Assessing vulnerability to panic:a systematic review of psychological and physiological responses to biological challenges as prospective predictors of panic attacks and panic disorder

Background Cognitive-behavioural theories of panic disorder posit that panic attacks arise from a positive feedback loop between arousal-related bodily sensations and perceived *** a recently develope computational model formalising these theories of pani attacks,it was observed that the response to a simulate perturbation to arousal provided a strong indicator of vulnerability to panic attacks and panic *** this/review,we evaluate whether this observation is borne o in the empirical literature that has examined responses biological challenge(eg,C02 inhalation)and their relatio to subsequent panic attacks and panic *** We searched PubMed,Web of Science and PsycINFO using keywords denoting provocation agents(eg,sodium lactate)and procedures(eg,infusion)combined with keywords relevant to panic disorder(eg,panic).Articles were eligible if they used response to a biologic challenge paradigm to prospectively predict panic attac or panic *** We identified four eligible *** effect sizes suggest that there is biological challenge response a moderate prospective association with subsequent pani attacks,but no prospective relationship with panic disorde Conclusions These findings provide support for the prediction derived from cognitive-behavioural theories an some preliminary evidence that response to a biological challenge may have clinical utility as a marker of vulnerab to panic attacks pending further research and developme Trial registration number 135908.

AIM To investigate hepatitis C virus(HCV) testing recommendations from the United States and other high-income *** A comprehensive search for current HCV testing recommendations from the top quartile of United Nations Human Development Index(HDI) countries(very high HDI) was performed using Google and reviewed from May 1-October 30, 2014 and re-reviewed April 1-October 2, 2017. RESUlTS Of the 51 countries identified, 16 had HCV testing recom-mendations from a government body or recommendations issued collaboratively between a government and a medical organization. Of these 16 countries, 15 had HCV testing recommendations that were primarily risk-based and highlight behaviors, exposures, and conditions that are associated with HCV transmission in that region. In addition to risk-based testing, the HCV Guidance Panel(United States) incorporates recommendations for aone-time test for individuals born during 1945-1965(the birth cohort) without prior ascertainment of risk into their guidance. In addition to the United States, six other countries either have an age-based testing recommendation or recommend one-time testing for all adults independent of risk factors typical of the region. CONClUSION This review affirmed the similarities of the HCV Guidance Panel's guidance with those of recommendations from very high HDI countries.

AIM:To investigate possible differences in dendritic cells(DC)within intestinal tissue of mice before and after induction of colitis. METHODS:Mucosal DC derived from intestinal tissue,as well as from mesenteric lymph nodes and spleen,were analyzed by fluorescence activated cell sorting(FACS) *** of these cells were analyzed for secretion of different pro-and anti-inflammatory cytokines. Immunohistochemistry and immunofluorescence were performed on cryosections of mucosal tissue derived from animals with colitis as well as from healthy mice. RESUlTS:It was shown that DC derived from healthy intestinal lamina propria(lP)represented an immature phenotype as characterized by low-level expression of costimulatory *** contrast to DC from spleen and mesenteric lymph nodes(MlN)that secreted proinflammatory cytokines,lP-DC produced high levels of the anti-inflammatory cytokine *** induction of mu-rine colitis in a CD4 + CD62l + transfer model or in chronic Dextran sulfate sodium-colitis,a marked increase of activated CD80 + DC could be observed within the inflamed colonic ***,in contrast to splenic DC,a significant population of DC within MlN and colonic lP expressed the mucosal integrin CD103 which was lost during colitis. CONClUSION:The constitutive secretion of antiinflammatory cytokines by immature DC within the intestinal lP might regulate the homeostatic balance between mucosal immunity and ***103 + DC could mediate this important function.

AIM:To evaluate the role of sequential therapy and lactobacillus reuteri (l. reuteri ) supplementation, in the eradication treatment of Helicobacter pylori (H. pylori ). METHODS:H. pylori infection was diagnosed in 90 adult dyspeptic patients. Patients were excluded if previously treated for H. pylori infection or if they were taking a proton pump inhibitor (PPI), H2-receptor antagonist or antibiotics. Patients were assigned to receive one of the following therapies:(1) 7-d triple therapy (PPI plus clarithromycin and amoxicillin or metronidazole) plus l. reuteri supplementation dur- ing antibiotic treatment; (2) 7-d triple therapy plus l. reuteri supplementation after antibiotic treatment; (3) sequential regimen (5-d PPI plus amoxicillin therapy followed by a 5-d PPI, clarithromycin and tinidazole) plus l. reuteri supplementation during antibiotic treatment; and (4) sequential regimen plus l. reuteri supplementation after antibiotic treatment. Successful eradication therapy was defined as a negative urea breath test at least 4 wk following treatment. RESUlTS:Ninety adult dyspeptic patients were en- rolled, and 83 (30 male, 53 female; mean age 57 ± 13 years) completed the study. Nineteen patients were administered a 7-d triple treatment:11 with l. reuteri supplementation during and 8 after therapy. Sixty-four patients were administered a sequential regimen:32 with l. reuteri supplementation during and 32 after therapy. The eradication rate was significantly higher in the sequential group compared with the 7-d triple regimen (88% vs 63%, P = 0.01). No difference was found between two types of PPI. No difference in erad- ication rates was observed between patients submitted to l. reuteri supplementation during or after antibiotic treatment. Compliance with therapy was excellent in all patients. Subclinical hypothyroidism (SCHT) is common, with an occurrence of up to 10% of the adult population and defined biochemically only by elevated TSH and normal T4. SCHT affects negatively on lipid and carbohydrate metabolism increasing the risk of ischemic heart disease, affects negatively on cardiac performance and have a close correlation with renal function. The aim of this study is to compare presentation characteristics and outcomes between euthyroid pts and pts with SCHT who underwent cardiac surgery. Methods: 474 pts from June 2003 through September 2004 had TSH and T4 measured. 365 pts were euthyroid (Group 1), 41 pts had SCHT (Group 2). Groups were compared by demographics and EuroSCORE (ES) risk profiles. Operative and hospital outcomes were compared as was follow-up mortality up to 96 months. Results: There were more females in Group 2, p = 0.04, more pts with CHF and number of NYHA III-IV pts (p levated s-crea (p l fibrillation (p = 0.007). Comparing the Groups by EuroSCORE (ES) showed higher risk scores in Group 2 pts (Additive ES 6.8 vs 8.5 and logistic ES 12.3% vs 18.1%, p = 0.01 and 0.03). Hospital mortality was higher in Group 2 (12.2% vs 4.1%, p = 0.04) and the number of pts needing extended care was higher in Group 2 (p = 0.01). Follow up mortality was doubled in Group 2 pts up to 96 months compared to Group 1 (p lusions: Presentation characteristics and risk scores are different and worse in SCHT pts compared with euthyroid pts. Hospital and follow-up mortality are increased in SCHT pts.

Magnetic resonance imaging(MRI)is a clinically relevant,real-time imaging modality that is frequently utilized to assess stroke type and ***,specific MRI biomarkers that can be used to predict long-term functional recovery are still a critical ***,the present study sought to examine the prognostic value of commonly utilized MRI parameters to predict functional outcomes in a porcine model of ischemic *** was induced via permanent middle cerebral artery *** 24 hours post-stroke,MRI analysis revealed focal ischemic lesions,decreased diffusivity,hemispheric swelling,and white matter *** deficits including behavioral abnormalities in open field and novel object exploration as well as spatiotemporal gait impairments were observed at 4 weeks *** graphical models identified specific MRI outputs and functional recovery variables,including white matter integrity and gait performance,that exhibited strong conditional *** correlation analysis revealed a prognostic relationship between lesion volume and white matter integrity and novel object exploration and gait ***,these analyses may also have the potential of predicting patient recovery at chronic time points as pigs and humans share many anatomical similarities(e.g.,white matter composition)that have proven to be critical in ischemic stroke *** study was approved by the University of Georgia(UGA)Institutional Animal Care and Use Committee(IACUC;Protocol Number:A2014-07-021-Y3-A11 and 2018-01-029-Y1-A5)on November 22,2017.

This brief overview highlights the global crisis of perinatal psychiatric illness(PPI).PPI is a major contributor to many adverse pregnancy,childbirth,and childhood development *** contributes to billions of dollars in spending worldwide each year and has a significant impact on the individual,their family,and their *** is also highly *** recommendations for intervention and management of PPI are limited and vary considerably from country to ***,there are several significant challenges associated with implementation of these *** challenges are magnified in number and consequence among women of color and/or minority populations,who experience persistent and negative health disparities during pregnancy and the postpartum *** paper aims to provide a broad overview of the current state of recommendations and implementation challenges for PPI and layout a framework for overcoming these *** equityinformed model of care that provides universal intervention for pregnant women may be one solution to address the preventable consequences of PPI on child and maternal ***,this model emphasizes the importance of managing and eliminating known barriers to traditional health care *** and contextually specific challenges must be overcome to fully realize the impact of improved management of PPI.
