Bi-revealed utilities in a defaultable universe: A new point of view on consumption

This paper investigates the inverse problem of bi-revealed utilities in a defaultable universe,defined as a standard universe(represented by a filtration F)perturbed by an exogenous defaultable time τ.We assume that the standard universe does not take into account the possibility of the default,thus τ adds an additional source of *** defaultable universe is represented by the filtration G up to time τ(τ included),where G stands for the progressive enlargement of F by *** basic assumption in force is that τ avoids F-stopping *** bi-revealed problem consists in recovering a consistent dynamic utility from the observable characteristic of an *** general results on bi-revealed utilities,first given in a general and abstract framework,are translated in the defaultable G-universe and then are interpreted in the *** decomposition of G-adapted processes X^(G) provides an interpretation of a Gcharacteristic X^(G)_(τ) stopped at τ as a reserve *** to the characterization of G-martingales stopped at τ in terms of F-martingales,we establish a correspondence between G-bi-revealed utilities from characteristic and F-bi-revealed pair of utilities from characteristic and *** a financial framework,characteristic can be interpreted as wealth and reserves as *** result sheds a new light on the consumption in utility criterion:the consumption process can be interpreted as a certain quantity of wealth,or reserves,that are accumulated for the financing of losses at the default time.

Stenosis of the Subglottic Trachea Revealing Relapsing Polychondritis of a Young Adult

Relapsing polychondritis is a rare inflammatory disease involving essentially cartilaginous structures. Other systemic manifestations can be encountered as the eye and ear disturbance. Aortic aneurysms affect few cases. If affection of airway cartilages occurs, prognosis may be worsened by eventual stenotic lesions. We report a 22-year-old woman with Relapsing polychondritis. When she was referred to our hospital 4 weeks after the onset of respiratory symptoms, she was having severe breathing difficulty. Immediate tracheostomy followed by steroid therapy improved her respiratory condition, although the treatment was complicated because of her diabetes. While airway involvement of Relapsing polychondritis can be life threatening, it is curable with steroid therapy. Clinicians should keep in mind that airway obstruction could be caused by this disease.

The impact of round window vs cochleostomy surgical approaches on interscalar excursions in the cochlea:Preliminary results from a flat-panel computed tomography study

UNASSIGNED:To evaluate incidence of interscalar excursions between round window (RW) and cochleostomy approaches for cochlear implant (CI) insertion.;UNASSIGNED:This was a retrospective case-comparison. Flat-panel CT (FPCT) scans for 8 CI users with Med-el standard length electrode arrays were collected. Surgical technique was identified by a combination of operative notes and FPCT imaging. Four cochleae underwent round window insertion and 4 cochleae underwent cochleostomy approaches anterior and inferior to the round window.;UNASSIGNED:In our pilot study, cochleostomy approaches were associated with a higher likelihood of interscalar excursion. Within the cochleostomy group, we found 29% of electrode contacts (14 of 48 electrodes) to be outside the scala tympani. On the other hand, 8.5% of the electrode contacts (4 of 47 electrodes) in the round window insertion group were extra-scalar to the scala tympani. These displacements occurred at a mean angle of occurrence of 364° ± 133°, near the apex of the cochlea. Round window electrode displacements tend to localize at angle of occurrences of 400° or greater. Cochleostomy electrodes occurred at an angle of occurrence of 19°-490°.;UNASSIGNED:Currently, the optimal surgical approach for standard CI electrode insertion is highly debated, to a certain extent due to a lack of post-operative assessment of intracochlear electrode contact. Based on our preliminary findings, cochleostomy approach is associated with an increased likelihood of interscalar excursions, and these findings should be further evaluated with future prospective studies.

Meso-American Nephropathy in el Salvador: Increasing Farmer Participation in MeN Prevention Efforts

Objectives: This paper highlights and discusses major factors affecting Salvadoran farmers’ involvement in National Health System-led efforts to prevent MeN in el Salvador and includes some recommendations to enhance their involvement. Methods: The study, conducted in el Salvador in June 2018, involved 10 life-long farmers residing in five MeN-affected communities in Chalatenango and Usulutan Departments who were either MeN-free or MeN-controlled, to understand through semi-structured, qualitative interviews their experience of participation in broader epidemic control efforts. Complementary interviews held with two nephrology teams and a senior representative of the grassroots-level National Health Forum operating in the same area, provided necessary contrast. Interview questions were informed by reference to the Adaptation and Development after Persecution and Trauma (ADAPT) model [1] as applicable in post-conflict environments like el Salvador to analyze contextual factors influencing community participation. Findings: Farmers are scarcely involved in wider MeN prevention efforts in el Salvador despite greater National Health System emphasis on encouraging community participation since the approval of the 2009 Health Reform [2]. This study found that widespread insecurity due to gang warfare, declining family and social networks due to high murder rates and international migration, and unresolved sense of injustice over unremitting poverty are among major factors with potential destructive effect on farmer participation. Conclusions: Health system-led MeN prevention responses need to encompass attention to persisting social vulnerabilities in economically and socially marginalized communities in order to enhance farmer participation in this effort. This paper concludes with some recommendations on how this can be done.

Somatic molecular changes and histo-pathological features of colorectal cancer in Tunisia

AIM:To determine correlations between family history,clinical features and mutational status of genes involved in the progression of colorectal cancer(CRC).METHODS:Histo-pathological features and molecular changes[KRAS,BRAF and CTNNB1 genes mutations,microsatellite instability(MSI)phenotype,expression of mismatch repair(MMR)and mucin(MUC)5AC proteins,mutation and expression analysis of TP53,MLH1promoter hypermethylation analysis]were examined in a series of 51 unselected Tunisian CRC patients,10 of them had a proven or probable hereditary disease,on the track of new tumoral markers for CRC susceptibility in Tunisian ***:As expected,MSI and MMR expression loss were associated to the presence of familial CRC(75%vs 9%,Pelationship has been observed between the presence of MSI and TP53 accumulation(10.0%vs 48.8%,P=0.0335)in CRC tumors,suggesting different molecular pathways to CRC that in turn may reflect different environmental ***,MUC5AC expression was significantly associated to the presence of MSI(46.7%vs 8.3%,P=0.0039),MMR expression loss(46.7%vs8.3%,P=0.0039)and the presence of familial CRC(63%vs 23%,P=0.039).CONCLUSION:These findings suggest that MUC5AC expression analysis may be useful in the screening of Tunisian patients with high risk of CRC.

Extraprofessional and Occupational Risk Factors for Colorectal Cancer

Colorectal cancers (CRC) account for frequent and serious cancers which result from the interaction between individual genetic factors and environmental factors, and in particular widely studied nutritional ones. The role of other occupational factors remains a controversial subject. The objective of this study is to evaluate the possible impact of occupational factors on the risk of developing CRC. Materials and Methods: This is a retrospective case-control study. The cases and the control group were enlisted in the general surgical ward of Farhat Hached Teaching Hospital of Sousse (Tunisia) during the period extending from 2004 to 2008, and they were age and gender-matched. The data were analyzed using SPSS 11.0 software with a signification threshold fixed at 5%. A univariate analysis was carried out as well as a multiple binary logistical regression. Results: During the period of the study, 40 cases of colorectal cancers have been colligated including 28 men and 12 women with a sex ratio of 0.43. The average age of the cases was 61.55 ± 13.3 years and 60.40 ± 12.84 years for the control group, with a non significant difference (P = 0.69). The univariate analysis has objectivized significant associations between colorectal cancer and the housing conditions, the neoplastic and digestive family history, the occupational activity sector, exposure to pesticides, and lack of periodic medical supervision. After logistical regression, the occurrence risk of CRC was significantly associated with: alcohol and smoking (ORa = 3.43; Pa = 0.05), meat consumption (ORa = 3.34; Pa = 0.03), exposure to pesticides (ORa = 20.44; Pa = 0.012) and lack of periodic medical supervision (OR = 7.45; P = 0.004). Conclusion: The occupational risk factors might play a role in the etiopathogenesis of colorectal cancers.
