Hyperfibrinogenemia and Reduced Plasma Protein C Levels in HIV-Infected Patients

Background: Heamatological problems have been associated with human immunodeficiency virus infection. Hypercoagulability, in particular, thrombosis is becoming more common in HIV-positive patients. Aim: The goals of this study were to determine levels of plasma fibrinogen, protein C, Hemoglobin, and ESR among Sudanese HIV-positive patients. Materials and Methods: This is a case-control study, for this investigation, a total of 100 participants were recruited for this study. Fifty people were diagnosed with HIV, 25 of whom were males (50 percent) and 25 of whom were females (50 percent), with an average age of 35.5 years. Further fifty healthy people, 26 (52%) of whom were men and 24 (48%) of whom were women, with a mean age of 37.1 years, matched the case group. Fresh Poor Plasma was obtained by centrifuging citrated venous blood samples at 3000 rpm for 15 minutes. The fibrinogen level was determined using an automated coagulation analyzer. Total protein C level was measured by a fully-automated blood coagulation analyzer (SYSMEX CA-500’JAPAN). The haemoglobin parameter was measured from EDTA anticoagulant samples using the Sysmex KX 21-N automated haematological analyzer. In one hour, the ESR was done using a Westergren tube. Data was collected using a structured direct questionnaire. SPSS version 21 was used to analyse the data. Results: The current study discovered that in Sudanese HIV infection, the mean and standard deviation of plasma fibrinogen levels were statistically substantially higher than in the normal control group (370.5 ± 67 vs 214.7 ± 21 with P value 0.001). Protein C levels were statistically significantly lower in HIV positive patients compared with control group (0.6 ± 0.1 vs 1.3 ± 0.2 with P value 0.001). In HIV positive patients, haemoglobin was statistically substantially lower than in healthy people (10.8 ± 1.8 vs 13.7 ± 1.9, P value 0.01). Background: Anemia is one of the complications of chronic kidney disease (CKD) which necessitates multiple blood transfusions. These multiple transfusions are usually accompanied by Iron over load. The current study aimed to assess the Iron profile in Sudanese patients with chronic renal failure received regular blood transfused. Materials and Methods: A descriptive analytical case control study was performed in Alturki Teaching Hospital, Khartoum, Sudan. A total of 90 Sudanese were enrolled in this study (30 CKD patients under hemodialysis and undergo multiple blood transfusion, 30 CKD patients under hemodialysis but not blood transfusion and 30 healthy controls, 51 (57%) were male and 39 (43%) were females, their age ranged between 28 to 75 years. Iron profile (serum iron, serum ferritin and total iron binding capacity (TIBC)) were measured using full automated Chemical analyzer Mindray BS 200 (China). Data were analyzed by using statistical package for social sciences (SPSS) verssion16. Results: The present study showed that serum iron and serum ferritin of those under hemodialysis and received regular blood transfusion patient were significantly higher compared to those under hemodialysis and did not received regular blood transfusion patients (P value 0.000 and 0.01) respectively. Conclusion: Patients with chronic renal failure and under hemodialysis and receive regular and multiple transfusions are prone to iron overload.

Background: Nutritional knowledge and practise is an important measure to improve dietary habits and food choices. The daily food should contain balanced nutrition as needed to support optimal growth, avoid deficiency diseases and prevent the diseases which might interfere with the children health. Materials & Methods: A descriptive cross-sectional non-interventional hospital-based study was carried out among 122 mothers and 122 children by using a non-probability convenience sampling technique in the period of 15 March to 15 April 2023, and using interviews with structural questionnaires, the data was analyzed using SPSS version 26. Results: We found that most of the children had moderate health, while most of the mothers had moderate nutritional knowledge and practise towards their children, also we found that the relationship between mothers’ knowledge and practise and child health was significant (P Conclusion: Mothers’ knowledge and practise among children was moderate and children had moderate health. Wrong beliefs and cultural taboos play major roles in decreasing knowledge and practise.

The nonlocal nonlinear Schr?dinger equation(NNLSE)with competing weakly nonlocal nonlinearity and parabolic law nonlinearity is explored in the current work.A powerful integration tool,which is a modified form of the simple equation method,is used to construct the dark and singular 1-soliton *** is shown that the modified simple equation method provides an effective and powerful mathematical gadget for solving various types of NNLSEs.

Epilepsy is one of the most prevalent neurological disorders with no age, racial, social class, and neither national nor geographic boundaries. There are 50 million sufferers in the world today with 2.4 million new cases occur each year. Electroencephalogram (EEG) has become a traditional procedure to investigate abnormal functioning of brain activity. Epileptic EEG is usually characterized by short transients and sharp waves as spikes. Identification of such event splays a crucial role in epilepsy diagnosis and treatment. The present study proposes a method to detect three epileptic spike types in EEG recordings based mainly on Template Matching Algorithm including multiple signal-processing approaches. The method was applied to real clinical EEG data of epileptic patients and evaluated according to sensitivity, specificity, selectivity and average detection rate. The promising results illuminate that hybrid processing approaches in temporal, frequency and spatial domains can be a real solution to identify fast EEG transients.

Percutaneous Mitral Valvotomy in Children and Adolescents: Immediate and Midterm Results

Objective: To analyze immediate and midterm results of percutaneous mitral valvotomy (PMV) in patients ≤ 18 year, a single centre experience of Sohag university hospital. Methods: Between May 2010 and May 2012, 64 procedures of Inoue technique mitral valvotomy were performed on 64 patients. The mean age was 15.8 ± 2.2 years;75% were females. Results: Success was obtained in 96.8% of patients. Immediate complications were severe mitral regurgitation (3.1%), cardiac tamponade (1.6%) and embolic stroke in (1.6%). Late follow-up was obtained in 93.7% of the patients (at 6 and 24 months). Restenosis developed in 3 patients. Most of the patients had no symptoms at discharge but most of them had NYHA class I to II at 6 months to 2 years follow up. No patients died. Patients who operated early with MVA around 1.4 had best outcome. Conclusion: PMV represents a safe therapeutic option in young patients with mitral valve stenosis. In these patients, recurrence of symptoms and restenosis occurs at an earlier stage than in elderly. Earlier intervention carries better outcome and decreases restenosis rate.

Prevalence and Associated Factors of Back Pain among Medical Students in Sudan

Background: Back pain affects old and young adults, and it is a common complain among medical students. Objective: To estimate the prevalence of back pain and associated factors among medical students in Sudan. Method: An observational multi-universal cross-sectional study was conducted from December 2021 to February 2022, involving 433 medical students selected by simple random sampling. Data were collected by electronic Google Form questionnaire designed to assess demographic features, back pain characteristics, and triggering factors. Data analysis was done by statistical package for social sciences. Result: The prevalence of back pain among medical students in Sudan is 69.3%, with a statistically insignificant difference between preclinical and clinical students (p value: 0.378), and between male and female students (p value: 0.826). The lower back is the commonest site of pain (75%). The majority of the students experience moderate pain (53.9%). They did not use medications (81%). Sitting for long hours is the most common trigger (87.9%) and the pain did not interfere with the daily activities (71%). Conclusion: Back pain is a common complain among medical students in Sudan. Medical schools should raise the awareness of the students about back pain.
