Efficacy of 14-d vs 7-d moxifloxacin-based triple regimens for second-line Helicobacter pylori eradication

AIM: To evaluate the efficacy of the 14-d moxifloxacinbased triple therapy for the second-line eradication of Helicobacter pylori(H. pylori) ***: Between 2011 and 2013, we conducted a retrospective review of the medical records of 160 patients who had experienced failure of their first-line proton pump inhibitor-based eradication therapy and subsequently received the moxifloxacin-based triple therapy as a second-line eradication treatment regimen. The patients who were treated with the moxifloxacinbased triple therapy(oral 20 mg rabeprazole b.i.d., 1000 mg amoxicillin b.i.d., and 400 mg moxifloxacin q.d.) for 7 d were assigned to the raM-7 group(n = 79) while those who took them for 14 days were assigned to raM-14 group(n = 81). The eradication rates for both groups were determined by intentionto-treat(ITT) and per-protocol(PP) analyses. ITT analysis compared the treatment groups as originally allocated while the PP analysis including only those patients who had completed the treatment as originally allocated. Successful eradication therapy for H. pylori infection was defined as the documentation of a negative 13C-urea breath test 4 wk after the end of the eradication ***: The overall ITT eradication rate was 76.2%(122/160). The final ITT eradication rates were 70.8%(56/79; 95%CI: 63.3%-77.1%) in the raM-7 group and 81.4%(66/81; 95%CI: 74.6%-88.3%) in the raM-14 group(P = 0.034). The overall PP eradication rate was 84.1%(122/145), and the final PP eradication rates were 77.7%(56/72; 95%CI: 70.2%-85.3%) in the raM-7 group and 90.4%(66/73; 95%CI: 82.8%-98.1%) in the raM-14 group(P = 0.017). The H. pylori-eradication rates in the raM-14 group were significantly higher compared with that of the raM-7 group according to both the ITT(P = 0.034) and the PP analyses(P = 0.017). 气候变化威胁人类的生存.化石燃料的过度使用导致CO_(2)持续排放是引起全球变暖和气候变化的主要因素,须尽快减少CO_(2)排放.捕获CO_(2)并将其转化为高附加值的燃料和化学品从而循环利用是减少CO_(2)净排放的有效措施.其中,CO_(2)催化加氢制甲酸(HCOOH)是碳捕获与利用(CCU)的有效途径之一,其产物HCOOH可用于防腐剂、牲畜饲料抗菌剂和食品添加剂,也可作为氢载体和直接甲酸燃料电池的燃料.目前,工业上生产HCOOH的工艺包括两个步骤:甲醇和CO在强碱条件下反应生成甲酸甲酯(HCO_(2)CH_(3)),随后水解为HCOOH.该工艺利用来自化石燃料的两种原料,排放出大量CO_(2),同时在水解步骤中使用了过量的酸性水,因此对生态环境产生一定的危害.近年来,以CO_(2)为可再生原料合成HCOOH被广泛研究,人们探索了电化学、光化学、生物和热化学反应等多种途径将CO_(2)还原为HCOOH,以期实现CO_(2)循环利用.上述方法距实际应用尚有一定距离,其中,CO_(2)催化加氢制HCOOH过程最先进,它与当前许多化学工业过程相似.由于CO_(2)分子的化学稳定性,CO_(2)加氢生成HCOOH在热力学上不利,可通过有机碱和无机碱形成甲酸-碱配合物,推动反应向生成HCOOH方向移动.然而,从甲酸基配合物中提取HCOOH需要额外的分离步骤,因此在无碱条件下直接将CO_(2)加氢生成HCOOH非常重要.本课题组致力于研究无碱条件下g-C_(3)N_(4)负载的Pd(记为Pd/CN)催化剂催化CO_(2)加氢合成HCOOH,前期研究(***.,2016,52,14302-14305和***.,2021,396,395-401)发现,通过控制Pd负载量可改变催化剂中Pd单原子/Pd纳米团簇的比例,且当两种状态Pd达到最佳比例时,催化剂催化活性最高.这表明在无碱CO_(2)加氢过程中,Pd单原子与纳米团簇间存在协同作用.此外,最近有关于单原子和纳米团簇在负载金属催化剂中不同作用的报道,如NixMg1-xO催化逆水气变换反应,Ir1/In_(2)O_(3)和Pd/Fe_(3)O_(4)催化CO_(2)加氢制乙醇反应等,这些工作得出相似的结论:单原子金属对CO_(2)活化有效,而纳米团簇对H_(2)活化有效.这对于设计以CO_(2)和H_(2)为反应物的金属催化剂很重要.本工作旨在考察类似的机理是否适用于Pd/CN催化剂在无碱条件下催化CO_(2)加氢制HCOOH过程.通过控制Pd负载量,合成了含有不同比例Pd单原子和Pd纳米团簇的Pd/CN催化剂,Pd与g-C_(3)N_(4)表面碱性位间的强静电相互作用使合成含有大量Pd单原子的Pd/CN催化剂成为可能.在Pd/CN催化剂催化无碱CO_(2)加氢制HCOOH过程中,当两种状态Pd达到最佳比例时,Pd单原子和Pd纳米簇之间存在协同作用,此时催化剂活性最高.实验表明,Pd纳米簇主要通过解离吸附参与H_(2)活化,Pd单原子对CO_(2)活化更有效.在最佳单原子比(单原子数/(单原子数+纳米簇)为41%)时,该催化剂表现出最高的活性,优于文献报道的相似反应条件下Pd催化剂活性.这些发现将为以CO_(2)和H_(2)分子为反应物的反应的催化剂设计提供一定的参考.

A Pilot Study on the Effect of Pelvic Exercise on Standing Balance in Patients with Incomplete Cervical Spinal

The purpose of this study was to evaluate the effect of pelvic exercise on standing balance in patients with incomplete cervical spinal cord injury for healthy rehabilitation. 11 patients (8 men and 3 women) with levels of incomplete injury between cervical 3 and cervical 6 who were participated in a study measuring the standing balance using an interactive balance system (IBS) before and after pelvic exercise. The index of standing balance after pelvic exercise was significantly lower than before exercise in patients with incomplete cervical cord injury. The greatest improvements were observed in incomplete cervical spinal cord injury with neurologic levels of cervical 3 and 4 with Brown-Sequard syndrome. These results suggest that the pelvic exercise for rehabilitation significantly improved in part standing balance in patients with incomplete cervical spinal cord injury.

Noble strain of Sparassis latifolia produces high content of β-glucan

Objective: To classify four new Sparassis strains(CLM1, CKM1, CKM2, and KJM1) using the internal transcribed spacer sequence and to elucidate their β-glucan content and mycelial ***: Two different microbiological media were used to determine growth rate. The β-glucan contents were analyzed using the Megazyme Mushroom and Yeast Beta-Glucan kit. To determine the genetic relationships, phylogenetic trees were constructed using ClustalX. Multiple sequence alignments were printed and shaded with the BOXSHADE 3.21 program. Results: In this study, four new Sparassis strains were isolated from the southern region of the Korea Peninsula. They were all classified into the Sparassis latifolia clade as a monophyletic group based on the internal transcribed spacer sequence. Mycelial growth rate of the CLM1 strain was highest in potato dextrose agar and potato dextrose agar larch. The β-glucan content of the CLM1 strain was highest at 29.5%(w/w). A high degree of sequence divergence was detected in the RNA polymerase second largest subunit II gene(RPB2) within Sparassis spp. tested. The putative amino acid sequences of the RPB2 had a distinct sequence. The nucleotide sequences of the RPB2's intron were also divergent among Sparassis spp., even though their nucleotide length was well conserved within Sparassis latifolia. Conclusions: These results indicate that the nucleotide sequences and the amino acid sequences of RPB2 can be used to identify individual Sparassis sp. The Sparassis strain CLM1 may be best for developing a remedy to prevent or treat cancer and other chronic diseases.

The Effects of Hippotherapy on Standing Balance in Patients with Incomplete Cervical Spinal Cord Injuries: A Pilot Study

The present study was designed to evaluate the effects of hippotherapy on standing balance in patients with incomplete cervical spinal cord injuries. Ten patients (8 men and 2 women) who were diagnosed with cervical levels of injury between 3 and 6, and D according to the ASIA Impairment Scale. All patients were tested by the physiotherapists prior to and after hippotherapy treatment. Standing balance was measured using the Interactive Balance System (IBS). The weight distribution index was significantly increased in application of hippotherapy compared with control groups in patients with a neurologic level of cervical 3 and 4, and with Brown-Sequard syndrome, respectively. However, most of the patients exhibited no significant changes in the standing balance index, such as F1, F2-4, F5-6, F7-8, and SI scores, after hippotherapy. These results suggest that hippotherapy did not have a significant effect on the standing balance of patients with incomplete cervical spinal cord injuries.

High-strength bainitic steels have created a lot of interest in recent times because of their excellent combination of strength,ductility,toughness,and high ballistic mass *** steels have great potential in the fabrication of steel armor *** various approaches and methods have been conducted to utilize the retained austenite(ra)in the bainitic matrix to control mechanical properties,very few attempts have been conducted to improve ballistic performance utilizing transformationinduced plasticity(TRIP)*** this study,high-strength bainitic steels were designed by controlling the time of austempering process to have various volume fractions and stability of ra while maintaining high *** dynamic compressive and ballistic impact tests were conducted,and the relation between the effects of TRIP on ballistic performance and the adiabatic shear band(ASB)formation was *** results show for the first time that an active TRIP mechanism achieved from a large quantity of metastable ra can significantly enhance the ballistic performance of high-strength bainitic steels because of the improved resistance to ASB ***,the ballistic performance can be effectively improved by a very short austempering time,which suggests that the utilization of active TRIP behavior via tuning ra acts as a primary mechanism for significantly enhancing the ballistic performance of high-strength bainitic steels.

A case of hypereosinophilic syndrome presenting with intractable gastric ulcers

We report a rare case of hypereosinophilic syndrome （HES） presenting with intractable gastric ulcers. A 71-year-old man was admitted with epigastric pain. Initial endoscopic findings revealed multiple, active gastric ulcers in the gastric antrum. He underwent Helicobacterpylori （Hpylon） eradication therapy followed by proton pump inhibitor （PPI） therapy. However, follow- up endoscopy at 4, 6, 10 and 14 mo revealed persistent multiple gastric ulcers without significant improvement. The proportion of his eosinophil count increased to 43% （total count： 7903/mm3）. Abdominal-pelvic and chest computed tomography scans showed multiple small nodules in the liver and both lungs. The endoscopic biopsy specimen taken from the gastric antrum revealed prominent eosinophilic infiltration, and the liver biopsy specimen also showed eosinophilic infiltration in the portal tract and sinusoid. A bone marrow biopsy disclosed eosinophilic hyperplasia as well as increased cellularity of 70%. The patient was finally diagnosed with HES involving the stomach, liver, lung, and bone marrow. When gastric ulcers do not improve despite H pylon eradication and prolonged PPI therapy, infiltrative gastric disorders such as HES should be considered.
