The Correlation of NT-Pro BNP with Echocardiographic Indices Including 3D Vena Contracta Width in the Assessment of Severity of Mitral Regurgitation

Introduction: Mitral regurgitation (MR) is a common valvular disease that causes significant morbidity and mortality. The use of multiple echocardiographic indices and hormonal parameters in combination can provide an accurate assessment of MR severity and LV dysfunction in most cases. Aim of study: The aim of this study was to correlate the levels of serum NT-pro BNP with various echocardiographic parameters including 3D vena contracta width in patients with MR. Methods: 74 Patients diagnosed with primary mitral regurgitation in sinus rhythm with no conduction disturbance were included in the study. 2D and Doppler echocardiography indices were calculated. A full-volume 3D color Doppler acquisition was obtained and 3D vena contracta width was calculated and serum BNP levels were obtained. Results: NT-pro BNP levels were increased with symptoms in patients with mitral regurgitation (NYHAI: 4.48 ± 0.06, NYHAII: 5.54 ± 0.78, NYHAIII: 6.68 ± 1.21 pg/ml, p 0.01). NT-pro BNP plasma levels were significantly correlated with MPI (r = 0.945, p 0.01), LVESV (r = 0.706, p 0.01), LVESVI (r = 0.677, p 0.01), LAVI (r = 0.709, p 0.01), MR 2D VC (r = 0.430, p 0.01), 3D VC (r = 0.441, p 0.01), PISA (r = 0.440, p 0.01) and negative correlation with LV ejection fraction (r = 0.846, p 0.01), dp/dt (r = 0.795, p BNP plasma level and LA jet area ratio (P = 0.33). It was observed that there was a significant correlation between 3D VC and PISA levels in patients of mitral regurgitation. However correlation is better in MR with central jet compared to eccentric jet in moderate and severe MR (r = 0.807, 0.817 vs r = 0.617, 0.572). Conclusion: This study showed that NT-pro BNP measurement has the same sensitivity and specificity as echocardiographic indices. Three-dimensional VCA may provide a reliable measurement of ROA, independent of geometric and flow assumptions.

Drug-Induced Hypothyroidism during Anti-Tuberculosis Treatment of Multidrug-Resistant Tuberculosis: Notes from the Field

We followed 188 euthyroidic persons undergoing treatment for multidrug resistant tuberculosis (MDR-TB) in the state of karnataka, India to determine the incidence of hypothyroidism during anti-tuberculosis treatment. Overall, among MDR-TB patients with valid thyroid stimulating hormone (TSH) values, about 23% developed hypothyroidism (TSH value ≥10 mIU/ml) during anti-tuberculosis treatment;the majority (74%) occurring after 3 months of treatment. Among 133 patients who received a regimen that contained ethionamide, 42 (32%) developed hypothyroidism. Among 17 patients that received a regimen that contained para-aminosalicylate sodium, 6 (35%) developed hypothyroidism. Among 9 HIV positive patients on antiretroviral treatment, 4 (44%) developed hypothyroidism. These results differ from previously reported 4% incidence of hypothyroidism amongst patients who passively reported thyroidal symptoms during treatment, suggesting routine serologic monitoring of TSH throughout the course of treatment for MDR-TB is warranted.

Background:?According to the GLOBOCAN 2018 report, the estimated incidence of lung cancer in India was 67,795 in both sexes. The treatment of advanced Non-small cell lung cancer (NSCLC) saw a major paradigm shift with recent advances in molecular-targeted therapy. Immune checkpoint blockade therapy is one such novel strategy with promising clinical benefits in advanced NSCLC. Programmed cell death receptor-1/Programmed cell death ligand-1 (PD-1/PD-L1) pathway is one such checkpoint and has thus become the current area of interest in the treatment of lung carcinoma. The PD-1/?PD-L1 pathway is under active investigation as it represents a promising therapeutic target in NSCLC. The expression of PD-L1 in tumor cells has been suggested as a predictive marker of the clinical response to PD-1/?PD-L1-targeted therapy. Methods: This study was carried out at the Department of Pathology, Health Care Global Specialty Hospital, Bangalore from May 2018 to May 2019. In this study, we analyzed pattern of PD-L1 expression by immunohistochemistry testing using our own Laboratory Developed Test (LDT) in NSCLC patients. Results:?Our study group comprised 50 patients of NSCLC. Among our study population, 40% of the patients exhibited PD-L1 immunopositivity (≥1%) with 28% of them having any expression (TPS 1% - 49%) and 12% of them having a high expression (TPS ≥ 50%) of PD-L1. Majority of them exhibited adenocarcinoma type of NSCLC under which the solid subtype showed a direct correlation with PD-L1 positivity (p-value: 0.004) with a poorly differentiated tumor histology being common in our population in relation to PD-L1 positivity (p-value: 0.043). PD-L1 expression did not correlate with age, gender, smoking status or clinical stage in our study. No association was found between tumor histology (SCC or AC) and driver mutation status with expression of PD-L1 in the present study. Objective. To determine the potential benefits of ISC (intermittent self-catheterisation) over SPC (supra-pubic catheterisation) in the post-operative bladder care of women following radical hysterectomy. Methods. A prospective randomised controlled trial of women treated by radical hysterectomy for early stage cervical cancer. Results. 40 women were recruited to the study, 21 to ISC and 19 to SPC. All patients randomised to ISC were able to learn the technique of ISC satisfactorily following a period of pre-operative training. The day 3 and day 5 positive CSU (catheter specimen of urine) rate was significantly higher in the ISC group (42%and 63%) compared to the SPC group (6%and 18%), P = 0.05 and P = 0.004, respectively). Eight of 17 patients randomised to SPC (47%)-documented having symptoms/problems arising from the SPC site of which 4 (23%) were shown to have a positive wound swab. There was no significant difference in length of period for bladder care between the two groups, P = 0.83. However, there were significant differences in patient acceptability (P = 0.009), freedom to lead a normal life (P = 0.000), disturbance at night (P = 0.006) and patient anxiety/embarrassment (P = 0.005) between the two groups. Conclusions. Patients are able to learn the technique of ISC without difficulty. Despite a greater urinary tract infection rate, the high incidence of SPC site problems can be avoided by use of ISC. The technique of ISC was seen to be more acceptable to patients allowing fewer disturbances at night, greater freedom to lead a normal life during the day and less anxiety/embarrassment compared to SPC.

Spin switching in antiferromagnets using Néel-order spin-orbit torques

Antiferromagnets offer considerable potential for electronic device applications. This article reviews recent demonstrations of spin manipulation in antiferromagnetic devices using applied electrical currents. Due to spin–orbit coupling in environments with particular crystalline or structural symmetries, the electric current can induce an effective magnetic field with a sign that alternates on the lengthscale of the unit cell. The staggered effective field provides an efficient mechanism for switching antiferromagnetic domains and moving antiferromagnetic domain walls, with writing speeds in the terahertz regime.

Large number of rain water samples, at 7 rural locations in the semi-arid region of the DeccanPlateau were collected during 4 consecutive monsoon seasons （1979-1982）.pH, conductivityand the major ionic components (C1-, SO4=, NO3-, NH4+, Na+, k+, Ca++, Mg++of the abovesamples were determined. The pH of rain water was found to be highly alkaline and the valuesvaried from 6.4 to 7.8. Soil-oriented elements showed good correltioan （r0.6） with pH valuesof rain water. The high concentration of soil-oriented elements, specially Ca++, is found to play animportant role in neutralizing the acidity of rain water and maintaining high alkaline pH. The studysuggested that the contribution of atmospheric aerosol of natural sources （sea and soil） to thechemical composition of rain water is more than that of anthropogenic origin.

Synthesis,Characterization and Biocompatibility of Potassium Ferrite Nanoparticles

In the present study, morphology, size distribution, structure, biocompatibility and magnetic properties of potassium ferrite nanoparticles （kFeO2 NPs）, synthesized by conventional sol-gel method have been reported. The formation of spherical nanoparticles with orthorhombic structure has been confirmed by scanning electron microscopy and X-ray diffraction. The particle size, as obtained by transmission electron microscopy has been found to be in the range of 4-7 nm. Further, the size distribution has been scrutinized using Analyse-it software, where a platykurtic feature in the size distribution was observed. Fourier transform-infrared spectroscopy and thermogravimetric analysis showed the formation of metal （Fe, k） bonds at neel temperature of 337℃. Vibrating sample magnetometer analysis revealed the superparamagnetic behaviour of the synthesized kFeO2 NPs, with saturation magnetization of 25.72 emu/g. In vitro cytotoxicity test, using MTTassay, on T cell lines （Jurkat cells） showed that kFeO2 NPs are biocompatible at a particle concentration of 100μg/ml.

Contamination of Environment with Polycyclic Aromatic Hydrocarbons in India

Environment in India is contaminated with polycyclic aromatic hydrocarbons (PAHs) due to occurring of large anthropogenic activities i.e. fuel combustion, mineral roasting and biomass burning. Hence, contamination of 13 toxic PAHs: phenanthrene, anthracene, fluoranthene, pyrene, benz (a) anthracene, ben-zo (b) fluoranthene, benzo (k) fluoranthene, benzo (a) pyrene, benzo (ghi) perylene, dibenz (ah) anthracene, indeno1,2,3-(cd) pyrene, coronene and coronene in the environment (i.e. ambient particulate matter, road dust, sludge and sewage) of the most industrialized area: Raipur city, India is described. The ΣPAH13 concentration in the 16 environment materials was ranged from 7980 - 1,051,300 μg/kg with mean value of 172,613 ± 154,726 μg/kg. The concentration variations, toxicities and sources of the PAHs in various environmental compartments are discussed.

Florogenesis and Female Gametophyte Development in <i>Allium cepa</i>L. cv. krishnapuram

Florogenesis is one of the most complicated and interesting processes in the nature. This process involves developmental, physiological and molecular events leading to transformation from vegetative to reproductive phase for optimal seed production and the continuation of species. The basic knowledge about flowering processes, male and female sexual systems support basic and applied research and breeding programs. Most of the onion varieties from India are short day varieties, more diverse than other exotic germplasm and useful as a source of new alleles for supporting breeding programs. The present investigation was focused to study for the first time florogenesis process by scanning electron study for the first time florogenesis process by scanning electron microscopy (SEM) and development of female gametophytes by light microscopy in order to acquire basic knowledge useful for optimizing in vitro process to produce gynogenic haploid to support and speed breeding program in short-day onion Allium cepa L. cv. krishnapuram (kP) or Bangalore Rose. This study revealed that shoot primordium differentiated into inflorescence meristem in the month of December, while seeds were planted in the field in the September. The individual florets are preceded by a variying number of floral initials. The female gametophyte developed from chalazal side megaspore. The embryo sac development is a bisporic Allium type showing short-lived antipodals. The histological study suggests that the use of big or preanthesis flower buds with embryo sac for production of gynogenic haploids to support breeding program in onion cv. krishnapuram (kP). However further studies are needed for confirmation of this observation.

Automatic Clustering Using Teaching Learning Based Optimization

Finding the optimal number of clusters has remained to be a challenging problem in data mining research community. Several approaches have been suggested which include evolutionary computation techniques like genetic algorithm, particle swarm optimization, differential evolution etc. for addressing this issue. Many variants of the hybridization of these approaches also have been tried by researchers. However, the number of optimal clusters and the computational efficiency has still remained open for further research. In this paper, a new optimization technique known as “Teaching-Learning-Based Optimization” (TLBO) is implemented for automatic clustering of large unlabeled data sets. In contrast to most of the existing clustering techniques, the proposed algorithm requires no prior knowledge of the data to be classified rather it determines the optimal number of partitions of the data “on the run”. The new AUTO-TLBO algorithms are evaluated on benchmark datasets (collected from UCI machine repository) and performance comparisons are made with some well-known clustering algorithms. Results show that AUTO-TLBO clustering techniques have much potential in terms of comparative results and time of computations.
