AIM:To evaluate the efficacy of the aspartate aminotransferase/platelet ratio index(APrI)and neutrophillymphocyte(N/L)ratio to predict liver damage in chronic hepatitis B(CHB).METHODS:We analyzed 89 patients diagnosed with CHB by percutaneous liver biopsy and 43 healthy *** biopsy materials were stained with hematoxylin-eosin and Masson’s ***’fibrosis scores and histological activity index(HAI)were calculated according to the Ishak scoring *** score was recognized as follows:F0-1 No/early-stage fibrosis,F2-6 significant fibrosis,F0-4 non-cirrhotic and F5-6 *** liver fibrosis was defined as an Ishak score of≥*** and N/L ratio calculation was made by blood test ***:The hepatitis B and control group showed no difference in N/L ratios while there was a significant difference in terms of APrI scores(Pregression analysis revealed that the only independent predictive factor for liver fibrosis in CHB was platelet *** score was significantly higher in cirrhotic patients than in non-cirrhotic ***,this significance was not confirmed by multiple logistic regression *** optimum APrI score cut-off point to identify patients with cirrhosis was 1.01with sensitivity,specificity,positive predictive value and negative predictive value of 62%(36%-86%),74%(62%-83%),29%(13%-49%)and 92%(82%-97%),*** addition,correlation analyses revealed that N/L ratio has a negative and significant relationship with HAI(r=-0.218,P=0.041).CONCLUSION:N/L ratio was negatively correlated with *** score may be useful to exclude cirrhosis in CHB patients.

This research introduces an innovative approach to image classification, by making use of Vision Transformer (ViT) architecture. In fact, Vision Transformers (ViT) have emerged as a promising option for convolutional neural networks (CNN) for image analysis tasks, offering scalability and improved performance. Vision transformer ViT models are able to capture global dependencies and link among elements of images. This leads to the enhancement of feature representation. When the ViT model is trained on different models, it demonstrates strong classification capabilities across different image categories. The ViT’s ability to process image patches directly, without relying on spatial hierarchies, streamlines the classification process and improves computational efficiency. In this research, we present a Python implementation using TensorFlow to employ the (ViT) model for image classification. Four categories of animals such as (cow, dog, horse and sheep) images will be used for classification. The (ViT) model is used to extract meaningful features from images, and a classification head is added to predict the class labels. The model is trained on the CIFAr-10 dataset and evaluated for accuracy and performance. The findings from this study will not only demonstrate the effectiveness of the Vision Transformer model in image classification tasks but also its potential as a powerful tool for solving complex visual recognition problems. This research fills existing gaps in knowledge by introducing a novel approach that challenges traditional convolutional neural networks (CNNs) in the field of computer vision. While CNNs have been the dominant architecture for image classification tasks, they have limitations in capturing long-range dependencies in image data and require hand-designed hierarchical feature extraction.

Allogeneic mesenchymal stem cells may be a viable treatment modality in cerebral palsy

Background:The impact of the coronavirus disease 2019(COVID-19)pandemic on patient decision making remains *** study aimed to investigate the effect of the pandemic on treatment preferences of patients with proximal ureteral stones.Materials and methods:retrospective data regarding treatment preferences of patients diagnosed with symptomatic proximal ure-teral stones between July 2018 and November 2021 at a single center were *** from 493 patients were analyzed according to 2 groups,including patients diagnosed during the COVID-19 pandemic and those diagnosed during an equivalent period of time be-fore the pandemic.results:Preference for conservative treatment increased during the COVID-19 pandemic(p=0.009).In patients who had previously undergone shock wave lithotripsy(SWL),the preference for SWL decreased and the preference for conservative treatment increased during the COVID-19 pandemic(p=0.042).Multiple logistic regression analysis revealed a significant correlation between a preference for conservative treatment during the pandemic and no prior spontaneous stone passage(p=0.003;odds ratio[Or],2.48;95％con-fidence interval[CI],1.45-4.23),no hydronephrosis(p=0.035;Or,3.57;95％Cl,1.34-9.49),and a visual analog scale score of 4 or less(p=0.018;Or,1.97;95％Cl,1.15-3.38).Conclusions:A significant increase in the preference for conservative treatment was observed among patients diagnosed during the pandemic,and patients with a history of SWL demonstrated a preference shift from SWL to conservative treatment.

Background: Chronic Kidney Disease (CKD), associated with a slow and progressive loss of kidney function over a period of several years, is an important clinical disaster with an increasing rate of morbidity and mortality especially in the least developed countries. Many hematological parameters are thought to alter dramatically during the course of the disease. These include white blood cells, red blood cells, and platelets. Methods: We tried, retrospectively, to evaluate the peripheral blood hematological alterations in a group of patients undergoing hemodialysis in an eastern Sudan dialysis center to add local medical information. results: Anemia (Low hemoglobin and hematocrit) was detected in 94% of the patients’ group. Mean Erythrocyte count (3.32vs.4.76 (×109/L)), Hemoglobin concentration (9.4vs.13 (g/dl)), Hematocrit (28.7vs.38.7 (L/L)) and platelet count (296 vs. 238 (×109/L)) were significantly lower in the patients’ group than in the control group (P-values Conclusion: Five out of eight studied parameters (red cell count, hemoglobin, hematocrit, mean cell hemoglobin concentration, and platelets count) have shown a significant alteration in CKD patients. As the complete blood count (CBC) test is the most utilized test in clinical laboratory practice, these alterations may be considered as early indicators for CKD. Furthermore, all patients with CKD must be routinely checked for these alterations.
