Affective responses to supervised 10-week programs of resistance exercise in older adults

Background:Resistance exercise has numerous,well-documented benefits on the health and functional performance of older ***,little information exists on the affective responses to resistance exercise in this *** affective responses can predict continued exercise behavior,examining if and how they differ between resistance exercise intensities and frequencies in older adults may provide important data to improve resistance exercise ***:We monitored the affective responses of older adults when performing high-velocity,low-load(HVLL)or low-velocity,high-load(LVHL)resistance exercise once or twice weekly.A total of 40 moderately to highly active,but resistance-exercise naïve,older adults(60-79 years)completed 1 of 4 randomly assigned progressive resistance exercise conditions:HVLL once weekly(HVLL1:n=10),HVLL twice weekly(HVLL2:n=10),LVHL once weekly(LVHL1:n=10),or LVHL twice weekly(LVHL2:n=10).The Physical Activity Affect Scale,Felt Arousal Scale,Feeling Scale,rating of perceived exertion,Visual Analog Scale,and the Physical Activity Enjoyment Scale were used to assess enjoyment and affective ***:The results from Physical Activity Enjoyment Scale(out of 35)suggest that all exercise conditions were largely and similarly enjoyable to older adults(HVLL1:30.9±2.4;LVHL1:31.9±3.6;HVLL2:30.9±3.5;LVHL2:30.2±3.7)despite greater fatigue(p=0.033;ηp^2p=0.22)and perceived workload(p=0.042;ηp^2=0.20)in LVHL(results from Visual Analog Scale).Conclusion:Moderately to highly physically active older adults may tolerate higher intensities of resistance exercise performed once or twice weekly without experiencing a negative impact on ***,the role that supervision and social interaction played in shaping the participants’responses is unclear.

British Soccer Academy Personnel Perceive Psychological and Technical/Tactical Attributes as the Most Important Contributors to development

Purpose Understanding the desired attributes for talented soccer players may give insight into the process of(de)selection and player *** study aimed to explore soccer academy personnel’s perceptions of attributes associated with talent and *** Thirty English and Scottish academy personnel(managers,coaches,recruitment,sports scientists)provided per-ceptions into what attributes contribute to‘talent’,via an online *** an E-delphi method,seven experts refined these inputs over several rounds until a consensus was reached,resulting in 82 agreed *** terminology was resubmitted via a second online survey,where 45 academy personnel rated each using a Likert *** A ranking of attributes by importance was produced,finding Psychological and Technical/Tactical attributes con-sidered of greatest *** were observed,whereby recruitment personnel consistently over-emphasised the importance of several attributes compared to other personnel(P=0.02-0.04).When analyzed within each age phase,11 variations in the perceived importance of attributes were demonstrated in the youth phase(11-16 years,P=0.01)compared with 5 in the professional phases(17-23 years,P=0.01-0.05)Conclusion The present study demonstrates that‘talent’requires multifaceted developments,with academy personnel per-ceiving psychological attributes the most important contributor to development.

Multimodality treatment of potentially curative gastric cancer: Geographical variations and future prospects

After much controversy, multimodality therapy is now accepted worldwide as the gold standard for treatment of resectable bulky localized gastric cancer. There is significant regional variation in the style of multimodality treatment with adjuvant chemoradiation the North American standard, neoadjuvant chemotherapy preferred in Europe and Australasia, whilst adjuvant chemotherapy is preferred in Asia. With further standardization of surgery and d1+/d2 resections increasingly accepted world wide, and in particular in the West, as the surgical standard of care for potentially curable disease, it is timely to reassess the multimodality regimes being used. The challenge in the use of multimodality therapy is how current outcomes can be standardized and improved further. Recent studies indicate that mere intensification of the regime in time, dosage or addition of further agents does not improve localized gastric cancer outcomes. More novel strategies including early commencement of adjuvant therapies, intra-peritoneal chemotherapy or assessing neoadjuvant response with positron emission tomography scanning may give improvements in outcomes. The introduction of targeted therapies means that the adjuvant use of biological agents needs to be explored. By proper assessment of the patient’s co-morbidities, full tumour staging, and a better understanding of the tumour’s molecular pathology, multimodality therapy for gastric adenocarcinoma may be individualized to optimize the likelihood of cure.

Colorectal cancer surveillance in inflammatory bowel disease:Practice guidelines and recent developments

Patients with long-standing inflammatory bowel disease(IBd)involving at least 1/3 of the colon are at increased risk for colorectal cancer(CRC).Advancements in CRC screening and surveillance and improved treatment of IBd has reduced CRC incidence in patients with ulcerative colitis and Crohn’s *** cases of CRC are thought to arise from dysplasia,and recent evidence suggests that the majority of dysplastic lesions in patients with IBd are visible,in part thanks to advancements in high definition colonoscopy and *** practice guidelines have supported the use of chromoendoscopy with targeted biopsies of visible lesions rather than traditional random *** are encouraged to endoscopically resect visible dysplasia and only recommend surgery when a complete resection is not *** technologies such as virtual chromoendoscopy are emerging as potential tools in CRC *** with IBd at increased risk for developing CRC should undergo surveillance colonoscopy using new approaches and techniques.

NMR relaxation analysis provides a unique and non-invasive probe of fluid dynamics within porous materials,and may be applied to the interpretation of a wide variety of material and interfacial ***,we report two-dimensional^(1)H T_(1)-T_(2)relaxation correlation measurements of a range of three-carbon adsorbates(1-propanol,2-propanol and propanoic acid)imbibed within the mesoporous metal oxide *** data,acquired across field strengths of 2 MHz,12.7 MHz and 43 MHz,clearly reveal two populations in each measurement,identified as the alkyl and hydroxyl moieties of each *** results expand the range of materials in which such functional group resolved relaxation is known to occur,and demonstrate the clear persistence of such phenomena using a range of typical benchtop NMR systems employed to study fluid-saturated porous media.

Prostate cancer （PCa） preferentially metastasizes to the bone marrow stroma of the axial skeleton. This activity is the principal cause of PCa morbidity and mortality. The exact mechanism of PCa metastasis is currently unknown, although considerable progress has been made in determining the key players in this process. In this review, we present the current understanding of the molecular processes driving PCa metastasis to the bone.

An uncommon source of upper gastrointestinal bleeding:epiphrenic esophageal diverticulum

Esophageal diverticula are rare findings that have an estimated incidence of 1 per 500000 people per year,even though acute upper gastrointestinal bleeding is a relatively common medical emergency with an incidence of up to 150 per 100000 people per year and a mortality rate of 7-14%.An 83-year-old man presented with hematemesis and *** upper endoscopy revealed an esophageal diverticulum,within which was an adherent *** of the clot identified a bleeding vessel within the diverticulum;this was successfully clipped and hemostasis was *** the best of our knowledge,this is the first case report of a bleeding epiphrenic esophageal diverticulum that was successfully managed endoscopically with hemostatic clips *** rare,our case serves as a reminder that bleeding epiphrenic esophageal diverticula can present as massive upper gastrointestinal bleeding and urgent endoscopic therapy can be life-saving.

Hepatic lipogranulomas in patients with chronic liver disease: Association with hepatitis C and fatty liver disease

AIM: To study the significance and clinical implication of hepatic lipogranuloma in chronic liver diseases, including fatty liver disease and hepatitis C. METHOdS: A total of 376 sequential, archival liver biopsy specimens were reviewed. Lipogranuloma, steatosis and steato-fibrosis were evaluated with combined hematoxylin and eosin and Masson’s trichrome staining. RESULTS: Fifty-eight (15.4%) patients had lipogranuloma, including 46 patients with hepatitis C, 14 patients with fatty liver disease, and 5 patients with other diseases. Hepatic lipogranuloma was more frequently seen in patients with hepatitis C (21%) and fatty liver disease (18%), and its incidence was significantly higher than that in control group (P d P ddition, 39 out of the 58 patients with lipogranuloma were associated with steatosis and/or steato-fibrosis. Of the 18 lipogranuloma patients with clinical information available for review, 15 (83%) had risk factors associated with fatty liver disease, such as alcohol use, obesity, hyperlipidemia, and diabetes mellitus. Although the incidence of these risk factors was greater in patients with lipogranuloma than in control group (60%), it did not reach statistical significance. CONCLUSION: Hepatic lipogranuloma is not limited to mineral oil use and commonly associated with hepatic steatosis, hepatitis C and fatty liver disease. With additional histological features of steato-fibrosis, lipogranuloma can also be used as a marker of prior hepatic steatosis.

Controlling human platelet activation with calcium-binding nanoparticles

Human platelets aggregate at sites of blood vessel damage in response to a rise in their cytosolic calcium *** these cytosolic calcium rises would provide a method to inhibit platelet activation and prevent the unwanted blood clots that causes heart attack and *** we have predicted that calcium accumulation within the lumen of an infolded portion of the platelet plasma membrane called the open canalicular system(OCS)is essential for maintaining this cytosolic calcium *** to its nanometer dimensions of the OCS,it has been difficult to measure or interfere with the predicted luminal calcium *** we utilise iron oxide magnetic nanoparticles coated with the known calcium chelator,citrate,to create calcium-binding *** were used to assess whether an OCS calcium store plays a role in controlling the dynamics of human platelet activation and *** demonstrate that citrate-coated nanoparticles are rapidly and selectively uptaken into the OCS of activated human platelets,where they act to buffer the accumulation of calcium *** with these calcium-binding nanoparticles reduced thrombin-evoked cytosolic calcium rises,and slowed platelet aggregation and clot retraction in human *** contrast,nanoparticles that cannot bind calcium have no *** study demonstrates that the OCS acts as a key source of calcium for maintaining cytosolic calcium rises and accelerating platelet aggregation,and that calcium-binding nanoparticles targeted to the OCS could provide an anti-platelet therapy to treat patients at risk of suffering heart attacks or strokes.
