3-Bromopyruvate(3BP) is a new, promising anticancer alkylating agent with several notable functions. In addition to inhibiting key glycolysis enzymes including hexokinase II and lactate dehydrogenase(LDH), 3BP also selectively inhibits mitochondrial oxidative phosphorylation, angiogenesis, and energy production in cancer cells. Moreover, 3BP induces hydrogen peroxide generation in cancer cells(oxidative stress effect) and competes with the LDH substrates pyruvate and lactate. There is only one published human clinical study showing that 3BP was effective in treating fibrolamellar hepatocellular carcinoma. LDH is a good measure for tumor evaluation and predicts the outcome of treatment better than the presence of a residual tumor mass. According to the Warburg effect, LDH is responsible for lactate synthesis, which facilitates cancer cell survival, progression, aggressiveness, metastasis, and angiogenesis. Lactate produced through LDH activity fuels aerobic cell populations inside tumors via metabolic symbiosis. In melanoma, the most deadly skin cancer, 3BP induced necrotic cell death in sensitive cells, whereas high glutathione(GsH) content made other melanoma cells resistant to 3BP. Concurrent use of a GsH depletor with 3BP killed resistant melanoma cells. survival of melanoma patients was inversely associated with high serum LDH levels, which was reported to be highly predictive of melanoma treatment in randomized clinical trials. Here, we report a 28-year-old man presented with stage IV metastatic melanoma affecting the back, left pleura, and lung. The disease caused total destruction of the left lung and a high serum LDH level(4,283 U/L). After ethics committee approval and written patient consent, the patient received 3BP intravenous infusions(1-2.2 mg/kg), but the anticancer effect was minimal as indicated by a high serum LDH level. Background: Minimal hepatic encephalopathy (MHE) is defined as hepatic encephalopathy (HE) without symptoms on clinical/neurological examination, but with deficits in some cognitive areas that can only be measured by neuropsychometric testing. Aim: Our aim was to study serum zinc levels in cirrhotic patients with and without MHE and correlate it with the results of neuropsychological tests and P300 event related potential. Patients and Methods: Fifty-five patients with liver cirrhosis were recruited for the present study. All patients were subjected to full history taking, clinical examination, abdominal ultrasonography, laboratory investigations including liver function tests and serum zinc, psychometric studies (line tracing test and serial dotting test) and P300 event related potential. According to the results of psychometric studies and P300 event related potential, the patients were divided into 3 groups in addition to the control group. Results: Among the 55 patients included in the study, 30 patients had no clinical evidence of HE and 25 patients had overt HE with different grades. In patients with no clinical evidence of HE, psychometric tests and P300 event related potential revealed that 13 patients had MHE, while 17 had no HE. There were significantly lower serum zinc levels in patients with MHE and in cirrhotic patients without HE compared with the healthy controls. Also, zinc levels were significantly lower in higher grades of hepatic encephalopathy. serum zinc levels showed positive correlation with serum albumin levels and inverse correlation with serial dotting test and P300 latency. Conclusion: Reduced serum zinc level and prolonged P300 latency can serve as predictors of minimal hepatic encephalopathy in cirrhotic patients. Zinc deficiency is common in cirrhotic patients with MHE. These findings may have important prognostic and therapeutic implications in MHE and overt HE patients with zinc deficiency.

Biofertilizers consist mainly of beneficial microorganisms that can release nutrients from raw materials and plant residues in the soil and make them available commercially where specific strains are used as biological fertilizers. They become recently, positive alternatives to chemical fertilizers because they help bring down the costs of chemical fertilizers especially N and P and improve soil fertility by maintaining the physical properties of the soil. They may help in improving crop productivity and quality by increasing the biological N fixation, the availability and uptake of nutrients and stimulating the natural hormones. They are safe for humans, animals and environment and using them is accompanied with reducing the pollution occurring in our environment.

Background: Patient falls in hospitals are common and affect approximately 2% to 17% of patients during their hospital stay. Patient falls are a nursingsensitive quality indicator in the delivery of inpatient services. Objective: To assess the effect of educational training program on nurses’ knowledge and performance regarding prevention of fall at one of the health insurance organization hospitals in Alexandria. setting: The study was conducted at 284 bed general hospital affiliated with the Health Insurance Organization in Alexandria. Design: A quasi-experimental design was followed. Participants: The study sample included all nurses of different ranks working at four departments namely, orthopedic, medical, surgical, ICU unit. Results: There was a significant difference regarding all factors under study before and after the educational programme except for two individual factors, old age (p = 0.84), overall poor health status (p = 0.38), and two health factors, uses aids (p = 0.50), treatment by heparin (p = 1.00), and two environmental factors, poor lighting (p = 0.34), loose cords or wires (p = 0.30) and bells (p = 0.30), and one miscellaneous factor, patient education (p = 0.85) and tidy environment(p = 0.85). All departments showed posttest performance improvement, the total performance median for departments regarding environmental factor (p = 0.04) and health education (p = 0.001). Conclusion: Education programmes should be regularly, updated in view of changing knowledge and work practices.

Objective： Over 50% of the world populations are infected with Helicobacter pylori （H. pylori）. Most subjects are asymptomatic; however, in 1994, H. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between H. pylori infection and gastric cancer. Methods： Thirty gastric cancer patients （GCs） and 30 gastritis patients were enrolled in the study. H. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the H. pylori 16s rRNA gene in addition to cagA, vacA and iceA genes. Results： H. pylori could be cultured from 100% of specimens obtained from all patients. The density of H. pylori was higher in cancer cases than in gastritis patients. The 16s rRNA was detected in all GC patients （100%） while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients （by at least one of its alleles） while it was only found in 33.3% of gastritis patients. The vacA slml combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients （10%）. The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. simultaneous presence of multiple H. pyiori strains was proved, both phenotypically and genotypically. Conclusion： The development of GC is linked to infection with H. pylori harboring certain virulence genes. Higher infection density of H. pylori was found in GC patients. Co-existence of more than one strain of H. pylori in the same patient occurs in both malignant and benign lesions.

Nephroprotective Effect of <i>Nigella sativa</i>and <i>Matricaria Chamomilla</i>in Cisplatin Induced Renal Injury—supportive Treatments in Cisplatin Nephrotoxicity

Nigella sativa and Matricaria chamomilla are extensively consumed as tea or tonic. Despite their widespread use as a home remedy, relatively few trials evaluated their benefits in nephroprotection. Hence, this study evaluates the nephroprotective effects of supportive treatments (N. sativa, M. chamomilla and vitamin E) in cisplatin nephrotoxicity rat model. Eighty rats divided into 10 groups, of 8 animals each. The first group (G1) injected with saline intra-pretoneal (i.p). G2 injected with 5 mg/kg cisplatin i.p on zero day of experiment and repeated 4 times, with 5 days free interval. G3-G10 received daily supportive treatments, started 5 days before the experiment (–5day). Concomitantly G4, G6, G8 and G10 injected with 5 mg/kg cisplatin i.p like G2. On day sixteen, animal scarified, serum and/or kidney tissue were used to determine kidney function tests (serum urea, creatinine, NAG, β-gal), oxidative stress indices (NO, LPO), antioxidant activities (sOD), sulphur compounds (GGT, GsH, total thiols ), apoptotic indices (cathepsin D, DNA fragmentation), two minerals (Ca2+ and zn2+). Cisplatin caused marked elevation in serum GGT that reduced signifi-cantly in group received M. chamomilla with cisplatin (P s a correlation between GGT and NAG in cisplatin group (r = 0.731 p suggest one of possible mechanisms of renal injury by cisplatin. M. chamomilla followed by N. sativa and vitamin E improved the biochemical and pathological renal injury, as determined by increasing the body weight, normalizing the kidney functions, decreasing the oxidative stress markers, improving the apoptotic markers, minimizing the pathological changes. Hence, N. sativa and M. chamomilla will be a promising nephroprotective agents for reducing cisplatin nephrotoxicity, most probably, by antioxidants effects and inhibition GGT production, respectively.

Relation between serum progranulin, inflammatory markers and visceral fat in childhood obesity

Aim: To study serum progranulin (PGRN) level in children with obesity and its relationship to inflamamatory markers and visceral fat. Methods: Fifty obese children and 50 controls aged 10-18 years were recruited. Demographic, anthropometric and biochemical features were collected according to a standard protocol. serum progranulin levels, serum IL-6 and hsCRP were measured using elIsA. Insulin resistance was calculated by the homeostasis model (HOMA-IR) using the following formula: HOMA-IR = fasting insulin (mU/L) × fasting glucose (mmoL/L)/ 22.5. The maximum visceral fat thickness (VFT) and the minimum subcutaneous fat thickness (sFT) were measured by ultrasonography. Results: In the obese group, a significant increase was found in serum PGRN (48.87 ± 42.33 ng/mL) compared to control group (30.18 ± 23.82 ng/mL). Progranulin correlated significantly with VFT (r = 0.475), IL6 (r = 0.368), Insulin(r = 0.440) and HOMA-IR (r = 0.379). The mean serum progranulin in the high tertile VFT group was significantly higher than those in the low tertile and middle tertile groups (P = 0.030 and P = 0.039 respectively). VFT was highly positively correlated to progranulin, sFT, IL6, insulin, HOMA-IR and hsCRP (P = 0.001, 0.000, 0.001, 0.001, 0.003 and 0.003). However, the correlation coefficient between sFT and progranulin was insignificant. summary: we demonstrated for the first time that serum PGRN concentrations increased in Egyptian obese children. The concentrations of serum PGRN correlated closely with visceral fat and IL6. Conclusion: PGRN may contribute to the pathogenesis of chronic inflammation in obesity. It could be a novel marker of visceral fat in obesity. Thus PGRN could be a potential therapeutic target for management of chronic inflammation in obesity.

The concept of the effective mass in crystals shows that the electron mass is affected by the crystal field and was experimentally verified. A useful expression for effective mass was obtained. Unfortunately this expression showed that the effective mass vanishes in the ab-sence of the external field. This is in conflict with observations which show that it reduces to the ordinary mass. To cure this defect a new model is developed assuming the existence of vacuum force as verified experimentally as shown by Casimir effect. Using Newton’s second law and the quantum expression of momentum, useful expressions were found. The same expression was found using generalized special relativity. strikingly the three models reduced to the conventional one in the absence of vacuum, they also reduced to the ordinary electron mass in the absence of all forces.

A pressure dependent schrodinger equation is used to find the conditions that lead to superconductivity. When no pressure is exerted, the superconductor resistance vanishes beyond a critical temperature related to the repulsive force potential of the electron gass, where one assuming the electron total energy to be thermal, where applying mechanical pressure destroys sc when it exceeds a certain critical value. However when the electron total energy is an assumed to be that of the free electron model and that the pressure is thermal and mechanical, the situation is different. The quantum expression for resistance shows that the increase of mechanical pressure increases the critical temperature. such phenomenon is observed in high temperature cupper group.
