Despite the National Policy for Solid Waste(PNRS)in 2010,nothing has changed to waste disposal in *** to reach 100%of all Municipal Solid Waste(MSW)collected and treated in landfills by Aug.2nd,2014,until nowadays,42%of this total remains in *** the most important national economic region treating its urban waste in landfills,what it has is no more than 4%of recycling and its landfills reaching the *** other ones is getting harder year by year,due to water reservoirs around the region,high freight costs,waste disposal and the severe control of emissions associated with its *** article comes to break the paradigm of investment and profitability proposing an alternative to the land-use,achieving higher rates of *** economic viability,carried out through well-known financial variables and Monte Carlo analysis,has taken into account proven local waste characteristics and market prices for recyclables and energy without *** considered a proposal highly intensive in capital and people,the revenues from the sales would be enough to guarantee the viability of 100%equity with IRR of 33.7%and ROI of 24.5%per year within the confidence of 99%.

Serrated adenocarcinoma is a recently described subset of colorectal cancer(CRC),which account for about10%of all CRCs and follows an alternative pathway in which serrated polyps replace the traditional adenoma as the precursor lesion to *** polyps form a heterogeneous group of colorectal lesions that includes hyperplastic polyps(HPs),sessile serrated adenoma(SSA),traditional serrated adenoma(TSA)and mixed *** are the most common serrated polyp followed by SSA and *** distinct histogenesis is believed to have a major influence in prevention strategies,patient prognosis and therapeutic ***,serrated polyps exhibited also a distinct pattern,with KRAS and BRAF having an important contribution to its *** other molecular changes that have been implicated in the serrated pathway include microsatellite instability and the CpG island methylator *** the present review we will address the current knowledge of serrated polyps,clinical pathological features and will update the most recent findings of its molecular *** understanding of their biology and malignancy potential is imperative to implement a surveillance approach in order to prevent colorectal cancer development.

Adjuvant Therapy on Cancer of the Lower Rectum. Evaluation of the Effects of Preoperative Radiotherapy on the Prognosis of Patients with Cancer of the Lower Rectum

Aims: The prognosis on treatment of the cancer of the rectum has not changed in the last fifty years. Survival rates of 50 to 55% seems immutable in several published series. The main cause for those results is the high incidence of recurrence, either local or widespread. Local recurrence is directly related to the number of undifferentiated cells and to the grade of wall invasion. Widespread recurrence depends specifically on the lymphatic and vascular spreading. So any kind of treatment that would diminish the number of undifferentiated cells and the size or the tumor wall penetration would certainly decrease the local recurrence rate, lengthening the interval free from cancer and, perhaps, modifying the long term survival rate. Between 1978 and 2009, a total of 538 patients with adenocarcinoma of the lower rectum (from the pectinate line to 10 cm above) were treated by preoperative radiotherapy. Methodology: The same protocol was used in all the patients – 400 cGy, 200 cGy/day, during 4 consecutive weeks (anterior and posterior pelvic fields) by means of a Linear Megavoltage Accelerator (25 MeV). Surgery was performed 2 months after completion of the radiotherapy. Results: Statistical analysis of the whole group showed that preoperative radiotherapy does decrease frequency of undifferentiated cells. Moreover, the incidence of local recurrence diminished after irradiation by 3.4%. Preoperative radiotherapy reduces tumor volume (ERUS) and wall invasion, as well as the mortality rate due to local recurrence (2.4%) and alters long-term survival rate (80.1%). Conclusion: Preoperative radiotherapy is really effective in reducing the number of undifferentiated cells and in diminishing the tumor volume and the carcinomatous infiltration of the rectal wall.

Neoadjuvant Radiotherapy in Stage I Cancer of the Lower Rectum

Introduction: The mortality rate in cancer of the lower rectum is related to the incidence of local recurrence, in the first 5 years. For stage I tumors, local excision has being increasingly used, but recent studies showed a higher incidence rate of local recurrence. Therefore, preoperative radiotherapy should be considered even for these tumors, as an attempt to prevent recurrence and provide cure. Objective: To show the effectiveness of neoadjuvant radiotherapy in stage I cancer of the lower rectum of a cohort population. Materials and Method: A cohort study in a prospective database was made with a total of 75 patients considered as stage I cancer of the lower rectum. Preoperative long course of 4500 cG radiotherapy was performed in this selected group of patients and followed up for a minimum period of five years. Results: Stage I/TI group had 27 patients. All of them presented complete response to the treatment and did not need to be submitted to surgery. Five years follow up with no recurrence. The stage I/TII group had 48 patients. After neoadjuvant radiotherapy, 8 patients had to be submitted to surgery for persistent tumor. All were submitted to full total local excision (FTLE), but anatomopathological examination showed no residual cancer. Conclusion: Preoperative long course of 4500 cG irradiation, not only reduced the local recurrence and mortality rate in lower rectal cancer, but also reduced indication for surgery in patients with stage I cancer of the lower rectum.

Epidemiological studies have found that the risk for cardiovascular disease is increased in patients with epilepsy. The Renin Angiontensin System (RAS), an important player in vascular tone control, is also involved in many neurological disorders, including seizures and epilepsy. Although it has been reported that Angiotensin II (Ang II) release and Angiotensin receptors expression are altered in many cerebral areas in patients/animal models with neurological disorders, there are no data on the vascular function. We evaluated Ang I and Ang II-mediated vascular responses and to correlate their contractile responses to the pres- ence of endothelium and the protein levels of components of the RAS (AT1, AT2, Mas and ACE) in aorta isolated from genetically epileptic rats (WAR strain). The major finding was that the vascular contractile response induced by Ang I and Ang II is endothelium-dependent. Ang II induced contractions in aortas from Wistar rats either with intact endothelium (E+) (1.16 ± 0.04 g, n = 6) and endothelium-denuded (E-) (1.24 ± 0.04 g, n = 6). Maximum contractile response (ME) induced by Ang I was lower in Wistar E+ (0.45 ± 0.03 g, n = 6) compared with Wistar E- (1.13 ± 0.08 g, n = 6). Ang I and Ang II failed to induce contraction in WAR E+, whereas the ME induced by Ang I in WAR E- was lower (0.52 ± 0.04 g, n = 11) than in the Wistar. ME induced by Ang II in aortas from WAR was also lower (0.40 ± 0.03 g, n = 11) compared with Wistar. AT1 receptor expression in both E+ WAR and Wistar was lower than in both E- WAR and Wistar. AT2 and Mas receptor expression was higher in Wistar E- and E+ as compared to WAR E- and E+. ACE expression was higher in both E+ WAR and Wistar, but it was lower in both E- WAR and Wistar. Endothelium impairs the contractile response induced by Angiotensin in WAR, suggesting that endothelial relaxing factors play important role on the aorta contraction.

This paper aims to conduct a study of the problems associated with the wear of the needles and fuel injection nozzles utilized in diesel engines. The wear found on the needles is mainly associated to impurities in the fuel oil and microcavitation occurred due to high pressure in the phase of the air compression for combustion of the combustible fluid. These pressures associated with the temperature and the fluid velocity results in the occurrence of vaporization, which releases shock waves that cause damage to the affected surface. The impurities solid particles from the fuel oil cause problems inside the nozzles as obstruction of the holes and wear on the needle tip and nozzle seat surface. These failures affect in the atomization of the fuel, since the deterioration of the internal passages of the nozzles interferes in the spray formation and in the end passage of the fluid. For the execution of this study it will be used digital microscopic analysis in specimens that suffered damage, in order to investigate the effects of fuel property, and the temperature conditions and pressure in the formation of the wear on the needles and injector nozzle.

Occupational Performance of Women with Chronic Pelvic Pain and the Potential of Coping as an Unassisted Intervention

Objective: To assess the satisfaction and occupational performance of women with chronic pelvic pain and to discuss unassisted intervention strategies by the occupational therapist for the care of these patients. Methods: A case-control study was conducted on 75 women with chronic pelvic pain and 75 apparently healthy women. The study was approved by the Research Ethics Committee of the institution and all subjects gave written informed consent to participate. Pain intensity was determined using a visual analogue scale and each patient was submitted to psycho-metric assessment using the Patient Health Questionnaire, the Self-Reporting Questionnaire of Psychiatric Screening and the Tampa Scale of Kinesiophofobia in their Brazilian version. The main outcomes regarding occupational performance and satisfaction were evaluated using the Canadian Measure of Occupational Performance. Results: The performance and satisfaction scores of women with chronic pelvic pain were significantly lower than those of healthy women. The presence of pain and kinesophobia was directly and independently correlated with low performance and satisfaction scores regardless of ethnicity, marital status, schooling, or psychometric scores. Conclusion: women with chronic pelvic pain present significant impairment of satisfaction and occupational performance. Coping is a potential unassisted intervention strategy to be applied to this population by occupational therapists.

Conservative Surgery for Subungual Melanoma In Situ Using Matriderm^(■)

Background: Standard treatment for subungual melanoma is wide local excision including digit amputation in order to obtain safety margins. Level of amputation has been discussed in order to achieve appropriate oncological results along with preservation of limb function wherein local excision of subungual melanoma in situ without amputation is an option. Methods: We report two cases of subungual melanoma in situ treated by using a conservative approach. Both cases were treated with local excision with the resected tumor defect covered with an artificial dermis, Matriderm?. One case was of a 65-year-old male with a left thumb subungual melanoma in situ and another of a 41-year-old female with the same type of melanoma in the left hallux. Full-thickness skin graft was used for reconstruction after 7 weeks from the first surgery in the latter, and in the former case re-epithelization occurred within 90 days after surgery. To date, no clinical signs of recurrence have been found, and finger/toe functions were completely preserved with a minimized scar and little graft bed sensitivity. Follow-up time is two years and ten months for the left thumb case, and one year and six months for the left hallux subungual melanoma in situ. Conclusion: We present two cases of subungual melanoma in situ treated with digit sparing surgery including excision of the periosteum followed by the use of an artificial dermal template to guarantee coverage of the tumor bed defect, providing good functional and cosmetic outcome.

Background:Hemodialysis(HD) per se is a risk factor for *** the growing body of evidence on blood-flow restriction(BFR) exercise in HD patients,identification of possible risk factors related to the prothrombotic agent D-dimer is required for the safety and feasibility of this training *** aim of the present study was to identify risk factors associated with higher D-dimer levels and to determine the acute effect of resistance exercise(RE) with BFR on this ***:Two hundred and six HD patients volunteered for this study(all with a glomerular filtration rate of 70 years old),and HD ***,RE+BFR significantly increases D-dimer after 4 *** with borderline baseline D-dimer levels(400-490 ng/mL) displayed increased risk of elevating D-dimer over the normal range(≥500 ng/mL).Conclusion:These results identified factors associated with a heightened prothrombotic state and may assist in the screening process for HD patients who wish to undergo RE+BFR.D-dimer and/or other fibrinolysis factors should be assessed at baseline and throughout the protocol as a precautionary measure to maximize safety during RE+BFR.
