ-sparing technique is safer than transrectal and transurethral approaches typical of other techniques (97% of agreement among experts). Aging represents an emerging challenge for public health due to the declined immune responses against pathogens, weakened vaccination efficacy, and disturbed tissue homeostasis. Metabolic alterations in cellular and systemic levels are also known to be cardinal features of aging. Moreover, cellular metabolism has emerged to provide regulations to guide immune cell behavior via modulations on signaling cascades and epigenetic landscape, and the aberrant aging process in immune cells can lead to inflammaging, a chronic and low-grade inflammation that facilitates aging by perturbing homeostasis in tissues and organs. here, we review how the metabolic program in T cells is influenced by the aging process and how aged T cells modulate inflammaging. In addition, we discuss the potential approaches to reverse or ameliorate aging by rewiring the metabolic programming of immune cells.

Objective:Robot-assisted partial nephrectomy(RAPN)has become widely used for treatment of renal cell carcinoma and it is expanding in the field of complex renal masses.The aim of this systematic review was to analyze outcomes of RAPN for completely endophytic renal masses,large tumors(cT2-T3),renal cell carcinoma in solitary kidney,recurrent tumors,completely endophytic and hilar masses,and simultaneous and multiple tumors.Methods:A comprehensive search in the PubMed,Scopus,Web of Science,and Cochrane Central Register of Controlled Trials databases was performed in December 2022 for English language papers.The primary endpoint was to evaluate the role of RAPN in the setting of each category of complex renal masses considered.The secondary endpoint was to evaluate the surgical and functional outcomes.Results:After screening 1250 records,43 full-text manuscripts were selected,comprising over 8500 patients.Twelve and thirteen studies reported data for endophytic and hilar renal masses,respectively.Five and three studies reported outcomes for cT2-T3 and solitary kidney patients,respectively.Four studies focused on redo-RAPN for recurrent tumors.Two studies investigated simultaneous bilateral renal masses and five reports focused on multiple tumor excision in ipsilateral kidney.Conclusion:Over the past decade,evidence supporting the use of RAPN for the most challenging nephron-sparing surgery indications has continuously grown.Although limitations remain including study design and lack of detailed long-term functional and oncological outcomes,the adoption of RAPN for the included advanced indications is associated with favorable surgical outcomes with good preservation of renal function without compromising the oncological result.Certainly,a higher likelihood of complication might be expected when facing extremely challenging cases.however,none of these indications should be considered per se an exclusion criterion for performing RAPN.Ultimately,a risk-adapted approach should be employed.

BACKGROUND Gastrointestinal schwannomas are slow-growing benign mesenchymal neoplasms that originate from Schwann cells of the nerve sheath of Auerbach′s plexus or less frequently from Meissner′s plexus.The main differential diagnosis of gastric schwannomas are the gastrointestinal stromal tumors(GISTs),which are classified by their immunohistochemistry.The treatment of choice for gastric schwannomas is surgery where laparoscopy plays an important role.Wedge resection,subtotal or total gastrectomy can be done.In its counterpart,esophageal schwannomas are benign tumors of the esophagus that are very uncommon since they comprise less than 2%of all esophageal tumors.The main differential diagnosis is the leiomyoma which corresponds to the most common benign esophageal tumor,followed by GIST.The treatment consists on tumoral enucleation or esophagectomy.AIM To review the available literature about gastrointestinal schwannomas;especially lesions from de stomach and esophagus,including diagnosis,treatment,and follow up,as well as,reporting our institutional experience.METhODS A systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyzes guidelines.The following databases were used for reviewing process:Pub Med,Ovid,MEDLINE,and Scopus.Only English language manuscripts were included.All gastrointestinal schwannomas specifically located in the esophagus and stomach were included.Cases that did not report long-term follow-up were excluded.RESULTS Gastric localization showed a higher prevalence in both,the literature review and our institution:94.95%(n=317)and 83%(n=5)respectively.With a follow-up with disease-free survival greater than 36 mo in most cases:62.01%(n=80)vs66.66%(n=4).In both groups,the median size was>4.1 cm.Surgical treatment is curative in most cases CONCLUSION Schwannoma must be taken into account in the differential diagnosis of gastrointestinal mesenchymal tumors.It has a good prognosis,and most are benign.A disease-free survival of more than 36 mo can

Experimental analysis of volatile liquid injection into a fluidized bed

Experiments were conducted on a lab-scale fluidized bed to study the distribution of liquid ethanol injected into fluidized catalyst particles. Electrical capacitance measurements were used to study the liquid distribution inside the bed, and a new method was developed to determine the liquid content inside fluidized beds of fluid catalytic cracking particles. The results shed light on the complex liquid injection region and reveal the strong effect of superficial gas velocity on liquid distribution inside the fluidized bed, which is also affected by the imbibition of liquid inside particle pores. Particle internal porosity was found to play a major role when the changing mass of liquid in the bed was monitored. The results also showed that the duration of liquid injection affected liquid-solid contact inside the bed and that liouid-solid mixin~ was not homogeneous durin~ the limited liouid injection time.

Noether’s theorem is one of the fundamental laws in physics,relating the symmetry of a physical system to its constant of motion and conservation law.On the other hand,there exist a variety of non-hermitian parity-time(PT)-symmetric systems,which exhibit novel quantum properties and have attracted increasing interest.In this work,we extend Noether’s theorem to a class of significant PT-symmetry systems for which the eigenvalues of the PT-symmetry hamiltonian hPTchange from purely real numbers to purely imaginary numbers,and introduce a generalized expectation value of an operator based on biorthogonal quantum mechanics.We find that the generalized expectation value of a time-independent operator is a constant of motion when the operator presents a standard symmetry in the PT-symmetry unbroken regime,or a chiral symmetry in the PT-symmetry broken regime.In addition,we experimentally investigate the extended Noether’s theorem in PT-symmetry single-qubit and two-qubit systems using an optical setup.Our experiment demonstrates the existence of the constant of motion and reveals how this constant of motion can be used to judge whether the PT-symmetry of a system is broken.Furthermore,a novel phenomenon of masking quantum information is first observed in a PT-symmetry two-qubit system.This study not only contributes to full understanding of the relation between symmetry and conservation law in PT-symmetry physics,but also has potential applications in quantum information theory and quantum communication protocols.

Gas-liquid separation is a very common process in industrial plants,where often high Efficiency of Gas Separation(EGS)is demanded.In the upstream oil industry,gas separation is also crucial for the proper operation of Electrical Submergible Pumps(ESP).Recent studies report the excellent performance of a gravitational separator known as Inverted-Shroud separator(IS-separator).Laboratory results indicate that this kind of separator can achieve total gas separation for a wide range of operation conditions under continuous two-phase flow.The original intent is to use the IS-separator for downhole gas separation in oil production wells.however,by account of its simple design and relatively compact size,it may be suitable for using in industrial plants.In this paper,we present the IS-separator in details,including geometry characteristics and phenomenology.The equipment performance is also discussed.In addition,the challenges related to the deep understanding of the gas separation process inside the IS-separator and the possible practical solutions are outlined.
