An Early Experience of Stapled Hemorrhoidectomy in a Medical College Setting

Background: Stapled hemorrhoidectomy, popularly known as Longo technique is in use for the treatment of hemorrhoids since its first description to surgical fraternity in the world congress of endoscopic surgeons in 1998. Objectives: To evaluate the feasibility, patient acceptance, recurrence and results of stapled haemorrhoidectomy in our early experience. Methods: Between Jan 2012 and Dec 2013, 42 patients with symptomatic GRADE III and IV hemorrhoids were operated by stapled hemorrhoidectomy by a single surgeon at our surgery department. The evaluation of this technique was done by assessing the feasibility of the surgery;and recording operative time, postoperative pain, complications, hospital stay, return to work and recurrence. Results: All the procedures were completed successfully. The mean (range) operative time was 30 (20 - 45) min. The blood loss was minimal. Mean (range) length of hospitalization for the entire group was 1 (1 - 3) days. Only 3 patients required more than 1 injection of diclofenac (75 mg) while as rest of the patients were quite happy switching over to oral diclofenac (50 mg) just after a single parenteral dose. All the patients returned to their routine work in less than a week’s time. The mean (range) follow-up was 6 (4 - 12) months. There were no major intraoperative or postoperative complications except for retention of urine in 8 patients. The patients are still on regular follow-up and have not had recurrence as yet. Conclusion: Stapled hemorrhoidectomy technique is a safe alternative to the traditional Milligan-Morgan technique. It can be performed as an office procedure, is well tolerated by patients and is cost effective than conventional surgical therapy.

Mobile communication through 5G technology is the key objective of this work. Existing research works in mobile communication through 5G technology in world submitted a great necessary development towards 5G technology in different work approaches including hardware and software. 4G technology includes several standards under a common umbrella, similar to previous generations of communication technologies. Actually 4G is good for now, however if look at it in five or ten years, 4G will obviously not be able to meet requirements for new applications coming up in the next few years. With 5G will increase the data rate, reduce the end-to-end latency, and improve coverage. These properties are particularly important for many applications related to IoT and D2D, which they are recognized as ones of the technology components of the evolving 5G architecture. The major contribution of this paper is the key provisions of mobile communication through 5G (Fifth Generation) technology of which is seen as consumer oriented. In 5G technology and mobile consumer has given top priority over others. 5G technology is to make use of mobile phones within very high bandwidth. The consumer never experienced the utmost valued technology as *** 5G technologies comprise all types of sophisticated features which make 5G technology most governing technology in the vicinity of future.

Cost and outcomes of postacute care after primary hip and knee arthroplasty

BACKGROUND AND OBJECTIVE With the aging of the population of the United States, significant increases are expected in cases of total hip arthroplasty (THA) and total knee arthroplasty (TKA). Joint arthroplasty is now the largest Medicare procedural cost, at 6.3%. This study examined the duration of care, charges and readmission rates after primary THA or TKA. METHODS Data were obtained from the Medicare Provider Analysis and Review (medPAR) database for individuals undergoing primary TKA ( n ＝136，842) or THA ( n ＝329，233) in 2008. Data collected included subsequent inpatient rehabilitation, extended-care facility stays, home healthcare and outpatient encounters, costs and readmissions within 90 days of discharge. RESULTS Those discharged to post-acute care facilities had longer hospitalizations and higher charges than those discharged to home, with or without home health care. Admission to inpatient rehabilitation was associated with a 30% increase in total charges for patients with THA and a 26% increase for patients with TKA. Mortality and readmission within 90 days were both significantly higher for patients discharged to extended care facilities and inpatient rehabilitation, than for those discharged to home ( P <0.001 for both). The number of comorbidities was associated with increased cost and length of stay. CONCLUSION This study of patients undergoing elective total knee or total hip arthroplasty found that inpatient post-acute care results in increased costs and an increased risk of 90-day readmission and mortality.

Myzus persicae(***)is now considered a threat to agricultural crops due to economic *** synthetic insecticides applied every year against ***,are reported to be unsafe for environment,humans,and beneficial ***,several species of Myzus have been found to develop resistance due to over application of these ***,it is required to find some novel insecticide that would be safe for the environment as well as for *** the current study,two major pure constituentsα-pinene andβ-caryophyllene were evaluated for their insecticidal potential against *** using a fumigant toxicity ***,impact ofα-pinene andβ-caryophyllene on expression of five different genes,e.g.,HSP 60,FPPS I,OSD,TOL and ANT responsible for reproduction,dispersion,and growth of *** has also been *** perform fumigant toxicity assay,five different concentrations(3.5,4,4.5,5 and 6μL L−1)ofα-pinene andβ-caryophyllene were *** concentration(LC)was calculated,and gene expression studies were executed through qRT PCR at LC30 ofα-pinene andβ-*** constituents demonstrated excellent fumigant toxicity effects against *** at all five ***,α-pinene shows significantly better results(98%)as compared toβ-caryophyllene(80%)after 72 h at 6μL L−1 of *** highest upregulation in expression was demonstrated at LC30 dose ofα-pinene in five in three out of five genes understudy(TOL,ANT,and FPPS I).Conversely,two genes HSP 60 and OSD demonstrated downregulation at LC30 dose ofβ-***,our results highlighted the promising insecticidal potential of both compoundsα-pinene andβ-caryophylleneby interfering with the reproduction and development related processes in ***,allowing us to recommend the phytoconstituents under investigation as an ecofriendly alternative to synthetic insecticides.

Design of a Fuzzy Logic Based Controller for Fluid Level Application

In industrial process control, fluid level control is one of the most basic aspects. Many control methods such as on-off, linear and PID (Proportional Integral Derivative) were developed time by time and used for precise controlling of fluid level. Due to flaws of PID controller in non-linear type processes such as inertial lag, time delay and time varying etc., there is a need of alternative design methodology that can be applied in both linear and non-linear systems and it can be execute with fuzzy concept. By using fuzzy logic, designer can realize lower development cost, superior feature and better end product. In this paper, level of fluid in tank is control by using fuzzy logic concept. For this purpose, a simulation system of fuzzy logic controller for fluid level control is designed using simulation packages of MATLAB software such as Fuzzy Logic Toolbox and Simulink. The designed fuzzy logic controller first takes information about inflow and outflow of fluid in tank than maintain the level of fluid in tank by controlling its output valve. In this paper, a controller is designed on five rules using two-input and one-output parameters. At the end, simulation results of fuzzy logic based controller are compared with classical PID controller and it shows that fuzzy logic controller has better stability, fast response and small overshoot.

Review of stem cells as promising therapy for perianal disease in inflammatory bowel disease

Those patients with perianal Crohn's disease or ul-cerative colitis experience a difficult to treat disease process with a delayed state and often inability to heal despite current therapies. The approaches currently used to treat these patients with corticosteroids,antibiotics,immunomodulators, anti-tumor necrosis factor-α drug, and surgical repair are limited in their healing ability. This review presents all current literature since emergence in the early 2000s of stem cell therapy for patients with perianal inflammatory bowel disease and analyzes the efficacy, outcomes and safety within these studies.

Quality milk production in modern dairy systems is facing many *** in them is mastitis which is responsible for decline in milk production,altered milk composition and compromised udder *** malaise consists of multiple bacterial etiologies which can be broadly classified into contagious pathogens and environmental pathogens *** is being isolated invariably in all epidemiological studies,followed by *** virulence in mastitis is often accounted due to microbial ability of producing wide array of virulence factors that enhances pathogenicity and sustainment potential in the epithelial linings of *** affects quality parameters of milk *** as well as mineral profile due to local damage and inflammatory *** decreases the lactose secretion because of oxidative stress generated due to the formation of free radicals in the *** mastitic milk,IgG2 becomes the predominant antibody which is thought to be the main opsonin supporting neutrophil phagocytosis in the bovine mammary ***,it plays a significant role in the battle against mastitis *** infected cow shows a notable elevated level of the sodium and chloride and demoted level of calcium,potassium and inorganic *** micro minerals,mastitis effects are pretty much same as in most macro minerals *** down their concentration in milk *** preventive strategy alongside strict surveillance and biosecurity is recommended for combating this challenge.
