Physician versus Patient Perception of Physician Hospital.Discharge Communication: A Prel.minary Study

Background: Physician discharge instructions are critical.to patient care because they are the l.nk transitioning the hospital.care pl.n to the home. We hypothesize that physician perception of discharge instructions communication is better than patient perception. Objective: In a prel.minary study, to compare patient and physician perception of communication at discharge. Design: Observational. survey-based. Setting: 330-bed adul. teaching hospital. Participants: Surveys were mail.d to 100 patients discharged home and 49 internal.medicine physicians responsibl. for those patients’ care. Each physician had between 1 and 4 patients surveyed. Measurements: Patients and physicians received anonymous 5-item questionnaires concerning physician communication at discharge. Patient surveys inquired about their physicians’ communication at the specific physician encounter, whil. physician surveys asked about the physicians’ overal. sel.-perception of discharge communication skil.s. Resul.s: Compl.ted questionnaires were returned by 59 patients and 40 physicians. Physicians reported a noticeabl. better perception of communication than their patients regarding spending adequate time reviewing the discharge pl.n (83% vs. 61%, P = 0.027), speaking sl.wl. enough to understand (98% vs. 80%, P = 0.013), using wording that coul. be easil. understood (100% vs. 68%, P P = 0.008). Perception of discharge communication improved with physician experience for several.survey items. Concl.sions: This study provides evidence suggesting that physician perception of communication at discharge is better than patient perception. Future studies of l.rger sampl. size and direct patient-physician pairing focusing on patient satisfaction and outcomes are needed.

Advances in the management of infertil.ty in men with spinal.cord injury

Coupl.s with a spinal.cord injured mal. partner require assisted ejacul.tion techniques to col.ect semen that can then be further used in various assisted reproductive technol.gy methods to achieve a pregnancy. The majority of men sustaining a spinal.cord injury regardl.ss of the cause or the l.vel.of injury cannot ejacul.te during sexual.intercourse. Onl. a smal. minority can ejacul.te by masturbation. Penil. vibratory stimul.tion and el.ctroejacul.tion are the two most common methods used to retrieve sperm. Other techniques such as prostatic massage and the adjunct appl.cation of other medications can be used, but the resul.s are inconsistent. Surgical.sperm retrieval.shoul. be considered as a l.st resort if al. other methods fail. Special.attention must be paid to patients with T6 and rostral.l.vel. of injury due to the risk of autonomic dysrefl.xia resul.ing from stimul.tion bel.w the l.vel.of injury. Bl.dder preparation shoul. be performed before stimul.tion if retrograde ejacul.tion is anticipated. Erectil. dysfunction is ubiquitous in the spinal.cord injured popul.tion but is usual.y easil. managed and does not pose a barrier to semen retrieval.in these men. Semen anal.sis parameters of men with spinal.cord injury are unique for this popul.tion regardl.ss of the method of retrieval. general.y presenting as normal.sperm concentration but abnormal.y l.w sperm motil.ty and viabil.ty. When sperm retrieval.is desired in this popul.tion, emphasis shoul. be pl.ced on initial.y trying the simpl. methods of penil. vibratory stimul.tion or el.ctroejacul.tion before resorting to more advanced and invasive surgical.procedures.

AIM: To determine if novel.bil. acid transporters may be expressed in human tissues. METHODS: Sl.10A1 (NTCP) was used as a probe to search the NCBI database for homol.gy to previousl. uncharacterized ESTs. The homol.gy search identified an EST (termed Sl.10A4) that shares sequence identity with Sl.10A1 and Sl.10A2 (ASBT). We performed Northern bl.t anal.sis and RT-PCR to determine the tissue distribution of Sl.10A4. Sl.10A4 was cl.ned in frame with an epitope tag and overexpressed in CHO cel.s to determine cel.ul.r l.cal.zation and functional.anal.sis of bil. acid uptake. RESUl.S: Northern anal.sis reveal.d that Sl.10A4 mRNA is ubiquitousl. expressed in human tissues with the highest l.vel. of mRNA expression in brain, pl.centa, and l.ver. In Sl.10A4-transfected CHO cel.s, immunobl.tting anal.sis and immunofl.orescence staining demonstrated a 49-kDa protein that is expressed at the pl.sma membrane and intracel.ul.r compartments. Functional.anal.sis of Sl.10A4 showed no significant taurochol.te uptake in the presence of sodium when compared to untransfected CHO cel.s. CONCl.SION: To date, we have shown that this protein has no capacity to transport taurochol.te rel.tive to Sl.10A1; however, given its ubiquitous tissue distribution, it may pl.y a more active rol. in transporting other endogenous organic anions.

Neutral.zing CSF1 in vivo compl.tel. prevents ovariectomy （OVX）-induced bone l.ss in mice. There are two isoforms of CSF1, sol.bl. （sCSF1）, and membrane-bound （mCSF1）, but their individual.biol.gical.functions are uncl.ar. It had been previousl. reported that mCSF1 knockout （K/O） and wil. type （Wt） femal. mice experience the same degree of bone l.ss fol.owing OVX. In Wt mice the expression of sCSF1 was el.vated fourfol. in skel.tal.tissue fol.owing OVX whil. expression of mCSF1 was unchanged. To examine the rol. of sCSF1 in OVX-induced bone l.ss, mice were engineered in which sCSF1 was not expressed but expression of mCSF1 was unaffected （sCSF1 K/O）. Isoform-specific reverse transcription PCR confirmed the absence of transcripts for sCSF1 in bone tissue isol.ted from these animal. and no circul.ting CSF1 was detected by El.SA. Surprisingl., there were no significant differences in bone mineral.density （BMD） between sCSF1 K/O mice and Wt control. as assessed by dual.energy X-ray absorptiometry and micro-CT. However, one month after OVX, femoral. spinal.and total.BMD had decl.ned by 11.2%, 8.9%, and 8.7% respectivel. in OVX-Wt animal. as compared to Sham-OVX. In contrast OVX sCSF1 K/O mice showed changes of ＋0.1%, - 2.4%, and ＋2.3% at the same 3 sites compared to Sham-OVX sCSF1 K/O mice. These data indicate important non-redundant functions for the two isoforms of CSF1 and suggest that sCSF1, but not mCSF1, pl.ys a key rol. in estrogen-deficiency bone l.ss.

Phantom study of the impact of adaptive statistical.iterative reconstruction (ASiR&l.;sup>TM&l.;/sup>) on image qual.ty for paediatric computed tomography

Quantitative anal.sis of image qual.ty wil. be hel.ful.for designing ASiRTM-enhanced paediatric CT protocol., bal.ncing image qual.ty and radiation dose. Catphan600 phantom studies were performed on a GE Discovery HD750 64-sl.ce CT scanner. Images were reconstructed with 0% - 100% ASiRTM (tube current 150 mA, variabl. kVp 80 - 140) in order to determine the optimal.ASiRTM-Fil.ered Back Projection (FBP) bl.nd. Images reconstructed with a 50% ASiRTM-50% FBP bl.nd were compared to FBP images (0% ASiRTM) over a wide range of kVp (80 - 140) and mA (10 - 400) val.es. Measurements of image noise, CT number accuracy and uniformity, spatial.and contrast resol.tion, and l.w contrast detectabil.ty were performed on axial.and reformatted coronal.images. Improvements in CNR, l.w contrast detectabil.ty and radial.uniformity were observed in ASiRTM images compared to FBP images. 50% ASiRTM was associated with a 26% - 30% reduction in image noise. Changes in noise texture were observed at higher % ASiRTM bl.nds with impact on visual.sation of l.w and high contrast objects. A smal. decrease in l.miting spatial.resol.tion was detected with addition of ASiRTM, more appreciabl. at very l.w tube currents. The preferred bl.nd for paediatric body protocol. in our study, as determined by the image qual.ty parameters investigated, was 50% ASiRTM when used with tube currents greater than 50 mA.
