Intra-arterial thrombolytics during endovascular thrombectomy for acute ischaemic stroke in the MR CLeAN Registry

Introduction The efficacy and safety of local intra-arterial(IA)thrombolytics during endovascular thrombectomy(eVT)for large-vessel occlusions is uncertain.We analysed how often IA thrombolytics were administered in the Multicenter Randomized Clinical Trial of endovascular Treatment for Acute Ischemic Stroke in the Netherlands(MR CLeAN)Registry,whether it was associated with improved functional outcome and assessed technical and safety outcomes compared with eVT without IA thrombolytics.Methods In this observational study,we included patients undergoing eVT for an acute ischaemic stroke in the anterior circulation from the MR CLeAN Registry(March 2014-November 2017).The primary endpoint was favourable functional outcome,defined as an modified Rankin Scale score≤2 at 90 days.Secondary endpoints were reperfusion status,early neurological recovery and symptomatic intracranial haemorrhage(sICH).Subgroup analyses for IA thrombolytics as primary versus adjuvant revascularisation attempt were performed.Results Of the 2263 included patients,95(4.2%)received IA thrombolytics during eVT.The IA thrombolytics administered were urokinase(median dose,250000 IU(IQR,193750-250000))or alteplase(median dose,20 mg(IQR,12-20)).No association was found between IA thrombolytics and favourable functional outcome(adjusted OR(aOR),1.16;95%CI 0.71 to 1.90).Successful reperfusion was less often observed in those patients treated with IA thrombolytics(aOR,0.57;95%CI 0.36 to 0.90).The odds of sICH(aOR,0.82;95%CI 0.32 to 2.10)and early neurological recovery were comparable between patients treated with and without IA thrombolytics.For primary and adjuvant revascularisation attempts,IA thrombolytics were more often administered for proximal than for distal occlusions.Functional outcomes were comparable for patients receiving IA thrombolytics as a primary versus adjuvant revascularisation attempt.Conclusion Local IA thrombolytics were rarely used in the MR CLeAN Registry.In the relatively small study sample,no statistical difference was observe

Amyotrophic lateral sclerosis(ALS)is associated with proteostasis collapse:ALS is an unrelenting neurodegenerative disease that is characterized by the loss of motor neurons in the brain and spinal cord,resulting in the progressive atrophy,and eventual paralysis,of skeletal muscles.Death due to respiratory failure usually occurs within 2-5 years from symptom onset.Approximately 90%of ALS cases are of unknown etiology and are termed sporadic ALS(sALS).The remaining 10%of ALS cases present with a family history(familial ALS;fALS)and are associated with genetic mutations in a range of over 20 functionally heterogeneous genes.Regardless of disease origin,the pathological hallmark of ALS is the accumulation of ubiquitylated protein inclusions in motor neurons and surrounding glial cells.The presence of these inclusions,compromised largely of misfolded and aggregated proteins,implies a collapse in proteostasis.Proteostasis refers to the maintenance of the proteome in a state of balance or equilibrium so that a cell can perform its proper function.

Ultraviolet light exposure and its penetrance through the eye in a porcine model

AIM:To determine the amount of ultraviolet(UV)light irradiance that various layers of the eye receive as sunlight passes through the eye,and to investigate the protective benefits of UV light-blocking contact lenses.MeTHODS:Twenty-four porcine eyes were prepared in one of three ways:isolated cornea,cornea and lens together,or whole eye preparation.UV light irradiance was measured with a UV-A/B light meter before and after the eye preparations were placed over the meter to measure UV light penetration in each eye structure.In the whole eye preparation,a hole was placed in the fovea to measure light as it passed through the vitreous.Subsequently,UVprotective contact lenses were placed over the structures,and UV light penetrance was measured.Measurements of UV light exposure were taken outdoors at various locations and times.ReSULTS:Cornea absorbed 63.56%of UV light that reached the eye.Cornea and lens absorbed 99.34%of UV light.Whole eye absorbed 99.77%of UV light.When UV-protective contact lenses were placed,absorption was 98.90%,99.55%,and 99.87%,respectively.UV light exposure was dependent on directionality and time of day,and was greatest in areas of high albedo that reflect significant amounts of light,such as a beach.CONCLUSION:Cornea absorbs the majority of UV light that reaches the eye in this model.UV-protective contact lenses reduce UV exposure to the eye.Locations with high albedo expose the eye to higher levels of UV light.

Associations of self-reported and device-assessed physical activity with fatigue,quality of life,and sleep quality in adults living with and beyond cancer

Background Greater physical activity is associated with improved outcomes in people living with and beyond cancer.However,most studies in exercise oncology use self-reported measures of physical activity.Few have explored agreement between self-reported and device-based measures of physical activity in people living with and beyond cancer.This study aimed to describe physical activity in adults affected by cancer across self-reported and device-assessed activity,to explore levels of agreement between these measures in terms of their utility for categorizing participants as meeting/not meeting physical activity guidelines,and to explore whether meeting guidelines is associated with fatigue,quality of life,and sleep quality.Methods A total of 1348 adults living with and beyond cancer from the Advancing Survivorship Cancer Outcomes Trial completed a survey assessing fatigue,quality of life,sleep quality,and physical activity.The Godin-Shephard Leisure-Time Physical Activity Questionnaire was used to calculate a Leisure Score Index(LSI)and an estimate of moderate-to-vigorous physical activity(MVPA).Average daily steps and weekly aerobic steps were derived from pedometers worn by participants.Results The percentage of individuals meeting physical activity guidelines was 44.3%using LSI,49.5%using MVPA,10.8%using average daily steps,and 28.5%using weekly aerobic steps.Agreement(Cohen'sκ)between self-reported and pedometer measures ranged from 0.13(LSI vs.average daily steps)to 0.60(LSI vs.MVPA).After adjusting for sociodemographic and health-related covariates,meeting activity guidelines using all measures was associated with not experiencing severe fatigue(odds ratios(ORs):1.43–1.97).Meeting guidelines using MVPA was associated with no quality-of-life issues(OR=1.53).Meeting guidelines using both self-reported measures were associated with good sleep quality(ORs:1.33–1.40).Conclusion Less than half of all adults affected by cancer are meeting physical activity guidelines,regardless of measure.Meeting guidelines is asso

Bandicoots and bilbies(Order Peramelemorphia)occupy a broad range of habitats across Australia and New Guinea,from open,arid deserts to dense forests.This once diverse group has been particularly vulnerable to habitat loss and introduced eutherian predators,and numerous species extinctions and range retractions have occurred.Understanding reasons for this loss requires greater understanding of their biology.Morphology of the pinnae and tympanic bullae varies markedly among species.As hearing is important for both predator avoidance and prey location,the variability in ear morphology could reflect specialization and adaptation to specific environments,and therefore be of conservation relevance.We measured 798 museum specimens representing 29 species of Peramelemorphia.Controlling for phylogenetic relatedness and head length,pinna surface area was weakly negatively correlated with average precipitation(rainfall being our surrogate measure of vegetation productivity/complexity),and there were no environmental correlates with effective diameter(pinna width).Controlling for phylogenetic relatedness and skull length,tympanic bulla volume was negatively correlated with precipitation.Species that inhabited drier habitats,which would be open and allow sound to carry further with less obstruction,had relatively larger pinnae and tympanic bullae.In contrast,species from higher rainfall habitats,where sounds would be attenuated and diffused by dense vegetation,had the smallest pinnae and bullae,suggesting that low-frequency hearing is not as important in these habitats.Associations with temperature did not reach statistical significance.These findings highlight linkages between hearing traits and habitat that can inform conservation and management strategies for threatened species.

Severe acute respiratory syndrome coronavirus (SARS-CoV-2) is a newly identified member of the coronavirus family that has caused the coronavirus disease 2019 (COVID-19) pandemic. This rapidly evolving and unrelenting SARS-CoV-2has disrupted the lives and livelihoods of millions worldwide. As of 23 August 2021, a total of 211,373,303 COVID-19cases have been confirmed globally with a death toll of 4,424,341. A strong understanding of the infection pathway of SARS-CoV-2, and how our immune system responds to the virus is highly pertinent for guiding the development and improvement of effective treatments. In this review, we discuss the current understanding of neutralising antibodies(NAbs) and their implications in clinical practice. The aspects include the pathophysiology of the immune response,particularly humoral adaptive immunity and the roles of NAbs from B cells in infection clearance. We summarise the onset and persistence of IgA, IgM and IgG antibodies, and we explore their roles in neutralising SARS-CoV-2, their persistence in convalescent individuals, and in reinfection. Furthermore, we also review the applications of neutralising antibodies in the clinical setting—from predictors of disease severity to serological testing to vaccinations, and finally in therapeutics such as convalescent plasma infusion.

Membrane bioreactors for hospital wastewater treatment: recent advancements in membranes and processes

Discharged hospital wastewater contains various pathogenic microorganisms,antibiotic groups,toxic organic compounds,radioactive elements,and ionic pollutants.These contaminants harm the environment and human health causing the spread of disease.Thus,effective treatment of hospital wastewater is an urgent task for sustainable development.Membranes,with controllable porous and nonporous structures,have been rapidly developed for molecular separations.In particular,membrane bioreactor(MBR)technology demonstrated high removal efficiency toward organic compounds and low waste sludge production.To further enhance the separation efficiency and achieve material recovery from hospital waste streams,novel concepts of MBRs and their applications are rapidly evolved through hybridizing novel membranes(non hydrophilic ultrafiltration/microfiltration)into the MBR units(hybrid MBRs)or the MBR as a pretreatment step and integrating other membrane processes as subsequent secondary purification step(integrated MBR-membrane systems).However,there is a lack of reviews on the latest advancement in MBR technologies for hospital wastewater treatment,and analysis on its major challenges and future trends.This review started with an overview of main pollutants in common hospital wastewater,followed by an understanding on the key performance indicators/criteria in MBR membranes(i.e.,solute selectivity)and processes(e.g.,fouling).Then,an in-depth analysis was provided into the recent development of hybrid MBR and integrated MBR-membrane system concepts,and applications correlated with wastewater sources,with a particular focus on hospital wastewaters.It is anticipated that this review will shed light on the knowledge gaps in the field,highlighting the potential contribution of hybrid MBRs and integrated MBRmembrane systems toward global epidemic prevention.

Hepatic transcriptome profiling reveals early signatures associated with disease transition from non-alcoholic steatosis to steatohepatitis

Background and aim:Non-alcoholic fatty liver disease(NAFLD)is becoming a leading cause of chronic liver disease worldwide.The molecular events that influence disease progression from non-alcoholic fatty liver(NAFL)to aggressive non-alcoholic steatohepatitis(NASH)remain incompletely understood,leading to lack of mechanism-based targeted treatment options for NASH.This study aims to identify early signatures associated with disease progression from NAFL to NASH in mice and humans.Materials and methods:Male C57BL/6J mice were fed a high-fat,-cholesterol,and-fructose(HFCF)diet for up to 9 months.The extent of steatosis,inflammation,and fibrosis was evaluated in liver tissues.Total RNA sequencing(RNA-seq)was conducted to determine liver transcriptomic changes.Results:After being fed the HFCF diet,mice sequentially developed steatosis,early steatohepatitis,steatohepatitis with fibrosis,and eventually spontaneous liver tumor.Hepatic RNA-seq revealed that the key signatures during steatosis progression to early steatohepatitis were pathways related to extracel-lular matrix organization and immune responses such as T cell migration,arginine biosynthesis,C-type lectin receptor signaling,and cytokine-cytokine receptor interaction.Genes regulated by transcription factors forkhead box M1(FOXM1)and negative elongation factor complex member e(NeLFe)were significantly altered during disease progression.This phenomenon was also observed in patients with NASH.

Initiation of Programmed Cell Death in Self-Incompatibility： Role for Cytoskeleton Modifications and Several Caspase-Like Activities

Programmed cell death （PCD） is an important and universal process regulating precise death of unwanted cells in eukaryotes. In plants, the existence of PCD has been firmly established for about a decade, and many components shown to be involved in apoptosis/PCD in mammalian systems are found in plant cells undergoing PCD. Here, we review work from our lab demonstrating the involvement of PCD in the self-incompatibility response in Papaver rhoeas pollen. This utilization of PCD as a consequence of a specific pollen-pistil interaction provides a very neat way to destroy unwanted ＇self＇, but not ＇non-self＇ pollen. We discuss recent data providing evidence for SI-induced activation of several caspase-like activities and suggest that an acidification of the cytosol may be a key turning point in the activation of caspase-like proteases executing PCD. We also review data showing the involvement of the actin and microtubule cytoskeletons as well as that of a MAPK in signalling to caspase-mediated PCD. Potential links between these various components in signalling to PCD are discussed. Together, this begins to build a picture of PCD in a single cell system, triggered by a receptor-ligand interaction.
