Acute effects of partial-body vibration in sitting position

AIM To investigate the acute effects of sinusoidal and stochastic resonance partial-body vibration in sitting position, including muscle activity, heart rate variability, balance and *** Fifty healthy participants were assigned randomly to two training conditions: A sinusoidal partial-body vibration(SIN, 8 Hz) or a stochastic resonance partial-body vibration(STOCH, 8 ± 2 Hz). For baseline assessment participants sat on the vibration platform without vibration. Both training conditions consisted of five series of a one-minute vibration training and a one-minutebreak between them. In this experimental study surface electromyography(EMG) of the erector spinae(ES), one of the back muscles, and heart rate variability(HRV) was measured at baseline and during training. Balance and flexibility were assessed at baseline and immediately after training. Balance was measured with the modified star excursion balance test(mS EBT) and flexibility was assessed through the modified fingertip-to-floor method(mF TF).RESULTS Paired sample t-test showed a significant increase in balance that was restricted to STOCH(t =-2.22, P = 0.018; SIN: t =-0.09, P = 0.466). An increase in flexibility was also restricted to STOCH(t = 2.65, P = 0.007; SIN: t = 1.41, P = 0.086). There was no significant change of muscle activity in the ES-EMG in STOCH or SIN conditions. In both training conditions, HRV decreased significantly, but remained in a low-load range(STOCH: t = 2.89, P = 0.004; SIN: t = 2.55, P = 0.009).CONCLUSION In sitting position, stochastic resonance partial-body vibration can improve balance and flexibility while cardiovascular load is low. STOCH can be a valuable training option to people who are unable to stand(e.g., people, who are temporarily wheelchair-bound).

Methodological Issues for a Landscape-Changing Analysis in Adaptive Excess Water Management

Hungary is located in the deepest part of the Pannonian Basin,which is affected by *** areas are particularly vulnerable to *** natural and anthropogenic processes have contributed to the formation of these flood patches,after river regulation(decisively from the middle of 18th century)and continue to affect *** objective of this research is to reveal the processes of landscape-change in areas of waterlogging,or areas threatened by excess water inundation in order to establish methods of adaptive excess water *** analysis focuses on examining the previously prevalent land-use structure and linear infrastructure elements(green,blue infrastructures)of the past 200-250 years that can be identified using historical,military maps and aerial *** maps compare different periods of 50-60 years *** a result of this analysis,the role of watercourses in shaping landscapes and human intervention process can be monitored and supported with maps including *** processing consisted of geographic information system(GIS)methods:georeference the historical maps and digitalize the well-separable land-uses of the sample *** results confirmed the process of landscape-change and the trend of green areas:cultivated areas and the spread of *** conclusion,landscape-changing analyses of landscape-changes provide valuable data onto identifying changes in land-use,which are complemented by hydrological databases,especially those related to waterlogging areas,as they adequately support adaptive excess water management methods in areas with specific *** results show which land-uses can be considered as permanent,stable land-uses in the given sample area and broach whether changes within the green and blue infrastructure network contribute to the increase or decrease of excess water formation.

Combinatorial Optimization for Electric Vehicles Management

Growing concerns about environmental quality of cities are calling for sustainable road transportation technologies. EV （electric vehicles）, for public and private transport, can contribute significantly to the lowering of the current pollution levels. However, the EV use is currently facing several weaknesses among which are： limited driving range, high cost and overall limited efficiency. Electric vehicles management is a relatively recent problem; its purpose is to expedite the establishment of a costumer convenient, cost-effective, EV infrastructure. Inspire the relevance of the problem, a few small research communities in this field work on some of its aspects. In this work, some important issues of this problem are discussed and the contribution of combinatorial optimization tools for solving some challenging subproblems is studied.

Impact of Epidural Analgesia in Labour on Neonatal and Maternal Outcomes

Aim: To evaluate the effect of epidural analgesia during labour on neonatal-maternal outcomes. Methods: A retrospective cohort study of nulliparous parturients who gave birth in Vasternorrland County, Sweden, over a 2-year period between 2015 and 2016. Neonatal outcomes (Apgar score at 5 min and umbilical cord arterial blood gases), maternal outcomes (perineal injury, total bleeding volume and maternal satisfaction with birth) and labour parameters (mode of delivery and the durations of labour and postpartum hospital stay) were evaluated. Results: The study cohort consisted of 1449 women with singleton pregnancies. Patients were divided into two groups according to whether during labour they were administered epidural analgesia using bupivacaine and sufentanil (EDA group, n = 615) or not (non-EDA group, n = 834). The rate of assisted vaginal delivery was significantly higher in the EDA group than in the non-EDA group (15.6% and 11.3%, respectively, p < 0.05), whereas the rates of caesarean section were similar. The duration of the active phase of labour was significantly longer in the EDA group than in the non-EDA group (489 ± 217 min versus 371 ± 210 min, respectively, p < 0.001). The Apgar score at 5 min and umbilical cord blood pH were lower and the base deficit greater in the EDA group (p < 0.001, p < 0.001 and p < 0.01, respectively). Bleeding volume was similar between the groups after adjusting for gestational age. Women in the EDA group were more satisfied with their labour experience, as measured by the visual analogue scale (p Conclusion: The results of this study suggest that EDA affects delivery and neonatal-maternal outcomes negatively, but increases the mother’s satisfaction with labour.

Gastroesophageal reflux disease after diagnostic endoscopy in the clinical setting

AIM: To investigate the outcome of patients with symptoms of gastroesophageal reflux disease (GERD) referred for endoscopy at 2 and 6 mo post endoscopy. METHODS: Consecutive patients referred for upper endoscopy for assessment of GERD symptoms at two large metropolitan hospitals were invited to participate in a 6-mo non-interventional (observational) *** two institutions are situated in geographically and socially disparate areas. Data collection was by selfcompletion of questionnaires including the patient assessment of upper gastrointestinal disorders symptoms severity and from hospital records. Endoscopic finding using the Los-Angeles classification, symptom severity and it's clinically relevant improvement as change of at least 25%, therapy and socio-demographic factors were assessed. RESULTS: Baseline data were available for 266 patients and 2-mo and 6-mo follow-up data for 128 and 108 patients respectively. At baseline, 128 patients had erosive and 138 non-erosive reflux disease. Allmost all patient had proton pump inhibitor (PPI) therapy in the past. Overall, patients with non-erosive GERD at the index endoscopy had significantly more severe symptoms as compared to patients with erosive or even complicated GERD while there was no difference with regard to medication. After 2 and 6 mo there was a small, but statistically significant improvement in symptom severity (7.02 ± 5.5 vs 5.9 ± 5.4 and 5.5 ± 5.4 respectively); however, the majority of patients continued to have symptoms (i.e. , after 6 mo 81% with GERD symptoms). Advantaged socioeconomic status as well as being unemployed was associated with greater improvement. CONCLUSION: The majority of GORD patients receive PPI therapy before being referred for endoscopy even though many have symptoms that do not sufficiently respond to PPI therapy.
