Evaluation of urea breath test as a diagnostic tool for Helicobacter pylori infection in adult dyspeptic patients

In this editorial,we discuss the article in the World Journal of *** article conducts a meta-analysis of the diagnostic accuracy of the urea breath test(UBT),a non-invasive method for detecting Helicobacter pylori(***)infection in *** is based on radionuclide-labeled *** methods,both invasive and non-invasive,are available for diagnosing *** infection,inclu-ding endoscopy with biopsy,serology for immunoglobulin titers,stool antigen analysis,and *** guidelines recommend UBTs as the primary choice for diagnosing *** infection and for reexamining after eradication *** is used to be the first choice non-invasive test due to their high accuracy,specificity,rapid results,and ***,its performance remains unaffected by the distribution of *** in the stomach,allowing a high flow of patients to be *** its widespread use,the performance characteristics of UBT have been inconsistently described and remain incompletely *** are two UBTs available with Food and Drug Administration approval:The 13C and 14C *** tests are affordable and can provide real-time *** may prefer the 13C test because it is non-radioactive,compared to 14C which uses a radioactive isotope,especially in young children and pregnant *** there was heterogeneity among the studies regarding the diagnostic accuracy of both UBTs,13C-UBT consistently outperforms the *** makes the 13C-UBT the preferred diagnostic ***,the provided findings of the meta-analysis emphasize the significance of precise considerations when choosing urea dosage,assessment timing,and measurement techniques for both the 13C-UBT and 14C-UBT,to enhance diagnostic precision.

Diagnostic and Prognostic Value of Nuclear Factor Kappa-B in Diffuse Large B Cell Lymphoma in Egyptian Patients with Hepatitis C Virus Genotype 4

Background: Members of the NFκB [p65] family have potential diagnostic and prognostic role in various inflammatory diseases and Lymphomas. Aim: We studied NFκB [p65] in paraffin blocks of hepatitis-C-virus [HCV] positive genotype-4 and HCV negative diffuse large B-cell lymphoma [DLBCL] patients, aiming at identification of its differential expression and prognosis in DLBCL and its subtypes;GCB and ABC. This is to establish its relation to HCV infection and its role in lymphogenesis. Besides assessing the role of new directly acting antiviral drugs [Sofusbuvir/Ledipasvir] concomitantly administered to [CHOP] combination in HCV positive DLBCL. Subjects and Methods: NFκB [p65] expression was assessed using Anti-NFκB [p65] antibody semi-quantitative technique in 30 newly diagnosed DLBCL patients [HCV positive [n = 15], HCV negative [n = 15]. Results: NFκB [p65] expression was higher in the HCV positive DLBCL patients than their HCV negative counterpart, with a positive correlation with the viral load [r = 0.536, p = 0.088]. NFκB [p65] expression was significantly more frequently detected in the ABC subtype than GCB subtype [p = 0.04]. Patients who expressed NFκB [p65] had higher incidence of extranodal involvement, advanced stages, higher LDH levels and IPI score. Besides, the expression of NFκB [P65] revealed an inferior overall response [OR] [p = 0.044]. Higher complete response rates to CHOP concomitantly with antiviral [ledipasvir/sofosbuvir] were encountered in the HCV positive group. In HCV positive group, NFκB [P65] displayed a positive relationship with the viral load and liver enzymes [p = 0.04], besides an inverse relation with serum albumin. This raises the possibility that NFκB [p65] expression is suggestive of the hepatic necro-inflammation in HCV patients. The ABC group presented more in advanced stages than GCB. The Diagnostic and Prognostic Impact of Serum miRNA-21 in a Sample of Hepatitis C/None Hepatitis Diffuse Large B Cell Lymphoma Egyptian Patients

Background: Circulating microRNAs are potential biomarkers of diagnostic and prognostic impact in various inflammatory and malignant diseases. Aim: Linking inflammation with malignancy, we studied miRNA-21 in sera of hepatitis-C-virus (HCV) and none hepatitis diffuse large B-cell lymphoma (DLBCL) patients, aiming to identify its differential expression and prognosis in DLBCL with its subtypes;germinal center B-cell (GCB) and activated B-cell-like (ABC) and to evaluate its relation with HCV. Subjects and Methods: MiRNA-21 expression was measured using TaqMan quantitative RT-PCR in sera of 30 newly diagnosed DLBCL patients (HCV positive (n = 10), HCV negative (n = 20)) and 20 controls (HCV positive (n = 10), HCV negative (n = 10)). Results: MiRNA-21 expression was significantly higher in DLBCL patients than in control (p = 0.00). Significant positive correlations between miRNA-21 and LDH, IPI and disease stage were detected (p Conclusion: Our study shows that miRNA-21 is over expressed in our patients with DLBCL, displaying higher levels in ABC than in GCB subtypes. MiRNA-21 is associated with poor response to treatment and survival in DLBCL. MiRNA-21 is a potential marker of necro-inflammation independent of its role in tumorogenesis, showing higher expression in HCV positive DLBCL patients compared to none hepatitis patients.

Diagnostic and prognostic value of plasma level of microRNA-92a in acute myeloid leukemia

Background: MicroRNAs (miRNAs) are short noncoding RNAs of ~21 to 23 nucleotides in length that post-transcriptionally regulate mRNA expression. Highthroughput methodologies have shown deregulated miRNA expression in an increasing number of human cancers. MiRNA expression patterns have been found to distinguish tumors of different developmental origin, even better than traditional mRNA expression profiling. Aim: To assess the plasma level of microRNA-92a in adult acute myeloid leukemia and to correlate it with prognostic factors and therapeutic response. Patients and Methods: This study was carried out on fifty AML patients as well as fifty healthy subjects as control. Conventional cytogenetics was performed on patients group only while measurement of the plasma level of miRNA-92a using TaqMan quantitative RT-PCR with miRNA-638 as endogenous reference for standardization and FLT3/ITD mutation was performed on patients and controls. Results: The differences in the ratio or relative quantitation (RQ) of plasma miRNa-92a to miRNA-638 in patients group to the control group have confirmed statistical significance. Also there was significant negative correlation between RQ of miRNA-92a and white blood count in patient group. Patients who achieved a response after induction chemotherapy had a mean RQ of miRNA-92a higher than non-responder with statistical significance. With regard to cytogenetics, favorable risk cytogenetics had meant RQ of miRNA-92a that was comparable to intermediate risk cytogenetics. While poor risk cytogenetics had a mean RQ which is significantly lower than both favorable and intermediate risk cytogenetics. Summary/Conclusions: Our data suggest the potential importance of the microRNA-92a as noninvasive cancer biomarkers helping in diagnosis, clinical prediction and therapeutic response.

The global burden of hepatitis B virus(HBV)and hepatitis C virus(HCV)infections and coinfection represents a major public health concern,particularly in resource-limited *** of HCV by 2030 has become foreseeable,with effective direct-acting antiviral oral therapies and the availability of affordable generics in low-and-middle-income countries(LMICs).However,access to oral nucleos(t)ide therapy for HBV remains critical and is limited outside the existing global HIV program platforms despite affordable *** of mother-to-child transmission of HBV through scaling up of birth dose implementation in LMICs is essential to achieve the 2030 elimination *** individuals living with HBV and/or HCV in resource-limited settings are unaware of their infection,and with improved access to medications,the most significant barrier remains access to affordable diagnostics and preventive *** coronavirus disease 2019 pandemic interrupted hepatitis elimination programs,albeit offered opportunities for improved diagnostic capacities and raised political awareness of the critical need for strengthening health care services and universal health *** review underpins the HBV and HCV management challenges in resource-limited settings,highlighting the current status and suggested future elimination strategies in some of these *** efforts should continue to improve awareness and political *** resources should be secured to access and implement comprehensive strategies for diagnosis and linkage to care in resource-constrained settings to fulfill the 2030 elimination goal.

Accuracy between clinical and radiological diagnoses compared to surgical orbital biopsies

AIM:To assess the concordance between diagnosing orbital lesions by clinical examination,orbital imaging,and histological evaluation,in order to help guide future research and clinical ***:A retrospective analysis was undertaken at a large regional tertiary referral centre of all surgical orbital biopsies performed over a 5-year period,from 1st January 2015 until 31st December *** and concordance between clinical,radiological and histological diagnoses are reported as percentage sensitivity and positive predictive ***:A total of 128 operations involving 111 patients were ***,sensitivities of 47.7%for clinical and 37.3%for radiological diagnoses were found when compared to the histological gold *** lesions that have characteristic clinical and radiological features had the highest sensitivity at 71.4%and 57.1%,*** conditions showed the lowest sensitivity in both clinical(30.3%)and radiological(18.2%)*** PPV for inflammatory conditions were 47.6%for clinical and 30.0%for radiological ***:Accurate diagnoses are difficult to reach by relying on clinical examination and imaging *** orbital biopsy with histological diagnosis should remain the gold standard approach for definitively identifying orbital *** larger scale prospective studies would help further refine concordance and guide future research avenues.

Serologic, endoscopic and pathologic findings in pediatric celiac disease: A single center experience in a low/middle income country

BACKGROUND Studies in Africa,Asia,and Latin America are needed to provide a comprehensive picture of the global incidence of celiac disease(CD).AIM To describe the serology,endoscopic and histological findings in typical and atypical presentations of pediatric CD at a tertiary referral hospital in an African low/middle income country(LMIC).METHODS This observational study was conducted on 199 patients with CD from 2010 to *** patients were divided into typical and atypical groups according to the presenting symptoms including 120 and 79 patients ***,upper gastrointestinal endoscopy with duodenal biopsy were performed for patients who had symptoms suggestive of *** severity of the intestinal damage was graded according to the histo-pathologic Marsh-Oberhuber *** Chronic diarrhea was the main intestinal presentation in the typical *** was the most common extraintestinal symptom in both the typical and atypical ***-Oberhuber type 3b and 3c was significantly higher in the seropositive patients with a P value of 0.007.A significant correlation was observed between the histological grade of the biopsied duodenal mucosa and the clinical presentation(P<0.001).Age was significantly higher in the atypical group(P value<0.001).CONCLUSION Although typical CD was observed in 120 patients in this study,the clinical variability of the condition was frequently *** only was a significant predictor for the appearance of atypical ***,CD presentations in LMIC are not different from industrialized countries.

In most isolated sites situated in south Algeria, the diesel generators are the major source of electrical energy. Indeed, the power supply of these remote regions still poses order problems （technical, economical and ecological）. The electricity produced with the help of diesel generators is very expensive and responsible for CO2 emission. These isolated sites have significant wind energy potential. Hence, the use of twinning wind-diesel is widely recommended, especially to reduce operating deficits. The objective of this paper is to study the global modeling of a hybrid system which compounds wind turbine generator, diesel generator and storage system. This model is based on the control strategy to optimize the functioning of the hybrid system and to consolidate the gains to provide proper management of energy sources （wind, diesel, battery） depending on the load curve of the proposed site. The management is controlled by a controller which ensures the opening/closing of different power switches according to meteorological conditions （wind speed, air mass, temperature, etc）.

Background:Management of severe velopharyngeal dysfunction is best performed by a multispecialty *** team could include a speech-language pathologist,otolaryngologist,prosthodontist,and a plastic *** most commonly performed surgical procedures in complicated cases with scarred soft palate are sphincter pharyngoplasty and pharyngeal *** this study,a multidisciplinary approach was applied for proper assessment and surgical intervention using sphincter pharyngoplasty for velopharyngeal insufficiency after cleft palate ***:Twenty patients underwent sphincter *** diagnosis was performed using auditory perceptual assessment,nasoendoscopy assessment,nasometry,and ***:There were statistically significant differences between the preoperative and postoperative *** occurred in two *** sleep apnea occurred in three patients and was resolved spontaneously within three months,and one patient experienced slight wound ***:Velopharyngeal dysfunction after cleft palate repair is best treated by a multidisciplinary team through speech therapy together with sphincter pharyngoplasty.
