Postoperative Complication in Overweight Patients Undergoing Coronary Artery Bypass Graft Surgery

Background: Obesity has a great impact on overall mortality and morbidity in cardiac surgery. The magnitude of obesity is defined by means of body mass index (BMI). At this study we aim to correlate between grade of BMI and postoperative complications in patients undergoing coronary revascularization. Methods: A prospective observational study was conducted in Ain Shams University hospitals and the National Heart Institute of Egypt. 98 patients with multi-vessel CAD and candidate for CABG were included in the study and divided into two groups Group I: 53 patients with BMI over 30. Group II: 45 patients with a BMI under 30. The primary endpoint was wound infection and mediastinitis. Secondary endpoints included mortality, prolonged ICU and hospital stay, stroke, renal and hepatic impairment. Results: As regard primary ending point, eight patients of Group I had their wounds infected which is significantly higher than the other group (p-value 0.034). The total ward stay in days was significantly higher in Group I (p-value 0.027). Conclusion: BMI more than 30 is associated with increased rates of wound complications either superficial or deep wound infections. Robust measures should be taken to prevent such grave complications.

Water security is a very important purpose facing the world in the last decay as one of the main consequences related to the risks of climate change. Water quality of Litani River’s at Quaraoun Lake station in Lebanon was evaluated by measuring various physicochemical parameters between 2008 and 2018. Samples were performed and analyzed the first of every month. Different analytical methods were used to determine the values of 11 physico-chemical parameters. Different statistical methods, such as Principal Component Analysis and the Times Series Representation, were applied to the results to evaluate the water characteristics, determine the operation of the ecosystem and study the correlation between the different parameters in the Quaraoun station. Industrial, agricultural and sewage water pollution of the Litani River were demonstrated by the high concentration of sulfate, phosphate and ammonia. High bacterial activity was proved from the high decreasing of the dissolved oxygen. Important correlations between the different parameters and between the parameters of the ecosystem and the weather were proved.

A simple, sensitive, precise and robust high-performance thin-layer chromatographic (HPTLC) method for analysis of tricin has been developed and validated for the determination of tricin in genus Spergularia. Analysis of tricin was performed on pre-coated TLC aluminum plates with silica gel as the stationary phase. Linear ascending development was carried out in double twin trough glass chamber saturated with a mobile phase consisting of chloroform: methanol (37:3 v/v). Spectro-densitometric scanning was performed by TLC scanner III (CAMAG) in absorbance mode at the wavelength of 270 nm. The system was found to give compact spots for tricin (Rf value of 0.34 ± 0.04). The linear regression analysis data for the calibration plots showed a good linear relationship with r2 = 0.9981 in the concentration range 2-15 μl with respect to peak area. According to the International Conference on Harmonization (ICH) guidelines, the method was validated for precision, recovery, and robustness. The tricin content quantified from Spergularia marina and S. daiandra was found to be 0.398 mg/100g and 1.642 mg/100g fresh weight, respectively. Statistical analysis of the data showed that the method is reproducible and selective for the estimation of tricin.

Objectives: Diabetic retinopathy (DR) is one of the most common microvascular complications of type 2 diabetes mellitus (T2DM). It is multifactorial with the contribution of multiple genetic factors. We questioned the association of polymorphisms in the peroxisome proliferator-activated receptor ?2 (PPAR?2) gene (Pro12Ala and C161T) with DR in an Egyptian population. Methods: This case control study included one hundred healthy individuals and 252 T2DM among them 122 with DR and 130 without DR. Genotyping was done by polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP). Results: The Pro12Ala Ala allele was associated with decreased risk of DR with an odds ratio (OR) of 0.484, 95% confidence interval (CI) (0.254 - 0.920), and a p value = 0.024. The C161T T allele was associated with increased risk of DR with OR = 2.593, 95% CI (1.672 - 4.020), p 1c) in multivariate regression analysis only C161T was associated with increased risk of DR with OR = 3.479, 95% CI (1.907 - 6.346), p 1c was higher in Pro/Pro genotype when compared to those with Ala/Ala and Pro/Ala genotypes. Conclusion: We report that T allele of C161T increased risk of DR in the studied population. Further studies are warranted to investigate functional implications of polymorphisms of the PPAR-? gene in DR development.

A direct synthesis method is introduced to prepare nano-porous silicon-nickel nanocomposite (nPS/Ni) powder for thermal isolation applications. In this paper, we study the thermal stability of nanocomposites consisting of nanoparticles metal incorporated into the pores of a porous silicon by a very simple method. The nickel element is chemically deposited whereas the nanoparticles are precipitated on the pore surfaces. The (nPS) and (nPS/Ni) nano-materials are thermally measured under nitrogen at temperatures of 40℃ - 1000℃, noticeable, demonstrating better thermal stability of (nPS/Ni) until 900℃ than in the case of (nPS) at 600℃. Then, the improving of the thermal stability of the nPS powder is facilitated using it in many applications of the thermal insulation process.

Objective: This study aimed to assess the relationship of cognition, depression and anxiety to glycemic control in elders with diabetes. DM is a chronic medical condition. Its control depends on adherence to medical therapy and making decisions related to lifestyle changes. This decision making capacity is affected by many factors including cognition and psychological status. Design: It was a case control study. Setting: It was done in Ain Shams University Hospital inpatients and DM outpatient clinic, Cairo, Egypt. Participants: Of the one hundred diabetic patients aged ≥ 60 years, 50 had Hemoglobin A1c (HbA1c) ≥ 7.5 (cases) and 50 had Hb A1c 7.5 (controls). Measurements: Cognition was assessed using minimental status examination (MMSE) test, Mattis Organic Mental Syndrome Screening Examination (MOMSSE) and Cambridge Cognitive Examination (CAMCOG) test. Geriatric depression scale-15 (GDS-15) was performed for depression assessment, while anxiety was assessed by DSM IV criteria. Laboratory investigations included: fasting blood sugar (FBS), post-prandial blood sugar (PPBS), glycated haemoglobin (Hb A1c), low density lipoprotein (LDL), high-density lipoprotein (HDL), total cholesterol, and triglycerides (TG). Results: Significant difference was found between the two groups regarding scores of cognitive tests: MMSE score (p = 0.004);below average (p = 0.02) and average scores (p = 0.05) of MOMSSE;CAMCOG score (p = 0.015);and CAMCOG divided items score including orientation (p = 0.003), comprehension (p = 0.005), expression (p = 0.020), attention (p = 0.002), and abstraction (p = 0.008) as well as depression screening scores (P = 0.002). Using Receiver Operating Characteristic, CAMCOG had better sensitivity and MOMSSE had better specificity. Conclusion: Cognitive impairment was associated with poor glycemic control, and impairment in attention and abstraction, related to executive function, functions were found to be associated with poor glycemic control.
