Non-pharmacological cognitive intervention for aging and dementia:Current perspectives

In recent years, cognitive difficulties associated with normal aging and dementia have been receiving increased attention from both public and scientific communities. With an increase in overall lifespan, promoting healthy cognition has become a priority and a necessity for minimizing and preventing individual and societal burdens associated with cognitive dysfunctions in the elderly. The general awareness concerning the efficacy of preventive(e.g., lifestyles) and palliative treatment strategies of cognitive impairments, related to either healthy or unhealthy trajectories in cognitive aging, is continuously rising. There are several therapeutic strategies which can be broadly classified as either pharmacological or non-pharmacological/psychosocial. In face of the modest evidence for success of pharmacological treatments, especially for dementia related impairments, psychosocial interventions are progressively considered as a complementary treatment. Despite the relative spread of psychosocial interventions in clinical settings, research in this area is rather scarce with evidence for success of these therapies remaining controversial. In this work we provide an evidence based perspective on cognitive intervention(s) for healthy aging, pre-dementia(mild cognitive impairment), and dementia populations. Current evidence and future directions for improving cognitive functions in the elderly are discussed as well.

Noninvasive methods for prediction of esophageal varices in pediatric patients with portal hypertension

AIM: To evaluate clinical and laboratory parameters for prediction of bleeding from esophageal varices (EV) in children with portal hypertension. METHoDS: Retrospective study of 103 children (mean age: 10.1 ± 7.7 years), 95.1% with intrahepatic portal hypertension. All patients had no history of bleeding and underwent esophagogastroduodenoscopy for EV screening. We recorded variceal size (F1, F2 and F3), red-color signs and portal gastropathy, according to the Japanese Research Society for Portal Hypertension classification. Patients were classified into two groups: with and without EV. Seven noninvasive markers were evaluated as potential predictors of EV: (1) platelet count; (2) spleen size z score, expressed as a standard deviation score relative to normal values for age; (3)platelet count to spleen size z score ratio; (4) platelets count to spleen size (cm) ratio; (5) the clinical prediction rule (CPR); (6) the aspartate aminotransferase to platelet ratio index (APRI); and (7) the risk score. ResULTS: Seventy-one children had EV on first endoscopy. on univariate analysis, spleen size, platelets, CPR, risk score, APRI, and platelet count to spleen size z score ratio showed significant associations. The best noninvasive predictors of EV were platelet count [area under the receiver operating characteristic curve (AURoC) 0.82; 95%CI: 0.73-0.91], platelet: spleen size z score (AURoC 0.78; 95%CI: 0.67-0.88), CPR (AURoC 0.77; 95%CI: 0.64-0.89), and risk score (AURoC 0.77; 95%CI: 0.66-0.88). A logistic regression model was applied with EV as the dependent variable and corrected by albumin, bilirubin and spleen size z score. Children with a CPR old more likely to have EV compared to children with CPR > 114. A risk score > -1.2 increased the likelihood of EV (odds ratio 7.47; 95%CI: 2.06-26.99). An innovative inversion code, named “Curupira v1.0”, has been developed using Matlab to determine the vertical distribution of resistivity beneath the subsoil. The program integrates Vertical Electrical Sounding (Ves), successful in shallow subsurface exploration and Time Domain Electromagnetic (TEM) techniques, better suited for deeper exploration, both of which are widely employed in geophysical exploration. These methodologies involve calculating subsurface resistivity through appropriate inversion processes. To address the ill-posed nature of inverse problems in geophysics, a joint inversion scheme combining Ves and TEM data has been incorporated into Curupira v1.0. The software has been tested on both synthetic and real-world data, the latter of which was acquired from the Parana sedimentary basin which we summarise here. The results indicate that the joint inversion of Ves and TEM techniques offers improved recovery of simulated models and demonstrates significant potential for hydrogeological studies.

3D Printed Scaffolds as a New Perspective for Bone Tissue Regeneration: Literature Review

Due to the high incidence of bone fractures in the population, it became necessary to produce scaffolds that are able to assist in tissue regeneration. It is necessary to find an appropriate balance between the mechanical and biological properties, in order to mimic the natural tissue, these properties are directly related to the architecture and their degree of porosity, as well as the size of their pores and their interconnectivity. In this perspective, the 3D printing stands out, where the structure is obtained layer by layer, according to a predetermined computational model which provides a greater control of architecture and scaffold geometry and overcomes, in this way, the limitations of traditional techniques of scaffolds manufacturing. In this way, the objective of this seminar is to present the state of the art of the polymer scaffolds produced by 3D printing and applied to bone tissue regeneration, highlighting the advantages and limitations of this process.

Depending on the treatment and weight of the breasts or abdomen, they may exceed volumes considered giant and morbidly obese. In these cases, and when the patient’s BMI is high above 40 kg/m2, the weight of the breasts or abdomen produces what we consider suffocation when the patient is placed in horizontal position on surgical tables, decreasing his respiratory capacity and increasing the difficulty in treating respiratory or embolic risks. An 8-kg breast on the patient’s chest prevents normal breathing. An abdomen with a volume of 30 kg causes difficulties in all senses, making the physiological expansion of the lungs impossible and even preventing surgical assistance to patients. These patients are almost always customers who sleep in the sitting position to breathe better. The gigantic extirpation of the surgical parts facilitates a better respiratory expansion reducing by a large percentage the risk of death, what we call body relief. This relief does not free the patient from bariatric surgery for a possible weight loss, which is vital for the proper functioning of the organs and decreasing arterial hypertension and diabetes.

Efficacy and safety of limited endoscopic sphincterotomy before self-expandable metal stent insertion for malignant biliary obstruction

AIM To evaluate the safety and efficacy of limited endoscopic sphincterotomy (es) before placement of selfexpandable metal stent(SEMS).METHoDS This was a retrospective analysis of 244 consecutive patients with unresectable malignant biliary obstruction, who underwent placement of SEMSs following limited es from December 2008 to February 2015. The diagnosis of malignant biliary obstruction and assessment of patient eligibility for the study was established by a combination of clinical findings, laboratory investigations, imaging and pathological results. All patients were monitored in the hospital for at least 24 h following endoscopic retrograde cholangio pancreatography(ERCP). The incidence of immediate or early post-ERCP complications such as post-ERCP pancreatitis(PEP) and bleeding related to limited es were considered as primary outcomes. Also, characteristics and complications according to the cancer type were *** Among the 244 patients included, the underlying diagnosis was cholangiocarcinoma in 118 patients,pancreatic cancer in 79, and non-pancreatic or nonbiliary malignancies in the remaining 47 patients. Early post-ERCP complications occurred in 9 patients(3.7%), with PEP in 7 patients (2.9%; mild, 6; moderate, 1) and mild bleeding in 2 patients (0.8%). There was no significant association between the incidence of post-ERCP complications and the type of malignancy(cholangiocarcinoma vs pancreatic cancer vs others, P = 0.696) or the type of SEMS used (uncovered vs covered, P = 1.000). Patients who had more than one SEMS placed at the first instance were at a significantly higher risk of post-ERCP complications (one SEMS vs two SEMS, P = 0.031). No other factors were predictive of post-ERCP *** Limited es is feasible and safe, and effectively facilitates the placement of SEMS, without any significant risk of PEP or severe bleeding.

objective: As isolates of Cryptococcus are frequently kept collection stocks in institutions, sometimes without proper characterization, we sought to determine the genotype profiles, protease and phospholipase activities “in vitro” and the susceptibility testing for azoles and amphotericin B. Methodology: 84 isolates from several regions of Brazil (40 samples from clinical origin and 44 isolates from environmental origin) were maintained at the microorganism’s bank of the Biomedical Science Institute (ICB-USP) of the São Paulo University, in São Paulo, Brazil. This isolates was submitted fungal strains determination, DNA extraction and purification, determination of genotype by URA5 gene RFLP of CGB-positive isolates, protease and phospholipase activity and susceptibility to antifungals. Results: of six CGB positive isolates tested by RFLP-PCR, only four presented a genomic profile consistent C. gattii species (VGII), while two other were C. neoformans (VNI and VNIII), indicating the existence of canavanine-resistante C. neoformans isolates in the culture collections. The clinical isolates secreted higher levels of phospholipase and environmental isolates but no differences were observed for the protease levels. Almost all isolates were sensible to azoles and amphotericin B. Conclusion: We point out in this research the existence of C. neoformans strains resistant to canavanine and intrinsic characteristic of C. gatti. These results demonstrate the importance to perform a detailed characterization of isolates kept in culture collections.

Bacterial spot caused by Xanthomonas arboricola pv. pruni (Xap) is considered as a major problem in peach orchards. Copper and antibiotics are used to control, and biocontrol should be a new alternative with low environment impact. The objective was evaluated by the antibiotic activity of the metabolite produced by Pseudomonas aeruginosa LV strain. The free cells supernatant was fractionated with a serial organic solvent with crescent polarity and a dichloromethane phase was concentrated and lyophilised, and after was fractionated using vacuum liquid chromatography. The antibiotic activity of the F3 fraction containing an organometallic compound was tested on Xap in vitro and in a greenhouse conditions. Plants were sprayed with F3 before or after Xap infection and the results showed changes in exopolysaccharides and cell morphology. The F3 concentration of 450 μg·mL-1 was more effective. The results showed that F3 fraction could be a new alternative to control bacterial spot.
