Evaluation of proximal tubule functions in children with COVID‑19:a prospective analytical study

Background There are limited numbers of studies focusing on renal efects of coronavirus disease 2019(COVID-19)infection and proximal tubular dysfunction in children with COVID-19 infections.The purpose of this study was to evaluate the functions of the proximal tubule in hospitalized children with confrmed acute COVID-19.Methods The children who were hospitalized for confrmed COVID-19 were included in this prospective descriptive analysis.The presence of at least two of the following four abnormalities was used to diagnose proximal tubule injury:abnormal tubular reabsorption of phosphate,normoglycemic glycosuria,hyperuricosuria,and proteinuria.Results A total of 115 patients were included in the study.About a third of the individuals had elevated blood creatinine levels or proteinuria.In addition,abnormal renal tubular phosphate loss measured by renal tubular phosphate loss was found in 10(8.7%)patients,as was hyperuricosuria in 28.6%.As a result,total proximal tubular dysfunction was found in 24(20.9%)patients.Conclusions One in every fve children with acute COVID-19 infections had proximal tubular dysfunction,according to our data.Although,the rate of proximal tubular dysfunction was lower than in adults,it should be noted.The recovery of proximal tubular function in children with COVID-19 should be followed.

In this editorial review,we comment on the article published in the recent issue of the World Journal of Gastrointestinal Surgery.Carcinoembryonic antigen(CEA)is a fetal glycoprotein and can be secreted in very small amounts from healthy adults after birth.CEA is widely used not only for diagnostic tumor markers but also importantly for the management of some gastrointestinal tumors.The most common clinical use is surveillance for the monitoring of colorectal carcinoma.However,CEA can become elevated in several malign or benign characterized pathologies.Serum CEA level may vary depending on the location of the lesion,whether it metastasizes or not,and its histopathological characteristics.It has been determined that cases with high preoperative CEA have a more aggressive course and the risk of metastasis to the lymph tissue and liver increases.In this editorial review,we focused on evaluating the role of CEA in clinical practice with a holistic approach,including the diagnostic and prognostic significance of CEA in patients with focal liver lesions,the role of CEA in follow-up after definitive surgery,and also hepatic resection for metastasis,and the management of all patients with raised CEA.

Symbol Detection Based on Back Tracking Search Algorithm in MIMO-NOMA Systems

One of the most important methods used to cope with multipath fading effects,which cause the symbol to be received incorrectly in wireless communication systems,is the use of multiple transceiver antenna structures.By combining the multi-input multi-output(MIMO)antenna structure with non-orthogonal multiple access(NOMA),which is a new multiplexing method,the fading effects of the channels are not only reduced but also high data rate transmission is ensured.However,when the maximum likelihood(ML)algorithm that has high performance on coherent detection,is used as a symbol detector in MIMO NOMA systems,the computational complexity of the system increases due to higher-order constellations and antenna sizes.As a result,the implementation of this algorithm will be impractical.In this study,the backtracking search algorithm(BSA)is proposed to reduce the computational complexity of the symbol detection and have a good bit error performance for MIMO-NOMA systems.To emphasize the efficiency of the proposed algorithm,simulations have been made for the system with various antenna sizes.As can be seen from the obtained results,a considerable reduction in complexity has occurred using BSA compared to the ML algorithm,also the bit error performance of the system is increased compared to other algorithms.

Dealing with hepatic artery traumas:A clinical literature review

BACKGROUND The hepatic artery(HA)is one of the most threatened vascular structures during hepatopancreatobiliary(HPB)surgeries and interventional procedures.It can be affected by many clinical pictures,especially tumors,due to its anatomical position and neighborhood.AIM To reveal the evolution and recent developments in the management of HA traumas in the light of the literature.METHODS In this article,100 years of MEDLINE(PubMed)literature and articles including cases and series of HA injuries were reviewed,and the types of injury occurrence,treatment,and related complications and their management were compiled.RESULTS The risk of HA injury increases during cholecystectomies and pancreatoduodenectomies,among the most common operations.HA anatomy shows anomalies in approximately 15%-25%of the cases,further increasing this risk.The incidence of HA injury is not precisely known.Approaches that have evolved in recent years in managing patients with HA injury(laceration,transection,ligation,resection)with severe morbidity and mortality risk are reviewed in light of the current literature.CONCLUSION In conclusion,complications and deaths due to HA injury are less common today.The risk of complications increases in patients with hemodynamic instability,jaundice,cholangitis,and sepsis.Revealing the variations in the preoperative radiological evaluation will reduce the risks.In cases where HA injury is detected,arterial flow continuity should be tried to maintain with primary anastomosis,arterial transpositions,or grafts.In cases where bile duct injury develops,patients should be directed to HPB surgery centers,considering the possibility of accompanying HA injury.Large-scale and multicentric studies are needed to understand better the early and long-term results of HA ligation and determine preventive procedures.

A seed rain community in a reforested post-agricultural field and adjacent secondary forest of Mount Papandayan Nature Reserve,West Java,Indonesia

Understanding the mechanisms and barriers to the restoration of degraded land,especially post agriculture,will help provide protocols on effective ways of restoration into functional ecosystems.One of the barriers in early stages of forest restoration is the arrival and availability of propagules.Seed rain and factors affecting it(i.e.,distance to the forest edge,species diversity and surrounding vegetation)were measured in a reforested post-agricultural field and in an adjacent secondary forest.Multivariate g lm analysis was used on the seed rain community data and univariate lm analysis on the most abundant seed captured(i.e.,Schima wallichii(DC.)Korth.).After 8 months of seed rain collection,there was a total of 3596 seeds from eight tree species.Seeds were more abundant and more diverse in the secondary forest(74.9%,8 species)compared to the reforested field(24.1%,2 species).There was a limitation on seed dispersal in reforested field from the adjacent forest.The abundance of S.wallichii seeds determined by the dominance of adult trees(136 trees/ha in the forest and 115 trees/ha in the reforested field).Our study suggests,that after 7 years of planting,the reforested field has received limited seed rain and has not yet recovered.

Is omentectomy necessary in the treatment of benign or malignant abdominal pathologies?A systematic review

BACKGROUND The omentum is an organ that is easily sacrificed during abdominal surgery.The scope of omentectomy and whether a routine omentectomy should be performed are still unknown.AIM To review the literature in order to determine the physiological functions of the omentum and the roles it plays in pathological events in order to reveal the necessity for removal and preservation of the omentum.METHODS A clinical review of the English language literature based on the MEDLINE(PubMed)database was conducted using the keywords:“abdomen”,“gastrointestinal”,“tumor”,“inflammation”,“omental flap”,“metastasis”,“omentum”,and“omentectomy”.In addition,reports were also identified by systematically reviewing all references in retrieved papers.RESULTS The omentum functions as a natural barrier in areas where pathological processes occur in the abdominal cavity.The omentum limits and controls inflammatory and infectious pathologies that occur in the abdomen.It also aids in treatment due to its cellular functions including lymphatic drainage and phagocytosis.It shows similar behavior in tumors,but it cannot cope with increasing tumor burden.The stage of the disease changes due to the tumor mass it tries to control.Therefore,it is considered an indicator of poor prognosis.Due to this feature,the omentum is one of the first organs to be sacrificed during surgical procedures.However,there are many unknowns regarding the role and efficacy of the omentum in cancer.CONCLUSION The omentum is a unique organ that limits and controls inflammatory processes,foreign masses,and lesions that develop in the abdominal cavity.Omental flaps can be used in all anatomical areas,including the thorax,abdomen,pelvis,and extremities.The omentum is an organ that deserves the title of the abdominal policeman.It is generally accepted that the omentum should be removed in cases where there is tumor invasion.However,the positive or negative contribution of omental resection in the treatment of abdominal pathologies should be questioned.

Role of contrast-enhanced serial/spot abdominal X-rays in perioperative follow-up of patients undergoing abdominal surgery:An observational clinical study

BACKGROUND Many imaging methods such as ultrasonography,computed tomography(CT),magnetic resonance imaging,and endoscopy are used to identify the problems or complications that occur in the perioperative period and to determine the appropriate therapeutic approach.Specialists at surgical clinics and intensive care units sometimes need diagnostic procedures that can give quick results or reveal unexpected results.In particular,rapid on-site evaluation of patients followed under intensive care conditions has several advantages.AIM To determine the problems developing in patients in the perioperative period by contrast-enhanced abdominal X-ray(CE-AXR),revealing their current status or defining the effectiveness of CE-AXR.METHODS The files of the patients who underwent hepatopancreatobiliary or upper gastrointestinal surgery,whose CE-AXR film was taken,were reviewed retrospectively.Abdominal X-ray radiographs taken after ingestion of a watersoluble contrast agent(iohexol,300 mg,50 cc vial)and its application in a drain,nasogastric tube,or stent were evaluated.The contribution of the data obtained in patients who underwent CE-AXR to the diagnosis,follow-up,and treatment processes and the effectiveness of the application were investigated.RESULTS CE-AXR was applied to 131 patients in our clinic,most of whom underwent hepatopancreatobiliary or upper gastrointestinal surgery.It was determined that the data obtained from CE-AXR films taken in 98(74.8%)of the patients contributed to the diagnosis,treatment,and follow-up expectations and positively affected the clinical processes.CONCLUSION CE-AXR is a simple procedure that can be applied anywhere,especially in intensive care patients and at bedside,with a portable X-ray device.The simplicity of the procedure,less radiation exposure for the patients,less time wastage,reduction in the CT and endoscopy procedure burden and costs,quick results,rapid assessment of the situation,and enabling the monitoring of processes with repetitive procedures are important advantages.X-rays t

Optical, Energetic and Exergetic Analyses of Parabolic Trough Collectors

Parabolic trough collectors generate thermal energy from solar energy. Especially, they are very convenient for applications in high temperature solar power systems. To determine the design parameters, parabolic trough collectors must be analysed with optical analysis. In addition, thermodynamics （energy and exergy） analysis in the development of an energy efficient system must be achieved. Solar radiation passes through Earth＇s atmosphere until it reaches on Earth＇s surface and is focused from the parabolic trough collector to the tube receiver with a transparent insulated envelope. A11 of them constitute a complex mechanism. We investigate the geometry of parabolic trough reflector and characteristics of solar radiation to the reflecting surface through Earth＇s atmosphere, and calculate the collecting totM energy in the receiver. The parabolic trough collector, of which design parameters are given, is analysed in regard to the energy and exergy analysis considering the meteorological specification in May, June, July and August in 1sparta/TUrkey, and the results are presented.

Differences between radiological findings of COVID-19 and non-COVID-19 infections in pediatric patients

Background This study aimed to reveal the differences between coronavirus disease 2019(COVID-19)infections and non-COVID-19 respiratory tract infections in pediatric patients.Methods Sixty pediatric patients admitted to the hospital between March 11,2020 and April 15,2020 with respiratory tract infections were evaluated retrospectively.Among them,20 patients with reverse transcription-polymerase chain reaction(RT-PCR)tests and chest computed tomography(CT)examinations were included in the study.According to the RT-PCR test results,the patients were divided into the COVID-19 and non-COVID-19 groups.The clinical observations,laboratory results,and radiological features from the two groups were then compared.Results According to the RT-PCR test results,12 patients were assigned to the COVID-19 group and 8 to the non-COVID-19 group.There were no significant differences between the two groups in terms of clinical or laboratory features.In terms of radiological features,the presence of bronchiectasis and peribronchial thickening was statistically significantly higher in the non-COVID-19 group(P=0.010 and P=0.010,respectively).Conclusions In pediatric cases,diagnosing COVID-19 using radiological imaging methods plays an important role in deter-mining the correct treatment approach by eliminating the possibility of other infections.

Objective response rate assessment in oncology: Current situation and future expectations

The tumor objective response rate(ORR)is an important parameter to demonstrate the efficacy of a treatment in oncology.The ORR is valuable for clinical decision making in routine practice and a significant end-point for reporting the results of clinical trials.World Health Organization and Response Evaluation Criteria in Solid Tumors(RECIST)are anatomic response criteria developed mainly for cytotoxic chemotherapy.These criteria are based on the visual assessment of tumor size in morphological images provided by computed tomography(CT)or magnetic resonance imaging.Anatomic response criteria may not be optimal for biologic agents,some disease sites,and some regional therapies.Consequently,modifications of RECIST,Choi criteria and Morphologic response criteria were developed based on the concept of the evaluation of viable tumors.Despite its limitations,RECIST v1.1 is validated in prospective studies,is widely accepted by regulatory agencies and has recently shown good performance for targeted cancer agents.Finally,some alternatives of RECIST were developed as immune-specific response criteria for checkpoint inhibitors.Immune RECIST criteria are based essentially on defining true progressive disease after a confirmatory imaging.Some graphical methods may be useful to show longitudinal change in the tumor burden over time.Tumor tissue is a tridimensional heterogenous mass,and tumor shrinkage is not always symmetrical;thus,metabolic response assessments using positron emission tomography(PET)or PET/CT may reflect the viability of cancer cells or functional changes evolving after anticancer treatments.The metabolic response can show the benefit of a treatment earlier than anatomic shrinkage,possibly preventing delays in drug approval.Computer-assisted automated volumetric assessments,quantitative multimodality imaging in radiology,new tracers in nuclear medicine and finally artificial intelligence have great potential in future evaluations.
