In egypt,Sofosbuvir(SOF)in combination with Dataclasvir(DCV)is the broadly used DAAs with excellent therapeutic *** study is designed to explore the relation between IL28B/TLR4 genetic variants and each of the followings;HCC development post SOF/DCV treatment,progression to HCC in naı¨ve patients and SOF/DCV therapy outcome.A total of 493 blood samples were collected(controls(n = 70);HCV patients treated with SOF/DCV(n = 252)of whom 65 patients developed HCC,187 patients didn’t develop HCC(125 responders,62 relapsers);naı¨ve HCV patients(n = 171)had early(n = 48),late liver fibrosis(n = 21)and HCC(n = 102)).Both SNPs were genotyped using a TaqMan 50 allelic discrimination *** IL28B rs12979860 SNP,the C allele was significantly correlating with the response rate more than T allele(OR 1.9,95%CI 1.29e2.9,p=0.004),while at TLR4 rs4986791 SNP,no association was found(OR 6.5,95%0.57e75.28,p = 0.09).Both SNPs couldn’t detect the probability for HCC emergence after *** naı¨ve patients,the protective alleles were detected in their lowest frequency in HCC patients(p = 0.1,for rs12979860 and,p = 0.001 for rs4986791).SOF/DCV combination improved SVR rates in HCV genotype 4a infected patients regardless of IL28B genotype,with the best rates in those lacking the T allele.

AIM:To investigate cell cycle proteins in chronic hepatitis C virus infection in order to analyze their role in the process of hepatocyte transformation and to characterize their prognostic properties. MeTHODS:Subjects of the current study included 50 cases of chronic hepatitis C(CHC) without cirrhosis,30 cases of CHC with liver cirrhosis(LC) ,and 30 cases of hepatitis C-related hepatocellular carcinoma(HCC) admitted to the Department of Hepato-Gastroenterology,Theodor Bilharz Research Institute(TBRI) ,Giza,*** wedge liver biopsies,taken during laparoscopic cholecystectomy,were also included as normal *** investigations including urine and stool analysis,liver function tests and prothrombin concentration;serologic markers for viral hepatitis and ultrasonography were done for all cases of the study together with immunohistochemical analysis using primary antibodies against Cyclin D1,Cyclin e,p21,p27 and Rb/p105 proteins. ReSULTS:Normal wedge liver biopsies didn't express Cyclin e or Rb/p105 immunostaining but show positive staining for Cyclin D1,p21 and *** D1 expressed nuclear staining that was sequentially increased from CHC to LC(Pes;meanwhile,Cyclin e revealed nuclear positivity only in the case of HCCs patients that was directly correlated to Rb/p105 *** expression of p21 and p27 was significantly increased in CHC and LC cases compared to normal controls and HCCs with no significant difference between well-and poorlydifferentiated tumors.p21 showed only a nuclear pattern of staining,while,p27 presented with either cytoplasmic and/or nuclear reactivity in all studied *** analysis revealed a direct relation between Cyclin D1 and p21 in CHC cases(Petween Cyclin D1 and Cyclin e in HCCs(Pever,an inverserelationship was detected between Cyclin D1 and p21 or p27(Petween p21 and Rb/p105(Pegulation of Cyclin D1 in CHC p

The outbreak of Covid-19 has taken the lives of many patients so *** symptoms of COVID-19 include muscle pains,loss of taste and smell,coughs,fever,and sore throat,which can lead to severe cases of breathing difficulties,organ failure,and ***,the early detection of the virus is very ***-19 can be detected using clinical tests,making us need to know the most important symptoms/features that can enhance the decision *** this work,we propose a modified multilayer perceptron(MLP)with feature selection(MLPFS)to predict the positive COVID-19 cases based on symptoms and features from patients’electronic medical records(eMR).MLPFS model includes a layer that identifies the most informative symptoms to minimize the number of symptoms base on their relative *** the model with only the highest informative symptoms can fasten the learning process and increase *** were conducted using three different COVID-19 datasets and eight different models,including the proposed *** show that MLPFS achieves the best feature reduction across all datasets compared to all other experimented ***,it outperforms the other models in classification results as well as time.

AIM: To investigate the relationship between low molecular polypeptide-7 (LMP-7) gene polymorphism and response to interferon (IFN) therapy in chronic hepatitis C virus (HCV) patients. MeTHODS: LMP-7 polymorphism at codon 49 with nucleotide substitution from A to C was amplified in 104 chronic HCV patients of genotype 4. The amplicons were digested with restriction endonuclease Bsm I and the produced restriction fragment length polymorphism was analyzed. Patients received IFN + regional blood volume therapy for 48 wk and the frequency of thissingle nucleotide polymorphism (SNP) was statistically correlated with treatment response. The exclusion criteria for these patients were stated by the national health program for treating viral hepatitis. Main exclusion criteria included co-infection with hepatitis B virus or schistosomiasis, thyroid dysfunction, uncontrolled diabetes mellitus, history of long term drug or alcohol intake and autoimmune hepatitis. Multivariate analyses were done to correlate LMP-7 SNP plus several factors such as age, gender, weight, serum alpha-fetoprotein (AFP) and alanine aminotransferase levels, liver activity, fibrosis score and viral load with response to therapy. ReSULTS: The data presented in this study clearly demonstrated statistically significant differences between sustained virological response (SVR) (defined as the absence of HCV RNA levels in the patient's sera at least 6 mo after discontinuation of treatment) and non-response (NR) (where HCV RNA levels in the patient's sera never become undetectable for 6 mo during or after treatment). Variables were described as odds ratio with 95%CI. Internet of Medical Things (IoMT) is a breakthrough technologyin the transfer of medical data via a communication system. Wearable sensordevices collect patient data and transfer them through mobile internet, thatis, the IoMT. Recently, the shift in paradigm from manual data storage toelectronic health recording on fog, edge, and cloud computing has been *** advanced computing technologies have facilitated medical services withminimum cost and available conditions. However, the IoMT raises a highconcern on network security and patient data privacy in the health caresystem. The main issue is the transmission of health data with high security inthe fog computing model. In today’s market, the best solution is blockchaintechnology. This technology provides high-end security and authenticationin storing and transferring data. In this research, a blockchain-based fogcomputing model is proposed for the IoMT. The proposed technique embedsa block chain with the yet another consensus (YAC) protocol building securityinfrastructure into fog computing for storing and transferring IoMT data inthe network. YAC is a consensus protocol that authenticates the input datain the block chain. In this scenario, the patients and their family membersare allowed to access the data. The empirical outcome of the proposedtechnique indicates high reliability and security against dangerous *** major advantages of using the blockchain model are high transparency,good traceability, and high processing speed. The technique also exhibitshigh reliability and efficiency in accessing data with secure transmission.
