Enhanced recovery after surgery protocols for outpatient operations in otolaryngology:Review of literature

Objective:Enhanced recovery after surgery(ERAS)protocols are patient-centered,evidence-based pathways designed to reduce complications,promote recovery,and improve outcomes following *** protocols have been successfully applied for the management of head and neck cancer,but relatively few studies have investigated the applicability of these pathways for other outpatient procedures in *** goal was to perform a systematic review of available evidence reporting the utility of ERAS protocols for the management of patients undergoing outpatient otolaryngology ***:A systematic literature review was conducted using MEDLINE,EMBASE,SCOPUS,and gray *** identified studies that evaluated ERAS protocols among patients undergoing otologic,laryngeal,nasal/sinus,pediatric,and general otolaryngology *** assessed the outcomes and ERAS components across protocols as well as the study design and ***:A total of eight studies fulfilled the inclusion criteria and were included in the *** of procedures evaluated with ERAS protocols included tonsillectomy and adenoidectomy,functional endoscopic sinus surgery,tympanoplasty and mastoidectomy,and septoplasty.A reduction in postoperative length of stay and hospital costs was reported in two and three studies,*** studies between ERAS and control groups showed persistent improvement in pre-and postoperative anxiety and pain levels,without an increase in postoperative complications and readmission ***:A limited number of studies discuss implementation of ERAS protocols for outpatient operations in *** clinical pathways appear promising for these procedures as they may reduce length of stay,decrease costs,and improve pain and anxiety postoperatively.

Modern internet search analytics and thyroidectomy:What are patients asking?

Objectives:Thyroidectomy is among the most commonly performed head and neck surgeries,however,limited existing information is available on topics of interest and concern to *** Design:***:***:A search engine optimization tool was utilized to extract metadata on Google‐suggested questions that“People Also Ask”(PAA)pertaining to“thyroidectomy”and“thyroid surgery.”These questions were categorized by Rothwell criteria and topics of *** Journal of the American Medical Association(JAMA)benchmark criteria enabled quality ***:A total of 250 PAA questions were ***‐oriented PAA questions describing what to expect during and after the surgery on topics such as postoperative management,risks or complications of surgery,and technical details were significantly less popular among the“thyroid surgery”group(Pvely).PAA questions about scarring and hypocalcemia were nearly threefold more popular than those related to pain(335 and 319 vs.113 combined search engine response page count,respectively).The overall JAMA quality score remained low(2.50±1.07),despite an increasing number of patients searching for“thyroidectomy”(r(77)=0.30,P=0.007).Conclusions:Patients searching for the nonspecific term“thyroid surgery”received a curated collection of PAA questions that were significantly less likely to educate them on what to expect during and after surgery,as compared to patients with higher health literacy who search with the term“thyroidectomy.”This suggests that the content of PAA questions differs based on the presumed health literacy of the internet user.

Climate-change habitat shifts for the vulnerable endemic oak species(Quercus arkansana Sarg.)

Quercus arkansana(Arkansas oak)is at risk of becoming endangered,as the total known population size is represented by a few isolated *** potential impact of climate change on this species in the near future is high,yet knowledge of its predicted effects is *** study utilized the biomod2 R package to develop habi-tat suitability ensemble models based on bioclimatic and topographic environmental variables and the known loca-tions of current distribution of *** predicted suitable habitats across three climate change scenarios(SSP1-2.6,SSP2-4.5,and SSP5-8.5)for 2050,2070,and *** findings reveal that the current suitable habitat for *** is approximately 127,881 km^(2) across seven states(Texas,Arkansas,Alabama,Louisiana,Mississippi,Georgia,and Florida);approximately 9.5%is encompassed within state and federally managed protected *** models predict that all current suitable habitats will disap-pear by 2050 due to climate change,resulting in a northward shift into new regions such as Tennessee and *** large extent of suitable habitat outside protected areas sug-gests that a species-specific action plan incorporating pro-tected areas and other areas may be crucial for its ***,protection of *** habitat against climate change may require locally and regionally focused conservation policies,adaptive management strategies,and educational outreach among local people.

Machine learning for predicting the outcome of terminal ballistics events

Machine learning(ML) is well suited for the prediction of high-complexity,high-dimensional problems such as those encountered in terminal *** evaluate the performance of four popular ML-based regression models,extreme gradient boosting(XGBoost),artificial neural network(ANN),support vector regression(SvR),and Gaussian process regression(GP),on two common terminal ballistics’ problems:(a)predicting the v50ballistic limit of monolithic metallic armour impacted by small and medium calibre projectiles and fragments,and(b) predicting the depth to which a projectile will penetrate a target of semi-infinite *** achieve this we utilise two datasets,each consisting of approximately 1000samples,collated from public release *** demonstrate that all four model types provide similarly excellent agreement when interpolating within the training data and diverge when extrapolating outside this *** extrapolation is not advisable for ML-based regression models,for applications such as lethality/survivability analysis,such capability is *** circumvent this,we implement expert knowledge and physics-based models via enforced monotonicity,as a Gaussian prior mean,and through a modified loss *** physics-informed models demonstrate improved performance over both classical physics-based models and the basic ML regression models,providing an ability to accurately fit experimental data when it is available and then revert to the physics-based model when *** resulting models demonstrate high levels of predictive accuracy over a very wide range of projectile types,target materials and thicknesses,and impact conditions significantly more diverse than that achievable from any existing analytical *** with numerical analysis tools such as finite element solvers the ML models run orders of magnitude *** provide some general guidelines throughout for the development,application,and reporting of ML models in terminal ballist

Adverse events associated with the gold probe and the injection gold probe devices used for endoscopic hemostasis:A MAUDE database analysis

BACKGROUND Gastrointestinal(GI)bleeding accounts for over half a million admissions annually and is the most common GI diagnosis requiring hospitalization in the United *** electrocoagulation devices are used for the management of gastrointestinal *** is no data on device-related adverse events for gold probe(GP)and injection gold probe(IGP).AIM To analyze this using the Food and Drug Administration(FDA’s)Manufacturer and User Facility Device Experience(MAUDE)database from 2013 to *** We examined post-marketing surveillance data on GP and IGP from the FDA MAUDE database to report devicerelated and patient-related adverse events between *** MAUDE database is a publicly available resource providing over 4 million records relating to medical device *** analyses were performed using IBM SPSS Statistics v.27.0(IBM Corp.,Armonk,NY,United States).RESULTS Our search elicited 140 reports for GP and 202 reports for IGP,respec-tively,during the study period from January 2013 to August *** reportedly occurred in 130 cases for GP,and actual patient injury or event occurred in 10 patients.A total of 149 patients(74%)reported with Injection GP events suffered no significant consequences due to the device failure,but 53 patients(26%)were affected by an *** GP and IGP are critical in managing gastrointestinal *** study of the FDA MAUDE database revealed the type,number,and trends of reported device-related adverse *** endoscopist and support staff must be aware of these device-related events and be equipped to manage them if they occur.

A simple and efficient coupled displacement field method is developed to study the large amplitude free vibration behavior of the moderately thick square plates.A single term trigonometric admissible displacement field is assumed for one of the variables,say,the total rotations(in both X,Y directions).Making use of the coupling equation,the spatial variation for the lateral displacement field is derived in terms of the total *** coupled displacement field method makes the energy formulation to contain half the number of unknown independent coefficients,in the case of a square plate,contrary to the conventional RayleighRitz *** lesser number of undetermined coefficients significantly simplifies the vibration *** expressions for the linear and nonlinear fundamental frequency parameters for the all edges simply supported moderately thick square plates are *** numerical results obtained from the present formulation are in very good agreement with those obtained from the existing literature.

Weibull Probability Model for Fracture Strength of Aluminium (1101)-Alumina Particle Reinforced Metal Matrix Composite

In recent times, conventional materials are replaced by metal matrix composites （MMCs） due to their high specific strength and modulus. Strength reliability, one of the key factors restricting wider use of composite materials in various applications, is commonly characterized by Weibull strength distribution function. In the present work, statistical analysis of the strength data of 15% volume alumina particle （mean size 15 um） reinforced in aluminum alloy （1101 grade alloy） fabricated by stir casting method was carried out using Weibull probability model. Twelve tension tests were performed according to ASTM B577 standards and the test data, the corresponding Weibull distribution was obtained. Finally the reliability of the composite behavior in terms of its fracture strength was presented to ensure the reliability of composites for suitable applications. An important implication of the present study is that the Weibull distribution describes the experimentally measured strength data more appropriately.
