Factors Affecting Treatment Interruption among TB Patients in Lagos Nigeria: Is There Any Need for Treatment Supporters?

Background: This study assessed treatment interruption of tuberculosis (TB) patients managed by treatment supporters and health care workers and other predictors of treatment interruption. Methods: A descriptive cross-sectional study was conducted. Four hundred and seventy new smear positive TB patients above 14 years of age were consecutively recruited between October 1 and December 31 2012 from 34 (23 public and 11 private) directly observed treatment short course (DOTS) facilities that offered TB treatment and microscopy services. They were followed up till treatment was completed. Logistic regression was used to assess the predictors of treatment interruption. Results: A significantly higher proportion of smokers (58.6% vs 38.3%, p = 0.030), patients supervised by treatment supporters (44.4% vs 34.7%, p = 0.032), patients not counselled before initiation of treatment (55.6% vs 38.2%, p = 0.041), patients managed at private DOTS facilities (50% vs 36.3%, p = 0.010) and TB/HIV co-infected patients (54.2% vs 38.6%, p = 0.038) had treatment interruption. Predictors of treatment interruption were supervision by treatment supporters, smoking, lack of pre-treatment counselling and TB/HIV co-infection. Conclusion: A higher proportion of patients supervised by treatment supporters had treatment interruption than those supervised by health care workers. There may be a need to review the concept of treatment supervision by treatment supporters in Lagos state Nigeria.

Setting: Private and public tuberculosis (TB) treatment centers in Lagos State, Nigeria. Objective: To compare adherence of private and public providers of directly observed treatment short course (DOTS) in the Lagos State TB control program, Nigeria (LSTBLCP) with the national TB guidelines. Design: A retrospective review of treatment cards of TB patients managed within the first and second quarter of 2012 in 34 DOTS facilities {23 public, 7 private for profit (PFP), and 4 private not for profit (PNFP)} involved in the private public mix of the LSTBLCP. Results: Of the 1896 treatment cards reviewed, 1524 (80.4%), 132 (7.0%) and 240 (12.6%) were from public, PFP and PNFP DOTS facilities, respectively. About 19%, 25% and none of the patients managed at the public, PNFP, and PFP DOTS facilities were treated in full adherence with the national guidelines respectively. A significantly higher proportion of adults and sputum smear positive TB patients were treated in full adherence with the national guidelines (p < 0.05). Treatment success was associated with full adherence with the national guidelines. Conclusion: There is a need to reorient health care providers in public and private health facilities in Lagos State Nigeria to ensure full adherence with the national TB guidelines.

Landslide is a geological hazard typically associated with extreme events such as earthquakes,heavy rainfall,volcanic eruptions,changes in groundwater level,*** study was carried out in Okemesi-Ekiti(also known as Okemesi),Southwest Nigeria,with the purpose of using remote sensing and GIS technologies to analyze the environmental factors(grain size,direct shear strength resistance,rainfall data,wet density,surface,and slope)resulting in the occurrence of the Okemesi *** study also aimed to conduct a vulnerability analysis in the study area to identify regions with a probability of landslide *** grain size analysis of the soil in the Okemesi landslide area showed that slope materials comprised 17.14%gravel,59.31%sand,and 19.48%fines,thus the soil type could be classified as poorly graded gravely sand with a high possibility of landslide *** geomorphic characteristics of the study area was characterized by slopes ranging from 0.00°to 49.00°,while most slopes in the area were less than 8.00°.The slope aspect direction was mainly in south(157.51°–202.50°),southwest(202.51°–247.50°),west(247.51°–292.50°),and north(0.00°–22.50°and 337.51°–360.00°).The highlands were primarily bounded by the slope directions of north(0.00°–22.50°and 337.51°–360.00°),northeast(22.51°–67.50°),east(67.51°–112.51°),and southeast(112.51°–157.50°),which indicated the potential direction of mass *** study area can be divided into three vulnerability zones:high,medium,and low,with the area percentages of 9.00%,61.80%,and 29.20%,*** analysis suggested that the Okemesi landslide was likely triggered by rainfall,which might have weakened the physical structure of slope *** the causes and impacts of landslides is crucial for policymakers to implement measures to mitigate landslide hazards,protect infrastructure,and prevent the loss of life in the landslide-prone regions.

Power-flow-tracing-based congestion management in hydro-thermal optimal power flow algorithm

This paper presents a method for managing congestion constraints in a hydro-thermal optimal power flow solution procedure. The congestion constraint is handled in this paper as an active power generation constraint. To achieve this solution, a power flow tracing technique is used to detect the generators contributing to line congestion and penalize themby reducing their outputs. The congestion is then removed by setting the maximum power of the affected generators to the penalized value. The proposed algorithm is implemented using MATLAB software. Finally, the performance of the proposed algorithm is tested and the results for the 5-bus,30-bus, and 34-bus Nigerian power networks are presented.

In this study,biogas production from co-digestion of pig dung,water hyacinth,and poultry droppings was investigated.500 ml bio-digesters were connected in series and water displacement method was used to estimate the amount of biogas *** ratio of the percentage weight distribution of pig dung to water hyacinth to poultry dropping was varied as:(0:40:60),(30:40:30),(15:40:45),(45:40:15),and(60:40:0)in digesters A1,B2,C3,D4 and E5,*** highest biogas cumulative yield of 307 cm^(3) CH_(4)/g was obtained in Digester *** gas chromatography analysis showed 22.50 wt%(CO_(2)),64.92 wt%(CH_(4))and 4.83 wt%(N_(2))which implied that the biogas produced can be used for cooking and heating of homes in rural and urban communities.

Assessment of Heavy Metals Pollution in Sediments from Ologe Lagoon, Agbara, Lagos, Nigeria

This study was carried out to assess the level of heavy metals pollution in Ologe Lagoon, Agbara, Lagos, Nigeria. The Lagoon receives effluents from industries in the Agbara Industrial Estate. Cored Soft sediments were retrieved from six random sampling points within the Lagoon and labeled (S1, S2, S3, S4, S5 and S6). Subsamples of the sediments were prepared and analyzed for heavy metal concentration. The result from the heavy metal analysis of the sediments shows that average concentration of Mn, Zn, Cr, Ni, Cr, Ni, Co, Cu and Pb in the samples was 278.1 ppm, 21.7 ppm, 14.2 ppm, 6.6 ppm, 6.4 ppm, 6.4 ppm and 4.4 ppm, respectively. The correlation between the heavy metals varies from 0.71 to 0.98 shows that they have a strongly positive degree of association which suggests that they are from the same source. The geo-accumulation values for Cu in all stations ranged between (1 - 2.8 ppm) and it can be deduced that the sediments are moderately to strongly polluted. Pb varies between 1 and 2.1 suggesting that the sediments are moderately polluted. Zn exhibits strong pollution levels with a range from 2.5 and 3.5 ppm. Co is moderately polluted with 1.5 - 2.4 while Mn shows the highest level of pollution in all stations with Igeo values ranging between 4.1 and 5.8. This study showed that the Lagoon accumulates heavy metals as a result of anthropogenic activities evident in the dumping of municipal, sewage, industrial waste in the environment and domestic activities such as auto mobile transportation and mechanic repair workshop. Based on sediment quality guidelines, the Ologe Lagoon level of pollution ranged from Cu > Pb > Zn > Ni > Co > Mn > Cr > Al with Mn having the highest level of pollution.
