Oxytocin and early parent-infant interactions: A systematic review

Sonoelastography in the musculoskeletal system:Current role and future directions

Ultrasound is an essential modality within musculoskeletal imaging, with the recent addition of elastography. The elastic properties of tissues are different from the acoustic impedance used to create B mode imaging and the flow properties used within Doppler imaging, hence elastography provides a different form of tissue assessment. The current role of ultrasound elastography in the musculoskeletal system will be reviewed, in particular with reference to muscles, tendons, ligaments, joints and soft tissue tumours. The different ultrasound elastography methods currently available will be described, in particular strain elastography and shear wave elastography. Future directions of ultrasound elastography in the musculoskeletal system will also be discussed.

Influence of Land Use Activities on Riparian Vegetation, Soil and Water Quality: An Indicator of Biodiversity Loss, South West Mau Forest, Kenya

Watershed and riparian areas of Mau Forest Complex in Kenya are experiencing increased threats due to unsustainable land use activities geared towards economic growth amidst growing population. This study was carried out to examine effects of land use activities on riparian vegetation, soil and water quality along two major rivers (Chemosit and Kipsonoi) of South West Mau Forest (SWMF). Land use activities adjacent to these rivers and biodiversity disturbance on the riparian zone were identified and underpinned to changes on Total Nitrogen, Total Phosphorous, Potassium, Sulphur, Cadmium, Copper, Lead, Total Suspended Solids and soil Organic Carbon. Three sampling sites designated(upstream, midstream and downstream) were identified and established along each river as guided by existing land use activities represented by forest, tea plantation and mixed agricultural farming respectively. At each sampling site, a 200 m × 50 m section was systematically marked on each side of the river bank;the longest side being parallel to the river flow and divided into three belts transects each 20 m × 50 m, spaced 70 m apart. Six distinct land use activities (indigenous forest, food crop, tree and tea farming, livestock keeping and urban settlement) were identified as the major land use activities in SWMF. Plant species richness decreased and overall riparian disturbance increased from upstream (intact canopy with native vegetation) to mid-stream and downstream as epitomized by the structure, biodiversity disturbance resulting from extensive and intensive farming, intrusion of exotic species to livestock grazing and urban settlement. Variation among sampling sites in Total Suspended Solids, pH, Total Nitrogen, Phosphorus and Potassium were associated to different land use activities along the riparian zone. endoscopic submucosal dissection （eSD） with a knife is a technically demanding procedure a ssociated with ahigh complication rate. The shortcomings of this meth-od are the inability to fix the knife to the target lesion,and compression of the lesion. These can lead to major complications such as perforation and bleeding. To reduce the risk of complications related to eSD, we developed a new grasping type scissors forceps （GSF）,which can grasp and incise the targeted tissue using electrosurgical current. Colonoscopy on a 55-year-old woman revealed a 10-ram rectal submucosal *** histological diagnosis of the specimen obtained by biopsy was carcinoid tumor. endoscopic ultrasonog-raphy demonstrated a hypoechoic solid tumor limitedto the submucosa without lymph node involvement. Itwas safely and accurately resected without unexpectedincision by eSD using a GSF. No delayed hemorrhage or perforation occurred. Histological examination confirmed the carcinoid tumor was completely excisedwith negative resection margin.

Interprofessional Clinical Simulation-Based education to Develop Professional Identity and Professionalism in Japanese Medical and Nursing Students

Introduction: We conducted a multi-occupational team simulation training for medical and nursing students and clarified how professional identity and professionalism attitudes change with interprofessional education (IPe). Methods: Thirty-nine 4th-year medical students and 48 2nd-year nursing students were enrolled and distributed to the educational intervention group and the control group. We used a vocation identity scale including lower four subscales, a scale for professionalism including lower five subscales, a readiness for inter-professional learning scale (RIPLS), and an interdisciplinary education perception scale (IePS). Results: Among the medical students, IPe using an advanced patient simulator improved the scores on the vocational identity scale, scale for professionalism, RIPLS, and IePS. Among the nursing students, IPe improved the scores on the vocational identity scale, scale for professionalism, RIPLS, and IePS. Conclusion: On-the-job training using simulated clinical training by a multi-occupational team improved vocational identification and professionalism.

Immunological aspects of age-related diseases

The proportion of elderly people rises in the developed countries. The increased susceptibility of the elderly to infectious diseases is caused by immune dysfunction, especially T cell functional decline. Age-related hematopoietic stem cells deviate from lymphoid lineage to myeloid lineage. Thymus shrinks early in life, which is followed by the decline of na?ve T cells. T-cell receptor repertoire diversity declines by aging, which is caused by cytomegalovirus-driven T cell clonal expansion. Functional decline of B cell induces antibody affinity declines by aging. Many effector functions including phagocytosis of myeloid cells are down regulated by aging. The studies of aging of myeloid cells have some controversial results. Although M1 macrophages have been shown to be replaced by antiinflammatory(M2) macrophages by advanced age, many human studies showed that pro-inflammatory cytokines are elevated in older human. To solve this discrepancy here we divide age-related pathological changes into two categories. One is an aging of immune cell itself. Second is involvement of immune cells to age-related pathological changes. Cellular senescence and damaged cells in aged tissue recruit pro-inflammatory M1 macrophages, which produce pro-inflammatory cytokines and proceed to agerelated diseases. Underlying biochemical and metabolic studies will open nutritional treatment.

endoscopic appearance of AIDS-related gastrointestinal lymphoma with c-MYC rearrangements: Case report and literature review

Acquired immune deficiency syndrome (AIDS)-related lymphoma (ARL) remains the main cause of AIDS-related deaths in the highly active anti-retroviral therapy (HAART) era. Recently, rearrangement of MYC is associated with poor prognosis in patients with diffuse large B-cell lymphoma. Here, we report a rare case of gastrointestinal (GI)-ARL with MYC rearrangements and coinfected with epstein-Barr virus (eBV) infection presenting with various endoscopic findings. A 38-yearold homosexual man who presented with anemia and was diagnosed with an human immunodeficiency virus infection for the first time. GI endoscopy revealed multiple dish-like lesions, ulcerations, bloody spots, nodular masses with active bleeding in the stomach, erythematous flat lesions in the duodenum, and multiple nodular masses in the colon and rectum. Magnified endoscopy with narrow band imaging showed a honeycomb-like pattern without irregular microvessels in the dish-like lesions of the stomach. Biopsy specimens from the stomach, duodenum, colon, and rectum revealed diffuse large B-cell lymphoma concomitant with eBV infection that was detected by high tissue eBV-polymerase chain reaction levels and epstein-Barr virus small RNAs in situ hybridization. Fluorescence in situ hybridization analysis revealed a fusion between the immunoglobulin heavy chain (IgH) and c-MYC genes, but not between the IgH and BCL2 loci. After 1-mo of treatment with HAART and R-CHOP, endoscopic appearance improved remarkably, and the histological features of the biopsy specimens revealed no evidence of lymphoma. However, he died from multiple organ failure on the 139 th day after diagnosis. The cause of his poor outcome may be related to MYC rearrangement. The GI tract involvement in ARL is rarely reported, and its endoscopic findings are various and may be different from those in non-AIDS GI lymphoma; thus, we also conducted a literature review of GI-ARL cases.

Background: Multiple system atrophy (MSA) is a neurodegenerative condition characterized by variable combinations of parkinsonism, autonomic failure, cerebellar ataxia and pyramidal features. Although the distribution of synucleinopathy correlates with the predominant clinical features, the burden of pathology does not fully explain observed differences in clinical presentation and rate of disease progression. We hypothesized that the clinical heterogeneity in MSA is a consequence of variability in the seeding activity of α-synuclein both between different patients and between different brain regions. Methods: The reliable detection of α-synuclein seeding activity derived from MSA using cell-free amplification assays remains challenging. Therefore, we conducted a systematic evaluation of 168 different reaction buffers, using an array of pH and salts, seeded with fully characterized brain homogenates from one MSA and one PD patient. We then validated the two conditions that conferred the optimal ability to discriminate between PD- and MSA-derived samples in a larger cohort of 40 neuropathologically confirmed cases, including 15 MSA. Finally, in a subset of brains, we conducted the first multi-region analysis of seeding behaviour in MSA. Results: Using our novel buffer conditions, we show that the physicochemical factors that govern the in vitro amplification of α-synuclein can be tailored to generate strain-specific reaction buffers that can be used to reliably study the seeding capacity from MSA-derived α-synuclein. Using this novel approach, we were able to sub-categorize the 15 MSA brains into 3 groups: high, intermediate and low seeders. To further demonstrate heterogeneity in α-synuclein seeding in MSA, we conducted a comprehensive multi-regional evaluation of α-synuclein seeding in 13 different regions from 2 high seeders, 2 intermediate seeders and 2 low seeders. Sources of Sexual Knowledge among Vietnamese High School Students

This study determined the sources from which Vietnamese high school students currently acquire sexual knowledge as well as the sources from which they believe they should acquire it. Participants were eleventh-grade students at three public high schools in Hanoi, Vietnam. We conducted a questionnaire-based survey from September to October 2012, with 1672 students participating. Responses were received from all 1672 students, a response rate of 100%. Complete data were received from 1653 respondents (98.9%), including 896 males (54.2%) and 757 females (45.8%). The survey was a self-administrated questionnaire. Although many participants believed that they were knowledgeable about sex, only a small number of them actually possessed accurate sexual knowledge. Few participants in this study, targeted to a specific age (eleventh grade), had used the Internet as a major source for obtaining sexual knowledge, in contrast to results from previous studies which covered a wider age demographic. Future research should include comparative studies between eleventh and twelfth graders as well as between those of the same age who are and who are not enrolled in high school. Additionally, it might be beneficial to consider the relationship between adolescent age and Internet use to study the influence of the Internet on sexual knowledge. Our results suggest that parents and schoolteachers should shoulder more responsibility in providing sexual education, particularly by improving their own knowledge and ability to provide such education, because many participants indicated that sexual knowledge should be acquired from parents and schools.
