Major liver resections,perioperative issues and posthepatectomy liver failure:A comprehensive update for the anesthesiologist

Significant advances in surgical techniques and relevant medium-and long-term outcomes over the past two decades have led to a substantial expansion in the indications for major liver *** support these outstanding results and to reduce perioperative complications,anesthesiologists must address and master key perioperative issues(preoperative assessment,proactive intraoperative anesthesia strategies,and implementation of the Enhanced Recovery After Surgery approach).Intensive care unit monitoring immediately following liver surgery remains a subject of active and often unresolved *** postoperative complications,posthepatectomy liver failure(PHLF)occurs in different grades of severity(A-C)and frequency(9%-30%),and it is the main cause of 90-d postoperative ***,recently redefined with pragmatic clinical criteria and perioperative scores,can be predicted,prevented,or *** review highlights:(1)The systemic consequences of surgical manipulations anesthesiologistsmust respond to or prevent,to positively impact PHLF(a proactive approach);and(2)the maximal intensivetreatment of PHLF,including artificial options,mainly based,so far,on Acute Liver Failure treatment(s),to buytime waiting for the recovery of the native liver or,when appropriate and in very selected cases,toward *** a clinical context requires a strong commitment to surgeons,anesthesiologists,and intensivists towork together,for a fruitful collaboration in a mandatory clinical continuum.

Potential role of the gut microbiota in synthetic torpor and therapeutic hypothermia

Therapeutic hypothermia is today used in several clinical settings, among them the gut related diseases that are influenced by ischemia/reperfusion injury. This perspective paved the way to the study of hibernation physiology, in natural hibernators, highlighting an unexpected importance of the gut microbial ecosystem in hibernation and torpor. In natural hibernators, intestinal microbes adaptively reorganize their structural configuration during torpor, and maintain a mutualistic configuration regardless of long periods of fasting and cold temperatures. This allows the gut microbiome to provide the host with metabolites, which are essential to keep the host immunological and metabolic homeostasis during hibernation. The emerging role of the gut microbiota in the hibernation process suggests the importance of maintaining a mutualistic gut microbiota configuration in the application of therapeutic hypothermia as well as in the development of new strategy such as the use of synthetic torpor in humans. The possible utilization of tailored probiotics to mold the gut ecosystem during therapeutic hypothermia can also be taken into consideration as new therapeutic strategy.

Reflectance Spectra Measurements of Mt. Etna: A Comparison with Multispectral/Hyperspectral Satellite

We present a collection of eight reflectance spectra representative of Mt. Etna volcano lava flows. The reflectance spectra were measured with a FieldSpecPro from 350 nm to 2500 nm during a fieldwork in June 2007. The reflectance has been compared with reflectance obtained by multispectral Advanced Space borne Thermal Emission and Reflection Radiometer (ASTER) and by hyper spectral EO1-Hyperion satellites. Prior the comparison, reflectance spectra have been convolved with ASTER and EO1-Hyperion spectral functions. The results show percentage errors in accordance to those present in literature in the ASTER SWIR range. However, some differences have been confirmed for the ASTER reflectance product (ASTER_07) in visible channels. Regarding EO1-Hyperion, a good agreement of reflectance against field measurement has been found resulting in 5% percentage maximum error in the VIS and up 30% in SWIR spectral range. The capacity of reproducing spectral feature by EO1-Hyperion has been checked on bright pixels (ice-snow) in the acquired image.

UAV Thermal Infrared Remote Sensing of an Italian Mud Volcano

Extreme environments like active volcanoes exhibit many difficulties in being studied by in situ techniques. For example, during eruptions, summit areas are very hard to be accessed because of logistics problems and/or volcanic hazards. The use of remote sensing techniques in the last 20 years by satellite or airborne platforms has proven their capabilities in mapping and monitoring the evolution of volcanic activity. This approach has become increasingly important, as much interest is actually focused on understanding precursory signals to volcanic eruptions. In this work we verify the use of cutting-edge technology like unmanned flying system thermally equipped for volcanic applications. We present the results of a flight test performed by INGV in collaboration with the University of Bologna (Aerospace Division) by using a multi-rotor aircraft in a hexacopter configuration. The experiment was realized in radio controlled mode to overcome many regulation problems which, especially in Italy, limit the use of this system in autonomous mode. The overall goal was not only qualitative but also quantitative oriented. The system flew above an Italian mud volcano, named Le Salinelle, located on the lower South West flank of Mt. Etna volcano, which was chosen as representative site, providing not only a discrimination between hot and cold areas, but also the corresponding temperature values. The in-flight measurements have been cross-validated with contemporaneous in-situ acquisition of thermal data and from independent measurements of mud/water temperature.

Background: Dermatological surgery is a relatively new and expanding subspecialty within dermatology. Little information is available about complications in this kind of surgery in the European setting. Objectives: The aim of this study was to assess the incidence of anaesthetic, haemorrhagic and infectious complications in dermatological surgery and to highlight the factors associated with these complications. Methods: Data were collected prospectively over a 3- month period for all surgical procedures performed by a network of dermatologists (n=84 dermatologists) in France, including the excision of all benign or malignant tumours but excluding sebaceous cysts and pyodermas. Information was collected regarding dermatologists, patients, procedures and complications. Results A total of 3788 surgical procedures were available for review; 236 complications, mostly minor, occurred in a total of 213 surgical procedures (6% ), bleeding being the most common (3% ). Vaso- vagal syncope was the main anaesthetic complication (51 of 54). Infectious complications occurred in 79 patients (2% ). Superficial suppuration accounted for 92% of surgical site infections. Only one patient had a systemic infection. Complications requiring additional antibiotic treatment or repeat surgery accounted for only 22 cases of 3788 (1% ). No statistically significant correlation was found with the characteristics of the dermatologists, especially with respect to their training or amount of surgical experience. Similarly, no link could be established between complications and surgical conditions. Multivariate analysis showed that anaesthetic or haemorrhagic complications were independent factors for infectious complications. Sex, administration of an anticoagulant or immunosuppressant, type of procedure performed and duration exceeding 24 min were independent factors for haemorrhagic complications.
